BACK OVER PREVENTION: REARVIEW SYSTEM BRAKING

My 2024 Toyota Tundra 1794 Edition, VIN [XXX], is subject to Toyota Safety Recall 25TA06, Interim 25TB06, NHTSA Recall 25V322, concerning reverse lamps that may become inoperable. The recall was announced in May 2025, but a repair remedy has still not been made available for my vehicle. The truck is primarily used for family transportation, including trips and nighttime driving. I experienced a minor, low-speed property-damage incident while reversing in a poorly lit garage area. Visibility behind and beside the vehicle was limited, and the truck contacted a garage-door support column before the garage lighting came on. The incident caused minor cosmetic side damage. There were no injuries, no structural damage, no police report, and no insurance claim. I previously advised Toyota that I had experienced an issue related to the recall. Toyota opened Customer Experience Case [XXX]. Toyota offered to keep my truck at a dealership indefinitely and provide alternate transportation until a remedy became available, but it could not guarantee a comparable Tundra 1794 or provide an estimated repair date. Toyota later opened repurchase review Case [XXX]. I promptly provided the requested documentation, but despite repeated telephone and email follow-ups, Toyota has not provided a meaningful review timeline, vehicle inspection date, or final determination. My primary complaint to NHTSA is the extended period without an available safety remedy and the continuing risk to owners operating affected vehicles. I respectfully request that NHTSA review the adequacy and timeliness of Toyota’s recall remedy process for this defect. No warning light was illuminated. The contact received recall notifications for NHTSA Campaign Numbers: 25V322000 (Exterior Lighting), 25V767000 (Engine and Engine Cooling), and 26V038000 (Back Over Prevention); however, no remedy was available. The vehicle was towed to the local dealer but was not diagnosed or repaired. The contact was provided a loaner vehicle. FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

[XXX], [XXX] PM. 2024 Toyota Tundra Hybrid (i-FORCE MAX, V35A engine), 36k miles, original owner. Clear, dry pavement, evening daylight. Exiting [XXX] westbound at [XXX] , I merged into the far right lane at the second light for a right turn. A vehicle was stopped ahead at the light. Traveling 15-20 mph, I applied the brake for controlled deceleration. Right foot squarely on brake, not touching accelerator. Vehicle did not decelerate as expected. For 3-4 seconds the brake felt unresponsive — pedal felt soft and loose, lacking normal firmness. I pressed the brake fully to the floor. A dashboard warning appeared: “gas and brake pedals are being pressed simultaneously.” I was not, and had not been, touching the accelerator. I then heard engine RPM increase audibly, consistent with an acceleration command, while my foot remained fully on the brake. I felt forward thrust working against my braking. The warning cleared and brakes engaged. Vehicle stopped approximately half a car length from the vehicle ahead, narrowly avoiding rear-end collision. Automatic Emergency Braking did NOT engage despite conditions clearly meeting its criteria — stationary vehicle directly ahead, my vehicle approaching at 15-20 mph with no observable deceleration for 3-4 seconds. No collision warning sounded; no auto-braking intervened. Notably, this same AEB has previously engaged falsely in benign situations (e.g., backing near low vegetation), suggesting reliability/calibration issues. Prior manufacturer notice: On [XXX] at 15,412 miles, I reported to Camelback Toyota, Phoenix (RO #[XXX], on file): “vehicle lurches forward going from reverse to drive (now more consistent)” — dispositioned by tech 10210 as “vehicle is operating to spec,” no diagnosis performed. Same RO documented “rattling in neutral/at stop” — dispositioned “unable to duplicate.” Toyota was on notice of throttle-response symptoms 21 months before this safety event. I previously reported the safety issue truck pulls to the right after roughly 20 alignments. I leave the keep lane assist off because it exacerbates the safety issue. Recently, while trying to self diagnose the problem, I turned on the system before using an on ramp to I820. While accelerating on the on ram, the keep lane assist caused the truck to swerve left and then right. I immediately turned off the feature and the symptom stopped. This is a serious safety issue and can can injury and harm to the public.

2 issues to report: 1. Truck has a pull to the right since new. Found the front strut springs installed wrong. Attached photos show 2024 tundras on the dealer (pink dot on drivers side, yellow dot on passenger side) lot vs. mine (pink dot on passengers side, yellow dot on drivers side). Dealer corrected. Following repairs, pull is much better, but still present. Over 20 alignments completed; 3 by toyota and 3 by 3rd party. Problem continues. I believe it is a manufacturing defect in the safety equipment, suspension, or bent frame. 2. Toyota recalled V35A engines through my manufactured date. Because mine is a hybrid, it was excluded. Crankshaft main bearings seize due to a defect. I am forced to drive a vehicle on public roads while toyota recognizes the issue.

