Most recent incidents on Friday 08/07 car lost power while driving on freeway unable to accelerate, all engine/warning lights came on was thankfully able to pullover and shut off vehicle. This is not first incident this also happened about 5x in 2025 and had car inspected and serviced by Honda dealership each time being told they didn’t know what the issue is because the engine light wasn’t on at time of appointments. Final visit resulting in them adjusting the intake valves but they were even unsure if this was the correct “fix” well it did last a year without other incidents until recently, there seems to be a recall on 2023+ models but not my 2021 and this seems like may be similar issue. 12v battery and spark plugs are new, car is regularly maintenanced and have been told alternator is still good. Unsure where else to turn as I no longer feel safe driving this vehicle especially with my children. Where I live and work is almost impossible to avoid freeways.

2019 Honda Accord Hybrid Campbell, CA

Vehicle was being driven on the highway when suddenly engine hesitated and started stalling. Speed was reduced from around 65MPH down to the low 40s. Cars were stacking up behind and passing, causing potential hazardous conditions. Vehicle was taken to a mechanic shop and diagnosed with misfiring, after reading OBD II with many alerts coming on. Further diagnostics revealed cylinder head gasket was leaking coolant into engine, as shown by borescope images. This is a known problem on 2018-2020 Honda Accords with both 1.5T & 2.0 engines. The multiple alert messages started to come on a few weeks ago, but the engine hesitation and stalling just started this week. At first we suspected a 12V battery issue, but testing confirmed battery was at 12.8V with no issues.

1. Component: The engine head gasket and the combustion system (specifically Cylinder 2). Specifics: A leak-down test confirmed a failure in Cylinder 2, leading to coolant intrusion. This was initially masked by a water pump code, but after the water pump was replaced and the code returned, a borescope confirmed "clean" pistons (steam-cleaned by coolant) and tap scarring. Availability: Yes, the vehicle and all diagnostic records are available for inspection. 2. Emergency Maneuvers: On multiple occasions, the car entered "Limp Mode" while driving 70–75 mph on the freeway. This caused a sudden, dangerous drop in speed, forcing me to maneuver through high-speed traffic to reach the median/suicide lane. Stranded: The vehicle overheated 5 times in one trip, leaving me vulnerable on the side of a busy freeway. 3. Yes. An independent shop confirmed the failure via a leak-down test, a borescope inspection, and physical analysis of the spark plugs. They confirmed that despite the computer throwing a water pump code, the mechanical failure is the head gasket. 4.Status: It has been inspected by an independent service center. I am now using their findings to pursue a lawsuit/claim against Honda for selling a vehicle with a pre-existing internal engine defect. 5. Timeline: Symptoms began in April 2024 (two months after purchase). Initial Signs: An "Emission System" warning light appeared. The diagnostic code indicated a water pump failure, which was replaced in April. Recurring Symptoms: In November, the car suffered multiple misfires and "Limp Mode." In March, the "Emission System" light and the same water pump code returned, but the shop confirmed the pump was functional and the true cause was the head gasket leak into the cylinders.

Description of Incident: 1. Component/System Failure: The engine cooling system and head gasket failed. A certified shop has confirmed coolant intrusion and a failed leak-down test on Cylinder 2. The vehicle is available for inspection upon request. 2. Safety Risk: My safety and the safety of my family and other drivers were put at extreme risk. On March 1, 2026, the vehicle suffered a sudden loss of motive power while traveling at 70 MPH on the freeway. The car dropped to 15 MPH instantly, forcing me to maneuver across active lanes to a narrow shoulder with almost no space. I was nearly hit by high-speed traffic. This has occurred over 8 times since 2023. 3. Confirmation by Service Center: The problem has been reproduced and confirmed by an independent service center On Site Lube Auto & Tire. They utilized a borescope and leak-down test to verify the internal engine failure. 4. Inspections: The vehicle has been inspected by a certified independent technician. I am in the process of notifying the manufacturer (Honda) of these findings for their inspection and accountability. 5. Warning Lamps & Symptoms: Prior to the total power loss, the vehicle displayed engine temperature warnings and "check engine" lights. These symptoms first appeared approximately 8 months after purchase in 2023 and have progressively worsened, leading to the current engine failure. 6. Additional Notes: This failure is identical to issues cited in active class-action lawsuits regarding Honda's engine defects, affecting both standard and Hybrid models. 2018 Honda Accord Hybrid Cincinnati, OH

Over the past several years, there have been multiple instances where I or my wife are driving on the highway and the engine light comes on or starts flashing, and I am unable to go faster than 50 miles an hour, and the engine itself feels to be underpowered. We have taken it to the dealership for repair at least five times over the past five years and in each case they have attempted to do a repair and said the issue has been resolved, but the issue continue. At at least one of those visits, they have diagnosed the issue as an engine misfire issue. The issue has intensified from the initial symptoms, (which were as I described, but resolved by turning the car off, waiting 30 seconds to a minute and then turning the car on again) to how it is now: in addition to the check engine light, every other light and warning system has gone off when driving on the highway and the engine was unable to go faster than 40 miles an hour, and it does not resolve by restarting the car. The dealership and Honda America’s main phone line have as of yet been unable to give me a clear answer as to why this is happening or whether it can be fixed. A sudden drop in speed and power on the highway as we have experienced many times over the last five years is inherently extremely dangerous.

