Rollx Vans (Rollx) is recalling certain GMC Yukon, Sierra, Chevrolet Express 2500, Ford Transit, E-250, Toyota Sienna, Sienna Hybrid, Honda Odyssey, Chrysler Town and Country, Voyager, Pacifica, Dodge Grand Caravan, and Ram Promaster vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. Please refer to Rollx's recall report for specific model year information. The retractors may not lock, preventing the wheelchair from being properly secured.

Chrysler (FCA US, LLC) is recalling certain 2019-2021 Ram ProMaster and 2019-2020 Dodge Journey vehicles. Cracks in the microprocessor may cause the vehicle's rearview camera image not to display properly. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility.

Risk: A rearview image that does not display properly reduces the driver's view behind the vehicle, increasing the risk of a crash.

Chrysler (FCA US, LLC) is recalling certain 2019-2021 Ram ProMaster vehicles equipped with a 62TE transmission. The park pawl may not fully engage the park gear due to debris inside the transmission.

Risk: A partially engaged park pawl can result in a vehicle rollaway, increasing the risk of a crash or injury.

Chrysler (FCA US LLC) is recalling certain 2020 Ram Promaster vans and 1500 pickups. The affected vehicles may have an improperly crimped diffuser in the side curtain airbag which may result in the diffuser detaching from the inflator in the event of an air bag deployment.

Risk: In the event of a crash, if the diffuser detaches from the inflator during air bag deployment, the air bag may not inflate properly, increasing the risk of injury.

Issue: On certain vehicles, the wiring for the 400 W radiator cooling fan module may not have proper circuit protection. Additionally, the bearing for the fan could fail. If this happens, the wiring could overheat and melt before the fuse blows. Safety Risk: Wiring that overheats and melts could create the risk of a fire. Corrective Actions: FCA Canada will notify owners in writing and advise you to take your vehicle to a dealership to replace the cooling fan module and fuse. The dealer will also install a label on the power distribution center and add an insert to the owner's manual.

Issue: On certain vehicles, a problem with the rearview camera could cause the rearview camera image not to display correctly or at all. Safety Risk: A rearview camera image that doesn't display correctly could reduce a driver's ability to see behind the vehicle while backing up. This could increase the risk of a crash. Corrective Actions: FCA Canada will notify owners in writing and advise you to take your vehicle to a dealership to replace the rearview camera.

Issue: On certain vehicles, the tire and loading information label does not list that there is no spare tire in both official languages. The label reads as "NONE" when it should read as "NONE / AUCUN". Canadian regulations require the information to be listed on the label in both English and French. Safety Risk: This error does not affect vehicle safety. Corrective Actions: This is not a recall. The company has corrected the tire and loading information label for all new vehicles.

Issue: On certain vehicles, the transmission parking pawl may not fully engage when you shift to park. If this happens, and the parking brake isn't used, the vehicle could roll away. Safety Risk: A parked vehicle that moves suddenly could create the risk of an injury or a crash. Corrective Actions: FCA Canada will notify owners by mail and instruct you to take your vehicle to a dealership to update the powertrain control module (PCM) calibration software. The dealer will also inspect and, if necessary, replace the transmission or parts inside the transmission. To reduce the safety risk, FCA Canada recommends that you should always use the parking brake when you park.

Issue: On certain wheelchair-accessible vehicles, the compliance label has incomplete information. Also, the tire and loading information label may not have been updated to include the new seating capacity and the maximum weight of occupants and cargo. This could lead to overloading of the vehicle and cause poor handling or a tire failure. Safety Risk: Overloading of the vehicle could cause a loss of control and result in a crash. Corrective Actions: Dynamic Specialty Vehicles will mail new compliance and tire and loading information labels to each owner, along with instructions for how to install them.

ELECTRICAL SYSTEM

Pertaining to the open recall: Manufacturer Recall Number59C NHTSA Recall Number25V-552 We had called last year when the back camera started acting up; was told there was a pending recall to release by the end of the year (Dec 2025). Called back January 2026 to see about a fix on the recall, was notified they have the recall, but there wasn't a part for the fix yet and to call back later. Called back March 2026 because the issue seems to be getting worse and the rear camera sometimes just doesn't turn on at all; and was told there was still no part yet. So was going to wait another two months before calling again. But today (April 2026) one of our drivers accidently backed into a truck, luckily all of our drivers have been hyper cautious so they weren't going very fast so there were no damages to the other vehicle. Not having a fix for the rear view camera system still, has made our drivers feel uneasy when they have to take this van out. It's definitely a huge safety issue, especially since there is no visibility to look out the rear windows. A fix for this would be greatly appreciated.

