NHTSA complaint #11754267 — an unverified report by a vehicle owner.
Multiple vehicle systems have malfunctioned, including the transmission, infotainment/display screen, and backup camera. Most recently, the backup camera has stopped functioning, preventing a rear view while reversing. The vehicle is available for inspection upon request, along with any service records documenting prior repair attempts. The transmission issues created an unsafe driving condition due to unpredictable shifting and hesitation. In addition, the backup camera is no longer functioning, significantly reducing rear visibility while reversing and increasing the risk of striking a pedestrian, child, vehicle, or object. These defects pose a safety risk to both the driver and others around the vehicle. The vehicle has been presented to an authorized Toyota dealership on multiple occasions for repair of various issues. Most recently, I sought service for the inoperative backup camera but was refused service. Previous concerns have been documented through dealership service visits. The vehicle has been inspected by an authorized Toyota dealership during previous repair attempts. It has not been inspected by police or insurance representatives. The vehicle experienced ongoing drivability and infotainment issues before the backup camera failed. The backup camera eventually stopped functioning entirely.

FORWARD COLLISION AVOIDANCE: WARNINGS
Forward facing Camera's: After vehicle was recalled for a refacing camera issue and repaired for that recall, now the forward facing camera does not work properly. When coming to stop or while stopped the forward facing camera's no longer work. They only come on when I get close to something. They also no longer read speed limit signs. When stopped at a cross walk if a child runs in front of the vehicle I do not get a warning until the child is directly in front of the vehicle. This could cause me to hit a child based on the height of the child and being blocked by the hood of the vehicle. I need the forward camera's to activate at 5 miles per hour or slower and stay on when stopped until going over 5 miles per hour like they did before the backup camara recall was installed on my truck. This is an on going issue starting June 18,2026. I took it to the dealership July 14,2026 and they did not correct the issue. This issue could case a child to be run over!

My reverse stopped working, it just shows a black screen when I switch to it. The rest of my entertainment console works, it's only when I put it in reverse that it goes black. I saw that there was a recall issued for 2024 and 2025 Toyota Tundras, but I believe that the 2023 should be included as well. It looks to be a common issue, as I saw 2023 Tundras with a higher trim package also had a recall for the backup camera.

NHTSA SAFETY RECALL DELAY COMPLAINT Complainant: [XXX] State: Illinois Vehicle: •Year/Make/Model: 2023 Toyota Tundra SR5 Crew Max •VIN: [XXX] •Current Mileage: approx. 16,000 miles Recall Number: Toyota Safety Recall 25TB06 Dealer Holding Vehicle: Romeoville Toyota 684 S. Weber Road Romeoville, IL 60446 ________________________________________ Description of the Safety Issue and Delay I am filing this complaint to report an excessive and unreasonable delay in remedying a safety-related recall on my vehicle. My 2023 Toyota Tundra SR5 Crew Max has been out of service at an authorized Toyota dealership (Romeoville Toyota) since June 18, 2025 due to Toyota Safety Recall 25TB06. As of today, the vehicle has remained unrepaired for 224+ days with no confirmed completion date. Despite repeated inquiries, neither the dealership nor Toyota has provided a definitive timeline for completing the recall repair. While a rental vehicle has been provided, I remain without access to my own truck and have experienced an extended loss of use far exceeding what would be considered reasonable for a safety recall remedy. A manufacturer safety recall is intended to correct a defect within a reasonable period of time. A delay exceeding seven months leaves the consumer without their vehicle and raises concerns about Toyota’s ability to timely and adequately remedy safety defects once a vehicle is removed from service. This prolonged delay undermines the purpose of a safety recall and places an unfair burden on the consumer. I am requesting that NHTSA review this matter and intervene to ensure Toyota Motor North America provides a timely and appropriate remedy, including repair completion or alternative relief. The truck “detected” a warning collision when there was no one in front of me which made the engine lag. There for causing me to come to an abrupt stop from 40 mph. I stepped on the gas to get back up to speed with traffic beside and behind me and there was no power almost causing an accident. This is the 3rd time this truck has done this in 6 months.

