FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

The Pre-Collision System (PCS) failed to engage the automatic emergency braking during a stop-and-go traffic, thereby, resulting in rear-ending the vehicle ahead of our 2018 Highlander Limited vehicle. Our Highlander was at a complete stop, then when the traffic moved slowly forward, we too started moving forward but didn't realize the vehicle ahead of us had suddenly stopped again. Our Highlander alerted us with high-pitched beep-beep-beep sound, but it didn't stop before colliding with the rear of the vehicle ahead of us. The speed couldn't have been more than 10 MPH since it started out from a complete stop and the distance traveled before the collision was about 40 feet. There were no warning lamps, messages or other symptoms of the problem prior to the collision. It always beeped, like it should, when there were nearby obstacles, pedestrians, and when we got closer to the end of our garage parking space, etc. PCS sensitivity was set at the highest level (3 bars).

While driving one day i got the ABS light and brake light on on my car screen and the small display showed "Barke Malfunction" when i did the scan it is C1391 Code which is for ABS Accutator issue. This showed be fixed by Toyota dealership for free. this is design issue seems like based on the google search with Toyota Hybrid vehicles

I was backing up the vechial accelerated the steering wheel was turning rapid & a grinding noise causing me too hit another vechial about a month before my husband was driving & the vechial accelerated going toward also we had had some issues with back camera not doing a warning signal with a object behind vechial when the accident happened my vechial has braking system that stops you if something is behind you this time it didn’t I called the dealership when accident happed which was May 21 2026 & reported it they said they had accelerations before & advised me to call Toyota engadgment which I did they did a diagnostic test June 5 & to this day I’ve never heard back from them

LANE DEPARTURE: BLIND SPOT DETECTION

Four days after I took delivery of this used vehicle (April 25, 2026), multiple warning lights appeared at once: BRAKE, ABS, PARK, Check Engine, Blind Spot Monitor Unavailable, and RCTA OFF. An independent Toyota dealership diagnosed the vehicle April 27-28, 2026 (approx. 109,000 miles) and found (1) water leaking into the rear hybrid battery area through a sunroof drain "installed incorrectly," causing a Brake System Control Module low-voltage code, starting issues, a headlight only partially functioning on one side, possible air in the brake system, and a musty HVAC odor; and (2) blind spot monitor fault code C1AC2 (Slave Module Horizontal Axis Misalignment) - the blind spot and rear cross-traffic monitoring systems do not function reliably. At a second visit on May 4, 2026, recalibration was unsuccessful; per Toyota procedure the only remaining option is replacing the right-rear blind spot sensor. The technician wrote the cause is a mystery, "whether it be body damage related or electrical short." Toyota service records show the same C1AC2 code was documented during the selling dealer's own pre-sale inspection, and that in December 2024 another Toyota dealership pulled the rear bumper and found the RR blind spot sensor bracket bent, causing incorrect readings - a recurring safety defect that prior calibrations and repairs have not resolved. Separately, federal safety recall 23V865 (occupant classification sensors; the passenger airbag may not deploy as designed) was open and incomplete when I purchased the vehicle and remains incomplete, and Toyota service campaign 25TD01 (rear traction motor cable corrosion) is also outstanding. The defect has been reproduced and confirmed by two Toyota dealerships, reported to the selling dealer in writing, and reported to Toyota's customer-care center. The vehicle is available for inspection upon request. No crash, fire, or injury.

The parking sensor bracket has failed twice now and the sensor keeps falling through. There are no signs of scratches or bumps on the bumper. The dealer said that’s not something they will fix. I have had it fixed once before and now it’s happening again.

The contact owns a 2019 Toyota Highlander. The contact stated that while driving approximately 60 MPH under a local under path the vehicles the braking system suddenly activated causing the vehicle to come to a complete stop. Previously the while driving at the same location the vehicle suddenly reduced speeds. The contact indicated that each time the failure had occurred the cruise control feature was activated. The cause of the failure was not determined. The manufacturer and local dealer were not yet contacted. The failure mileage was 80,000.

The Pre-Collision System with Pedestrian Detection is a good thing, but there's a problem. It stops the car too abruptly on the highway, leading to rear-end collisions. When you release the brake pedal, the car remains stationary. To get it moving, you need to press the accelerator pedal. There have been four instances of the Pre-Collision System with Pedestrian Detection being activated in the last 12 months. Two of them ended in rear-end collisions. In the other two cases, there were no cars behind.
