I was pulling into (my usual) handicapped parking space, at a slow speed. As I approached the curb my brakes did NOT respond to stop my car. Not only did my brakes not respond, the car actually sped up - going up the curb and hitting the handicapped sign pole. The pole broke and stopped my vehicle! I was afraid my (out of control) vehicle would keep going through a store front - it did not. Folks gathered around- thinking I had put my foot on the gas . I DID NOT put my foot anywhere except on my BRAKES …which went ‘mushy’ . would not stop my vehicle. My research has found similar incidents reported by other drivers. I am NOT ALONE. Of course the dealership could not reproduce the incident. But they did suggest many other services for a car with 60,000 miles. The AUDI dealership took NO responsibility for the incident. There was a recall on the master cylinder that was supposedly handled before I took delivery of the vehicle in January 2019. Audi denied that anything was currently amiss with my master cylinder. There was a warning message when I tried to start my car about a week before. Because it made no sense - like your tires are low - I dismissed it as just a check engine light 🤷🏽‍♀️ I honestly can’t remember what that ‘odd’ warning said🤦🏽‍♀️ I’m not alone in reporting this - I’m almost afraid to drive my car😰

2025 Audi Q5 Schenectady, NY, NY

On XXX at approximately 5:00 PM, my 2025 Audi Q5 (VIN: [XXX]) initiated an unprovoked forward collision warning and emergency braking intervention while I was traveling at approximately 57–58 mph on [XXX]. Road conditions were clear and dry. Weather was sunny. Adaptive Cruise Control was not activated. I was the sole occupant of the vehicle. A vehicle was making a lawful turn onto the roadway from a T-intersection. There was substantial distance between my vehicle and the turning vehicle. No collision risk existed and no evasive action was required. Despite this, the vehicle's forward collision system overrode my control and initiated emergency braking without cause. As a result of the sudden braking and the force of the seatbelt restraint during the event, I sustained abdominal pain and lower back pain that began immediately following the incident and persisted thereafter. This incident did not occur in isolation. Prior to this event, the vehicle had already exhibited a documented pattern of electronic and driver assistance system malfunctions, including intermittent check engine warnings, repeated false-positive driver assistance alerts, power liftgate malfunctions, sunroof malfunctions, and additional electronic irregularities. All of these concerns were reported to Audi Albany prior to the emergency braking event. Following a service visit, I was told nothing further could be done. The vehicle is currently at Audi Albany undergoing inspection at the request of Audi of America, including a review of the Event Data Recorder. I am filing this complaint to ensure the incident is part of the federal safety record. A driver assistance system that initiates emergency braking at highway speed without a valid hazard present, on a vehicle with a prior history of false-positive alerts, represents a safety defect that I believe warrants federal documentation and review. •XXX: Purchased new 2025 Audi Q5. •XXX Dashboard displayed "Check Tire Pressure" warning for all tires. •XXX: Service at Audi Marin - Reported hatch malfunction, tire pressure concerns, and repeated rollback after brake release during service visit. •XXX: Vehicle displayed "P Not Possible. Risk of Rolling Away" warning. Vehicle could not be placed into PARK, required roadside assistance (never showed after 2-hour wait), and could not be safely secured. •XXX: While stopped at a traffic light, both front seat belts tightened aggressively without warning. Passenger was a [XXX] child. •Late XXX: Collision avoidance/emergency braking warnings continued. Vehicle remained in use while awaiting loaner car availability. •XXX Oil service reminder displayed at approximately 5,000 miles. Electronic issues continue. Additional warning lights appeared. 2019 Audi Q5 Bolingbrook, IL

Went to Mavis on Weber Rd in Bolingbrook, IL for front brake replacement on June 6, 2026. It took 6.5 hours when I was originally told when I called to schedule the appointment it would only take 1.5 hours, so it was obvious that something was wrong. Within 100 yards of leaving their parking lot after the service appointment, an obvious clattering was present. I immediately called them to let them know the replacement had not been performed correctly. Booked appointment with Audi of Naperville first thing Monday morning to have them fix whatever was wrong - the earliest appointment I could get was June 11. Drove the car until Wednesday evening when something went very wrong and constant clatter was replaced by loud scraping and brake lockup when I exited 355 on my way home from work. Called someone that night to have it towed to Audi for the next day's service appointment. Audi videoed their inspection and shared the video. Their inspection revealed one caliper entirely missing from the driver side brakes and the other caliper on that brake very loose. I paid Audi personally for that repair. I called Mavis customer service to notify them of my findings. After pulling the shop video from my brake appointment that day, they got back to me and said they would refund me for that appointment, the tow to Audi, and the Audi repairs. I have since been in touch with them and they said they mailed me a check on July 2. Since it has been 17 days since that date and it doesn't take mail that long to get to most of North America, I tend not to believe them. I have since learned that this Mavis brake replacement was negligent and could have resulted in an accident, with the potential of injury. I am contacting you because I don't want Mavis to take their responsibilities for the safety of their customers so casually.