Tundra has had and has throttle lag. Lag is so noticeable that other drivers notice. Following lag is a burst of power causing a jerk and or lunge forward. This is after a long pause. Happens on a roll, or start-stop situation. My truck has had the toyota tsb applied a long time ago.

I experience delayed throttle response and throttle lag daily. Acceleration is unpredictable. The vehicle barely moves in some cases when pressing the gas pedal. Other times, the tires will chirp. The vehicle will sometimes hesitate so long to accelerate, that I've almost been T-boned and I've held up traffic. It seems like the truck thinks I'm going to run into something, even when there's nothing in front of me. The delay is sometimes 3-5 seconds before it will allow any acceleration. This seems like a huge safety issue, not being able to get out of the way of oncoming traffic and having such an unpredictable, and often delayed throttle response.

1. What component or system failed or malfunctioned, and is it available for inspection upon request? Multiple safety systems failed simultaneously, including: SRS Airbag System Pre-Collision System Vehicle Stability Control (VSC/TRAC) Advanced Ultrasonic/Sonar System The engine is also subject to Safety Recall 25TA14 (potential loss of motive power). The vehicle is currently located at Crown Toyota in Ontario, CA and is fully available for inspection. 2. How was your safety or the safety of others put at risk? I was driving on the 60 freeway with my two small children when all of these safety systems failed at the same time. The truck also began pulling to the right and hesitating during acceleration. With the airbag system, pre-collision system, and stability control disabled, our safety was seriously at risk, especially at freeway speeds. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. Crown Toyota advised me not to drive the vehicle and instructed me to have it towed immediately as an emergency. They confirmed the warnings and safety alerts. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? The vehicle was inspected by Crown Toyota service technicians. They noted the safety system malfunctions and the active engine recall. Toyota Corporate has opened a repurchase case: #251118002114. 5. Were there any warning lamps, messages, or other symptoms of the problem prior to the failure, and when did they first appear? Yes. While driving, the dashboard displayed multiple malfunction warnings: Airbag malfunction Pre-collision system malfunction Vehicle Stability Control malfunction Sonar system malfunction The “Contact Dealer Immediately” message also appeared. These warnings first appeared on November 13, 2025, while I was on the freeway.

The engine has 13,441 miles on it and it has been consuming 4 quarts of oil in 1700 miles. The dealer sealed the engine and is performing an oil consumption test. Approximately 500 mile into the test the check engine light started flashing due to a misfire. The dealer found that the #1plug was fouled out and had to be replaced. At 1029 miles into the test the truck was idling in a parking lot for about 8 minutes when it started to misfire and stumble. After stepping on the throttle to clear out the misfire, the dash lit flashing multiple warnings including the breaking system was stuck on hindering the truck from being able to move. After several restarts and playing with the parking brake I managed to get the parking brake off. It still had the flashing check engine light on and flashing. The pre-collision light was flashing, along with the traction control light was flashing and none of the safety systems were working.

FORWARD COLLISION AVOIDANCE: WARNINGS
The issue is with the auto-braking or pre collision system. Have had multiple instances where system has engaged at low speed which also engaged the auto engine stop feature when it was not necessary. Have noticed system engages on roads with curves where it thinks the vehicle in the other lane is in front of me when its not. The most unsafe thing that happened was when a vehicle quickly changed lanes in front of me and slowed slightly. I saw the other vehicle make the unsafe lane change and was covering the brake and when i pressed the brake it went out from under my foot braking hard on its own when it was not necessary. Vehicle slowed from 65 to about 35 quickly almost causing vehicle behind me to rear end me with no way to stop the system. Since i have owned this vehicle auto brake system has engaged about 5 times and all were not necessary or unsafe. I have lowered sensitivity setting and still have the issue, and am unable to disable system.

