I would like to report a potential safety concern involving the Evenflo Revolve360 Slim 2-in-1 Rotational Car Seat. My child is [XXX] old and is able to independently press the harness release/loosening button located between her legs while seated in the car seat during normal vehicle operation. Once she presses the button, she is able to lean forward, loosen the harness straps significantly, and climb out of the seat on her own while the vehicle is in motion. This has happened repeatedly during everyday use and not during a crash. My concern is that the harness release mechanism is too accessible to a young child and allows the child to defeat the restraint system while riding in the vehicle. I am concerned that if this occurred during a collision or sudden stop, my child could be seriously injured or ejected from the seat due to the loosened harness. The seat has been used according to manufacturer instructions to the best of my knowledge. I believe this may represent a design or safety issue that should be reviewed further. I am submitting this complaint because I believe other caregivers may experience the same issue, and I am concerned about the risk of injury associated with a child being able to loosen the harness independently during normal travel. The contact owns an Evenflo Car Seat, Model Number: 36412348, Model Type: Nurture Max, Manufactured Date: 8/17/2022. The car seat was being utilized in a 1998 Honda Accord. The contact stated that she noticed that the latches on the left and right side of the safety seat failed to attach as designed. Additionally, the pull latch snapped from the safety seat. The safety seat was not inspected or replaced. The manufacturer was not made aware of the failure. The contact stated that the car seat was still in use, just without the base. The manufacturer instructed the contact to destroy the defective seat once the replacement arrived.

On 12/29/22, while in our driveway and preparing to leave home, our forward-facing daughter alerted us to a functionality issue with her car seat straps. The child was able to pull herself all the way forward while strapped into her car seat. After tightening the straps several times, the issue persisted and she continued to be able to fully loosen the straps by simply leaning forward. This demonstrated the way she would have flown forward in the event of an accident, because the straps were not properly restraining her. The car seat was removed from the vehicle and brought inside the home for closer inspection. The locking mechanism in the base of the seat, which typically allows a parent to tighten the straps but then locks the straps in place to prevent loosening, had clearly been rendered nonfunctional. Despite visually appearing functional and safe, the mechanism was not functioning correctly. At that time, a flashlight was used to inspect the mechanism closer. A very small shard of mulch was the culprit. We suspect this mulch must have fallen from our child’s clothing, fallen through the designated strap’s hole in the car seat cover, insidiously jammed itself into the locking mechanism during a routine tightening, and rendered the lock nonfunctional. We believe this to be a significant safety issue that could feasibly repeat without redesign of the mechanism and/or cover of the seat. The mechanism should not be designed in such a way that external items (mulch, food, etc) can interfere. We reported this issue to Graco as well yesterday, 12/29/22. We also have a video of the carseat prior to the removal of the mulch shard demonstrating the nonfunctional mechanism.

OUR HONDA ACCORD 2000 SE WAS IN A HEAD ON COLLISION WITH A PICKUP TRUCK. THE CAR WAS TOTALED, BUT I WOULD LIKE TO MAKE YOU AWARE HOW OUR SON WAS INJURED. OUR SON WAS IN A CENTURY 44164ELM CAR SEAT SITTING BEHIND THE DRIVER (MY WIFE). THE METAL BUCKLE THAT IS USED TO ADJUST THE SEAT BELT SIZE (IN THE BACK) SHEARED FROM THE PLASTIC DUE TO THE FORCE OF THE IMPACT. AS A RESULT THE UPPER PART OF OUR SON'S BODY BECAME A PROJECTILE AND THE SEAT BELT DID NOT KEEP HIM IN THE CAR SEAT. OUR SON'S HEAD LURCHED FORWARD HITTING THE SEAT REST IN FRONT HIM. DUE TO THE IMPACT HE SUFFERED A SKULL FRACTURE, AND FORTUNATELY EVERYTHING IS FINE NOW. I HAVE THE CAR SEAT SAVED IF CENTURY WANTS TO SEE IT. IN MY OPINION IT SHOULD BE RECALLED SINCE THIS IS UNSAFE.
