I am submitting this complaint regarding a serious safety concern involving the brake actuator/booster assembly on my 2014 Toyota Prius. My vehicle has experienced failure of this critical braking component and has been unable to be properly repaired since November due to difficulty obtaining the correct replacement parts. This involves a major safety system required for proper braking operation. Brake actuator/booster assembly. Diagnostic codes: B1206, B2321, C1252, C1253 C1256. Toyota previously acknowledged brake actuator concerns through Customer Support Program ZJB. However, my vehicle is outside the program limits despite experiencing what appears to be the same type of failure reported by many other Prius owners. Since November, I have faced difficulty obtaining the correct parts, reports of nationwide backorders or limited availability, conflicting information regarding updated Toyota part numbers, and concerns about repair providers advertising new OEM parts that may actually be aftermarket or incorrect components. This creates a safety concern because owners experiencing brake failures may be unable to obtain timely repairs using verified, appropriate parts. Repair estimates have ranged from approximately $3,000 to over $4,000, including a Toyota dealership estimate of approximately $4,100. This creates a significant barrier to restoring a vehicle to safe operation, especially involving a component Toyota has previously addressed through a customer support program. I request that NHTSA review this complaint and similar reports to determine whether further investigation is warranted regarding Toyota Prius brake actuator failures, parts availability issues, and whether additional manufacturer action or support is needed. A vehicle’s braking system is a fundamental safety component. Owners should be able to obtain a safe and reliable repair without months of delay or uncertainty regarding replacement parts.

The traction control, abs, and emergency brake light came on. We checked and the code produced was C1391. There’s a humming noise every 10 or so seconds when idle. The humming noise is also present when braking. We did some research and this appears to be an issue with Toyota Prius. We called the dealership and were informed that this was not a known issue. Thank you.

Dashboard Warning Lights: ABS, VSC, Parking Brake Light, Triangle with Exclamation Point, Codes: C1256 & C1391, Intermittent Audible Cabin Buzzing indicating critical pressure loss on start-up for about 10 seconds...then stops, not loss so much as anti-braking quick bursts at about 5 mph of stopping v slowly and seamlessly coming to a stop. I contacted Toyota Corporate Case Management; they refused to escalate or remediate the issue due to vehicle mileage, despite being notified of the active brake pressure leak codes and the audible safety alarm.

My 2013 Toyota Prius with only 99,000 miles is experiencing a brake system failure. The brake warning light, ABS light, and traction control light all illuminate. The brake actuator pump runs every 10 seconds, indicating loss of hydraulic pressure. Safety concern: This issue affects braking performance and disables ABS and traction control, creating a safety hazard, especially in emergency braking situations. This appears to be a known issue with Toyota Prius brake actuator assemblies. Toyota previously issued a warranty extension, but my vehicle is now outside the time window despite low mileage. This failure occurred prematurely and without warning. What happened: The warning lights temporarily went off after adding brake fluid but returned shortly after. The pump continues to cycle frequently, indicating system failure.

2024 Toyota Prius Indianapolis, IN Crash

I was driving my 2024 Toyota Prius and was involved in a front-end collision. The impact was strong enough to cause significant front-end damage, but the front airbag did NOT deploy. I was wearing my seatbelt at the time. This is a serious safety concern because the airbag should have deployed in this type of frontal impact. If the crash had been worse, I could have been seriously injured. I believe this may be a safety defect in the airbag system.

There has been no incident, however the concern what happens with regards if someone is not completely understanding of when the battery is completely discharged….per instructions(difficult to find, terrible diagram, not even as described)we were finally able to unlock and open our rear hatch. However you must have a screw driver in order to open a panel to get to the latch which is supposed to be able to open easily. Upon opening, there is another screw which must be loosened, and a very tiny unmarked leaver which can not be seen as it is not marked nor easily seen. In the manual as well as when i actually went to the dealership I was told that there should be a brightly colored in either bright green or red pull that will open the latch. Our concern is for the individual who needs to open this hatch, is older and not as limber. Toyota needs to do better with their instructions. Who drives around with a screw driver in a Prius? When i went to the dealership to ask how to get into my vehicle the gentleman had to go speak to someone and I was given misinformation as the manual and the service individuals directions were completely different.

I was driving and notice ABS light and other light came on, then noticed that when I tried to brake it felt weak, had to pump brake, then later noticed a leak on brake fluid. I took car to Toyota and they checked it and said brake booster and booster pump needed replacing and they wanted to charge me 3,000 dollars. I could not do the job so I checked around and the auto one in my area informed me that many Toyota Prius’s had a recall on that issue. I made an appointment with Parks Toyota in Deland, Florida to fix problem but when he got there they told me that my car was not on recall for that but was for battery EV fuse but couldn’t do because my engine light was off. I am having such a difficult time with this car. I cannot risk my life and that of others driving with this brake condition if I can’t have the manufacturer or who ever has to to take care of this issue. So far I was able to get a mechanic to do some adjustments so I can drive. I am being very careful and only use it as needed. I stopped looking for a part-time job because I need this fixed first. I am [XXX] , please tell me if I am able to get this recall show for me. Thanks INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

On Jan 5, 2026 I was driving locally at about 40-45 mph. All of a sudden 3 indicator lights came on ABS, Brake, and Brake System Warning Light. There were no noises, but the brakes felt different. They did stop when needed. I left space between myself and others just in case, did not want an accident. Called my regular mechanic. They advised to not drive the car, but to bring it into the shop at 8am the following day. On Jan 6th, the mechanic stated it was beyond their expertise. It was towed to the local Toyota Dealer for repairs. It ended up being the Brake Booster and Pump. Upon investigation online, we saw this was a recall, but we were not notified by Toyota.

Body of car has rust perforation starting from the inside working out. This is on the roof, so water incursion will effect airbags and electrical system. FIrst noticed rust bubbles a few years ago, but did not think much of it. Now two body shops have said roof panel needs to be replaced. This is big stuff!

In 2025 I decided to clean the hybrid battery fan. I removed the side panels and found that the fan to battery duct had never been installed. I am the original owner and no one had ever removed the inside panels or had reason to remove the duct. This has likely resulted in the overheated battery which now needs replacing even though I have only 161,000 miles on this car which I have always maintained without fail regarding oil changes and other maintenance tasks.

I purchased a 2011 Prius 4 years ago from a dealership. For the last 2 years, whenever I drive the car, the ABS, brake, and skid lights are on. also, there's a slightly whirring noise every few seconds from under the hood. Braking has been affected, and I've had to increase my stopping distance as a result. Also, the brake pads and rotors deteriorate faster. Apparently, this is a common problem with this type of car. I've called Toyota, and despite it being a known flaw with this car, they refuse to do anything about it.

Design of rear window, side mirrors and large rear pillar greatly reduce the visibility and checking of blind spot if you are a taller person. Blind spot electronic detection can not be trusted as it's performance is intermittent. Solution would be for Toyota of retrofit electronic mirror feature like installed in the Limited version of this car. This feature has live rear view camera enabled on the rearview mirror at all times. Prius without this feature is very dangerous to drive.

Toyota has a popup ad appear on their infotainment screen every time the car cranks. This does not allow me to use the map and other apps in the CarPlay system. I have on occasion driven the wrong route and got lost in traffic and confusing me as to turns and whatnot. I need to be able to use the maps for guidance purposes to safely get to my destination.
