The contact stated that while driving at various speeds with the cruise control feature engaged, the brakes independently activated without the brake pedal being depressed. 2015 Tesla Model S Greenwich, CT

Parking brake on my TESLA MODEL S will not release. My understanding is that this is from a defective parking brake caliper that was under a recall by TESLA. However, TESLA refuses to acknowledge or repair this issue which is a huge safety problem especially if the parking brake does not engage / release when on an uneven surface. Also, from looking at forums and speaking to other owners, this appears to be a very common issue.

2015 Tesla Model S at 170,000 miles Naples, NY

The contact stated that while driving approximately 45 MPH, and approaching a red traffic light, the brake pedal was depressed; however, the braking system became inoperable. The vehicle eventually regained braking functionality. During the failure, the regenerative braking and stability control warning lights were illuminated. The dealer was contacted; however, the vehicle was not diagnosed or repaired. The manufacturer was notified of the failure. The approximate failure mileage was 170,000.

2014 Tesla Model S Coronado, CA Crash 1 injured

I have been driving my Model S Tesla for 13 years. On 30 April as I was pulling into a parking space at my local grocery store, I applied the brakes to ease into the space and my car surged forward with unintended acceleration. I smashed into the concrete base of a light pole, the airbags deployed and my car ricocheted back about 8 feet. There were no warning messages. I was taken to the emergency room and had whiplash and a burn on my leg from the airbag. No one else was hurt. I know it was the car that malfunctioned because my foot was still on the brake after the collision and the airbags bruised the right side of my right leg. It could not have bruised it in that way if my foot had been on the accelerator. The insurance company inspected and declared a total loss. I obtained the black box data from Tesla and am uploading the documents below. The car is currently at the Tesla collision center. They do not seem interested in evaluating the problem.

2018 Tesla Model S Folsom, CA Crash

I pulled my Tesla Model S in a parking lot, and as I turned the car into a parking spot, the car suddenly accelerated, jumped the curb next to the parking lot and hit a tree. The car speed when I pulled into the parking lot and then into a parking spot was very low. so I was very surprised and startled to see the car suddenly on its own accelerating and jumping the curb.

I noticed it happened before but I couldn't quite figure out what happened! I was driving down the local road and got ready to go under a overpass and next thing u know my EV just slammed on brakes like I was about to hit someone! Seatbelt choked me and made me gag! But when I came through this time It caught me off but after that I realized there's a problem,especially going under that overpass! This couldn't been very dangerous to anyone! 2023 Tesla Model S Lehigh Acres, FL

Tesla just sold me a car with defective front half shafts axles. The car makes a reproducible and very concerning accelerator pedal shudder that makes the entire vehicle front vibrate, including the steering wheel and brake pedals while the car is accelerating between 30-60mph. This interferes with safety features like, lane departure assistance and blind spot steering wheel feedback, Confusing the driver and creating a distracting scenario where an accident can happen. Upon bringing this to the attention of the service center in Fort Myers Florida, the service center manager took a drive test with me, and was able to replicate the issue, but immediately started dismissing it as "normal" and within Tesla specs. This is becoming a regular practice at that service center, where they deem warrantable issues as normal and within (their tolerance). I insisted that this issue is not to be taken likely by Tesla, because it is something that involves a safety hazard. Upon further research, I was able to find that this issue is happening to many Teslas model S and X, where the front axles bend due to the weight and high torque of the vehicles. The issue falls completely under warranty and the half shafts should be replaced, but apparently Tesla has instructed their service centers to dismiss and disregard the symptoms, and just tell the customers that no warranty replacement will occur, because it is not necessary, we just have to drive the cars like this, until the car completely fails, or an accident happens because one of the bent axle shafts breaks on a highway during normal drive. I recording this report as evidence that the issue has been reported to the highest vehicle safety authorities, and I expect a corrective action so Tesla can fix my car under the existing unexpired warranty.

2022 Tesla Model S Fort Collins, CO

The center high-mounted stop lamp (CHMSL / third brake light) fails on 2021 and newer Tesla Model S vehicles (including Plaid). This is a common issue reported by many owners, particularly as vehicles exit the warranty period. No redesigned or corrected replacement part has been made available by Tesla. Pre-2021 CHMSL units are incompatible and will not fit. Repair requires removal and replacement of the entire rear liftgate glass assembly, which is extremely expensive and labor-intensive for owners. Prior model years did not experience this problem. A non-functional CHMSL reduces the vehicle's visibility to following traffic and increases the risk of rear-end collisions, especially at night or in poor weather. This appears to be a design or manufacturing defect affecting safety. 2013 Tesla Model S at 108,000 miles Waldrof, MD