2018 Audi Q5 Memphis, TN

I wrote Audi to request escalation of an unresolved issue involving my 2018 Audi Q5, related to Safety Recall 90S9 (NHTSA Campaign 21V947000) for the gateway control module. Summary of the issue: My vehicle recently experienced a complete gateway control module failure due to water intrusion. I lost steering and brake assist warnings while driving, and the vehicle would not restart after being pulled over safely. It was towed first to the nearest Audi mechanic who diagnosed the issue as Recall 90S9. We then contacted Audi Memphis for inspection by them since it was a recall issues and paid a 2nd towing charge. Audi Memphis (owned by Gossett Motor Cars) said they could not do anything until they looked at the car. I have since obtained a copy of the original repair order from when Recall 90S9 was performed on my vehicle. The vehicle at the time of recall repair was owned by my Mother. The recall requires two remedies: (1) installation of a protective cover over the gateway control module, and (2) sealing of an underbody seam on vehicles produced before September 2021. The repair order shows a technician comment reading "INSF" next to the seam sealant line item, which indicates the sealant application was noted as insufficient at the time of the original recall service. Despite this documentation, and despite towing my vehicle in specifically for a 2nd inspection, Audi Memphis has stated they have no findings and no liability for the current failure. Given that: • My vehicle is included in the affected range for Recall 90S9, • The original repair order appears to document an incomplete or insufficient sealant application, and • Audi's own 90S9 technical bulletin states that damages resulting from improper repair or failure to follow the work instructions are the dealer's responsibility, I am requesting that this case be escalated for review. The current gateway control module repair be repaired at no cost to me. I currently have no working vehicle.

CORRECTION to NHTSA COMPLAINT #11745968 The Electronic Parking Brake (EPB) did not disengage when the vehicle was placed in gear from P to R to D while/after parked in an incline postion (see DTCs -Bozeman Audi invoice, 6/23/26). The EPB is designed to disengage automatically. There was a dashboard warning light --RED--symbol--brake failure that flashed when the ignition was started. The occupants drove the vehicle approx 50 ft before the vehicle's Automatic EBS and/or Automatic Parking Braking (Maneuver Assist) shut down the vehicle completely because EPB was still engaged after gear shifts--R and D. |DTC| Diag 01500060-15TU (202729223) Terminal 75X (Relief Relay for X-Contact). [DTC 01500 / 015000] Diagnostic Lookup [Root Causes]: Blown Fuse: A fuse (typically on the main power distribution block) protecting the relief relay circuit has failed. Faulty Relay: The J59 X-Contact Relief Relay has mechanically stuck open or failed internally. Wiring/Ground Fault: Corroded, pinched, or damaged wires along the steering column or under the dashboard. Low Battery Voltage: Insufficient system voltage can trigger relay dropout during vehicle start-up. [Immediate Next Steps] Check Voltage: This was not done at Bozeman Audi dealership [DTC] The Audi repair sequence "27068950" Battery Charging / Testing (Labour Operation Number: 27068950) associated with the 12V/48V electrical system on an Audi. Automatic Emergency Braking system engaged when the vehicle was parked on an incline. Unable to release automatic emergency brake when vehicle was started with key. Ignition light warning — RED — symbol — brake failure. Required a tow to Audi dealership. Audi dealership in two states failed to use OMB II reader to scan codes at the time of emergency visit. Vehicle was released to owner without scanning codes at the time of the emergency therefore owner acquired more than 3-4 consecutive drive cycles before scheduled repair appointment 7 days after emergency incident described above because the Audi dealerships in two states (vehicle occupants were traveling long distance) failed to read and store computer codes immediately after emergency incident with the automatic braking system. This incident made the vehicle inoperable and placed the occupants of the vehicle in an unsafe situation. The failure at the Audi dealerships to retrieve codes at the time of the emergency incident prolonged a permanent repair and will make vehicle evaluation/assessment for safety less reliable given that no evaluation or assessment was done at the time of the emergency at the dealerships and before the vehicle codes were automatically cleared after 3-4 consecutive ‘drive cycles.’ Requested an incline field test with vehicle — place the vehicle in similar conditions— incline — and test automatic emergency braking system— during scheduled appointment 7 days after incident.

A professional diagnostic scan confirmed a permanent hard fault on the ABS control Module (J104) and Hydraulic Unit (N55). The vehicle has only 66,755 miles. Independent repair shops cannot service this unit because it requires highly restricted, specialized Audi factory programming and coding software, forcing consumers to face massive dealership replacement costs for a premature electronic failure ona critical safety system.

