I noticed that my tire was getting flat at my aunt townhouse in nephi Utah. so I took it to the maverick gas station to get a better look at it and investigate the possibility of a leak in it put some air in it and then enough to make it rise and I leaned in close to listen to see if I can hear a leak and I could hear a sound, but I couldn’t locate it and right when I was about to give up and actually move my head away from my tire. My tire exploded, giving me a major concussion to the point where two ambulance services were called. The police were called. I ended up going to the ER that night because I could not remember how to spell my name what day it was and where I was at exactly aka the hospital.

The van was going about 10 mph and the side wall just burst and went to flat right away and tire pressure warning came on. Yokohama had a recall for this same issue in 2022 but this DOT was not included. Nothing came in contact with tire or van when the issue occurred. This would have been a major issue if going full speed. Discount tire in Southaven MS stated the tire just gave out with no indication of anything hitting the tire. Yokohama refused to do anything as this is a BraunAbility handicap van which shows them as owner 1 to do the conversion through Chrysler. These Vans cannot be purchased through Chrysler directly. Yokohama did now even offer to report this issue internally as a complaint on the tire. I kept the tire for further inspection and investigation. I also have pictures and video of the tire prior to replacement. Tire Yokohama AVID S34 (S34RV) 236/65R17 104T. The P style is not indicated on the tire. The vehicle was towed to a nearby gas station where the tire was replaced. The contact contacted an unknown dealer and inquired about the spare tire kit. The dealer attempted to order the part unsuccessfully and later informed the contact that the part had been discontinued. The contact stated that he had contacted several other dealers in the USA and Canada and was informed that the part was no longer available. The manufacturer was notified of the failure and informed the contact that they could not assist and that the NHTSA could require them to install the spare tire kit inside the vehicle. The failure mileage was approximately 800.

My tire size is 245/50 R 20. At 39,106 miles I had a bulge in the side of my front passenger tire. I had the tire replaced at the dealership. The cost was covered by my extended warranty with road hazard. A month later at 40,468, I had a bulge in the rear passenger tire. I went immediately to the dealership. They did not have the Falken tires in stock or available from their suppliers. My remaining 2 tires were showing some normal wear for 40,000 miles. I made the decision to go with a different brand which would be available sooner. My warranty covered the cost of my second bulging tire. I of course had to pay for the 2 tires that were beginning to show some wear. I also chose to replace the front passenger Falken tire as well. The one that was replaced at 39,106 for a bulge in in the sidewall. I did this for 2 reasons. It is always best to have 4 tires of the same brand on a vehicle. Most importantly, I had already had 2 Falken tires develop a bulge in the sidewall. That is a failure rate of 50%. I did not feel comfortable with using Falken tires. In all my years driving, I have never had a bulge in a tire. I understand that it is usually caused by hitting potholes, driving over curbs, etc. but my driving habits are the same as before. I paid over a $1000.00 for 3 tires. Perhaps, this is not a safety issue with Falken tires that I should be reporting but I prefer erring on the side of caution. My feelings will not be hurt if you tell me I am wasting your time.

The contact owns a 2018 Chrysler Pacifica equipped with Kumho Solus Tire Size: 235/65R17, DOT Number: 275AYAY1. On three separate occasions while driving at various speeds, the contact's steering wheel would shake without warning as one of his tires would become defective. The contact had initially taken the vehicle to a dealer where they replaced the rear passenger side tire due to an internal band failure. The second time, the same failure occurred with the rear driver side tire as an independent mechanic replaced that tire. Soon after the second replacement, the front passenger side tire also experienced that same failure. The contact then decided to replace all four tires. The manufacturer had yet to be notified of the failure. All tires were replaced. Each tire that failed were original tires.The tire and vehicle failure mileage was approximately 30,000.

THERE WAS NO INJURY. THE TIRES WERE REPLACEMENT TIRES. THE CONTACT STATED THAT THE SIDEWALL WAS SHREDDED AND THE TIRE WAS FRACTURED INTO THREE SEPARATE PIECES. MR. TIRE IN BEXLEY, OHIO WAS CONTACTED AND ARRANGED TO REPLACE THE SET OF TIRES. THE TIRE WAS REPLACED WITH THE SPARE TIRE. THE MANUFACTURER WAS NOT NOTIFIED OF THE FAILURE. THE APPROXIMATE VEHICLE FAILURE MILEAGE WAS 200,570. THE APPROXIMATE TIRE FAILURE MILEAGE WAS 24.

WHILE COMING TO A STOP IN A PARKING LOT THE TIRE VALVE STEM BLEW OFF, COMPLETELY DEFLATING THE TIRE. SIX WEEKS LATER THE SAME THING OCCURRED TO ANOTHER WHEEL. IT APPEARS THAT VALVE STEM CORRODED JUST BELOW THE CAP. THE PRESSURE INSIDE THE TIRE THEN BLEW OUT THE SCHROEDER VALVE RESULTING IN A QUICK DEFLATE OF THE TIRE. IF THIS HAD HAPPENED AT HIGHWAY SPEED IT COULD HAVE RESULTED IN LOSS OF STEERING AND POSSIBLE ACCIDENT OR PERSONAL INJURY. BOTH TIRE VALVE STEMS HAVE BEEN REPLACED, BUT NOW WONDERING IF THE REMAINING TWO NEED TO BE REPLACED. SUCH A DESIGN OF THIS PRODUCT IS A POTENTIAL FATAL ACCIDENT WAITING TO HAPPEN.

