BACK OVER PREVENTION: SENSING SYSTEM: CAMERA

Ford Motor Company (Ford) is recalling certain 2015-2019 Lincoln MKC, Mustang, F-350 SD, F-250 SD, F-450 SD, 2015-2017 Lincoln Navigator, Expedition, 2015-2018 Edge, Transit Connect, 2016-2019 F-550 SD, Transit, 2017-2019 Econoline, and 2019 Ranger vehicles. The rearview camera may display a distorted, inverted, or blank image when the vehicle is in reverse.

Risk: A rearview camera that displays an inverted, distorted, or blank image can reduce the driver’s view behind the vehicle, increasing the risk of a crash.

Ford Motor Company (Ford) is recalling certain 2015-2020 Transit vehicles. An incorrect driveshaft torsional damper service kit may have been installed, which can allow the damper shield to contact the parking brake cable, possibly resulting in parking brake failure.

Risk: Parking brake failure can result in unintended vehicle movement, increasing the risk of a crash.

POWER TRAIN:DRIVELINE:DRIVESHAFT

Ford Motor Company (Ford) is recalling certain 2015-2017 Transit vehicles. This recall includes all full-size Transit vehicles previously recalled under 17V-408 except ones that received mechanical universal joint (u-joint) as a remedy and any 156" or 178" wheelbase Chassis Cab and Cut-Away bodies models. The flexible coupling connecting the transmission to the driveshaft may prematurely fail.

Risk: If the coupling fails, the vehicle will not be able to move under its own power and the disconnected driveshaft may impact and damage the brake and fuel lines. Additionally, the separated driveshaft may allow the vehicle to roll if the vehicle is exited when the transmission is in 'Park' but the parking brake is not applied. These scenarios may increase the risk of a crash or injury.

Mobility Specialists, Inc. (Mobility Specialists) is recalling certain 2014-2017 Ford E-150 and Transit vans modified to be equipped with Ricon S-Series and K-Series wheel chair lifts. The wheelchair lift positioning input cam may fail while the lift is in use, allowing the platform to travel higher than the vehicle's floor height.

Risk: If the wheelchair lift platform stops above the vehicle floor, the wheelchair user can tip inwards toward the vehicle when exiting the lift, increasing their risk of injury.

Visual System

Issue: On certain vehicles, there could be a problem with the connections inside the rearview camera. As a result, the rearview camera image may not display correctly or at all when the transmission is shifted into reverse. Note: For certain models, this recall also affects incomplete vehicles. Safety Risk: A rearview camera image that doesn't display could reduce a driver's ability to see behind the vehicle while backing up. This could increase the risk of a crash. Corrective Actions: Ford will notify owners by mail and advise you to take your vehicle to a dealership to inspect and, if necessary, replace the rearview camera.

Issue: On certain vans, the pivot points for the side mirrors could rust. If this happens, the mirrors could become loose or fall down. Note: This recall only affects vans equipped with short arm, manual fold, power-adjustable outside rearview mirrors. The recall applies to vehicles originally sold or currently registered in areas of heavy road salt usage (Ontario, Quebec, New Brunswick, Nova Scotia, Prince Edward Island, and Newfoundland & Labrador). Safety Risk: A side mirror that loosens or separates could reduce rear visibility and increase the risk of a crash. Corrective Actions: Ford will notify owners by mail and instruct you to take your vehicle to a dealer to replace the side mirrors.

Powertrain

Issue: On certain vehicles, the flexible coupling between the transmission and driveshaft may crack over time. If this happens, you may notice vibrations and noise from under the vehicle. If you continue driving with this problem, the driveshaft could separate, resulting in a loss of power to the wheels. If the driveshaft separates, and the parking brake is not used when parked, the vehicle could move suddenly. A driveshaft that separates can also cause damage to nearby components, including brake and fuel lines. Note: Cutaway chassis models with a 156" or 178" wheelbase are not involved in this recall. Safety Risk: A loss of power to the wheels could increase the risk of a crash. Additionally, unintended vehicle movement could increase the risk of injury and/or a crash. Corrective Actions: Ford will notify owners by mail and instruct you to take your truck to a Ford or Lincoln dealer to replace the front driveshaft section and install a universal joint (to replace the flexible coupling). Note: Ford does not recommend driving your vehicle if the driveshaft flexible coupling has travelled over 64,000 km.