I purchased my vehicle in March 25th 2024 a message on my dashboard saying engine is too hot so in August of this year I took it to the shop for a diagnosis test and they told me that the car didn’t run hot when they drove it so again in October 10th I was driving home and my car lost power on the freeway so I pulled over and then all my lights on the dashboard came on so I took it to the shop on October 13th and they told me that my head gasket was faulty and my coolant system was faulty. To me a major problem should have not happened within a years time and I had my maintenance done on my car like I should have.

2025 Honda Accord Hybrid Ann Arbor, MI

While driving ~70 mph on highway on adaptive cruise control, the car suddenly LOST ALL POWER/ACCELERATION, including POWER STEERING and ALL WARNING LIGHTS FLASHED ON THE DASHBOARD. I recalled some were BREAK SYSTEM, ACC, ABCS, CHECK ENGINE, POWER, TPMS...etc. Essentially any warning light Accord has was flashing. The car slowed with no power steering, I managed to parked on the shoulder and turned on hazard light. This is extremely dangerous as the highway traffic was extremely heavy. After I parked, I pressed the ENGINE START/STOP button thinking I needed to "turn it off" but the car already had lost power at that point anyway. After pressing it a couple times, the car restarted and I drove to the dealer with one error message remaining on the dashboard - "Emission system problem. Power may be reduced. See you dealer." and the CHECK ENGINE light was on.

2022 Honda Accord Hybrid Oxford, NC

Touch screen on console has lots of ghost-touches in warm weather operating conditions. The audio screen keeps switching from FM to USB MP3 player, and goes into settings or prompts for factory reset while driving, and without touching it. Also, Android AutoPlay navigation keeps jumping around on the map, and giving excessive warnings for next turn, without touching it. This is an EXTREME driver distraction, esp. while trying to use navigation. Local dealer was not able to reproduce in their shop on a cool day. They told me they had 12 other complaints of this issue, and no fix available from Honda, as of Oct. 2025.

2024 Honda Accord Hybrid Chula Vista, CA

Wiring harness failed randomly at 42,000 with no damage to the harness, engine, or vehicle itself. Without the wiring harness the engine does not run properly and shakes violently, causing the car to lose power. This happened with no prior warnings, problems, or damage. Additionally the steering rack seems “loose” and changes direction quite quickly when hitting a pothole or uneven roads even at lower speeds. I have to make sure I constantly have a firm grip on the wheel so no sudden unexpected direction changes occur due to imperfections in the road.

2015 Honda Accord Hybrid at 145,000 miles Edgewater, MD

The contact owns a 2015 Honda Accord. The contact stated that while driving approximately 65 MPH, the vehicle started losing motive power. The contact stated that the battery symbol and a message "12V Battery Inoperable" appeared on the instrument cluster. The contact depressed the accelerator pedal, but the vehicle failed to respond. The contact pulled over, and the vehicle had a power steering assist function; however, the brakes failed to respond as needed while depressing the brake pedal, and the brake pedal went to the floorboard. The vehicle stalled, and the contact coasted to a stop and attempted to restart the vehicle. The contact stated that the vehicle restarted but almost immediately stalled. The vehicle was towed to an independent mechanic. The mechanic scanned the vehicle and retrieved DTC: P0A94 related to the DC power converter. The vehicle was not repaired. The contact researched online and related the failure to NHTSA Campaign Number: 17V418000 (Electrical System); however, the VIN was not included in the recall. The manufacturer was informed of the failure and confirmed that the VIN was not included in the recall. The failure mileage was approximately 145,000.

Occasionally the touch screen will have phantom touches and start randomly acting like it was clicked in random parts. This makes it do random things like switch from navigation to radio to calling random numbers. While driving this is especially distracting and poses a safety hazard. The issue is reported many times online and seems to be common especially when they temperature outside is above 70 degrees.The only solution from Honda seems to be to pay to replace the screen but it’s not a true fix as it can happen again.

The screen randomly freezes or toggles between different screen without user input. It will randomly exit navigaton, end calls, increase volume, etc. This happens irrespective of day or night and whether I am using Android Auto of the factory navigation. Several times it has ended navigaton while driving cause me to have to pull over to avoid going in the wrong direction or missing turns. The system has also had its software updated the, factory reset and software reset with no improvement in the situation.

