The contact owned a 2015 Ford F-150. The contact stated that while driving at an undisclosed speed, the vehicle lost motive power while approaching a stop. The contact depressed the brake pedal; however, the vehicle unintendedly accelerated and crashed into a Chevrolet Tahoe. The wrench symbol warning light was illuminated. There were no reported injuries. The air bags deployed as intended. A Police report was filed. The vehicle was towed to a tow lot. The insurance company towed the vehicle to the total loss yard, where the vehicle was deemed a total loss. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The approximate failure mileage was 310,000. The VIN was not available.

My local Ford Dealer (Tallahassee Ford) which I had just purchased the vehicle from two weeks prior. Refused to repair NHTSA Recall Number26V237 they wanted to charge me over 2000 dollars to fix even though it has a recall. I ended up getting the lead frame replaced at a local transmission shop for little over 1000 dollars. I would like to be reimbursed for the money and time I spent.

The clock spring in the steering has failed causing the airbag to no longer work on the driver side. The cruise control turns on and shuts off on its own. Sometimes won’t even let you set it. Volume controls no longer work. Horn no longer works. Steering has a clicking catching sound now when turning. Steering controls no longer light up and none of the buttons work.

The truck exhibited the abrupt downshifting upon driving out of the parking lot, this caused great stress and possible accidents to me and family. The transmission continued to get worse and we were in fear of our lives to drive the vehicle on the highway. Once we got the vehicle home it literally would not shift into gear and has been disabled since the first week in XXX. We are plumbing and heat contractors and have lost a great deal of business due to not having a truck/this truck to use for work. We had it towed to Greico Ford in [XXX] on advice from Keith, the Service Mgr. We know the recall is due in XXX. This dealership is now trying to tell us that all the hoses are damaged and wants to charge us $2,000 to repair the hoses. We obviously don't want to do this since we believe the violent jarring and downshifting of the transmission caused by this MFG defect probably caused this and a lot more other problems. MFG Recall #26S28, NHTSA Recall# 26V237. I need to know when this recall remedy will be submitted and what are my rights to get this fixed? On Thursday, XXX, at approximately 5:10 AM, I was driving my Ford F-150 XLT (VIN: [XXX]) eastbound on [XXX]. While exiting the elevated bridge at the intersection of [XXX] and [XXX] at 55 mph (6th gear), the vehicle experienced a sudden, uncommanded transmission downshift directly from 6th to 1st gear. This caused immediate extreme deceleration, engine over-speed, and locked the rear tires, causing tire slippage and an absolute loss of traction control on a high-speed highway. This severe event put my life and my wife's life in imminent danger of catastrophic injury. After safely pulling over, the Malfunction Indicator Light (Check Engine) and the Powertrain Fault Warning Light (wrench icon) illuminated. The electronic cluster froze and failed to display the gear position. Drive (D) mode was completely unresponsive. I managed to vacate the active roadway using Manual (M) mode. While driving in 4th gear in manual mode under extreme caution, a second violent, unintended downshift to 1st gear unexpectedly occurred, confirming the persistent, dangerous nature of this failure. Consequential mechanical damage includes severe transmission clunking noises during gear engagement and a constant internal metallic knocking sound from the engine block caused by the forced over-speed. This failure perfectly matches the technical failure mode described in NHTSA Safety Recall Campaign 26V237 (OSS to PCM signal loss). On XXX, at 11:10 AM, I personally visited Arceneaux Ford Inc. (Authorized Dealership in [XXX]) to report this safety defect, open a service case, and request a safe vehicle inspection and loaner. The dealership explicitly refused to open a case or take action, stating that "Ford Motor Company does not currently have an engineered solution for this problem," leaving me stranded with an unroadworthy, highly hazardous vehicle. The truck has sporadically gone into “limp” mode for 6+ years. In the last 4 weeks it has happened a total of 6 times while driving. On Tuesday 6/9/2026 it happened while leaving a driveway. I was able to disconnect the battery and let the throttle body reset. I reconnected the battery, it started and drove without problems. I connected a loaded horse trailer, drove approximately 12 miles and it happened again, this time on a two lane road with a soft , uneven shoulder going from 60 miles per hour to 8 miles per hour very quickly. I was able to turn on my hazard lights and pull onto the grass, but only about 5 inches off the 60 mph road. In order to reset the throttle body, I disconnected the battery again, disabling the hazard lights while vehicles passed by narrowly colliding with us several times. The check engine light was illuminated, but I was not in a safe place to scan the truck. It usually disappears after resetting the throttle body, but did not this time. After 10 minutes, I heard the throttle body click, it started up and I drove another 15 miles when it happened again. This time at a temporary stoplight with no shoulder. I was able to scan the computer and the P2111 code was displayed. I reset the throttle body again, drove 5 miles and it happened again. I was able to navigate to a safer area, reset the throttle body again and drive less than two miles home. On 6/11, after letting the truck sit for two days, I started it with no issue to move the trailer and it happened again. I tried resetting the throttle body again and if did not work. The P2111 code is still showing and the throttle body will not reset. The truck is currently inoperable. It will only idle, roughly, and pressing the accelerator pedal had no effect on the truck.

