The contact owned a 2020 Volvo XC60 II. The contact stated while reversing, the rear view camera displayed a black screen. The contact stated that the accelerator pedal was depressed, however the vehicle failed to accelerate as intended. The vehicle's speed independently accelerated. The contact's vehicle crashed into a parked vehicle. The contact stated that there were two additional occupants in the vehicle. The contact sustained pain in the right knee, no medical treatment was provided. The passenger, who was seated in the rear seat, sustained neck and upper back injuries. The passenger received medical treatment at the local medical center. The vehicle was driven to the residence. The failure was intermittent. The contact stated that the vehicle was previously taken to the dealer, however the failure was not duplicated. The contact later received a recall notification, NHTSA Campaign Number: 25V908000 (Back Over Prevention). The manufacturer was not made aware of the failure. The failure mileage was 40,000

After a software update to the infotainment system (Android Automotive OS version 5.1.17, installed on 06/03/2026), my 2022 Volvo XC60 B5 intermittently loses all audio output. When the failure occurs, the vehicle produces no sound of any kind: no audible rear parking/proximity sensor warning while in reverse, no turn-signal indicator tones, and no media audio. The rear-view camera image still displays normally, so the loss of the audible backup warning is not obvious to the driver — the parking-assist system appears functional while its audible warnings are in fact disabled. On multiple occasions the failure was already present as I backed out of my garage and began driving, leaving me with no audible obstacle warning while reversing and no turn-signal tones while maneuvering. The display, navigation, and camera continue to function while audio is dead, which indicates the audio service stops responding rather than a hardware fault. The only way to restore audio is to fully restart (reboot) the infotainment head unit; the failure then recurs intermittently. The fault began immediately after the 5.1.17 update and was sporadic at first. After noticing a pattern, I documented occurrences on 06/15/2026, 06/17/2026, and 06/20/2026 with timestamped photographs, which are available on request. No crash or injury has occurred, but the loss of audible backup-warning tones while reversing is a safety hazard, particularly for pedestrians or obstacles behind the vehicle. I am not aware of a current recall covering this issue and am requesting investigation.

2022 Volvo XC60 T8 Recharge VIN: [XXX] Mileage at time of complaint: ~38,400 Purchased: November 2023 (Certified Preowned) DESCRIPTION OF PROBLEM: The vehicle's infotainment system and backup camera have experienced repeated failures since the time of purchase. The backup camera intermittently fails to display an image when the vehicle is placed in reverse, leaving the driver without rear visibility. The infotainment system freezes, goes black, or becomes unresponsive without warning. These failures affect the federally mandated rearview camera display required under FMVSS 111. The vehicle has been brought to authorized Volvo retailers (Jim Fisher Volvo, Portland, OR and Germain Volvo North Hills, Wexford, PA) more than eight times for in-person service for this concern. The same issues have also been troubleshot remotely by phone or text on approximately ten additional occasions. Software updates, system resets, and other attempted repairs have not permanently resolved the problem. On one occasion, the backup camera failed to display while reversing, which contributed to minor rear bumper damage. The driver was relying on the camera display as designed and federally required, and the system did not perform. I am aware that NHTSA Recall 25V282 (Volvo R10320) was issued in April 2025 for this defect across 413,000+ vehicles including the 2022 XC60. Despite the recall, the underlying infotainment and camera reliability issues in my vehicle have not been resolved. A second recall (25V392) was issued after the initial software fix caused braking concerns in some vehicles. No injuries have occurred. No crashes have occurred beyond the minor bumper contact described above. Volvo Customer Care Case #XXX was opened and a goodwill assistance request was denied. FORWARD COLLISION AVOIDANCE: WARNINGS

At the end of a downhill cloverleaf coming coming on and off a highway, the car slowed to a crawl but would not stop. It was going slowly as if "creeping" like when in B mode but was not in B mode. The brakes would not stop it. I was able to shift gears from Drive to N to P but no response, car continued to creep forward until it gently bumped into the back of an 18 wheeler. There was no collision alarm at the time. I turned the car on and off, and powering down seemed to re set the issue, but the collision alarm went off like 10 minutes later when there was nothing to set it off (seemed like it was just delayed). No error code came up, nothing. I submitted my claim to Volvo, esp since there are similar recalls so I thought it was a known thing, but apparently not reported on my exact make and model (there were not very many made). I did not know there was damage to my vehicle, and the truck driver and myself did not even get out, he didn't feel me, and I assumed the bumper just bumped him so no police report. This was an intermittant failure and after I re booted the vehicle I slowly drove home after brakes seemed to be working after power on/off. I did not know that it was my HOOD that bumped the truck. I did not know there was damage until I got home. If this car's make and model were NOT included in the recall, they should be. TERRIFYING. And volvo did not acknowledge or reassure. requested documentation on what they inspected, and did not receive.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL

I received the the recall for the rear backup camera system not working now the whole cars safety system doesn't work. The infotainment screen keeps crashing and showing a dead android on it thus killing the Turn signal noise, climate controls, safety systems blind spot monitoring, adaptive cruise control randomly turns off, rear backup cameras still don't work. Overall this could lead to an accident as the whole cars safety system is not working and it makes me wonder if this could trigger the car to fail while driving if the operating system is malfunction or bad. I have tried to fix the issue with a factory reset but nothing seems to be working.