WE OWNED A 2007 CHRYSLER PACIFICA CAR FOR 3 MONTHS, 46000 MILES APPROX, WHEN PURCHASED. AFTER THE FIRST 3 MONTHS OF OWNERSHIP, THE REAR TIRES WENT COMPLETELY BALD , WIRES POKING OUT, THE CAR WAS BOTTOMING OUT ON EVERY BUMP. THE SPRINGS WERE WEAK WE WERE TOLD. THE SHOCKS WERE BROKEN, THE TIRES WERE COMPLETELY OUT OF LINE AND THE DEALER DID FIX AT THEIR OWN EXPENSE. 2 NEW TIRES REPLACED , WHEEL ALIGNMENT, TWO NEW SHOCKS, AND SUPPOSEDLY SPRINGS FIXED. AT APPROX. 8 MONTHS LATER THE VEHICLE WAS AGAIN BOTTOMING OUT. ONCE AGAIN WE WERE TOLD SHOCKS BROKEN. CHRYSLER WAS CONTACTED AND REPLACED SHOCKS, DEALER DID WHEEL ALIGNMENT AND A TIE ROD END WAS REPLACED. IN APPROXIMATELY 10 MONTHS WE AGAIN NOTICED THE CAR WAS MAKING NOISE AND BOTTOMING OUT. ONCE AGAIN SHOCKS WERE BROKEN AND THE OTHER TIE ROD END NEEDED REPLACING. CHRYSLER CUSTOMER CARE WAS NOTIFIED FROM THE FIRST INCIDENT AND SUBSEQUENTLY AT EACH ADDITIONAL OCCURENCE. THE DEALER PAID AT THE FIRST OCCURENCE, CHRYSLER PROVIDED SHOCKS AT SECOND OCCURENCE, ON THE LAST OCCURENCE CHRYSLER REQUIRED AN INSPECTION BY A CHRYSLER DEALER. ANOTHER TIE ROD WAS REPLACED. WE WERE TOLD BY DEALER CAR WAS GOOD TO GO. WE TOOK TRIP TO PICK UP FAMILY AT AIRPORT AND HAD TO BE TOWED. UPON REINSPECTION 2 DAYS LATER, WE WERE TOLD 2 STUDS HAD COMPLETELY BROKEN OFF LEFT REAR TIRE, AND TIRE COULD HAVE COME OFF ON FREEWAY AT SPEEDS OF 65 MPH. THEY ALSO DISCOVERED TWO LUGS ON RIGHT REAR TIRE WERE LOOSE. THIS WAS THE SECOND INCIDENT WHERE OUR SAFETY WAS AT RISK. 4 SETS OF SHOCKS 2 TIRES 2 TIE ROD ENDS AND 3 WHEEL ALIGNMENTS LATER. TIRE ID # B9H2XKWX1910-----1 TIRE DOT # B9H2XKWX1911-----2 TIRES # B9H2WKWX2211----1 TIRE SIZE 235/65ZR17 PART # 33809 55,000 MILE WARRANTY TIRES WERE ROTATED EVERY 3,000 MILES AFTER THE JULY 2013 TIRE ROTATION THE TIRES BEGAN A WOOP WOOP SOUND AT ALL SPEEDS. SOUND VERY NOTICEABLE. TIRES AGAIN ROTATED AND REBALANCED(WITH NO CHANGE) 2 MONTHS LATER AND THEY STILL HAVE THIS SOUND. HAVE HAD THE CAR BACK TO THE TIRE DEALER SEVERAL TIMES FOR MORE ROTATIONS AND BALANCING( NO CHANGE NEEDED) WITH OUT ANY RESOLUTION TO THE PROBLEM. TIRES ARE STILL ON THE CAR WITH ONLY11,000 MILES ON THE TIRES SINCE PURCHASE.

WHEN ACCELERATES OR DECELERATES THE VEHICLE WILL LUNGE FORWARD. TOOK VEHICLE TO PEERLESS TIRE CO. AFTER MECHANIC POINTED OUT THE WRONG SIZE AND MODEL OF TIRE ON REAR PASSENGER SIDE OF VEHICLE. AFTER ACCEPTING FAULT WANTED TO TAKE VEHICLE TO [XXX] PAINT AND BODY TO PAY FOR THE REPAIRS. WHEN THE ESTIMATE WAS BEING DONE AND VEHICLE WAS ON LIFT...[XXX] AND MECHANIC POINTED OUT THE WHOLE MOTOR CARRIAGE HAD COMPLETELY RUSTED OUT AND CORRODED. THEY HAD NOT SEEN ANYTHING LIKE IT. TOOK VEHICLE BACK TO DEALER AND EXPLAINED THIS ISSUE. HAD TO GO AHEAD WITH INSURANCE CLAIM AND WAIT FOR ADJUSTORS TO DETERMINE FAULT OF ACCIDENT.THE VEHICLE IS TOTALLY UNSAFE TO DRIVE, FEELS LIKE THE ENGINE IS TIPPING FORWARD WHEN ACCELERATING OR DECELERATING. AFTER RESEARCHING THIS ISSUE IM SHOCKED AND APPALLED THAT 2 PREVIOUS MODEL YEARS HAVE SAME ISSUES WHICH ARE NOT BEING ADDRESSED FOR THE 2006. AFTER TAKING IT IN MANY TIMES TO GET AIR I WAS TOLD THE TPMS SENOR WAS BAD ON ONE TIRE AND THERE WAS CORROSION AROUND THE WHEEL. I REPLACED IT AND HAD SEALANT PUT AROUND THE CORROSION. I WAS INFORMED THAT THIS WAS A FREQUENT PROBLEM WITH THESE WHEELS AND CHRYSLER IS AWARE OF IT. I STILL HAVE TO CONTINUALLY PUT AIR IN THE TIRES EVERY 7-10 DAYS, AND THE "SERVICE TIRE SYSTEM" MESSAGE NOW NEVER GOES OUT.
