Description of Incident: 1. Component/System Failure: The engine cooling system and head gasket failed. A certified shop has confirmed coolant intrusion and a failed leak-down test on Cylinder 2. The vehicle is available for inspection upon request. 2. Safety Risk: My safety and the safety of my family and other drivers were put at extreme risk. On March 1, 2026, the vehicle suffered a sudden loss of motive power while traveling at 70 MPH on the freeway. The car dropped to 15 MPH instantly, forcing me to maneuver across active lanes to a narrow shoulder with almost no space. I was nearly hit by high-speed traffic. This has occurred over 8 times since 2023. 3. Confirmation by Service Center: The problem has been reproduced and confirmed by an independent service center On Site Lube Auto & Tire. They utilized a borescope and leak-down test to verify the internal engine failure. 4. Inspections: The vehicle has been inspected by a certified independent technician. I am in the process of notifying the manufacturer (Honda) of these findings for their inspection and accountability. 5. Warning Lamps & Symptoms: Prior to the total power loss, the vehicle displayed engine temperature warnings and "check engine" lights. These symptoms first appeared approximately 8 months after purchase in 2023 and have progressively worsened, leading to the current engine failure. 6. Additional Notes: This failure is identical to issues cited in active class-action lawsuits regarding Honda's engine defects, affecting both standard and Hybrid models. I am communicating via written report/email for documentation and accessibility purposes.

2025 Honda Accord Hybrid at 5,300 miles Smithtown, NY Crash 1 injured

LANE DEPARTURE: LANE KEEP: STEERING ASSIST

The contact owned a 2025 Honda Accord Hybrid. The contact stated that while driving approximately 5 MPH and making a right turn onto the driveway, the vehicle unintendedly pulled to the left. The contact stated that while attempting to turn the steering wheel back to the intended direction, the vehicle accelerated unintendedly, and the brakes and the steering wheel were inoperable. There was no warning light illuminated. The vehicle crashed through the garage door, hit cabinets on the side, and drove on top of the cabinets, 3 inches from the ground, and stopped. The driver's side and passenger's side knee air bags deployed. The contact stated that the driver's side knee air bag deployed and broke the plastic underneath the dashboard, which injured the contact's legs. The emergency call (eCall) system activated and connected the contact with an operator, who then called an ambulance and the police on the contact's behalf. A police report was filed. Both the contact and her husband, who was occupying the front passenger's side seat, were transported to the Emergency Room by ambulance. The contact sustained Hematomas on both legs, and the contact's legs were swollen. The contact was admitted for 5 days and was discharged to a rehabilitation center. The contact's husband did not sustain any serious injuries. The vehicle insurance company towed the vehicle to a tow yard. The dealer was notified of the failure and referred the contact to the manufacturer. The manufacturer advised the contact that the vehicle insurance company would need to file a claim with the manufacturer; however, the insurance company had already deemed the vehicle a total loss and had sold the parts of the vehicle. The failure mileage was approximately 5,300.

Component/System: Collision Mitigation Braking System (CMBS) or related software. Vehicle available for inspection. Safety Risk: Vehicle brakes or decelerates unexpectedly during normal driving, risking rear-end collisions. Most recent incident on August 23, 2025, required override by pressing accelerator and shifting to neutral in traffic. Reproduction: Dealer could not replicate issue during May 16, 2025 visit. No diagnostic codes found. Radar and braking systems reported as “operating normally.” A second evaluation occurred after September 5, 2025, with no resolution; issue persists. Inspection: Inspected by Honda dealership twice—on May 16, 2025 (Invoice #488382) and again after September 5, 2025. No third-party inspections. Symptoms: Braking occurs in Sport, Normal, and Econ modes. Happens when accelerating from stop or easing off accelerator. Brake light activates falsely. No warning lamps or messages prior to failure. When traveling on an Interstate Highway in the left lane of a 2-lane highway with the Adaptive Cruise Control turned on. Passing a tractor trailer going slower in the right lane, the car will apply the brakes unexpectedly. The dash will briefly show a "ghost" truck crossing into the left lane. The unexpected braking causes any vehicle behind the Accord to brake. Putting those vehicles and any behind them at risk. Smart Honda of Des Moines looked at the car on Sept. 22, 2025 reported no error codes and nothing could be done. If the Accord hugs the far left shoulder, no "ghost" breaking occurs. If the Adaptive Cruise Control is turn off and regular Cruise Control is turned on, no "ghost" braking occurs.

Left a store on a frontage rd, drove about 1/2 mile , car just breaking out of no where, stoped in the middle of the road, the dash stated breaking malfunction see Dealer immidentaly car died, I restatred it , the dash still stated the same , the picture of the car had back lights flashing and the red Break sign was on , Took it to the nearest dealer and when they started it the warning system had erased. They inspected it, told me that they could not find anything wrong. I just leased it 2wks ago and only have driven it 3 times , not even a hundred miles and this happens. There were no warning signs of any kind . If I would have been approaching the freeway I could have been killed.