There are many incidents, issues, and critical safety problems with my vehicle. I have had multiple repair attempts, with multiple Toyota Car Dealerships, and have continued to have the same amount of problems, and issues with this vehicle. Multiple leaks of fuel, smells of gas and burning, odometer reads brakes failing, and danger lights, Transmission shifting issues, Engine knocking while driving and at a complete stop, Brake issues, By the grace of God my family and I have not been in an accident yet. I have taken my car in for repairs multiple times and to this day my vehicle is still being repaired at the dealership as we speak. My vehicle has cumulatively been at the dealership for almost 3 months. My family and I are under an assortment of insurmountable mental, and physical stress with the level of unsafe, unfit, and unreliable attempts to not only trust this vehicle to operate safely, but trust the service technicians to safely, and correctly diagnose the many unsafe issues with this vehicle. I have made several attempts in filing cases, complaints, and attempts of contact, to resolve these multiple issues with the manufacturer, and have had no such luck. Please, for the safety and daily life continuity of my family. Please help me resolve these issues with the manufacturer. My family, and I do not feel safe and do not feel like our issues are being acknowledged or resolved. Please let me know if there is anything that can be done so that my family can safely, and cordially operate a functioning vehicle. Thanks and God Bless. The contact owns a 2023 Toyota Tundra. The contact stated that while driving at various speeds, the navigation screen would shut off and then it would restart with no information. The contact would have to stop driving and reenter the information. The vehicle was taken to the dealer and the contact was informed that he needed to show proof of the failure for the issue to be repaired. The driver-side seat functions failed to operate as well as the lane departure warning system was inoperable. The vehicle was not repaired and the failures continued to occur. The contact stated that while his mother was driving at an unknown speed merging into traffic, the front passenger side of the vehicle collided with a trailer. The contact stated that the vehicle collision warning audio system failed to warn of a collision. No injuries were sustained. A police report was not filed. I have been getting my Tundra alignment done every other month since I purchased it brand new and have actually run out of warranty on the alignments and have been coming out of pocket ever since. The vehicle pulls quite aggressively to the right unless extra pressure is applied to the left side of the steering wheel. It has steadily gotten worse after every alignment and I can’t even pull an empty trailer with a truck that has the towing package. I spoke with the manufacturer who believed that they have “not failed in repairing the vehicle” I am 8 months pregnant and am terrified to drive this vehicle. It sits while I drive my 25 year old beater car that feels safer because it can at least drive straight. All I want is for the manufacturer to take the truck back and return my money as I have no faith that I won’t receive another faulty truck.

Before this recall happened we had issues with screen blacking out and crashing even after the recall was “fixed” also the back seat outlet has never worked. The malfunction of the grille shutter and park support brake malfunction came last week and we had to have that fixed. Now this! The dealerships around kansas don’t have loaner vehicles or rentals available. How is this okay?!
NHTSA complaint #11551902 — an unverified report by a vehicle owner.
This vehicle is dangerous. There are many more, complaint level issues, but there are at least three real safety issues. First, the automatic braking system has engaged multiple times for no reason. Traveling down a straight, flat road in good weather, the brakes suddenly went fully engaged. On one occasion my dog was thrown forcefully into the dash and was injured and then afraid to ride in the truck. It makes me afraid of the truck as well and could have easily caused an accident. Second, there is the known fuel line issue. This could, according to the manufacturer, be a fire risk. However, there's no remedy. So I'm driving around this truck that could catch fire, that likes to slam on the brakes for no reason. Third, there's something wrong with the windshields in these trucks. Mine recently was broken while the vehicle was parked. It just cracked. Just driving it home made the crack quadruple in size. It's unsafe to drive as is. I contacted Toyota and they aren't interested in considering that there's something wrong with the windshields, yet they can't replace it, because they're back ordered due to so many being broken. So I have a truck that slams on the brakes unpredictably, that might catch fire, that the windshield broke for no reason but can't be replaced. I've spoken several times with Toyota corporate about this and they have no answers and keep referring to the dealer who tells me there are no answers and to call toyota. It's a classic runaround while my $70,000 truck is unsafe to drive. It's absolutely unsafe and the NHTSA needs to act to protect consumers from these vehicles.

When proceeding ahead from a stop with no one in front of me, after traveling ahead at a normal pace, the emergency braking system activated and the truck came to an abrupt stop. This occurred after traveling about 5 feet and luckily the person behind me did not rear end me. After this panic stop the truck then proceeded ahead as normal. This is the first time this has happened to me since purchased new. Currently has 3870 miles.
