Desiccated Air Bag Inflator Rupture

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. AIR BAGS:FRONTAL:PASSENGER SIDE:INFLATOR MODULE

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2003-2008 Corolla, 2005-2008 Corolla Matrix, and Pontiac Vibe vehicles equipped with a front passenger air bag assembly replaced under a prior recall. The air bag may not unfold as designed during inflation under high temperature conditions, possibly resulting in the air bag not inflating properly.

Risk: An improperly inflated air bag increases the risk of injury during a crash.

18V883000

AIR BAGS:FRONTAL:PASSENGER SIDE:INFLATOR MODULE

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2002-2005 Lexus SC and Toyota Sequoia, 2003-2005 Toyota Corolla and Tundra, and 2003-2007 Pontiac Vibe vehicles. These vehicles are equipped with certain air bag inflators assembled as part of the passenger frontal air bag modules used as original equipment or replacement equipment. In the event of a crash necessitating deployment of the passenger frontal air bag, these inflators may explode due to propellant degradation.

Risk: An inflator explosion may result in sharp metal fragments striking the driver or other occupants resulting in serious injury or death.

15V286000

AIR BAGS

Toyota Motor Engineering & Manufacturing (Toyota) notified the agency on May 13, 2015, that they are recalling 2005-2007 Lexus SC, Toyota Corolla, Corolla Matrix, Sequoia, and Pontiac Vibe, and 2005-2006 Toyota Tundra vehicles that were originally sold, or ever registered, in Florida, along the Gulf Coast, Puerto Rico, Hawaii, the U.S. Virgin Islands, Guam, Saipan and American Samoa. On June 16, 2015 Toyota expanded the recall to cover an additional 769,055 vehicles, including certain model year 2002-2004 Lexus SC, Toyota Sequoia, 2003-2004 Toyota Corolla Matrix, Toyota Corolla, Pontiac Vibe, and Toyota Tundra vehicles, as well the vehicles covered by the May 13, 2015 notification in the portions of Georgia, Alabama, Louisiana, Mississippi and Texas that were not previously included. The inflators in the front passenger air bags are susceptible to rupture in the event of a crash necessitating the deployment of the front passenger air bag.

Risk: In the event of a crash necessitating deployment of the passenger's frontal air bag, the inflator could rupture with metal fragments striking the vehicle occupants potentially resulting in serious injury or death.

15V285000

AIR BAGS

Toyota Motor Engineering & Manufacturing (Toyota) notified the agency on May 13, 2015 that they are recalling certain model year 2004 Sequoia vehicles manufactured August 18, 2003, to August 19, 2004, and 2003-2004 Tundra vehicles manufactured March 18, 2003, to August 25, 2004. On June 16, 2015, Toyota expanded the recall to include an additional 1,270,468 vehicles, including model year 2002-2003 and 2005-2007 Sequoia, 2005-2006 Tundra, 2003-2007 Corolla, 2003-2007 Corolla Matrix, 2003-2007 Pontiac Vibe and 2002-2007 Lexus SC vehicles. This is to address a safety defect in the passenger side frontal air bag inflator which may produce excessive internal pressure causing the inflator to rupture upon deployment of the air bag. This recall addresses both the passenger side frontal air bags that were originally installed in the vehicles, as well as replacement air bags that may have been installed as replacement service parts. A replacement air bag may have been installed, as one example, if a vehicle had been in a crash necessitating the replacement of the passenger side frontal air bag.

Risk: In the event of a crash necessitating deployment of the passenger's frontal air bag, the inflator could rupture with metal fragments striking the vehicle occupants potentially resulting in serious injury or death.

ENGINE AND ENGINE COOLING

GM IS RECALLING CERTAIN MODEL YEAR 2005-2008 PONTIAC VIBE VEHICLES. THE ENGINE CONTROL MODULE (ECM) FOR THE SUBJECT MODELS EQUIPPED WITH THE 1ZZ-FE (LV6) ENGINE AND TWO-WHEEL DRIVE MAY HAVE BEEN IMPROPERLY MANUFACTURED. THERE IS A POSSIBILITY THAT A CRACK MAY DEVELOP AT CERTAIN SOLDER POINTS OR ON VARISTORS ON THE CIRCUIT BOARD.