The truck often shifts from 6th gear down to 1st or second gear, sometimes at highway speeds and all warning light come on. It sometimes resolved itself upon starting it again, but seems to affect the throttle body. When the electrical system resets, you can hear a click from the throttle body indicating that the valve reset when the computer reset. We also have been affected by intense suffering from the transmission that started about the same time, around 2019 and just over 100,000 miles, after the warranty expired. Both the limp mode and shuddering seem to be results of the TRS signal. I have replaced the battery and throttle body (that repair was first suspected to be the sensor) at Lakeland Ford, then later at Midas in north Lakeland I had work done to the wheel mounts and other axle/wheel relate repairs. In total we have spent approximately $4000 on repairs caused by the improper TRS signal. In addition, we have had at least 4 near accidents caused by the faulty signature. I called Ford with no solution, such as a loaner very, for safety before the recall can be fixed.

"My 2015 Ford F-150 is experiencing an intermittent, complete loss of the Output Shaft Speed (OSS) signal within the 6R80 transmission lead frame. While driving at speed, the speedometer suddenly drops to zero, the 'wrench' light illuminates, and the vehicle performs an unintended, harsh downshift into a lower gear (Limp Mode/3rd gear). This creates an immediate safety hazard of rear-wheel lockup or a sudden loss of acceleration in traffic. i took my vehicle to a local repair shop where they could not duplicate the fault. i then called my local ford dealer who informed me unless it would duplicate the fault or show a code they could not repair the vehicle due to it being against warranty policy.

1. System Failed: Power Train (Transmission). Component Failed: The 6R80 6-speed automatic transmission, specifically the internal lead frame and the direct drum. Inspection: No, the failed unit was replaced on December 19, 2025, but itemized diagnostic reports and repair invoices from both a Ford dealer and an independent shop are available for inspection. 2. The vehicle experienced sudden, unexpected downshifting and a temporary loss of power while in motion. These incidents occurred frequently and randomly, particularly while accelerating or traversing inclines, creating "unsafe driving scenarios" where the vehicle could have been rear-ended or lost traction due to abrupt gear changes. Furthermore, the vehicle eventually entered "limp mode," where it would "buck" during shifts and fail to accelerate under load, leaving the driver vulnerable in traffic. 3. Yes, by both. Independent Center: Super Tech Automotive confirmed on December 11, 2025, that the transmission line pressure dropped from 158 psi to 20 psi under load, causing the vehicle to go into "limp mode". Dealer: South West Ford Service Department verified the concerns on December 16, 2025, documenting a "torque converter shudder" and "direct drum failure" with "excessive metal debris in fluid". 4. Yes. The vehicle was inspected by a manufacturer-authorized dealer, South West Ford Service Department, on December 16, 2025 (Service Advisor Ryan Forbis; Transmission Specialist 'Dietrich'). 5. Yes. Symptoms: The symptoms first appeared in August 2025 as a "sudden, temporary loss of power or unexpected down-shifting". While driving at normal speeds, the transmission hesitates and pops when shifting into 4th gear. The vehicle slows down unexpectedly before shifting and will suddenly downshift without warning. This causes abrupt deceleration and creates a safety concern, especially at highway speeds. The issue occurs intermittently and without driver input. Even when I’m in normal gears it will downshift and cause me to slow and lose control even when I am not breaking. It occurs at just 35 mph- highway speeds.