The contact stated that the Lane Departure, Lane Keep Assist, ACC, and Blind Spot Detection warning lights remained illuminated. The vehicle was taken to a local dealer, where it was diagnosed, and the contact was informed that the malfunction or warning lights were a result of the camera being faulty and that the camera needed to be replaced. The vehicle was not repaired. The manufacturer was notified of the failure. The approximate failure mileage was 105,000.

While traveling at approximately 55 mph yesterday, in daylight, with no other cars in front, back, or to the side of our car, it radically braked on its own. The collision mitigation system warning light has frequently flashed on the dashboard since we bought the car new in 2019 but never did it apply the brakes on its own. Dealerships over the years have told us "this just happens", and offered no fix for the false positive alerts. This is a dangerous situation which could result in an accident or even death of occupants if not resolved by Honda. I spoke with several dealerships since the phantom braking event and they tell me they hear this complaint often. Being an intermittent situation, it is extremely difficult to diagnose and resolve. Something needs to be done.

2024 Honda Accord Hybrid Morristown, TN

LANE DEPARTURE: ASSIST

Purchased my car brand new. Within 1.5 years I've had to have 2 steering racks replaced and another component replaced within the steering column. Currently my lane keeping assist problem is on and I'm waiting to get appt confirmation from the service dept manager. My infotainment center went black and quit working and had to be replaced a little over a year after purchasing the car. The paint began peeling off my front bumper within 6 months of purchasing the car. I'm currently dealing with the Honda I purchased from to try to come to a resolution of this issue since I don't want my brand new car to have a bumper repainted and possibly not match and have it show up on a carfax report. Also have what the dealership calls "fisheyes" under the clear coat of the factory paint. Approx 20-25 spots on my entire car. The dealership said they could "buff" the places but shouldnt have left the factory in that condition. The body shop that Honda sent me too said once they started buffing on the "fisheyes" that they couldn't guarantee that the buffing of the spots would correct the issue or if they would need to repaint which I'm not ok with since it is a brand new car and paint might not even match the rest of the car. Also', I should add that all the maintenance and repairs have been completed at the dealership where I purchased my car brand new.

I am reporting two safety-related issues with my 2022 Honda Accord Hybrid. 1. Head Gasket Failure At approximately 93,500 miles, the head gasket failed without any prior warning signs. The vehicle had been regularly maintained at Honda dealerships, with one service done by a third-party mechanic around 87,000 miles. There were no engine warning lights or coolant leak warnings before the incident. The engine temperature warning suddenly appeared while driving, and I immediately stopped the car. A Honda dealership confirmed the head gasket failure and quoted over $4,000 for repairs. This kind of major engine failure is highly unusual for such a new vehicle and raises serious concerns about long-term engine durability or possible design flaws in the cooling system or head gasket components. 2. False Activation of Automatic Emergency Braking (AEB) The vehicle’s automatic emergency braking system frequently activates or issues a warning while driving, even when there are no obstacles or vehicles nearby. This has happened multiple times and creates a sudden sense of panic, increasing the risk of being rear-ended or losing control of the vehicle. Both issues suggest potential safety defects in this model. I respectfully request that the National Highway Traffic Safety Administration investigate whether similar complaints have been reported for the 2022 Honda Accord Hybrid, as these failures could pose significant safety risks.

2025 Honda Accord Hybrid Kennewick, WA

FORWARD COLLISION AVOIDANCE: WARNINGS

The car was in cruise control going 73mph on the freeway. I was passing a semi on the left hand side. When I was parallel to the semi, the car brakes hard and a big "BRAKE" light came on the cluster. There were no cars in front of me and luckily no one behind me. If there was a car behind me there would have been a collision. It was a clear day and again the road was clear. It took me from 70 to about 40 mph until I hit the accelerator to get back to speed. The CMBS is faulty and I will take it in to a dealer asap.

All of sudden, no incident, no accident, a lot of things were disabled such as Adaptive Cruise Control , Forward Collision Warning , Lane Departure Warning and more while I was out of town (long range highway travel). When I went to Honda dealership, they blamed a few things and fixed them (expensive), then they said that the front camera needed a calibration. It was expensive. After that aiming correction, everything works fine. The camera was NOT broken. It just needed recalibration, and it costed about a thousand dollars. I would say it was caused by the poor design, which is Honda's fault. I googled this issue and found a lot of Honda owners had same issued and charged 1K ~2K for it. I will report it separately, but my Honda Odyssey has the exactly same issue now. I have 2 Honda cars, and both of them have the same issue. I strongly suggest that Honda should recall their front camera and they should reimburse the expense we already paid for it.

when i am driving in an area near my home with road construction- there is a certain area that has a metal plate in the road and when i approach in from the east my car will automatically brake hard. luckily this has not caused an accident but i suspect that it may because it is a busy road.