The contact owns a 2013 Tesla Model S. The contact stated that the parking brake seized while leaving a charging station, and the vehicle screeched into oncoming traffic and stalled. The parking brake warning light was illuminated. The local dealer was contacted, and an appointment was scheduled. The vehicle had not been charged prior to a snowstorm, and the battery became fully drained. The vehicle was taken to the local dealer, where it was determined that the main battery and 12-Volt battery needed to be replaced before the diagnostic test. The vehicle was repaired, but the failure reoccurred. The vehicle failed to shift into drive or move. The jack mode was disengaged. While driving, the rear wheels were screeching. The contact stated that the failure was related to TSB: SB-21-33-002). The vehicle was towed to another local dealer, Tesla Service Arlington-Glebe Rd (2710 S Glebe Rd, Arlington, VA 22206). Incident Description: While driving through a parking lot at 20 mi/hr, my vehicle's AEB system activated in response to another vehicle approaching from a perpendicular direction. However, the system's response was dangerously delayed and inappropriate stopping the car right in front of the other car: The system failed to provide any advance warning or alert before applying the brakes The braking occurred too late to create adequate separation from the approaching vehicle The vehicle stopped directly in the path of the oncoming car, increasing rather than decreasing collision risk The abrupt stop left my vehicle in a position where I was at greater risk of being T-boned Only the other driver's quick reaction prevented a collision. Had that driver not stopped in time, the AEB system would have contributed to, rather than prevented, a serious accident.

2018 Tesla Model S Palmdale, CA

My 2018 Tesla Model S has a recurring safety-critical defect involving the steering system and the Chassis CAN communication network. On October 20, 2025, the vehicle had to be towed to a Tesla Service Center because it would not power on and displayed multiple safety alerts (traction control disabled, stability control disabled, ABS disabled, and reduced steering assist). Tesla documented that the steering rack was “bringing down the Chassis CAN” and stated the issue might require a new steering rack and coolant pump if it returned. The warnings have now returned, and the vehicle is again showing active alerts for disabled traction control, disabled stability control, and previously reduced steering assist. This is a repeat of the same unsafe condition that Tesla attempted to repair. The car is unsafe to drive and could cause loss of steering control. This is a recurring steering/Chassis CAN failure with major safety system disablement. I am reporting this because the defect affects vehicle control and increases the risk of a crash.

On [XXX], my 2018 Tesla Model S (VIN [XXX] ) experienced a complete brake system failure while in motion. The brake pedal was pressed multiple times, but the vehicle did not stop, resulting in a collision. The emergency brake also failed to engage. 1. Component/System Failure: The braking system, including both the standard hydraulic brakes and the electronic emergency brake, failed to operate. The vehicle is available for inspection upon request. 2. Safety Risk: This put my safety and the safety of others at immediate risk, as I was unable to stop the vehicle to avoid an accident. 3. Confirmation of Problem: The problem has not yet been reproduced or confirmed by a Tesla service center since the accident, but the vehicle previously had recalls in 2021 and January 2024 for brake and emergency brake issues that Tesla claimed to have remedied. 4. Inspection: The vehicle has been involved in an accident and has been documented by police and insurance representatives. Tesla has not yet inspected the car after the most recent failure. 5. The contact owns a 2016 Tesla Model S. The contact stated while attempting to park the vehicle, the brake pedal was depressed and failed to respond. There were no warning lights illuminated. The contact depressed the brake and the vehicle kept rolling forward. The contact opened the door for the vehicle to come to a stop. On another occasion, the contact stated that the ADAS feature engaged and while driving at various speeds, and the vehicle failed to recognize stop signs, traffic lights, or pedestrians. The contact stated that the function was a paid feature; however, the system failed to operate as designed. On another occasion, the contact stated that the steering will shift to one side while driving at various speeds, and the vehicle became difficult to steer. •I’m not sure what component was responsble for either the braking issue or the fire. It is available for inspection. •When driving the brake pedal had a frightening delay that almost caused 2 accidents and unfortunately my car was charging on the 24th of October which could have something to do with arching that led to my garage catching fire rendering the car unable to move or turn on after the incident. I’ve insisted Tesla insurance to look into possible causes because the Fire dept. mandated the a LADWP (power company) to install new transformers to our power supply a couple of days after the fire, which they did. Tesla insurance seems uninterested in finding the cause and wants to compensate me for the vehicle but I believe it’s in their best interest to find a cause and document it. The car had no issues prior, in fact the service center sent a mobile tech to replace the 12volt battery on the car not even 10 months ago. After the fire the car died and has a bunch of codes that make it inoperable. • Service center rejected looking at it because of the risk. They said since it was possible salvage they could not look at it which is understandable but they worked on it before in decem •Insurance has been unwilling to look at any of this they just want pictures of the car to settle the issue. In my opinion they are ignoring a bigger problem. Tesla is my insurer. •The car had no issues prior to the fire and none during the near misses were the brake delayed.

I pulled into a driveway on a one lane road (due to construction) to allow a car traveling downhill to pass so they would not have to back up. I had come to a complete stop in the driveway with the car in drive. As I started to put the car in reverse it surged forward at a high rate of speed crashing into my neighbor's garage, tearing the door completely down pushing the door into the garage and my car entering the garage at least 15 feet hitting the car that was parked inside and pushing it with such force that the front end of the vehicle struck the garage cabinets tearing them down. My air bag did not deploy nor did the automatic braking system engage, it just sped up as it was doing the damage. The car has been deemed a total loss by my insurance company but i don't want this report to go unnoticed. To my knowledge the insurance company has not inspected the computer on the vehicle. I'm not sure how they will investigate this occurrence. I feel strongly that this was a malfunction with my car. The message that showed immediately was "driver assist unavailable", I have NEVER used driver assist.
