The contact owns a 2022 Toyota RAV4. The contact stated that while driving through a trench at 5 MPH, the brake pedal was depressed to avoid scraping the bottom of the vehicle however, the vehicle unintendedly accelerated and failed to stop. Additionally, the steering wheel failed to turn as needed, and the vehicle crashed into a picnic table and ran over the chairs. The vehicle came to a stop after crashing into a rock. The contact stated that the bumper and doors were damaged and had scratches from the crash. There was no injury sustained. A Police report was not filed. The dealer was contacted and performed an investigation and determined that the accelerator pedal was depressed during the crash and the vehicle was driving 25 MPH; however, the contact stated that the brake pedal was depressed during the crash. The manufacturer was made aware of the failure. The failure mileage was approximately 62,000.

The contact stated that while pulling into a vertical spot and making a right turn, the vehicle suddenly accelerated, jumped the curb, the sidewalk, and continued driving down the sidewalk and crashed into a building. The brake pedal was depressed but was stuck. The vehicle failed to turn off, and the parking brake failed to engage. There were no warning lights illuminated. The air bags deployed as needed. A Police report was filed. The EMS workers were unable to depress the brake pedal. The undercarriage of the dashboard ripped the skin off both shins. The contact was transported to the trauma hospital, where medical attention was provided for four weeks to close the wounds. A skin graft was performed on both legs. The vehicle was towed to a tow lot. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The vehicle was deemed a total loss by the insurance company. The black box data was retrieved, which showed the acceleration, but no information if the brake pedal was depressed. The approximate failure mileage was 25,000.

Sudden self-acceleration by new RAV4 hybrid with less than 1000 miles on it. I made a hard left at low speed onto an uphill residential street. Saw a car backing down the street 1/2 block ahead so I took my foot off accelerator to slow down. Instead, car self-accelerated while my foot was not pressing any pedal, and shot uphill and over a speed bump, throwing the car off course toward parked cars. I stepped hard on brake, and assisted braking probably also kicked in. Car slammed to a stop. No damage or injuries. I took the car to Toyota dealer service immediately to report and address self-acceleration. Was told Vehicle Control History confirmed an event but not its nature or cause; didn’t find problem. Dealer claimed they reported incident to Toyota national center as UA (Unintended Acceleration), as we requested. Returned the car to me without repair or further diagnostic steps. There was never any follow up by Toyota, local or national. One year later, a similar self-acceleration by this car after a hard left turn at low speed caused life-threatening event and damage. I was told then that Toyota corporate had no record of first UA incident. The 2nd incident is reported separately on this site. I have now realized it’s important to report each incident.

2 incidents of Sudden Unintended Acceleration. No warning lights etc in either. Car damaged but available for inspection. Now encountering difficulties initating Toyota investigation. 1st incident (4/18/25; mileage 967) occurred after a hard left turn. After turn onto uphill street, I took my foot from accelerator to slow and maintain distance from a car backing down hill ahead. Instead, car self-accelerated and shot uphill, over speed bump with increasing speed. Did not respond to braking until it was close to hitting a parked car. Stopped with a slam; probably brake assistance engaged. No damage or injury, but I was shaken. Took car immediately to Toyota dealer to inspect. Was told RoB confirmed an event but not its nature or cause; identified nothing wrong. They hypothesized cruise control might have been triggered accidentally (no evidence, not plausible). Claimed they submitted report to Toyota national center. After 2nd incident, Toyota national said they had no record of 1st event. Their standard safety inspection protocols were not followed. Returned car to me without repair or follow up. 2nd incident ensued. 2nd self-acceleration event (2026) also occurred after hard left turn to make U turn at low speed, braking while completing turn. When I took my foot off brake, before I could press it on accelerator, car shot diagonally across street. Jumped curb and traveled at considerable speed 100' up landscaped slope. Destroyed 6 large shrubs and 3 trees 9' tall. Did not stop despite constant driver braking. Forward collision prevention system did not engage to prevent uprooting trees. Car stopped only after I turned steering wheel sharply to left to avoid hitting a full-size tree trunk. Then car responded to braking, acceleration ended, car stopped next to tree without impact. Terrifying life-threatening event but only damaged landscape and car. Police came, assessed incident, determined not a police matter, did not file police report.