On certain vehicles equipped with a trailer lighting control module, or that had a trailer lighting control module installed as an aftermarket accessory, water may enter the module and connector, and could cause wiring corrosion and damage to the module. This could result in an electrical short to ground, which could cause an uncommanded seat belt pretensioner deployment, a burning smell and/or an increased risk of fire, even when the vehicle is parked and the ignition is off. This may also cause turn signals to flash rapidly as well as cause a loss of the instrument cluster display, heater and air conditioning controls, and multimedia system (radio, screens, SYNC). An electrical short could increase the risk of injury and/or damage to property. Correction: Dealers will add a drainage hole to the driver’s door stepwell and incorporate a fuse into the vehicle wiring harness. Note: This recall is an expansion of recall 2017-516.

Powertrain

On certain vans, chassis cabs and cutaway chassis', the driveline operating angle may affect the long term durability of the flexible coupling between the transmission and driveshaft. If the flexible coupling were to crack, it would result in vibration and noise from underneath the vehicle. Continued driving with a cracked flex coupling may cause separation of the driveshaft, resulting in a loss of motive power while driving, or unintended vehicle movement in Park without the parking brake applied. In addition, separation of the driveshaft from the transmission can result in damage to surrounding components, including brake and fuel lines. A loss of motive power may increase the risk of a crash, and unintended vehicle movement may result in property damage and/or injury to bystanders. Correction: As an interim remedy, for vehicles with more than 48,000kms, dealers will replace the driveshaft flexible coupling every 48,000kms until the final repair is available. Upon the final remedy being available, dealers will either install a redesigned flexible coupling, modified driveshaft bracket and shield, or revised driveshaft equipped with a universal joint. Note: Cutaway chassis’ with a 156" or 178" wheelbase are not involved in this campaign.

Fuel System

On certain vehicles equipped with a 3.2L diesel engine, defects in the high pressure fuel pump could result in excessive wear, leading to metallic wear particles. These particles could clog fuel volume control valves and/or fuel injectors, potentially resulting in the engine stalling or failing to start. A loss of motive power could increase the risk of a crash causing injury and/or damage to property. Correction: Dealers will replace the fuel injection pump and inspect the fuel system for metallic contamination. If metallic contamination is present, dealers will also clean/flush the fuel system and replace the fuel filter and fuel injectors.

Airbag

On certain vehicles equipped with side curtain airbags, the side curtain airbag assemblies may not have been manufactured and installed with the intended design fold orientation. If the side curtain airbag fold is not oriented properly, it could prevent the airbag from deploying as intended. A side curtain airbag that does not deploy as intended during a crash (where side curtain airbag deployment is warranted) could increase the risk of injury to vehicle occupants. Correction: Dealers will inspect the side curtain air bags on both sides of the vehicle, and will re-orient them if required.

POWER TRAIN

The contact owns a 2016 Ford Transit. The contact stated that while driving 50 MPH, the rubber-flexible coupling between the transmission became shredded into pieces and the vehicle swerved off the road. There were no warning lights illuminated. The vehicle was towed to the independent mechanic, who confirmed that the rubber transmission coupler had failed and needed to be upgraded to the metal version by the manufacturer. The local dealer was contacted and confirmed that the vehicle was previously repaired under NHTSA Campaign Number: 17V408000 (Power Train). The vehicle was not diagnosed or repaired. The contact called another local dealer, Winner Dover Auto Body Shop (591 S Dupont Hwy, Dover, DE 19901), but the vehicle was not diagnosed or repaired. The manufacturer was contacted, but no assistance was provided. The failure mileage was approximately 197,000.

I bought the car in 2017 with 7800 miles within the first few months. The rear camera began to freeze sometimes I once went to the dealer but they said they can’t find it because at that point in time it wasn’t freezing so they couldn’t do anything about it now at least 95% of the time the rear camera does not work either is upside down or it doesn’t work at all I see there has been a recall for my year similar vehicles for this problem of the rearview camera. For some reason my car was not included and I would like to log the complaint and hopefully get my car included. thank you so much.

The contact owns a 2016 Ford Transit. The contact stated that while attempting to reverse, the back over prevention camera was inoperable. The screen displayed a black screen or a blue screen. Occasionally, the back over prevention camera operated intermittently or displayed a message that the camera was currently unavailable. The contact referenced NHTSA Campaign Number: 25V572000 (Back Over Prevention); as a possible cause for the failure; however, the VIN was not included in the recall. FUEL/PROPULSION SYSTEM