I have had 2 instances of my F150 downshifting at highway speeds to 2nd gear while driving. I was not towing a trailer, just driving normally in both cases on my daily commute to work. The first instance was resolved by coming to a stop, shifting to park, and shifting into drive again. The second instance, I released the accelerator completely, shifted the gear selector to neutral and back to drive while coasting and it seemed to correct. Neither have led to loss of control but it is startling and not a great thing for the longevity and safety of my vehicle. I see how it could become a problem. After instance number 1, I thought perhaps it was a fluke. I now have learned that there have been prior model years of this truck transmission that also have had issues and I am more concerned that this get resolved. I now plan to have vehicle looked at by a dealership in the near future. first incident was early September and second incident was Oct. 20, 2025.

A mechanic confirmed the truck has an issue with the speed sensor. On the daily, the truck will experience shifting issues, gets stuck in 3rd, and will not go past 5th. I have to turn off the vehicle and restart and it resets. I've read this is a known issue, and F150s for model years prior to and after 2015 have been recalled for this same thing. I also believe there are already a significant number of complaints for the 2015 model year for this exact issue. Please issue a recall.

I noticed in the fall of 2024 that my transmission stared shifting hard between 30-40 mph during acceleration. I had the local Ford dealership check out the issue and they stated I needed a new transmission and included an estimate. I decided to seek information from 2 independent transmission shops and they could not confirm that I needed the transmission replaced at that time. The vehicle has been driving fine and I recently started to notice the hard shift again at 30 mph. Every once in a while, when the hard shift occurs, it seems like I lose power to the drive train and my vehicle slows down to about 15 mph and vehicles need to go around me. It only lasts for a few seconds and then the acceleration will continue to drive normal. I do notice an increase in RPM's from time to time as well while shifting around 30 mph but does not feel like it is slipping. Once through this area of shifting, the vehicle operates normally shifting through other gears. There were no warning lights or other symptoms leading to this issue. I did notice that there was an article posted with a possible transmission issue from this year vehicle that is being investigated concerning sudden down shifting while driving. I was traveling [XXX] near Liverpool, PA at 60mph, the truck flashed a quick code on the dash and then suddenly downshifted from 6th gear to 1st almost spinning the truck as the rear wheels locked up momentarily. It would not upshift past 3rd gear and had to travel 35 miles home like that. I called my local Ford dealer (Parsons) and they said bring the truck in for diagnostic testing, so I did. Meanwhile I searched the internet to see if others had the issue. Turns out they did, BUT my 2015 with the 6R80 was NOT in the recall notice. Ford said it was the lead frame and needed to be replaced. IF it was a known problem, why not inform owners of these trucks. That said, Ford wanted $1500 to fix it, BUT they did not have the part. So I drove the truck (never with my family in it ever again) after I cleared the code with my reader...again traveling [XXX] @ 75mph it downshifted IN TRAFFIC. Luckily I was in the right lane and made a quick move to the shoulder to avoid being run over. In Carlisle, PA there are hundreds of trucking companies and warehouses, so the truck traffic is very heavy. That was it. I searched out the part, paid for it myself and had a Ford mechanic that does work on the side fix the truck. I have been a loyal Ford guy for over 30 years...I reached out to Ford via telephone, chat and even Facebook. All they say is it is NOT under any recall. Hard to swallow that when I am driving a $75K truck THEY built and they will NOT stand behind a $2K fix. Well goodbye Ford, as a business owner for over 25 years. If I make an error I fix it NO MATTER what. I gave Ford the opportunity to stand behind truck and they did...JUST FAR behind it and passed me as I sit on the shoulder of the road. [XXX]: check engine light was on and codes P2196 and P2198 were present. The dealer suspected it was the purge valve, causing the problem so they replaced it and reset the codes. 10/10/24: I brought the truck in to have it looked at due to the check engine light being on again. I was told the lead frame needed to be replaced but was on backorder. The bill to diagnose this was 196.80 and I was told I could still drive it. On [XXX], I had to bring my truck in to be inspected because while I was going about 70 mph it all of a sudden downshifted and jerked me forward like it was going to abruptly stop. The check engine light came on and I had to drive pretty slow to avoid a breakdown on the interstate. Very scary to have this happen in interstate traffic It could have easily caused an accident. At this time, It was brought to my attention that the downshifting could also be related to technical bulletin 1495. Since we couldn’t get a real definitive answer on what was going on and the lead frame was on backorder, I decided to try and have the case connector in the bulletin removed and replaced as well as do a transmission tuneup on 10/23/24. This bill cost me $383.77. All codes were cleared and all appeared to be working correctly. On [XXX], I was driving on the interstate at about 70 mph and my truck seized up again and jerked forward, again scaring me and potentially causing an accident. I drove it very slowly directly to a repair shop. It was there until November 7, 2024 after the lead frame was replaced. That bill cost me $1321.68. I decided to file this complaint after I had seen a news article that this downshifting was being investigated for the same year as my 2015 Ford F150 for a safety issue. It's the same issue as Recall Notice 22S02/NHTSA Recall 22v011. The replacement part is MR3Z-7583-A 1X ($2-$5 part with 30 second install), but they do not show it under parts for my truck, they would prefer to sell me the whole brake pedal assembly ($50+ part and labor probably diagnosis charge in addition to part replacement). The part becomes brittle and breaks. This caused dead battery, 5 hours from home, due to lights staying on, allowed me to start and put truck into gear without touching the brake, lost acceleration between 45 & 50 mph, cruise control would not turn on and message presents that hill assist is not available.