COMPONENT: Electronic Throttle Control System (ETCS) / accelerator pedal. Vehicle is available for inspection at Autocentro Toyota, San Juan, PR, where it has been retained since 2/28/2026. INCIDENT: On 2/28/2026 at 7:45 AM, while entering a private driveway at under 5 MPH to park, my 2025 Toyota RAV4 Hybrid SE (VIN [XXX] , 3,164 miles) suddenly and involuntarily accelerated without driver input. I categorically affirm I did NOT press the accelerator. The vehicle struck a metal gate, causing frontal damage. Airbags did not deploy. OEM floor mat was properly secured; no aftermarket parts installed. SAFETY RISK: Sudden unintended acceleration at low-speed parking maneuver. Had a pedestrian, child, or another vehicle been in the path, this would have caused serious injury or death. The defect is unpredictable and the driver cannot anticipate or override it. INSPECTION: Vehicle inspected 3/20/2026 by Engineering Analysis Associates (EAA), a firm contracted and paid by Toyota — NOT independent. EAA downloaded electronic data and concluded the accelerator was pressed 1 second before impact. I dispute this finding: I did not press the pedal. No independent inspection has been allowed. Full EAA report was never provided to me. On 4/23/2026, Toyota Motor North America (Ms. Delve L., Legal Claims Administrator) declined assistance based solely on EAA's data. WARNING SIGNS: UNKNOWN. No warning lamps, messages, or prior symptoms appeared before the incident. The vehicle had functioned normally since purchase on 4/25/2025. ADDITIONAL: Vehicle retained by dealer 73+ days without repair or substitute vehicle. Consumer complaint filed with Puerto Rico DACO (Case XXX). We were on a funeral procession when suddenly the car accelerated at a very high speed and hit the end of guard rail head on. My wife on the passenger side and two people at the back were all taken to the emergency room. I got pinned on the steering column and taken out via the roof. I had severe abrasions on both legs, wife has 3 broken ribs,the 2 passengers suffered whiplash and complains of chest pains. Car was purchased 11/3/2025 and I took it for a recall fix about a month ago at Burien Toyota dealership where we bought it. Incident # 2026-55959 Seattle police. At present our car insurance is investigating and our claim # is 082 0356939 Allstate.

On 12/24/25 I was pulling into a parking space, took foot off accelerator to brake and vehicle suddenly accelerated and engine revved. I kept pushing the brake, but the vehicle kept lurching forward and slammed into car parked in front of me before finally stopping. My vehicle was going so hard forward that it pushed the car I hit 2 to 3 feet out of it's parking space and even punctured the radiator. It was not raining, the floor mat was not in the way, I did not have cruise control engaged. On 12/10/25 I took my car to Toyota dealership because I had a slow leak in one tire, which had to be replaced. At that time, the service department also updated parking assist software (could that be the problem?). In answer to above questions: * brakes and accelerator malfunctioned and it is available for inspection upon request. * no one was in the car I hit. I was not injured. * the problem has not been reproduced to my knowledge or confirmed. * the vehicle has been inspected by a third-party that Toyota engaged and also Erie Insurance. A police report is available. * there was no warning lamps, messages or other symptoms of the problem prior to the failure on 12/24/25.

Driving up a slope of about 20-30 degrees gradient of a straight lane going into a private parking lot. Braked and decided to make a 3-point turn to get back out. There was no traffic in the lane. Put gear on reverse while on the slope with the vehicle pointing in the up direction of the slope. Vehicle suddenly surge backwards, accelerating. Vehicle sped down the slope and crossed a street. Could not stop. Vehicle was stopped only after placing gear into park from reverse, and after jumping curb across the street. Fortunately, there were no cross traffic in either direction on the street, otherwise vehicle could have been T-boned. Tire wall on the rear passenger side busted. AAA was called to replace tire with spare. There were no warnings on the dashboard or any symptoms anywhere else, although the vehicle had all the safety features.. The next morning, vehicle was taken to Toyota of Carlsbad. Left there for diagnosis and tires change. Vehicle is left with Toyota of Carlsbad until this issue is answered/solved. Told by service manager of Toyota of Carlsbad to call Toyota USA at [XXX] to report incident by customer, and request Toyota USA to send expert from Toyota to diagnose issue. I have had multiple instances where the vehicle has failed to accelerate when making a left hand turn. Each time this has occurred, my car has rolled into the intersection from a stop at a small decline, but the gas seems to do nothing. My car silently rolls forward and then I have no power or gas and am trapped with no acceleration. I’m worried this is going to get me into an accident and seriously injured. It does not happen super often, but enough that I know there is a problem. Dealer has not been able to see any codes, and we have not been able to recreate the issue in front of the service advisors.

