On June 3, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE22005 after the agency received VOQs and several field reports concerning the Auto Idle Stop (AIS) feature on 2016-2019MY Honda Pilot vehicles. Issue: On certain vehicles, the suspension mounting points on the rear subframe could rust. As a result, rear suspension components could fail. This could lead to poor handling and a loss of vehicle control. Safety Risk: A loss of control could create the risk of a crash. Corrective Actions: Honda will notify owners by mail and advise you to take your vehicle to a dealership to inspect and install a rear subframe reinforcement kit or replace the subframe, as necessary.

Seats And Restraints

Issue: On certain vehicles, the weight sensor for the front passenger seat may not have been manufactured properly. As a result, the passenger-front airbag and knee airbag could deploy in a crash that doesn't require them to. Note: This recall is an expansion of Transport Canada recall no. 2024-059. If the weight sensor fails, the SRS warning light would turn on in the instrument panel. Safety Risk: An airbag system that doesn't work properly could increase the risk of injury in a crash. Corrective Actions: The company will notify owners by mail and advise you to take your vehicle to a dealership to replace the passenger-front seat weight sensor.

Issue: On certain vehicles, the recall repairs completed by a Honda or Acura dealer during recall AL65/AL66 (Transport Canada recall no. 2019-159) may not have been effective. As a result, a second repair is required. On certain vehicles, water could enter the tail lamps on the tailgate and cause a short circuit. As a result, the fuse could blow and cause a complete loss of rear lighting. Note: This recall replaces recall AL65/AL66 (Transport Canada recall no. 2019-159). Vehicles that previously had a fuse harness installed under that recall do not require this repair. Safety Risk: A loss of rear lighting could make the vehicle less visible and increase the risk of a crash. Corrective Actions: Acura will notify owners by mail and advise you to take your vehicle to a dealership to install a fuse harness. The dealer will also inspect and, if necessary, replace the tail lamps on the tailgate and repair the body dust sealer.

Safety Risk: An engine failure could cause a sudden loss of power and/or increase the risk of a vehicle fire. Both issues could increase the risk of a crash or injury. Issue: On certain vehicles, the teeth on the timing belt may separate due to a manufacturing issue. This could result in an engine stall. Safety Risk: An engine stall could increase the risk of a crash. Corrective Actions: The company will notify owners by mail and instruct them to take their vehicle to a dealer to inspect the lot number on the timing belt. If it is within the affected range, the belt will be replaced. Additionally, dealers will inspect the engine for internal damage.

This recall was replaced by Transport Canada recall no. 2025-212. Issue: On certain vehicles, water could enter the tail lamps located on the tailgate. This could cause the tail lamps not to work properly. This could also cause a fuse to blow, which could result in a complete loss of rear lighting. Safety Risk: A loss of rear lighting could make the vehicle less visible and increase the risk of a crash. Corrective Actions: The company will notify owners by mail. Owners will be instructed to take their vehicle to a dealer to inspect the tailgate lid lamps. If either lamp is not working, the dealer will replace both lamps and inspect/replace the fuse. If both tailgate lid lamps are working, the dealer will modify the lamps, and install updated gaskets and wiring.

ENGINE

The contact owned a 2018 Acura MDX. The contact stated that while the vehicle was parked in a garage there was a sudden odor of smoke. The contact stated upon discovering the smoke the contact noticed fumes and flames coming from the engine compartment. The contact stated the vehicle was unoccupied at the time of the fire. The fire was extinguished by the Fire Department however the vehicle was consumed by the fire. There was smoke damage to the home and there was damage to the driveway due to the fire. The contact's wife sustained emotional distress. A Fire Department report was filed. A Police report was not filed. The dealer was contacted. The vehicle was destroyed. The manufacturer was notified of the failure. The failure mileage was approximately 50,000.

Lititz, PA Fire

ELECTRICAL SYSTEM

Fire under the hood of my 2018 MDX - caused by weak battery and overloaded starter. My car was parked in the garage right under my daughter’s bedroom. I pressed the "Start" button on the car and the dashboard started to flash on and off quickly and I heard clicking sounds and other swooshing noises. The car did NOT start but it seemed to keep trying over and over again. I pressed the start/off button repeatedly but the car did not shut off. My wife came out and asked what I was doing and she smelled and saw smoke coming from under my hood. She ran into the house and called 911. I popped the hood and got out of the car. At this point, there was more smoke and a VERY strong burning electrical odor. The car continued to try to start and the only thing I could think of was to disconnect the battery. Just as I disconnected the battery, The fire department showed up and the smoke started to slowly subside since the car was no longer trying to start. I called AAA and had the car towed to the local Acura dealership that I just had it serviced at. Upon their evaluation, it was determined that the battery was not completely dead, but not strong enough to engage the starter. Therefore, the car continued to try to start and overloaded the starter, burning it out. I strongly feel that this is a catastrophic failure for Acura and completely unacceptable for a vehicle with all kinds of safeguards. It should not have caught on fire from a weak battery in the garage/under my daughter’s bedroom – The whole car and house could have burned down. After several weeks of discussions with the dealership and Acura customer care, it was determined they would split the cost of ~ $2000 for repairs. I felt I had no choice but to pay it and get my car back with a new starter and battery. ELECTRICAL SYSTEM