I went to the dealership near me to get the following recall completed: Manufacturer Recall Number26TA02, NHTSA Recall Number26V038. The recall was stated to say that the rear backup camera would sometimes not show despite being shifted into reverse, which I had experienced multiple times. After the dealership performed the software update to fix this issue, I have experienced the issue multiple times since. So the issue is not fixed and the update from Toyota failed.

The contact stated that the messages “Engine Power Reduced” and “Support Brake Malfunction” were displayed. The vehicle was able to restart. The contact stated that while driving approximately 45 MPH, the message “Malfunction with Throttle/Engine/Powertrain” was displayed. The vehicle was towed to the dealer, where it was diagnosed and determined that the vehicle had experienced a low-voltage failure and needed to be repaired. The vehicle was repaired; however, the failure recurred. The vehicle was taken back to the dealer, where it was diagnosed that the computer software had failed, and a software update was needed. The contact received notification of NHTSA Campaign Numbers: 26V038000 (Back Over Prevention), 25V322000 (Exterior Lighting), and 25V767000 (Engine and Engine Cooling); however, the parts for the recall repairs were not yet available. The manufacturer was made aware of the failure. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The approximate failure mileage was 7,278.

FORWARD COLLISION AVOIDANCE: WARNINGS

I have had an issue several times which nearly resulted in a traffic accident. The truck has a sudden loss of acceleration where the gas pedal is completely dead. It last a few seconds and then goes back to normal. This has nearly caused several accidents where I could not properly accelerate while traffic was heading in my direction. After talking with other Tundra owners, I am not the only one with this issue. This needs immediate attention. If it is not fixed as soon as possible, I fear that someone will get seriously injured or killed. Please take this matter seriously. I suspect it may have something to do with the pre collision sensor system on the truck. It seems that it happens when there is a slight grade or dip in the road. Perhaps the sensors are at a lower angle due to the dip, making it think there is an object it will hit. The Toyota service centers don’t appear to know why this is happening and cannot reproduce the issue in a short visit.

The lens on the forward facing camera in the front grill cracked making the camera useless and in need of replacement. I first noticed the problem about a year ago. It is available for inspection. I can't see any damage around the camera that would have been caused by an object hitting the front of the truck which leads me to believe that the camera lens cracked because of a defect. The camera has not been inspected by the dealer however I have gotten a replacement estimate at the dealer.

BACK OVER PREVENTION: REARVIEW SYSTEM BRAKING

Both passenger seats by controls plastic cracking. Infotainment center blacks out and marker bars and back up lines for both back up and front parking have disappeared. GPS even when linked to my phone screen will black out but phone gps will still work. I have complained about this multiple times. Toyota did a factory reset on it and didn’t fix the issue took it back and a tech had to re-install program said the Infotainment center has a known issue where it forgets the program. I also informed Toyota about the engine seems to be really sluggish when at a complete stop and then start again taking a few seconds to go along with shifting hard from stop and go. Toyota did say they saw what I saw and this usually happens when people double foot pedals. I told them I don’t do that. I am frustrated and don’t know what to do I have a 72k truck that just turned 17k miles on it with a lot of issues what will I have to deal with down the road once the warranty is up? I love Toyota but this time seems like I got a lemon from Toyota with all the issues and continued rising issues.

The contact owns a 2024 Toyota Tundra. The contact stated while driving 25 MPH at night, the low beam and high beam headlights were extremely dim. The vehicle was taken to the dealer, where the high beam headlights were adjusted; however, the failure persisted while driving. The contact's friend inspected the vehicle and informed him that the LED fog lights were not installed on the vehicle; however, the vehicle specifications indicated that the vehicle was designed with LED fog lights. The vehicle was taken to the dealer who informed the contact that they were unaware that the LED fog lights were not installed on the vehicle at the factory. The dealer advised the contact that the manufacturer had informed them that the LED fog lights were not installed on the vehicle because they could cause a glare to other drivers when driving with the suspension lifted. The dealer installed the LED fog lights on the vehicle. Additionally, the contact stated that while driving 30 MPH with the Adaptive Cruise Control engaged, the Lane Keep Assist caused the vehicle to drift outside the lane to the left and to the right continuously and failed to keep the vehicle in the lane. The dealer was notified of the failure and informed the contact that the failure occurred due to a crack on the front grill. The contact also stated that the hood molding was causing a sun reflection on his eyes while driving, which affected the visibility of the roadway even while wearing sunglasses. The contact stated that the vehicle design caused the water on the road to splash on the windshield while driving instead of parting the water to the sides, which temporarily reduced the visibility of the roadway. The contact stated that the trims on the windows were inconsistently sealed. The dealer was notified of the failures and informed the contact that they would need the vehicle and User data to repair the vehicle. The contact declined to provide the data to prevent a breach.