The contact stated that while driving approximately 45 MPH during nighttime, the Forward Collision Avoidance system engaged while there were no vehicles or objects in front of the vehicle. The contact stated that the failure caused pain in her rib cage when the brake was suddenly applied. Medical attention was not sought. No warning lights were illuminated; however, the message "Front Collision Aversion Engaged" was displayed on the instrument panel. The vehicle was taken back to the residence, and the dealer was contacted; however, the vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. Even after the Volvo R10320 - WAM Upgrade recall, the backup camera is STILL unavailable at times when reversing. The recall and software update did not fix the issue. I would start the car and begin to reverse and met with "Camera is temporarily unavailable". I do not know what else do to as Volvo is slow to respond or provide anytime of upgrade or timely fix. This has happened multiple times and almost five times a week.

Feb. 1, 2024: A short drive on electric during below zero weather, a car wash & a couple 100ft drive into a parking lot. Pulling into a parking space, foot on the brake pedal, ICE roared on accelerating the car over a curb, down an embankment & into a 3ft snow drift before I could stop it by pressing the brake pedal with both feet. We stopped 6in from a power pole. If forward crash avoidance system worked, didn't register in the EEPROM per the service tech! Feb. 17, 2024, during below zero weather, no electric charge, operating on ICE over about 2mi, Pulled into a parking space, foot on the brake pedal, ICE surged accelerating the car up over a 6" high curb onto sidewalk. Stopped car by depressing the brake pedal with both feet. Car stopped 2in before hitting a brick building. It didn't seem the forward crash avoidance system functioned to stop us. As a 35-year electronic design and repair professional, I have attempted to merge the two situations to try to find similarities. Two things: unexpected ICE engine acceleration surge & forward collision sensor not kicking on brakes. Per Volvo tech, Feb. 1, no register in EEPROM of forward crash avoidance or ICE issues. Second visit Feb. 22 to service will probably yield same. Obviously, this is not normal operation. We have been in two near collisions caused by the car doing things not designed to do.

I have had 5 visits with problems with engine , braking sensors, and automatic braking incidents when no obstruction is noted. Propulsion System failure has occurred 3+ times and turtle mode has come on and limited my speed to 30 MPG. It has also started bouncing and shaking while in reverse. I am extremely concerned about the safety threats and lack of consistency when taking it to be repaired.

The Google/Android (IHU) electrical system controls the entire car: brakes, engine, safety features, AC/Heat, etc. It is unreliable after 4 repair attempts. Problems started date of delivery (9/6/22) & have been ongoing. The ones that directly affect safety are the auto braking activating without cause, blind spot warning missing motorcycles, lane keeping will pull car towards a hazard in middle of road when you swerve to avoid it and the Google/Android system in general malfunctioning. I live in south Florida and I can assure you that air conditioning is a safety concern, especially in people like me who have medical conditions in which excessive heat exposure can result in an ER visit. My AC has failed upon starting the car as well as following the rebooting of the Google/Android system. The rebooting of the system must be done whenever the system malfunctions & it is wise to pull off the road to do it because everything but engine shuts down during the procedure. I have had to do this several times sitting off the side of the road & the system still wasn't working. It is not safe to have to pull off road so frequently. The automatic braking was particularly frightening, as both I and the car in front of me were both slowing for a stoplight, there was at least 5 car lengths between us & the distance between us was not narrowing. My car suddenly braked. Had someone been close behind me they would likely have rear-ended my car. They might have also assumed I brake-checked them & reacted with anger. Requested Volvo corporate involvement for 4th repair attempt & IHU was replaced. System began malfunctioning again, but blind spot worked. The Google system is apparently unstable, as the 2nd loaner car's backup camera & cross traffic warning froze & failed to alert. Dates: 9-6-22 to 1-25-23 (date they took car for repair try #5). About 4000 miles on it. Volvo says they can't replicate concerns. I have video of some malfunctions & sent to dealer.

The vehicle is basically new and I have only had it a few months. It has already been with the dealer three times because of technical/computer/infotainment issues. The dealer had the vehicle for about a week on each occasion. Today, I started to break so I could turn the car around and park it in the parking lot. I was moving very slowly, as you would expect in such a case. However, the car didn't stop but instead lurched forward in full throttle causing me to crash into a breezeblock wall. The automatic warning system did not sound and the crash detection system never engaged. I haven't been in an accident for 30 years and was not distracted when this happened. If the vehicle hadn't already had issues, I would accept user error as the problem, however in light of the car's history, I have doubts.

The car is only a few months old with less than 10k miles. After parking the car for about 2 hrs, came back and the battery was completely dead. This left me and my family stranded unexpectedly. After jumping the car, a warning displayed "Stop Safely, 12 V Battery Critical Charging Fault". I decided to try to make the short drive home as I was stranded with my family otherwise, the vehicle was in a limp home "turtle mode" which was not safe. The acceleration was surging unexpectedly and without warning. I realize that there was a warning not to drive, however I didn't have a lot of good options given the electrical failure. Also, in thinking back, there was a warning sometime in the last month regarding collision detection that needed to be fixed, but warning went away. In researching this issue online, seems that there are other complaints similar in nature (11472450, 11469166).

Continuous problems associated with the computer of the car that are affecting its ability to drive safely. While in reverse and in motion, the car's camera will often, an unpredictably, shut off. Additionally, the car's forward collision warning system will initiate when there are no cars in front of; rather, it sometimes engages when oncoming traffic going the opposite direction. Lastly, the car lost all power while on the highway due to a "battery error". It required a redownload of software; however, these other safety problems persist.