In San Diego on 11/15/2021, my daughter side swiped a parked vehicle in residential area driving about 30 miles. No airbags deployed. A minute later the car stopped and would not start and started to have smoke and flames come from under hood and through inside windshield and dash. My daughter was able to get out and take her 3 year old son out of his carseat and run for their lives as the car completely became engulfed in flames.Thank God they made it out alive. Police and Fire department showed up. Car was a total loss, everything was burnt. One day the Acura MDX fires will take someone's life. Called Hoehn Acura in Carlsbad Ca and sent service manager Skip about 12 pictures of the completely burnt vehicle. He never returned my calls after leaving several messages. Also left message with general manager and no return call. They don’t care. They only appear interested when you are buying the vehicle. I did file a claim with Acura corporate office and still no word from them either. I have read many cases about the MDX going up in flames for no reason, even in the driveway or drive in your neighborhood. Not sure why they have not recalled this or tried to solve the issue. I am asking for someone to look into this as this is not an isolated incident before lives are lost. Once I get police report I will also send you pictures as well. Thank you.

FORWARD COLLISION AVOIDANCE: WARNINGS

The car has started with an auto high beam, lane assist, lane departure, crash mitigation, adaptive cruise lights coming on one after the other noting there are issues with the systems and they are disabled. Code is U3000 using a scanner noting Multipurpose Camera Unit Internal Electronic Failure. Sometimes the code will clear, and sometimes not. If it clears an entire drive cycle may be completed just fine, sometimes it pops back up before even moving the car and sometimes during the drive. This is a safety concern as these systems not working or stopping working mid drive and affect safety. The repair for this is not clear and a failure of a safety system should not be occurring on cars without an inmediate and covered fix. The average car on the road is over 12 years old so this failure is occurring on cars well newer than the average car and compromising safety of the driver and surrounding cars especially features intended to prevent crashes that drivers rely heavily on. The fact it can stop working mid drive is a huge hazard since one may be relying on a system and it suddenly fails (like adaptive cruise control) and cause a crash. Please investigate his widespread issue and please fix it.

The center display continuously cycles through four warnings: Collision Mitigation System Problem Adaptive Cruise Control Problem Lane Keeping Assist Problem Road Departure Mitigation System Problem In addition, the Brake System, Crash Warning, and Lane Departure dash lights remain permanently illuminated. Diagnostic Findings An OBD-II diagnostic scan logged the following stored chassis code: DTC U3000-49: Internal Electronic Failure of Multipurpose Camera Unit / Millimeter Wave Radar I am aware of TSB #B23-008 regarding camera logic errors. However, clearing codes and software resets do not clear this persistent hardware failure within the monocular camera unit (Part # 36160-TZ6-A34). Safety & Request for Action The total shutdown of primary crash-prevention systems presents a clear safety hazard. Forcing owners to absorb $20,000+ out-of-pocket for camera replacement and mandatory dealer recalibration on a known hardware fault undermines Acura's commitment to safety and quality. Given NHTSA's oversight of active safety systems, I respectfully request: 1. Goodwill Repair: Acura/Honda assistance to replace and recalibrate the defective camera unit. 2. Quality Investigation: A broader review into premature failures of these Nidec monocular camera modules on 2017–2018 MDX vehicles. Thank you for your prompt attention to this critical issue. Diagnostic Findings An OBD-II diagnostic scan logged the following stored chassis code: DTC U3000-49: Internal Electronic Failure of Multipurpose Camera Unit / Millimeter Wave Radar I am aware of TSB #B23-008 regarding camera logic errors. However, clearing codes and software resets do not clear this persistent hardware failure within the monocular camera unit (Part # 36160-TZ6-A34). Safety & Request for Action The total shutdown of primary crash-prevention systems presents a clear safety hazard. Forcing owners to absorb $20,000+ out-of-pocket for camera replacement and mandatory dealer recalibration on a known hardware fault undermines Acura's commitment to safety and quality. Given NHTSA's oversight of active safety systems, I respectfully request: 1. Goodwill Repair: Acura/Honda assistance to replace and recalibrate the defective camera unit. 2. VISIBILITY:SUN/MOON ROOF ASSEMBLY

The contact owns 2018 Acura MDX. The contact stated while driving 35 MPH, there was a loud banging noise coming from the sunroof. The contact mention she pulled to the right side of the road and discovered it was shattered. An Independent mechanic or A dealer were not contacted. The contact mention she called her insurance company, and they working with a company through Safe lite. The vehicle was not taken to the dealer or independent mechanic. The vehicle was not repaired. ENGINE

Rob bearing recall was performed on a 2018 Acura MDX at around 78,000 miles. After a long ordeal with Acura because the recall had not yet been extended to the 2018 MDX's yet, Honda agreed to replace the motor. The new replacement motor has already seized again with only 30,000 miles. Dealer has taken motor apart and confirmed it is the same rod bearing issue as the recall. So the new motor that was replaced must have the same issue. After further investigation, it also appears that the Acura dealership that replaced the motor, did not tighten or replace many bolts during the repair. I have attached pictures of missing frame bolts, engine bolts, and the spun rod bearings.
