The contact owns a 2021 Ford Mustang Mach-E. While the contact's daughter was driving at an undisclosed speed, the vehicle independently accelerated. The brake pedal was depressed; however, the vehicle failed to stop as intended. No warning lights were illuminated. The contact stated that the failure occurred while the vehicle was being parked in the residence driveway. The vehicle crashed into the right side of the residence before coming to a stop. The air bags did not deploy. The contact stated that the right side of the garage was damaged. No injuries were sustained. The contact stated that the failure was a known failure with the vehicle. The contact stated that an unknown recall with a similar failure was completed in 2022. The vehicle was towed to a local tow yard. The vehicle was deemed a total loss by the insurance company. The manufacturer was not made aware of the failure. The failure mileage was approximately 65,000.

Vehicle: 2021 Ford Mustang Mach-E Component: Image Processing Module A (IPMA) / Front Camera / Forward Collision Avoidance System The IPMA and front camera system have failed, disabling Pre-Collision Assist, AEB, forward collision warning, and adaptive cruise control while driving. The vehicle is available for inspection upon request. Warning messages "Front Camera Fault Service Required" and "Pre-Collision Assist Not Available" first appeared August 24, 2022 and have recurred consistently over nearly four years. No accident, physical damage, or obstruction was present at any occurrence. This failure puts safety at risk by silently disabling AEB and forward collision warning during normal driving — systems a driver may depend on in an emergency braking situation with no advance notice they are unavailable. The problem was confirmed by a Ford dealership in February 2026. Their repair order documents: "RAN IPMA SELF TEST AND FOUND DTC SET U3000-49... HARDWARE TEST FAILED... FOUND TSB RELATED REC REPLACE MODULE." The dealer contacted Ford, who authorized 50% off the module replacement only, characterizing it as a "loyal customer discount" rather than a warranty or goodwill repair. I declined to pay for the fix. NHTSA Campaign 11028164 (superseding TSB 25-2156) covers this exact failure. Ford's own TSB acknowledges the defect and the dealer confirmed it applies to my vehicle, yet owners are being quoted up to $8,000 for a known safety system failure. The Mustang Mach-E Select has three driving modes. From least to most aggressive in terms of acceleration, they are Whisper, Engage, and Unbridle. When driving in 1-pedal mode where removing your foot from the accelerator causes the car to brake, the strongest application of brakes is in Whisper mode as well. It’s not aggressive, but it will bring the car to a stop over a shorter distance. There is a known and reported problem among drivers, shared often in driver forums like Reddit, where vehicles will change to Engage mode from either Whisper or Unbridle. Over a year ago, I first noticed my car doing this on a downhill exit from a highway to a stoplight. I engaged the brake when I realized I was approaching the stopped car in front of me faster than I should. Over the past 16 months, I’ve experienced this at least twice a month, with no way I’ve been able to discern of predicting when it will happen. I drive the car in the most energy-using and highest energy-recovering mode. I am well aware that the braking might change at any time, but am concerned my wife, who drives the car less than I do, or some other Mustang Mach-E driver will rear-end another vehicle by realizing too late the car is not slowing as expected. I’ve told my dealer, I called Ford, now I’m notifying you in the hopes you can get Ford to issue a software fix. This was something I just noticed again pulling in to my garage solely because I was showing my wife something, and I realized I had the time to report it. While picking up my daughter from school this week, I entered a school parking lot and began a low-speed right turn to pull into a parking space. During this maneuver, the car unexpectedly and abruptly accelerated forward. I reacted immediately and applied the brakes, but the car traveled approximately two car lengths beyond the intended parking spot. I came to a stop across two parking spaces. This occurred in an active school pickup area with parents and children nearby. There were no cars or pedestrians directly in front of me at that moment. I still fear what the outcome might have been if anyone had been present. Immediately after stopping, I observed that the adaptive cruise control speed indicator on the instrument cluster was set to 15 and was crossed out, indicating it had recently been active and disengaged. However, I did not engage adaptive cruise control during that trip. Based on the car’s behavior and the display state, my best assessment is that adaptive cruise control was unintentionally engaged during the parking maneuver causing the car to attempt to accelerate toward a previously set cruise speed despite being in a low-speed, turning scenario. Again, I did not use ACC on this trip. A sudden acceleration in a school parking lot presents an extreme hazard. The only reason this incident did not result in injury or death was immediate braking and luck. There were no warning lamps or messages prior to the incident. The car has not yet been inspected by a dealer or manufacturer. I am aware of multiple publicly shared videos from other Mach-E owners documenting similar incidents, some resulting in crashes. This appears to be a systemic adaptive cruise control design or software issue that warrants immediate investigation and correction to avoid serious injury or death.

Everyday I drive the vehicle There are 2 error messages 1. Front Camera fault, service required 2. Pre-collision Assist not available I have 32 Pre-collision warning messages in the last month, one for every day I drive. I had to drive 4 hours with no cruise control causing my leg to cramp at the 2 hour mark. There needs to be a recall for this issue. Many many other owners of the Mach-e have mentioned the same.

I was backing up the car and the car raced on it own at least couple of times. Luckily no-one was injured. I am worried about the safety of my family including a newborn. There was a Recall 22S29 for the mach-e but it seems to have not fixed the issue for my car.

I was proceeding to drive home from an appointment with a friend, the car suddenly lost power and I had no control over anything. I was able to keep the steering wheel in the locked position trying to get to the side of the road. Thankfully the speed was only 25mph, as someone had just switched lanes and came in front of me. I was in the process of turning to enter the highway. I've been having a problem with this car since I purchased. I told Ford after this incident, that I fear to drive the vehicle with my family, because that's how scared I was when I had no control. I requested multiple times for someone from Ford Corporate to contact me, and still have not heard from anyone. On 11/26 I received a recall for the 12v battery, which is pretty much what keeps the car on and off, and I still have not heard from anyone in Ford corporate.

