Whilst driving on highways which feature multiple speed limits - eg. 70 cars, 65 Trucks, 60 Buses. If full self driving more or when in adaptive cruise control - the car systems fail to recognize the restriction linked to the lower speed limits. As a result, you can be driving on Full Self Drive at 75mpm, the car sees a 65 followed by a 60 speed limit (neither applicable to cars), but the car brakes hard dropping the speed from 75 to 62 almost instantly and, on more than one occasion, a closely following car has almost collided with me. Until such time as the Tesla system can recognize the 'sub text' on a speed limit - the FSD and Adaptive cruise should not be certified. I also drive a BMW i4 and this reads all signs correctly, only applying those applicable to a car to the vehicle.

Tesla software update 2026.20.6.11 v14 Lite NA-2025.44 removed manual speed control for Full Self-Driving. The driver can no longer set a maximum speed. The vehicle chooses its own speed and it is frequently wrong and dangerous. Curves are the worst failure. The car enters curves too fast. It accelerates through curves. It does not slow to a safe speed before the curve begins. The driver has no way to cap the speed. The scroll wheel is dead. There is no override. Posted speed limits are ignored. The vehicle uses bad map data instead of reading visible signs. It will do 65 in a 55. It will not slow down when the sign says 55. The driver cannot force it to obey the posted limit. Speed control is gone. The driver is a passenger watching the car make bad decisions. This is not safe. This is not driver assistance. This is loss of control. NHTSA must require Tesla to return manual speed control immediately. A human driver must be able to cap the speed at all times. The current software removes that ability and creates a clear safety hazard.

U.S. registered Tesla operating in [XXX]. Full Self-Driving engaged. Dark, nighttime conditions. Vehicle was traveling toward low traffic zones near the edge of [XXX] when the system made a severe mistake. The car went off the main road. Driver was forced to take control off-road after the system lost control. Damage to the rims occurred. At the time of this incident, manual speed control was still available. The driver could cap the speed. That capability has since been removed by a software update. The current version is more dangerous. If the same incident happened today, the driver would have no way to limit speed or prevent the system from repeating the error. The defect is the progressive removal of driver control from the Full Self-Driving system. Earlier versions allowed manual speed caps. Current software does not. This vehicle is U.S. registered and runs U.S. software on American roads. The same defect that caused this off-road incident in Mexico now exists without the safety net of manual speed control. I was making a right turn from the parking lot onto the street, a maneuver I have made hundreds of times previously without any issue. My foot was on the accelerator pedal as normal for this turn. The vehicle accelerated far faster than intended for this maneuver, with tires screeching, and continued across the width of the street. The vehicle's front left wheel struck the curb on the opposite side of the street from the parking lot exit. The wheel sustained significant damage, and the wheel rim was torn. When I regained my bearings, I lifted my foot off the accelerator and the vehicle slowed down. At the time, I believed a serious crash was imminent. I was able to drive the vehicle home, which was about 1 1/2 miles away, but it is not safe to drive so I won't drive it until it is repaired. I am aware that Tesla and NHTSA have attributed many prior sudden acceleration incidents to drivers unintentionally pressing the accelerator when intending to press the brake. Due to the vehicle's regenerative braking, I rarely use the brake pedal during normal driving, which makes this explanation virtually impossible in my case. I am also aware that NHTSA reopened an investigation in 2023 after learning about a potential inverter-related design flaw that could cause sudden unintended acceleration in Tesla vehicles, and that this investigation remains open as of this writing.