My 2016 Ford Transit experienced a sudden loss of engine power while I was driving on the highway. The vehicle entered limp mode, stalled, and emitted excessive black smoke from the exhaust, creating a hazardous situation for me and surrounding motorists. I was forced to pull off the roadway because the vehicle could no longer operate safely. When a warning first appeared, I promptly took the vehicle to an authorized dealer, the dealership preformed some type of update and a DPF regeneration and instructed me to drive the vehicle on the highway for 50 miles to complete the regeneration process, I followed these instructions. The light returned and the vehicle was inspected by another authorized Ford dealership, All-State Ford, which confirmed the condition covered under Ford Customer Satisfaction Program (CSP) 23M01 involving the catalytic converter system. The dealership confirmed that the required repair cannot be completed because Ford has not supplied the necessary replacement part after I called to get an update. Approximately five months have passed since the vehicle was diagnosed, and the required part remains unavailable. During this time, my vehicle has remained unusable. Ford has been unable to provide a repair date or any estimated timeline for when the part will become available. Ford’s response has been to direct me back to the dealership. However, the dealership has confirmed that it cannot obtain the part, provide a repair timeline, authorize reimbursement, or offer any alternative remedy because those decisions are controlled by Ford Motor Company. In researching this issue, I found online discussions from owners describing similar catalytic converter failures and extended delays across multiple Ford vehicle models. While I cannot independently verify those reports, they suggest this may not be an isolated incident. I respectfully request the NHTSA review whether the remedy offered under CSP 23M01 is adequately addressing the safety issue.

I purchased this vehicle, used, back in 2024 from STG Auto Group of Montclair. At the time I purchased the vehicle, they informed me there were no open recalls and ensured me the vehicle was safe to operate. About a year later, the driveshaft coupling (NHTSA 17V408000) completely sheared and I lost control for a bit on the freeway while trying to stop safely. Before I got the vehicle towed, I noticed the coupling was destroyed and it was all made of rubber. At the time I didn’t know about the recall repair that was supposed to replace this rubber coupling with a more robust version. Since I had a service contract with STG, they performed the repair and installed the same type of rubber coupling that had just been destroyed. I got the vehicle back and it drove fine for a few months, and last month the coupling blew out again. I did some due diligence and found out about the recall. I informed STG about it and they said the vehicle was sent to a dealership that is now out of business for the recall repair. The car has been sitting on their lot for over a month while they “source” parts and try to figure out who’s going to pay for repairs. Please help, I need the vehicle back to transport my family.

The contact's son-in-law owns a 2016 Ford Transit. The contact stated that while her son-in-law was driving at highway speeds, an abnormal, popping sound began to emit from the vehicle. The contact also stated that white steam began to emit from the engine as her son-in-law pulled over and had the vehicle towed to an independent mechanic. The mechanic performed an inspection and determined that the driveshaft flexible coupling had cracked which lead to transmission damage. Upon investigation, the mechanic discovered NHTSA Campaign Number:19V767000(Power Train) which he linked to the failure. The contact then had the vehicle towed to a dealer where they acknowledged the recall; however, they informed the contact that the transmission was not covered under recall and the contact was given an estimate for the transmission repair. The contact contested that her son-in-law never received a recall notice about the recall; therefore, the manufacturer should be responsible for the repair. The manufacturer was notified of the failure and stated that it was the owner's responsibility to alert them of any address changes and they are not responsible for the contact not being aware of the recall. The contact was given a case number. The vehicle was not repaired. The failure mileage was approximately 105,000.

The contact owns a 2016 Ford Transit. While the contact's wife was driving at an undisclosed speed, the accelerator pedal was depressed; however, the vehicle failed to accelerate as intended. The contact stated that the engine revved, and the vehicle lost motive power. The vehicle was driven to the shoulder of the roadway. No warning lights were illuminated. The vehicle was turned off; however, while the contact's wife was attempting to restart the vehicle, the vehicle failed to restart. In addition, the contact stated that the failure had occurred while his wife was driving with children inside the vehicle. The contact then submitted a copy of the recall to the manufacturer and was informed that the repair was not covered because the recall was not performed in the required time frame. The vehicle back up camera goes out when reversing. Not every time, but intermittently, maybe 50% of the time. The screen goes blue, followed by the message "Rear View Camera Not Available." This makes it incredibly challenging to see pedestrians or cars directly behind the vehicle. It has not had it confirmed by a dealer; the local dealer said there was nothing yet in their system stating a recall on the vehicle. The NHTSA Advance Notice Safety Recall 25S89 specifically calls out the vehicle as being potentially affected, based on assembly plant and build dates. I have not had it inspected by the manufacturer, police, insurance representatives, or others. There have been no warning lamps. Just the screen going blue, followed by the message "Rear View Camera Not Available"