Dash completely turned off. We could not see speed, gear, battery life, etc. See attached photo of the dash board above the steering wheel. Tried turning car off and on, tried resetting the monitor. Dealership service advisor witnessed this. The technician could not replicate it, and they did not have a solution but it is now back on. Worried it may happen again, opened up a buy back claim but dealership thinks we don’t have a strong case since they couldn’t find anything.

Vehicle: 2023 Ford Mustang Mach-E (leased) The issue involves the cruise control system and steering wheel control layout (human-machine interface). Cruise control is unintentionally activated when attempting to use the turn signal due to the placement of the cruise control buttons. The vehicle is available for inspection upon request. On two separate occasions while using one-pedal driving, unintended cruise control activation caused sudden acceleration during low-speed turning maneuvers. The first incident occurred while signaling to turn into a parking spot. The second occurred while entering a left-turn pocket. In both cases, the unexpected acceleration created an immediate risk of collision with surrounding vehicles and required sudden panic braking to avoid an accident. The issue has occurred multiple times during normal driving but has not yet been formally reproduced or confirmed by a dealer or independent service center, and the vehicle has not yet been inspected by the manufacturer or other parties regarding this issue. No warning lamps, alerts, or messages appeared prior to the unintended acceleration. Cruise control engaged without driver intent and without advance warning.

Upon vehicle startup, the speedometer does not show on the instrument cluster (completely digital readout, analog readout not possible). In its place is the EV's remaining battery % with absolutely no way to access the speedometer and know the speed of the vehicle. This places the driver, other drivers, pedestrians, anyone else on the road at risk of excessive vehicle speed. There is no warning, error messages, or other symptoms prior to the issue happening. The only way rectify the issue is to fully restart the vehicle. However, this is not always a guaranteed fix, and the problem may persist between starts up. The issue does not occur during every start up event and is intermittent with a sudden onset (had the vehicle for 1.5 years at this point). The issue has occurred 3x in the last 2 weeks now. There is an issue with Cruise Control while moving out of a parking space. I am concerned for my wife who knows nothing about technology. If she were to MISTAKENLY press the Cruise Control button while parked and a car is in front of her, the car's Cruise Control will ACTIVATE AND LUNG FORWARD at a speed of 15MPH. This can be frightening and in a panic a person can crash. This has happened to me on SEVERAL occasions but I am able to handle the situation. If it happens to my WIFE, there surely will be a HUGE problem. I can reproduce the problem very easily. If you are parked behind a car Cruise Control can be activated. If you are STOPPED and there are no cars in front of you, you CANNOT activate Cruise Control. Therefore, when you need Adaptive Cruise Control (for traffic jams) a double click of the button seems feasible. Single click when you are over 30mph. This should be an easy software update.

Shortly after purchasing my vehicle on [XXX], specifically on [XXX] a significant malfunction was encountered, indicated by the 'Front Camera Fault Service Required' and 'Pre-Collision Assist Not Available' warnings. Returning to the dealership on May 5th, the manager misinformed, stating the issue was a recall and required service at a Ford dealership. However, upon contacting Ford Lincoln with the VIN, it was discovered these codes are not associated with any recall. I returned to the dealership, provided them the printout from Ford Lincoln explaining the codes and how it's not a recall. They then told me that they will not honor the repairs due to me exceeding my 30-day extension. I proceeded to leave and said I'll deal with it when it poses a problem. After a resent software update, a new code popped up saying "Door Fault Service Required". July 2025 Koons Ford picked my car up and they confirmed that the issues documented in the attached photos are not covered by any recalls. However, this experience is further corroborated by attached photos and testimony from others reporting identical recurring problems since 2022. A KNOWN faulty camera problem for 100s of Ford EVs. Starts off as very intermediate warnings until it stops working all together. This problem doesn't allow you to use cruise control and a host of other standard safety features. This should be a recall, I can't imagine someone getting hurt or worse because ford isn't getting ahead of this thing.

Sudden unintended acceleration event. I was pulling into a parking spot going very slow and lifted my leg off the accelerator when the car suddenly surged forward and crashed into a tree, a wall and a nearby parked car (no people in the car). This put my safety at risk. I was thankfully not severely hurt and had no other passengers in my car. I have sustained some muscle strains in my neck. I have reported the incident to Ford Credit and requested the data logs and investigation. My airbags were also not deployed and this seems very similar to other reports I have seen online. The police were called to the site and a case filed. There was a message flashing "parking sensors fault".

The contact stated that while driving approximately 20 MPH and pressing the brake pedal to stop, the vehicle suddenly accelerated forward and crashed into the rear of a second vehicle. Before the crash, the pre-collision warning had activated, but the vehicle did not stop. The front end of the had sustained severe damage but no bags deployed. No injuries were reported. A police report was taken at the scene, and the vehicle was later towed to a nearby collision facility. The cause of the failures has not yet been determined. The manufacturer and local dealer have not yet been contacted. The failure mileage was 19,100.

A software glitch is causing lag in the instrument panel which displays the speed and turn indicators. Both are experiencing 'lag' or 'skipping', so the speedometer is not updating in real-time and regularly displays inaccurate speed information. To elaborate, as I accelerate, the indicated speed jumps with the worst experienced so far being 28mph to 60mph. The car will do the same decelerating. The turn indicators/hazard light indicators are also lagging and not displaying correctly. The Ford dealership was not able to resolve the issue after resetting several systems and attempting a software update.