Risk: THERE ARE A VARIETY OF WARNINGS AND CONSEQUENCES ASSOCIATED WITH THE DEFECT. THE ENGINE WARNING LAMP COULD BE ILLUMINATED, HARSH SHIFTING COULD RESULT, THE ENGINE MAY NOT START, OR THE ENGINE COULD SHUT OFF WHILE THE VEHICLE IS BEING DRIVEN. AN ENGINE SHUTOFF WHILE THE VEHICLE IS BEING DRIVEN INCREASES THE RISK OF A CRASH.

Canada runs its own recall programme, and its campaigns do not always mirror the American ones.

Recall 2019436 19,620 units in Canada

Airbag

Issue: On certain cars, the passenger-front airbag that was installed during prior recall repair must be replaced. The airbag may not unfold properly when it deploys. This could cause the airbag to tear or not inflate properly in a crash. Safety Risk: An airbag that does not work properly could create an increased risk of injury in a crash. Corrective Actions: General Motors will notify owners by mail and instruct them to take their vehicle to a dealer to replace the passenger-front airbag assembly.

Recall 2018707 29,827 units in Canada

Airbag

On certain vehicles, long-term exposure to high absolute humidity and temperature, combined with high temperature cycling, could eventually degrade the propellant contained in the passenger frontal airbag. This could cause the airbag to deploy with more force than normal. As a result, if the vehicle were involved in a collision that warrants airbag deployment, fragments could be propelled toward vehicle occupants, and the airbag assembly could be damaged, preventing proper function. These issues could increase the risk of injury. Correction: Dealers will replace the passenger frontal airbag module.<br><br> Note: Canadian climate results in the propellant degrading slowly. The recall is being conducted as a precaution to address future risk, and is expected to replace airbag inflators before their function would be affected. Vehicles may have had the original front passenger airbag module replaced as a "like-for-like" replacement under recall 2015-272 (GM Recall #14491) as a temporary remedy while evaluating a permanent remedy. These vehicles will receive the permanent remedy passenger airbag assembly under this recall.

Recall 2016106 60,672 units in Canada

Airbag

On certain vehicles, the passenger (frontal) airbag inflator could produce excessive internal pressure during airbag deployment. Increased pressure may cause the inflator to rupture, which could allow fragments to be propelled toward vehicle occupants, increasing the risk of injury. This could also damage the airbag module, which could prevent proper deployment. Failure of the passenger airbag to fully deploy during a crash (where deployment is warranted) could increase the risk of personal injury to the seat occupant. Note: This recall supersedes recalls 2013-116, 2014-301 and 2015-272. Correction: Dealers are to replace the front passenger airbag assembly with one equipped with a newly specified inflator.

Recall 2015272 55,595 units in Canada

Recall 2010287 37,412 units in Canada

Engine

On certain vehicles equipped with a 1.8L MFI engine and a two-wheel drive transmission, the Engine Control Module (ECM) may have been improperly manufactured. There is a possibility that a crack may develop at certain solder points or on varistors on the circuit board. In most cases, if a crack occurs at certain points or on certain varistors, the engine warning lamp could be illuminated, harsh shifting could result, or the engine may not start. In some instances, if cracking occurs on particular solder points or varistors, the engine could stop while the vehicle is being driven, increasing the risk of a crash causing personal injury. Correction: Dealers will inspect and, if necessary, replace the ECM.

I HAD A RECALL ON THE PASSENGER SIDE AIRBAG OF MY CAR A FEW YEARS AGO. I HAD IT REPLACED WITH THE SAME, BUT NEWER PART. I THEN RECEIVED ANOTHER NOTIFICATION AND CALLED IN TO GET IT FIXED, BUT NO PARTS WERE AVAILABLE. I RECEIVED TWO OR THREE MORE NOTICES AND CALLED FOR THE FIX, BUT THE PART WAS STILL NOT AVAILABLE. TODAY, ABOUT A YEAR OR SO LATER, I RECEIVED ANOTHER NOTICE, MADE ANOTHER CALL, AND THEY WERE FINALLY ABLE TO ORDER A REPLACEMENT PART. HOWEVER, THEY STILL DON'T KNOW HOW LONG IT WILL TAKE TO COME IN.