Pertaining to the open recall: Manufacturer Recall Number59C NHTSA Recall Number25V-552 We had called last year when the back camera started acting up; was told there was a pending recall to release by the end of the year (Dec 2025). Called back January 2026 to see about a fix on the recall, was notified they have the recall, but there wasn't a part for the fix yet and to call back later. Called back March 2026 because the issue seems to be getting worse and the rear camera sometimes just doesn't turn on at all; and was told there was still no part yet. The contact owns a 2020 Ram Promaster. The contact stated that while driving at approximately 25 MPH another vehicle in the opposite lane had crashed head on into his vehicle. The contact stated that the air bag did not deploy upon impact , however after he hit his head on the steering wheel , the air bag deployed. The contact stated he and the other driver did sustain head injuries, and required medical attention. The contact stated that a police report was filed and the ambulance arrived to transport them to the hospital. The contact stated the police had the vehicle towed to collision center, where it remained. The dealer and the manufacturer were not notified of the failure. The failure mileage was 19,100.

Newark, CA Crash

VISIBILITY/WIPER

While traveling away from my home and over the course of 4,000 miles, the driver's and passenger sideview mirrors would intermittently blow inward suddenly. This impaired my ability to view anyone in my blind spots, but the passenger one was worse due to having to pull off the side of the road to pull it back into position. Both mirrors are powered but the buttons to make the mirror go back into position would not work when they were blown in. On 5/11/2021 @ approx. 6:20pm, I was driving on I-880S in Oakland, CA going 65mph . When I changed lanes to go into the fast/passing lane, the passenger side mirror blew in. Although I was following at least 3 car lengths in front of me, the 4 cars in front of me were bumper-to-bumper with the first car having nobody in front of them for several car lengths. That front car suddenly braked very hard due to noticing a tow truck pulling another vehicle off of the offramp (4 lanes away from them), which caused the cars following it to scramble. Since my vehicle is so heavy and I didn't want to possibly not stop in time, I had no choice but to pull into the emergency lane as I braked as I could not see if I could do an evasive maneuver by pulling into the lane on my right with the passenger mirror having blown in. Upon pulling into the emergency lane there was a stationary tire still on its rim that I tried to go around but hit with my front passenger tire. It popped the tire and caused body damage to the vehicle. Despite going through FCA's process of warranty replacement through their acting agent, Fremont CDJR dealership, the mirrors were not replaced with non-defective ones but with another set of broken mirrors. FCA's risk assessor came out to inspect the mirrors but FCA has not contacted me at all about the incident. Reporting this incident so that others will not be put in an unsafe predicament and be unable to replace the mirrors properly.

The contact owns a 2020 Ram Promaster 3500. The contact received notifications of NHTSA Campaign Numbers: 25V552000 (Back Over Prevention) and 25V720000 (Engine and Engine Cooling); however, the parts to do the recall repairs were unavailable. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repairs. The contact stated that while in reverse(R), the back over prevention camera failed to properly operate and failed to display an image of the rear of the vehicle. The failure was intermittent. The coolant level failed to remain at the proper levels and coolant needed to be added frequently. The contact stated that the battery suddenly lost power and need to be recharged more frequently. The dealer was contacted several times; however, the part to do the recall repair was not yet available. The dealer was recently contacted, and the contact was informed that an inspection was needed before ordering the parts. The dealer stated that it might take a day or two to complete the inspection and order the parts but could not provide a time for when the parts would be received or when the repair would be completed. Related recall: NHTSA 25V720000 / Chrysler 67C — "The electrical circuit for the radiator fan may overheat." Fire/injury: No fire / no injuries; component overheated and burned/melted. Complaint narrative (paste this into the form) The radiator cooling fan on my 2020 Ram ProMaster 3500 (3.6L gasoline V6) repeatedly became stuck running / would not shut off on multiple occasions. The cooling fan electrical circuit overheated, and the fuse and fuse box (power distribution center) burned and melted, which in turn damaged the adjacent wiring harness. The fuse did not protect the circuit before the overheating and thermal damage occurred. This is the identical failure mode described in NHTSA recall campaign 25V720000 (Chrysler recall 67C), which states that "the electrical circuit for the radiator fan may overheat" and that "electrical circuits that overheat increase the risk of a fire," with a remedy of replacing the cooling fan module and fuse. My vehicle is a 2020 ProMaster, within the 2018–2026 model years covered by that recall. However, when I contacted Ram/Chrysler, I was told my VIN is not included in the recall population. I am filing this complaint because my vehicle experienced the exact defect the recall was issued to address — a stuck-on radiator cooling fan causing the fan electrical circuit to overheat and burn the fuse box and wiring harness, creating a fire risk. I had to pay to repair this damage. I request that NHTSA review whether the recall population should be expanded to include my VIN and other affected vehicles exhibiting this same failure. Documentation available on request: photographs of the burned/melted fuse box and damaged wiring harness, and repair invoices showing replacement of the cooling fan/fan module, fuse box, and wiring.

The rear backup camera is intermediate and does NOT provide a visible image 90% of the time. When the vehicle is in reverse the display reports a message, "Camera System Unavailable". The van doesn't have rear or side windows and makes it nearly impossible to safely backup the vehicle.