I am reporting a potential safety issue with my 2020 Honda Accord Touring Hybrid. On September 4, 2025, my dealership identified a non-genuine purple quick-splice connector in the driver’s door harness, which they attribute to error codes B1142 (Driver’s Door Lock Knob Switch Circuit), U0199 (Lost Communication with Driver’s Door Module), and U3003-16 (Electric Servo Brake Control Unit Power Source Low Voltage). This connector, not installed by the recent audio installer who put only upgraded speakers and deemed non-Honda, may be an aftermarket modification (possibly an alarm/tracker or anti-theft device) causing electrical faults. While no water was found, the issue could impact brake assist (U3003-16), posing a safety risk. I request NHTSA investigate whether this reflects a widespread manufacturing or modification defect, especially given reports of similar quick-splices at dealerships for repos or fleet locking. A Reddit user said, “ We see these at our dealer frequently. Those are quick-splices, usually used to piggyback off your oem wires to supply voltage/signals to an aftermarket alarm/tracker. Other dealerships install this in the event of repos/auto locking an entire fleet/or additional anti-theft.”

It registers false inputs “ghost touches” without any user interaction. Yes, the vehicle and affected screen are available for inspection upon request. 2. How was your safety or the safety of others put at risk? The ghost touches cause the screen to randomly change menus, activate or deactivate features, and make it difficult to use navigation, audio, and android auto with maps. This creates distraction while driving, as I have to focus on regaining control of the screen instead of the road, putting myself and others at risk. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? The issue has not yet been officially confirmed by a dealer or independent service center, but it is recurring and can be demonstrated on demand. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? Were there any warning lamps, messages, or other symptoms of the problem prior to the failure, and when did they first appear? No warning lamps or messages were displayed. 2018 Honda Accord Hybrid Pleasant Hill, CA

At approximately [XXX]. Pacific, I was traveling southbound in the fastrak lane of the [XXX] departing from Pleasant Hill, California headed to Pleasanton, California. As I began to accelerate, the engine began to sputter. Immediately, the check engine light flashed on, then the dashboard reported a number of issues ranging from brake failure, TPMS failure, power assisted steering failure, vehicle stability assist failure and a number of other issues that I have never encountered since purchasing the vehicle. The sudden deceleration almost resulted in other cars impacting my car as this occurred during the morning commute. I was able to safely navigate off of the freeway. I was on speakerphone with Honda when this occurred. Had I not acted swiftly, a potential high speed rear end collision was likely to have occurred. I immediately took the car to Concord Honda. The team at Concord Honda performed a diagnosis and advised me that my head gasket was leaking water into the combustion chamber which resulted in the sudden loss of power. I was and continue to be dissatisfied with the handling of this issue by Honda as my car has never been handled by another mechanic. My car has a perfect or near perfect maintenance record and should not have a blown head gasket at 80k miles especially since my car is meticulously maintained and serviced by Concord Honda. I was driving on an expressway 355 on Friday July 25. It was raining hard. I slowed down and was getting ready to exit on 55 when the lights On the dash all started lighting up and telling me it required immediate attention at the dealer. The car would not accelerate. I was going slow and was able to idol to the space between 355 and 55. There were cars all around me. I put my hazards on and turned it off. I waited a minute and restarted it. Luckily it started and I was able to drive. This happened on an expressway during a storm I was terrified and worried that I would cause an accident by not being able to move the vehicle. This happened once before also while raining but was in a parking lot of a gas station. I took it in and they told me it’s the 12V battery. However was not failing while at dealer. This is a serious safety issue.

2025 Honda Accord Hybrid Virginia Beach, VA

On July 14, 2025, our 2025 Honda Accord was serviced at Hall Honda Virginia Beach for a TPMS warning. The repair was marked complete under warranty by 12:24 PM. We picked up the car [XXX] at 7:30 AM. By 4:32 PM, the passenger-side tire catastrophically failed no TPMS alert, no impact, no pothole, just sudden sidewall rupture. This was not a minor issue; it was a failure of a federally mandated safety system. Had it occurred at highway speed, it could have resulted in wrongful death due to service negligence. We towed the car back that night (Visit #2). The next morning, advisor Tito blamed us: “You must have hit something.” He then offered to replace the rim and tire under warranty and gave us two options a fast repair or a “proper” 3-day one. Framing a potentially fatal failure as a customer convenience choice is reckless. We picked the car up again July 21. On [XXX] at 3 PM, the TPMS failed again this time throwing a calibration warning. This was Visit #3. I returned to ask why it failed again. Tito grew defensive and again blamed us for the initial tire failure. I wasn’t there for that I was reporting a second failure of the same safety system. There was no concern shown that the car had been returned unsafe multiple times. No apology, no accountability just blame deflection and hostility. As of July 25, the TPMS still does not work. A fourth visit is now required. This is a repeat failure of a safety-critical system required under FMVSS 138, mishandled with negligence and denial. It is only luck—not competence—that prevented serious harm or death.
