Tesla, Inc. (Tesla) is recalling certain 2017-2023 Model 3 and 2020-2023 Model Y vehicles. The headlight low beams may be too bright and exceed the maximum light output. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 108, "Lamps, Reflective Devices, and Associated Equipment."

Risk: Headlight low beams that are too bright can reduce visibility for oncoming drivers, increasing the risk of a crash.

STEERING:AUTOMATED/ADAPTIVE STEERING

Tesla, Inc. (Tesla) is recalling certain 2016-2023 Model S, Model X, 2017-2023 Model 3, and 2020-2023 Model Y vehicles equipped with Full Self-Driving Beta (FSD Beta) software or pending installation.  The FSD Beta system may allow the vehicle to act unsafe around intersections, such as traveling straight through an intersection while in a turn-only lane, entering a stop sign-controlled intersection without coming to a complete stop, or proceeding into an intersection during a steady yellow traffic signal without due caution. In addition, the system may respond insufficiently to changes in posted speed limits or not adequately account for the driver's adjustment of the vehicle's speed to exceed posted speed limits.

Risk: FSD Beta software that allows a vehicle to exceed speed limits or travel through intersections in an unlawful or unpredictable manner increases the risk of a crash.

Issue: A previous over-the-air (OTA) software update could cause the vehicle not to display the correct visual warning indicators when there are problems with the brake system or when the parking brake is applied. As a result, the BRAKE, PARK, and antilock brake system (ABS) visual indicators would display as text instead of symbols. Safety Risk: While there is a low risk to safety, Canadian regulations require visual warnings to use common symbols, instead of words, to avoid driver confusion. Corrective Actions: Tesla will notify affected owners by email. Tesla has released an over-the-air software update to fix this problem. There is no further action required if your vehicle has software release 2024.3.25, 2024.14.5, or a release later than 2024.14.5.

Corrective Actions: Tesla will notify owners by email and supply an over-the-air software update to improve driver monitoring and alerts while using the Autosteer feature. Issue: On certain vehicles, the bolts that attach the front suspension lateral links to the sub-frame could loosen over time and the lateral links could separate from the sub-frame. If this happens, there could be sudden changes in wheel alignment and a loss of vehicle stability. Note: This is an expansion of Transport Canada recall 2021-649. Safety Risk: A loss of vehicle stability could cause a loss of control and increase the risk of a crash. Corrective Actions: Tesla will notify owners by e-mail and arrange for an inspection the front lateral link bolts. Tesla Service will tighten or replace the bolts, as necessary.

This could increase the risk of a crash if the driver doesn't intervene. Corrective Actions: Tesla will notify owners by email and supply an over-the-air software update to improve how the FSD Beta feature maneuvers the vehicle in certain traffic situations. Issue: On certain vehicles, the bolt attaching the left-rear seat belt buckle and center-rear seat belt anchorage may not have been reassembled correctly after previous service repair. As a result, the seat belts may not work properly in a crash. Safety Risk: A seat belt that moves or detaches in a crash could create an increased risk of injury. Corrective Actions: Tesla will notify owners by email and advise you to contact Tesla Service to arrange for an inspection of the left-rear seat belt buckle and center-rear seat belt anchorage bolt. The bolt will be re-tightened, if necessary.

Issue: The window automatic reversal system may not react correctly after detecting an obstruction. This could cause the window to close with too much force before it stops and retracts. Also, the window may not retract enough after detecting an obstruction. As a result, the power windows do not pass certain tests under Canadian safety regulations. Safety Risk: A window that closes with excessive force could pinch a person and create a risk of injury. Corrective Actions: Tesla will email a notice to owners and provide an over-the-air firmware update to fix this condition.

Adaptive cruise control was on, i was driving on the highway, traffic was speeding up and slowing down, and the tesla was braking accordingly. I was paying attention to the road and noticed, all of a sudden, that the tesla was not braking as traffic in front of us slowed. I immediately slammed my foot on the brake as hard as i could, yet there wasn't enough time or distance, and my car collided with the one in front of me. I wasn't injured, but there's a chance someone in the other car was. The emergency brake did not kick in whatsoever, and the adaptive cruise control completely failed, leading to an accident.

The door handles have a defect. The chrome peels off unexpectedly, outside of normal wear and tear, and may cause a deep cut to a person’s finger. It cut my son’s finger, causing bleeding. After researching, I found online that that this has happened to many other people.

Hello, I have a Tesla model 3 that I bought back in 2018 . On [XXX] when I sliced my finger while opening the door, because the chrome coating on the door handle started peeling. The cut was very deep ( see attached pictures). I contacted Tesla for resolution and they said that the car is not under warranty and I have to pay $340 for each handle to replace them. Apparently the peeling chrome happened to many other Tesla 3 owners and the company is aware of it and they told me that it might be caused by the car washing detergent (?!!) I think it was a defective design and the peeling chrome coating is a major health hazard. The child cand slice their fingers and if person has a bleeding issues and cut themselves while on the road it can be bad. I think it should be treated as a recall for bad design and should be fixed for free. I lost couple of days of work since I work in the Operating room and my finger was severely cut. The contact owns a 2018 Tesla Model 3. The contact stated that while driving at approximately 30 MPH through an intersection, another vehicle t-boned the driver's side from the driver's side front bumper to the rear passenger door. The contact stated that the air bags did not deploy as intended. Medical attention was needed after the contact sustained lower back, left shoulder, neck, tailbone, and internal bleeding injuries were sustained. A police report was filed. The vehicle was taken to a collision center, where it was currently pending an inspection. The manufacturer was not notified of the failure. The failure mileage was approximately 88,894.

SEAT BELTS

My 2018 Tesla Model 3 displayed alert RCM_a056, “Front Passenger Safety Restraint System Fault.” The warning may affect correct detection of a passenger and operation of the passenger restraint/airbag system. Tesla quoted approximately $705 but has not identified the failed component or explained whether the condition is related to known front-seat wiring or occupant-classification failures. Tesla Repair Bulletin SB-21-17-005 recognizes that electrical harnesses in 2017–2019 Model 3 front seats can be damaged through repeated seat movement and trigger restraint-system alerts, although that bulletin specifically lists codes RCM_a021 and RCM_a022. I am reporting RCM_a056 because it affects the same general front-passenger safety-restraint system and appears to be reported by other early Model 3 owners. I am concerned that an occupant may not be properly classified or protected in a collision.

Tesla replaced the front subframe, both front lower lateral links, both front lower compliance links, the left front halfshaft, and performed an alignment. Total cost was $6,328.96. The failed parts were replaced by Tesla; I do not know whether they remain available for inspection. This failure created a serious risk of loss of vehicle control at highway speed and could have endangered me and surrounding traffic. The problem was confirmed and inspected by Tesla Service. There was no collision or other discrete impact event that I recognized as causing the failure. On XXX, at approximately 101,728 miles, my vehicle experienced a severe suspension issue. The vehicle began producing a loud, frequent squeaking and rattling noise originating from the front passenger-side wheel area while driving under 40 mph. Due to the clear mechanical risk and safety concerns associated with a suspension failure, I brought the vehicle to the Tesla Service Center in [XXX]. Tesla technicians diagnosed the problem and confirmed a failure in the front suspension.