I'M SORRY IF THIS IS THE WRONG VENUE FOR THIS, BUT THERE IS AN OPEN RECALL ON MY CAR DEALING WITH AIRBAG ISSUES. IT HAS BEEN ALMOST AN ENTIRE YEAR AND THE RECALL STATUS IS "RECALL INCOMPLETE, REMEDY NOT YET AVAILABLE." FRANKLY, THIS IS UNACCEPTABLE. I SAW A NOTE ON YOUR WEBSITE SAYING THAT IF THE MANUFACTURER HAS FAILED OR IS UNABLE TO REMEDY THE RECALL IN A TIMELY MANNER, WE COULD FILE AN ONLINE COMPLAINT. I COULDN'T FIND THE FORM FOR THAT, SO I FIGURED I'D SUBMIT IT HERE. THANK YOU! MANUFACTURER RECALL NUMBER: N192273760 NHTSA RECALL NUMBER: 19V627

THERE HAS BEEN A KNOWN AIRBAG RECALL ON MY 2005 SINCE 08/29/19. TEN MONTHS IS FAR TOO LONG FOR THIS TO GO UNADDRESSED BY GM WHEN IT AFFECTS A MAJOR AIRBAG ON A VEHICLE IMPACTING THE SAFETY OF PASSENGERS. I HAVE GONE MOST OF A YEAR WITHOUT BEING ABLE TO HAVE PASSENGERS IN THE FRONT SEAT OF MY CAR BECAUSE OF THIS RECALL. THIS IS THE SECOND TIME THE FRONT PASSENGER AIRBAG HAS BEEN RECALLED ON MY CAR THE LAST RECALL WAS IN 2016 AND I HAD THE AIRBAG REPLACED THEN. GM NEEDS TO STEP UP AND WORK TO GET THE NECESSARY REPLACEMENT PARTS AVAILABLE OR THEY NEED TO PLAY A PART IN HELPING THEIR CUSTOMERS PURCHASE NEW CARS THAT ARE SAFE AT A DISCOUNTED PRICE TO MAKE UP FOR NEARLY A YEAR OF NOT BEING ABLE TO SAFELY CARRY A PASSENGER IN THE FRONT SEAT OF THE VEHICLE. THIS IS IN REFERENCE TO A RECALL DATED DEC 12,2018 THE MANUFACTURER HAS FAILED TO REMEDY THIS RECALL IN A TIMELY MANNER AND SINCE I CANNOT DRIVE WITH A PASSENGER IN THE FRONT SEAT UNTIL IT IS REMEDIED I AM REPORTING THIS TO NHTSA AS RECOMMENDED ON THE NHTSA WEBSITE. MANUFACTURER RECALL NUMBERN161644424 NHTSA RECALL NUMBER18V883 RECALL STATUS: RECALL INCOMPLETE, REMEDY NOT YET AVAILABLE --- I HAVE CHECKED WITH GM CUSTOMER SERVICE AND THERE IS STILL NO FIX FOR THIS PROBLEM - THE NHTSA WEBSITE STATES THAT UNTIL IT IS FIXED NO PASSENGER SHOULD SIT IN THE FRONT PASSENGER SEAT. I CALLED CUSTOMER SERVICE AND REQUESTED THAT AT LEAST THEY INSTALL A NEWER AIRBAG WHICH WOULD BE SAFER THAN THE OLDER POTENTIALLY DEGRADED AIRBAG. THEY DO NOT CHOOSE TO DO THAT AND STILL THERE IS NO FIX FOR THIS PASSENGER SIDE AIRBAG. I AM TRYING TO GET THE RECALL REPAIR FIXED BUT NOT HAVING ANY SUCCESS WITH LOCAL OR NATIONAL GM REPAIR SHOP OR CUSTOMER SERVICE AT THE NATIONAL LEVEL. THEY DO NOT HAVE ANY INFORMATION ON WHEN OR IF A REPAIR SOLUTION WOULD BE AVAILABLE. SINCE THE MANUFACTURER HAS FAILED TO REMEDY THIS RECALL IN A TIMELY MANNER AND SINCE I CANNOT DRIVE WITH A PASSENGER IN THE FRONT SEAT UNTIL IT IS REMEDIED I AM REPORTING THIS TO NHTSA AS RECOMMENDED ON THE NHTSA WEBSITE.