I was driving at 65 mph when my 2015 F150 shifted into 1st gear. I started skidding sideways and almost flipped but was able to run through the gears quickly and got it into Neutral. This has happened a few other times since the initial incident. I constantly receive codes related to lead frame assembly issue: P0720, P0721, P0722, P161B, etc. I am in desperate need of a new lead frame assembly so my vehicle does not crash on the highway. 6/25/24 I PUT MY 2015 F150 IN REVERSE AND GAVE IT JUST A LITTLE ACCELERATION AND IT IMMEDIATELY ACCELERATED FAST CROSSING THE STREET AND HITTING A UTILITY POLE TOTALING MY TRUCK. I DID SOME RESEARCH ON A FORD FORUM AN SAW WHERE OTHE PEOPLE HAD HAD RAPID UNEXPLAINED ACCERATION.ONE PERSON SAID IT WAS BECAUSE OF A FAULTY ACCELERATO PEDAL POSITON ALSO KNOWN AS APP. MY SAFETY AND ANYONE WALKING OR DRIVING BY MY HOUSE WERE GREATLY PUT AT RISK. THE PROBLEM HAS NOT BEEN REPRODUCED OR CONFIRMED BY ANYONE. THERE WERE NO WARNING LAMPS OR MESSAGES. THIS NIGHTMARE CAME OUT OF NOWHERE.

VEHICLE SPEED CONTROL

I am driving back to Florida on [XXX] and my engine light comes on and the wrench symbol on my dash, as I am going about 70 mph and accelerate suddenly my truck going from gear/speed 6 down to 5. I take my foot off the accelerate and it goes back to speed /gear 6. If i stay at one speed and do not accelerate its ok, the minute I accelerate it changes speed. I took it into the Ford Dealer in Daytona Beach, and they are fixing the problem, but I don't have all the details. I am wondering if this is related to your recall to the 2014 F-150 where the problem is exactly like mine. I have not gotten the truck back, only that it is costing around $2400.