On XXX, while Full Self-Driving (Supervised) was engaged, my 2022 Tesla Model 3 RWD abruptly decelerated without any visible vehicle, pedestrian, debris, or other obstacle requiring braking. Based on my best recollection, this occurred on approximately five separate occasions in clear daylight with clean cameras. On XXX, the first event occurred near XXX while traveling from [XXX] toward the [XXX]/[XXX] area. I took manual control for about 30 minutes. After I re-engaged FSD, a second event occurred and included approximately three rapid braking pulses. The occupants lunged forward during both episodes. On XXX, the drive from [XXX] to the Ukiah Costco was normal. On the return toward [XXX], FSD suddenly braked hard at approximately 63 mph and reduced the vehicle's speed by about 25 mph, to approximately 38 mph. Family members and friends in the vehicle were startled and moved forward. Later that day, there were two additional unexpected braking episodes while traveling from XXX toward XXX. I used accelerator input to override the unwanted braking. The unexpected highway-speed deceleration created a risk of being struck from behind and made the system unpredictable to supervise. No crash, fire, injury, or property damage occurred. I do not recall a warning message before the braking. The vehicle has not been inspected or diagnosed for this issue and is available for inspection upon request. On July 11 at approximately 6:47 a.m., I was operating my Tesla Model 3 on Interstate 65 in Hart County, Kentucky, with Full Self-Driving (FSD) engaged during normal highway travel. Approximately one hour earlier, I had departed a highway rest area after sleeping from the previous day’s travel. Rain developed during the trip, but I observed adequate visibility, apparent roadway traction, and I was not impaired. I was traveling in the far-right lane of a three-lane interstate. The posted speed limit was approximately 70 mph; however, FSD limited the vehicle to approximately 65 mph and displayed warnings indicating that the higher speed was unavailable. While FSD remained engaged, the system initiated a lane change from the far-right lane into the center lane. After entering the center lane, the vehicle began displaying repeated warning messages and FSD disengaged multiple times. I attempted to locate settings related to the warnings on the touchscreen but could not identify an available adjustment. During the third disengagement, I perceived that the vehicle continued braking without releasing while steering resistance increased significantly. The steering wheel then rotated abruptly, and I was unable to regain normal directional control before the vehicle departed its intended travel path and struck a concrete median. The collision caused personal injuries and substantial vehicle damage. I have not been able to reproduce the condition. The incident was investigated by the Kentucky State Police. Based on the sequence of repeated FSD warnings, disengagements, continued braking, and abrupt steering behavior, I believe this event warrants investigation. I request that NHTSA review the vehicle’s Full Self-Driving logs, telemetry, Event Data Recorder (EDR) data, steering and braking commands, diagnostic records, and software history to determine whether a vehicle or software malfunction contributed to the collision. Tesla Full Self-Driving (Supervised) repeatedly ignored the “Avoid Toll Roads” navigation setting and attempted last-second lane changes into Express Toll Lanes at freeway speeds. This has occurred approximately 10 times. I was repeatedly forced to take over steering immediately to avoid entering the toll lane, creating a serious safety risk due to sudden maneuvers in heavy traffic. On several occasions, I could not safely intervene before the lane change was completed. Tesla Service Center inspected the vehicle, found no hardware or camera faults, and stated the behavior appears to be software-related.

While turning right, I slowed down and put on my turn lights. The vehicle accelerated all of a sudden and did a sharp right. There was a vehicle in front of me in the opposite direction and my car turned very sharply to avoid it. The accelerator pedal was fully pressed down and for that fraction of a second, it didn’t allow me to control the speed.

The vehicle was purchased pre-owned directly from Tesla. On the day of pickup and five days later, while operating on Full Self-Driving (FSD), the vehicle phantom-braked abruptly and then dangerously accelerated through a solid red light into cross-traffic intersections. The driver had to forcefully override the vehicle to prevent a crash. Tesla Service Center was notified but dismissed the issue, performing only a camera recalibration and charging a fee. Following the service appointment, the red-light running behavior stopped, but the vehicle continues to suffer from aggressive, sudden phantom braking multiple times on every single trip, creating a severe and continuous risk of a rear-end collision on public highways.

The forward-facing camera and Autopilot computer system on my 2021 Tesla Model 3 has failed. The dashboard first showed intermittent warnings that "Automatic Emergency Braking is currently unavailable" and "Adaptive headlight features are unavailable." Over several weeks the problem worsened from intermittent to constant, and the vehicle progressively lost its forward road visualization and all active safety features. Using the vehicle's Service Mode diagnostics, the Autopilot computer's Primary (A) processing node reports a "CRITICAL" status. The fault log records repeated alerts including dasMIA (driver-assistance system not responding), aebFault (automatic emergency braking fault), and fisheyeCamInitFault and fisheyeCameraStreamExit (forward camera initialization and stream failures). The cameras themselves are physically clean and unobstructed. Safety risk: the failure disables Automatic Emergency Braking and forward collision avoidance, lane-keeping/driver assistance, and the adaptive auto high-beam headlights. AEB is a critical crash-avoidance system; losing it leaves the vehicle unable to brake automatically for an imminent collision, and the loss of adaptive headlights reduces nighttime visibility. A Tesla service center inspected the vehicle, confirmed the diagnosis, and determined the Autopilot computer — a sealed, solid-state control unit — had failed and required full replacement at approximately $3,400. The vehicle is well under 50,000 miles. This failure appears consistent with the same car-computer power-component shorting failure for which Tesla issued a recall on its newer model-year vehicles. The vehicle and the failed component are available for inspection on request. The vehicle has not been inspected by police or insurance.