I am the driver's husband and the driver was the only one in the car. I assist her to file this report The accident happened around 6 pm on 12.28.2025 My wife was involved in a single car accident in a parking lot. She made a left turn at the intersection of 192nd ave and 3rd st, and then entered into the parking lot from the north entrance. The car hit the curb as she entered the parking lot. Then the car (RAV4) started accelerating by itself, and was out of control. She cannot stop the car. To avoid going straight and hit the shops at the other end of the parking lot, she turned the wheel to one side. She hit a light post and then hit something else. The vehicle flipped over and landed on its roof. The vehicle was totaled. She was trapped inside the car unable to free herself until the ambulance, tow truck and the police arrived. By God's grace, she did not hit any other car or person and she only suffered minor injury. She was taken to the emergency room at Peace Health, examined and released after 3 hours. If you look at the RAV4, you cannot believe that she only suffered minor injury. Praise the Lord. She is still mentally shaken up by this traumatic event. Looking at the pictures of the damage to the vehicle, the car must have been traveling at a very high speed and there is no way that the driver stepped on the gas pedal intentionally. The insurance Company is Liberty Mutual Claim# [XXX] -XX The vehicle is considered total loss by the insurance company. EDR was inspected but data in the EDR was not collected. I suspect this incident was not caused by the driver, The data in the EDR will tell the true story. I asked Liberty Mutual to retrieve the data from the EDR to prove that the gas pedal was not depressed by the driver. If the car accelerated without the driver stepping on the gas pedal, then the vehicle has safety issues. I am waiting for Liberty Mutual to retrieve the data. My wife was going at about 10 - 15 mph coming up about 2 car lengths to a small curve in a residential area when all of a sudden she heard a pop or snap and the Rav4 went to a super high speed by itself. The acceleration could not be stopped even though she had her feet on the brakes. She had both hands on the steering wheel to avoid hitting a house and ended up going through a wall in the yard. There were no warning sounds and no signals indicating a problem. it appeared that the display was blank. There was no pre-collision warning or braking except as my wife was pushing on the brakes. She also hit the right side into side wall. No air bags deployed even though Windshield and all door glass on sides shattered. My wife the driver was okay and not hurt, and the grandchild in the car seat behind my wife was fine also. Neither had injuries. There were checked by emt. The police looked but all would not try to get in except to remove my granddaughter. My wife was removed also because she could not get out the door was jammed against shed and rock, and handles broken off This happened after the recall work was done by toyota dealer on 12/04/2025. There were no problems prior to the recall. Mud, water, or road salt could potentially enter the HV Floor Underwire connector housing and could cause corrosion over time. Excessive corrosion could cause AM Radio Static during certain driving cycles, or the vehicle may not start. Status Remedy Available Remedy Any authorized Toyota dealer will apply a wire harness protector seal to the HV Floor Under Wire Connector. Toyota corporate engagement is looking into this.

The front camera fails which does not allow for the cruise control to operate. The speed odometer does not specify the correct speed when driving the car. The feature of telling you how fast you are supposed to go does not operate correctly specifying the wrong speed you are supposed to go. The tires have already been replaced because the metal was coming out of the tires causing the car to not operate correctly there was wobbling in the tires causing the car to feel like the tires were about to come off the tires. The transmission not shifting properly in eco mode.

While driving the vehicle on an uphill stretch of highway with cruise control set at approximately 65 mph, the vehicle suddenly slowed down without warning. A distinct clunk was heard at the same moment. The cruise control system attempted to restore speed, and the engine became noticeably louder, but the vehicle struggled to return to the set speed. The driver tapped the brake to disengage cruise control and manually pressed the accelerator to resume normal speed. The trip was completed without additional events. After parking, the check engine light illuminated and the cruise control system became disabled on all subsequent drives. No warning indicators were present at the time of the initial failure, and there were no vehicles ahead that would have triggered the collision-avoidance system. Two days later, the vehicle became an intermittent no-start and eventually failed to start entirely. The vehicle was towed to a dealership for inspection. Technicians confirmed an electrical malfunction and reported that the multiplex ECU was likely experiencing an internal short. A parasitic draw test showed an abnormally high current draw of approximately 113 mA, which disappeared when the dome-light fuse and later the multiplex ECU were disconnected. The failure is intermittent and appears to involve an internal electrical short within the multiplex ECU. The sudden loss of cruise control function, abrupt deceleration, and subsequent intermittent electrical behavior created a safety risk, as the electrical malfunction occurred without warning while the vehicle was in motion. The issue was confirmed by a dealership service department.
