The LKA system steers the vehicle into the oncoming lane of travel under specific and repeatable conditions. The defect occurs as follows: While the vehicle is traveling and the LKA system is following the left lane marking, the vehicle approaches a marked left-hand turn lane or turnout where no fog line is present on the right-hand side of the road. In this situation, the system abruptly attempts to steer the vehicle toward the turn lane. Because the turn lane is positioned to the left of the driver’s actual lane of travel, this steering input pulls the vehicle toward and into the oncoming lane of traffic. This steering intervention happens without warning and without any input from the driver. Given that it moves the vehicle into a lane carrying oncoming traffic, it presents a direct risk of a head-on collision. Manufacturer Response I contacted Toyota regarding this issue and was told that this behavior is normal, and that the system will use whatever lane marking is available to determine its steering path. Toyota did not indicate that a fix or software update was planned. I do not consider “the system follows whatever line it has” an acceptable explanation for a feature that actively steers a vehicle into oncoming traffic. Why This Is a Safety Defect 1.The system takes an abrupt steering action with no driver input and no warning. 2.The result of that action is movement into a lane used by oncoming traffic, which is the most dangerous possible outcome of a lane keep assist malfunction. 3.The condition that triggers this behavior (a marked left turn lane with no right-side fog line) is common on ordinary two-lane roads and is not something a driver can always anticipate or avoid. 4.The manufacturer has acknowledged the behavior exists and has characterized it as expected operation rather than a defect.

Vehicle Back-up camera works intermittantly, sometimes working when placed in reverse, other times showing a black screen when placed in reverse. Has not been a consistent enough problem for dealer repair/troubleshooting. A google search shows many 5th Gen 4runner online web forms with lots of reports involving pinched, stretched, worn camera wiring harnesses inside the rear hatch or in the rubber wire boot between the body and rear hatch.

The car has the iforce max powertrain. At low speeds or in stop and go traffic, when the car has come to a stop, the car will lurch forward, which could potentially cause an accident. I took car to dealership and they said this is normal operation of the hybrid system and there is no fix. This is a huge safety issue if the car pushes itself forward and potentially causing an accident. The transmission system is very unrefined.

I am not sure this qualifies but I will submit the problem. All of a sudden, the center console display disappeared. Besides showing the radio tuner, it provides access to the GPS, cell phone connection and other devices. It also provides access to the backup camera which does light up when put in reverse. The cell phone connection provides hands free use of the phone which comes in handy during an emergency.

On at least 3 occasions the multimedia touch-screen display console has gone blank without warning lamps/messages. This causes a hazardous condition for 2 main reasons: 1. The GPS navigation system is not visible and one has to pull over in traffic, find a safe place to park, and then change to one’s cell phone system. 2. When this occurs, the reverse camera is no longer visible, thus increasing the chance to crashing into rear crossing traffic or pedestrians. On [XXX], (odometer 16339 mi) I reported it to SW Toyota in Lawton OK. They said that other owners had reported this problem but that it would take months to receive the replacement display, so I should take it in to the Toyota dealer in my home dealership in MS. I did so on 1/16/25, (odometer 17637mi). This dealership, J. Allen Toyota, Gulfport, MS, inspected the vehicle and said that they could not reproduce the problem. They advised me to return if the display turned off permanently. I asked if there was a TSB or recall on this problem and they replied in the negative. I have just read that Toyota has issued a recall on the 2024 & 2025 Tundra, which according to Gemini AI shares the same part as the 2025 4Runner. My backup camera does not work consistently. It will sometimes come on and show objects behind me; other times it is a black screen. I have read where Toyota models from 2022 to newer, but not including the 4 Runner, are having major issues with the back up camera and are under recall. I believe this recall should include the Toyota 4 Runner, as mine seems to be having issues which affect my safety and others. I called the nearest Toyota dealership to verify if my car was under recall and they said no. I have not taken the vehicle to be inspected by the dealership. This issue started about 6 -8 months ago. No lights or warning messages appear on the dashboard. I thought maybe my camera was dirty, but wash the car and it still is inconsistently working. It fails more than it works.

This had happened a year ago. I was driving and my anti collision brake just started to make a beeping noise and was telling me to call a toyota dealer. And before that the anti collision brake would go off when there was nothing in front of me. A few months later my cruise control and lane assist also stopped working. I took it in for a diagnostic and they couldn't tell me the real problem and that it would be 1500 to actually know what the issue is. I have never been in an accident in this car and its annoying that I bought it because of the safety features that I now cant use.

The parking collision assistance feature continues to beep loudly if there is snow on the sensors as it thinks there is something close to the sensor. If driving and unable to clear snow off the sensors in the front a fault will eventually show on the gauge cluster. This is a false reading and needs to be addressed. This is a program issue and needs fixing. As the loud noise while driving is a complete distraction and can cause an accident.

Rear backup camera has been failing or intermittently working, it is available for insp[ection upon request. This has created a situation upon reversing in driveway where I expect it to be on and dont see a vehicle or trailer behind me. I have hit a parked car because I was expecting it to work as I check rearview mirrors. This is now a habit and I did not realize it wasn't working at the time until i collided with the vehicle. The dealership will not confirm it or inspect it unless payment is rendered. No one has inspected it except for Virginia. Which I am unsure that is even something they inspect. They may have just inspected other things that allow for use on highway. No lamps or other warnings when it began in april 2023. I have not tried to get it fixed as this is a new vehicle and I wasn't willing to pay the dealer to diagnose and most likely decline my warranty coverage.

I have a 2016 Toyota 4Runner at 143,011 miles. When it hit 125,000 mile mark the backup camera stopped working intermittently. The screen in the car goes blank. Turning the car on/off or changing gears does not affect this issue. It’s random and seems to becoming more frequent as more miles are put on. I will take it in however when I called them they said since it’s not a known issue I will have to replace it and it’s not cheap!