-I was driving down into an indoor basement level garage very slowly as there is a steep incline. It descends from street level to basement level within a distance of approximately 30 to 35 ft. At the end of the incline is a sharp curve to the right, followed by a sharp left turn to proceed straight. -While on the incline, the car was slowing to a stop. I gently tapped the accelerator to keep it moving. -The car suddenly lunged forward, rapidly accelerating. The acceleration was not consistent with the amount of accelerator input I applied. -I quickly steered right to avoid hitting the wall, then immediately steered left to avoid hitting parked cars and garage columns. -I braked as hard as possible, pumping it. The brake pedal was stiff. The car kept moving forward despite my efforts and did not stop until I crashed into a parked car at the end of the garage. -My son was in the front passenger seat. We both had seatbelts on. Other than whiplash, there were no major injuries. -There were no warnings, messages or any indications that a problem was present prior to the incident. -There was no one in the parked vehicle that was struck and no other people in the garage. -My cell phone automatically alerted emergency services. The police responded, and I reported the incident.

I purchased the Full Self-Driving (FSD) package for approximately $10,000 on my 2022 Tesla Model 3 Long Range AWD. Tesla marketed FSD as a feature that would enable autonomous driving capabilities including Navigate on Autopilot, Auto Lane Change, Autopark, Summon, and city street driving - all to be delivered via software updates. Tesla has since confirmed my vehicle's HW3 (Hardware 3.0) computer cannot support FSD as advertised. There is no hardware upgrade path. The FSD Transfer Program that allowed owners to move their purchase to a new vehicle ended March 31, 2026 with no accommodation for affected owners. The system regularly fails to perform as advertised. It requires constant human supervision despite being sold as "Full Self-Driving." I have sent three formal complaints to Tesla's executive team (May 7, 8, and 12, 2026) with zero response. Tesla's own support chatbot confirmed I am not eligible for FSD transfer.

When using cruise control, there has been 4 times that the car has engaged in “phantom braking”. We were driving on highways going the posted speed limit of 70 mph, when the car suddenly slammed on the brakes and decelerated to less than 30 mph – all four times we were thankfully not being closely followed by another vehicle or we would’ve had a devastating crash. One time when we looked at the screen, it showed the outline of a person in the median but there was no person/ other obstacle at all. We have reported this to the Tesla service center and they said it is a known issue, and what we have to do is just click the scroll bar and report it so that the engineers can mark the timestamp and collect data so that some patches can be sent over the air/ uploaded at a future date. Nothing has been done to fix the issue and we feel dismissed about our concerns about our family’s safety, and the safety of drivers around us.

On February 5, 2026, at 12:21 PM PST, my 2026 Tesla Model 3 (Hardware 4) collided with a 5-inch wooden pillar while using the Autopark feature. The system failed to provide any audio chimes or visual warnings prior to the impact. I officially requested the driving logs from Tesla to investigate the cause of this failure, but Tesla refused to provide any logs or data. Furthermore, Tesla Service Center technicians insisted there were no hardware defects, effectively confirming that the collision was caused by a software/algorithmic failure of the Tesla Vision system to detect a stationary vertical object. Despite Tesla’s claim that the vehicle is "operating as designed," the system's inability to recognize common infrastructure and its failure to warn the driver constitutes a severe safety risk. This defect must be investigated to ensure the safety of the Hardware 4 autonomous platform, as the lack of proximity alerts could lead to far more serious accidents involving pedestrians or other obstacles.

I am writing to file a formal safety complaint against Tesla, Inc. regarding a software update failure that has disabled legally required vehicle functions on my Tesla for over two years. For more than 26 months, my vehicle has been unable to complete a software update through Tesla's over-the-air (OTA) update system. As a direct result of this failure, my vehicle's display screen does not function for the first several minutes of every drive. This prevents me from accessing my speedometer, turn signals, and other legally required safety features, rendering the vehicle non-compliant with federal and state motor vehicle safety standards during this period. I have made multiple documented attempts to resolve this issue through Tesla's customer support channels over the course of these two years. Despite these efforts, the issue remains unresolved. Tesla has now informed me that resolving the defect will require a paid service visit — despite the fact that the failure originates within Tesla's own software update infrastructure, not from any action or negligence on my part. I am requesting that NHTSA investigate this matter on the following grounds: 1. The software defect disables legally required vehicle safety functions, including the speedometer, turn signals, hazard lights, and more. 2. The defect has persisted for over two years despite repeated good-faith efforts by the vehicle owner to resolve it through the manufacturer. 3. Tesla's OTA update system is manufacturer-controlled infrastructure. A failure within that system constitutes a manufacturer defect, not a maintenance issue. 4. Tesla is requiring payment from the consumer to correct a defect in their own systems. I believe this issue may not be isolated to my vehicle and could affect other Tesla owners experiencing similar OTA update failures. I respectfully request that NHTSA investigate whether a broader safety defect exists and whether a recall or corrective action is warranted.