I am submitting this complaint regarding a serious brake-related safety issue involving my Audi vehicle after recent brake service performed by an authorized Audi Meadowland dealership in [XXX] . In December 2025, I brought my vehicle to an authorized Audi dealership for replacement of the brake pads. The repair was completed by the dealership, and I trusted that the vehicle was safe to drive afterward. However, only approximately six months later, and after driving only around 1,000 miles since the repair, the vehicle experienced a dangerous brake-related failure involving the brake pad assembly becoming loose/detached while the vehicle was in operation. This incident occurred while my entire family was inside the vehicle, creating a serious safety hazard and placing all occupants at risk. A brake system component should not fail after such a short period of time and limited mileage following dealership service. In addition to the safety concern itself, I am also extremely dissatisfied with the dealership’s handling of the matter. The dealership did not proactively contact me regarding the repair status, and multiple attempts to contact them by phone went unanswered. I believe this issue may involve improper installation, defective parts, insufficient inspection procedures, or other serious safety-related service failures. I respectfully request that NHTSA review and investigate this matter to determine whether similar incidents may affect other vehicles serviced under similar conditions. am writing to report a very concerning safety issue with my 2019 Audi Q5 and to request guidance on whether this may be covered under warranty or if my vehicle is experiencing a malfunction. On Saturday, while driving to an appointment, there was a bicyclist in the bike lane. Suddenly, the “Audi Presence” warning flashed red on my dashboard and the vehicle braked very hard on its own. I was not close to hitting the bicyclist and there was no imminent collision risk. The sudden braking was extremely frightening and could have caused an accident if a car had been behind me. It took quite a while for me to calm down and get my breathing back to normal after the incident. Then this morning, I received a dashboard warning that said: “Drive system: malfunction! Please contact service.” I am very concerned that this may be a malfunction of the automatic emergency braking or driver assistance system, especially after the unexpected hard braking incident. This feels like a serious safety issue. I am very concerned and very unhappy, as this situation felt dangerous and could have resulted in an accident.

Check engine light is on almost constantly on my car. It might turn off and stay that way for a couple of days, but always comes on again. Dealership had made two attempts to "fix" it by just clearing the faults. However, according to the service department the reason why check engine light came on was "faults for loss of communication to powertrain electronics and brake electronics" which i think is a real safety concern. After my last trip to the dealership the check engine light came on after just two days of normal driving. There was also a situation when i was unable to shift to park with the risk of rolling along with some other error messages on the dash. I was unable to lock the vehicle because of that and of course there was a risk of it rolling.

On 12/13/25 around 8:00am in the morning. I was driving between 20-25 MPH and started to brake for the corner stop sign. I applied the break as I normally would however, the car continued to roll and did not respond to my applying pressure to the brake pedal. No matter how much pressure I apply to the brake the car continued to coast without stopping. I continued to press hard on the break and even pumped the break. After 2 to 3 yards of coasting, the car stopped. I continued to drive the car for another 0.5 miles and the same break problem occurred again. There are no lights indicating problems on my dashboard. The weather was clear and sunny in the morning. There was no rain. There was no snow. All the roads were dry. I took the car to my dealership and they could not find anything wrong. The brake pads were well within the normal safety range at 8 mm. There was no computer system malfunction. They could not give me an explanation. The Audi dealership’s head mechanic said that because my front brake pads and rotor, and my back brake pads were replaced at Pep Boys two years ago that this could be a reason for the brake failure. although he was not 100% sure of this. He suggested that I pay $3500 for Audi to replace the current rotors and brake pads despite the original determination that there were nothing wrong with them. The auto mechanic test drove the car with me after the diagnostic evaluation. He could not detect any problems with the brakes no screeching sounds no noises. The anti brake system was working well. to this day, I still do not feel safe driving this car for fear that the incident with the brake failure will happen again.

On December 22nd i got into the vehicle and a few errors popped up which consisted of “loose wheel warning malfunction” “brakes: limited functionality drive with causation” “active front assist and emergency assist unavailable”. I called the nearest dealer and was told it’s ok to drive it’s just a software glitch and if i don’t feel comfortable driving i should take it to the dealer and come back tomorrow for a loaner since service was closed already. So i proceeded to drive home, about 10 minutes into the drive a warning for the coolant came up so i pulled off into a parking lot and as i went to park another warning came up saying coolant too high turn off vehicle and as i was approaching the parking spot the car started to overheat and smoke. This is a BRAND new car with BARELY 1000 miles on it.

This vehicle Misrecognizes intersections on two lane highways and causes the vehicle to unexpectedly slam on its brakes without warning in anticipation of an intersection that either does not exist or where my vehicle has the entire right way. this behavior has persisted since the date of purchase and has not been corrected at all. Audi has confirmed that the vehicle is operating as designed however, I have personally almost had two major traffic accidents when this system engaged in the process of overtaking a vehicle, slamming the brakes and almost causing a accident where he and I was rear ended. This is not a vehicle. I would let a child learn how to drive in.
