FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL

Adaptive cruise control was on, i was driving on the highway, traffic was speeding up and slowing down, and the tesla was braking accordingly. I was paying attention to the road and noticed, all of a sudden, that the tesla was not braking as traffic in front of us slowed. I immediately slammed my foot on the brake as hard as i could, yet there wasn't enough time or distance, and my car collided with the one in front of me. I wasn't injured, but there's a chance someone in the other car was. The emergency brake did not kick in whatsoever, and the adaptive cruise control completely failed, leading to an accident.

Also all of the driver assistance safety features stopped working at the same time, forward collision warning, emergency braking, lane keeping assist, adaptive cruise control, blind spot detection have all stopped working as well. I have been driving Tesla using FSD from [XXX] to [XXX] and passing [XXX] (XXX around 5 pm). We took an exit for rest room break near Gas Station. As soon as Car took exit using FSD, the car completely stopped suddenly in the middle of the road without any reason and warning. There was not any rain, visibility issue or driver distraction. There was not any alert or recording. As far as I remember, there was not a vehicle on the front 100ft. It was on a small bridge. My family was in the car (with [XXX]). Date: XXX, Time: 5.09.2026 State: [XXX] Location: [XXX] We were on Full Self Service Driving Mode A [XXX] came and hit my car in the passenger side behind the front wheel and front buffer. We were on left/fast moving lane and two semi-trucks were in the right lanes slight ahead of us (one had overweight sign). Something came out of the direction of trucks and hit our car with a big sound (moving object). When we stopped and checked the car later, it was evident that the damage was a sizable, it would have been something solid or heavy. Our concern is the Tesla was on Full Self Driving Mode, and it did not generate any alert, recording or more importantly, it did not break, change the driving direction nor try to avoid being hit by the object. This diminishes the reliability of Tesla FSD and raises serious safety concerns. We are in the process of filling insurance claim. After disengaging Tesla Full Self-Driving (FSD) and taking manual control of the vehicle, a feedback popup appears on the center touchscreen asking the driver to select a reason for disengaging FSD. The popup does not automatically disappear and remains on the screen unless the driver interacts with it. I believe this creates an unnecessary driver-distraction risk. The moment immediately following an FSD disengagement is precisely when the driver has resumed responsibility for manually controlling the vehicle and should be focused entirely on the road and surrounding traffic. Presenting a nonessential survey at that moment encourages the driver to look at and interact with the touchscreen. There should be an option to disable this feedback request, dismiss it without touchscreen interaction, or have it automatically disappear after a short period. I am reporting this because I believe requiring or encouraging touchscreen interaction immediately after transitioning from FSD to manual driving is a safety concern and should be reviewed.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL

The vehicle is experiencing severe, unpredictable software and hardware regressions related to the Full Self-Driving (Supervised) system (v14.3.7, Software build 2026.21.5) that create an immediate, active traffic hazard and a high risk of rear-end collisions. Primary Safety Issues: 1. TRAFFIC STALLS UPON DISENGAGEMENT: When Full Self-Driving (FSD) is engaged, auxiliary sensors malfunction, causing the headlights to cycle on/off and windshield wipers to run at full speed in clear, dry, midday daylight. When attempting to manually disengage FSD to correct these malfunctioning systems, the vehicle fails to smoothly transition control. Instead, it triggers critical red flashing alert lights, sounds severe warning chimes, and initiates an immediate, aggressive brake application—stopping the vehicle dead in active, live traffic. 2. PHANTOM BRAKING: The vehicle routinely experiences sudden, severe, and unprovoked phantom braking events while driving at speed, without any forward obstacles or environmental hazards present. 3. STEERING INSTABILITY: When navigating parking lot entrances, the vehicle exhibits extreme "indecisive behavior," resulting in violent, rapid shaking of the steering wheel before completing the maneuver. Tesla customer service was formally notified of these dangerous system behaviors on August 14, 2026. A formal remote engineering log review has been requested from the manufacturer, as these sudden braking actions in active traffic pose an immediate threat to public safety.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL

When disengaging Full Self Driving (FSD), a feedback notification pops up on the bottom left corner of the infotainment screen that fully blocks the media control panel and does not disappear until the driver provides feedback to Tesla for why FSD was disengaged. By forcing the driver to answer the feedback question in order to access the media controls, Tesla causes a distraction to the driver, who is forced to take their eyes off the road and select a feedback response to why they chose to disengage FSD, increasing the chance of an accident. This is an intentional but dangerous design decision.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL

Two separate incidents occurred on July 10th and July 12th 2026 involving adaptive cruise control. While adaptive cruise control was engaged my car slammed on the brakes for no reason. There was nothing in the roadway to cause abrupt braking. The second time it occurred my car was close to being rear ended by a vehicle driving behind me. I took the car which is still under warranty to Tesla who confirmed there is a known firmware issue. No time frame was provided as to when the issue will be fixed.

The horn on the steering wheel rarely works UNLESS it is pressed directly in the middle. I have tried to honk the horn when there is a safety event but it rarely works. This is definitely a safety issue. This has been going on since I bought the car June 2022. Full self drive is also unsafe and it will make sudden turns, accelerate too quickly, or have phantom braking. This can be unsafe for other drivers. I heard that this is improved with newer models, but if it is unsafe for older models like my 2022, it should not be available.

All Teslas vehicles running HW3 / AI3 Autopilot self driving computers have been known for years to have a defective design problem with the cooling system without its own dedicated cooling and which puts the self driving computer last in the cooling looping at the top of the cooling stack. When the computer fails and they will Tesla requires the owner to replace the computer. This does not result in a redesigned improvement to the computer or cooling system that reduces the thermal stress being applied to the new computer. Tesla is now pushing a new version of the self driving FSD software with the “v14 Lite” release over the last two weeks. This software release uses large complex software models which Tesla design to use every part of the HW4 / AI3 compute power which stressed the computer by pushing it to it compute limiting. All this compute is causing the computer to overheat and fail. Tesla then require owners who are all out of warranty now to pay thousands to replace the computer. This replacemen uses the same flawed cooling system and the same computer incapable of dealing passive compute requests on it by the latest v14 Lite software to eventually cause the computer to just require the replacement computer to eventually need yet another replacement of the same flawed design at owners cost. There is also supposed to systems which monitor the thermal stress on the computer and shut it off abruptly until it cools down which is not ideal at freeway speeds which is not happening. Recall to address and resolve the issue with improved cooling, more powerful computer that doesn’t reach maximum capacity and it’s each use, and less taxing software needs to be issue. This is a major safety design problem which Tesla has not addressed. There only solution is to just replace design constrained components with the exact same parts at owners cost. These multiple design issues can and often result is sudden shutdown of the computer, or worse case sudden complete failure.
