2024 Toyota RAV4 California, MD Crash 4 injured

Airbags, seats, and headrest all failed. State Farm insurance currently has the vehicle. I do not know if the people has been inspected by anybody. The vehicle had no warning lamps message or other symptoms of problems or failure before the collision. I was traveling in the far left lane when traffic came to abrupt stop and I was able to stop 10 to 15 ft from the vehicle in front of me. Two vehicles in my line behind me swerved out of the way. Then a semi truck with a trailer rear-ended me. The front end of the semi truck struck the rear end of my vehicle getting lodged inside my vehicle. My car was then pushed into the car in front of me and dragged 100 to 200 yards down The highway by the semi. The seat belts did not lock. The airbags did not deploy. Both front seat frames were bent and ripped apart. The rear seat that's became detached. The headrest for the front seats flew off. All four seats were occupied by adult males all weighing around 190 pounds or above. All four occupants were wearing seat belts. I the driver was knocked unconscious and received a concussion plus facial damage/injuries. Rear passenger side occupant received a broken jaw and a skull fracture. The other two passengers were also injured. All four passengers of the vehicle were transport to emergency room via ambulance.

2017 Toyota RAV4 New York, NY

This vehicle has twice had the same issue where the sunroof drain lines become clogged or dislocated and the water fills the liner in the front driver and passenger seats resulting in flooding in the compartment and mold damage throughout the interior. the mole spore are transmitted throughout the interior. The water damaged the airbags and the electrical harness in the dash of the car resulting in the dashboard electrical system shutting off and restarting spontaneously while driving. The leaks were repairs and the interior replaces in october 2023 and the same incident has recurred in august 2026. The claims adjuster recommends that the entire wiring harness, airbags, and carpets, seats and liners be replaced for health and safety of passengers.

2014 Toyota RAV4 Lakeland, FL Crash 1 injured

Request for Investigation – 2014 Toyota RAV4 Airbag Non-Deployment On [XXX], at approximately [XXX]., the vehicle was involved in a traffic collision in Lakeland, Florida. The vehicle sustained a significant impact to the driver’s-side front area. Despite the nature and location of the collision, the Supplemental Restraint System (SRS) airbags did not deploy. Law enforcement responded to the scene and completed Police Report. Emergency medical services also responded, and the driver was transported from the scene by ambulance due to injuries sustained in the collision. The primary concern is whether the airbag system, including the crash sensors and SRS components, functioned as designed during this crash. Based on the impact, I am seeking an investigation into whether the non-deployment of the airbags was the result of a defect or malfunction. I respectfully request that NHTSA review this incident, document it within its vehicle safety database, and determine whether similar complaints involving 2014 Toyota RAV4 vehicles exist that may indicate a broader safety concern warranting further investigation. Supporting documentation, including photographs of the vehicle damage, the police report, and any other relevant records, can be provided upon request. While I am driving my Toyota RAV4, the car will suddenly shut off and I can restart it and keep driving as the month is going on. The incidents are happening more often and now the dashboard is going black. I’ve had a couple of warning signs on the dashboard. I brought the car to Toyota and Pep Boys. Both of them checked the electrical systems and everything looks good according to their testing.

2021 Toyota RAV4 Rio Rancho, NM

I have been unable to get my vehicle inspected and repaired by a dealer despite months of waiting and asking two different Toyota dealers to order the necessary part or parts. This recall has been in place for years. For several years I was told it was only being done in certain states and that New Mexico would be last on list due to climate. Probably six months ago the message changed from Toyota and it was now ok in every state. Only problem is still no parts according to dealer. Last contact was on June 2, 2026 with the second dealer to order part. No call back and no contact. First effort was in Santa Fe NM where car was purchased new in 2021at Toyota dealer. Last effort over a month ago was at Albuquerque Larry Miller Toyota. So safety recall for faulty air bag sensor on my wife's side of car has been in place about 900 days without parts to fix a potentially deadly problem. VERY DANGEROUS

We were part of a 4 car collision on the freeway. A Ford Expedition that was slammed into then slammed into our RAV. The airbags never deployed and an error code came up saying fail for airbags.

2021 Toyota RAV4 at 62,000 miles Houston, TX Crash 1 injured

The contact owns a 2021 Toyota RAV4. The contact was traveling at approximately 50 MPH directly behind a van in the same lane of traffic. The van suddenly made an emergency avoidance maneuver to bypass a stationary vehicle completely stopped in the lane. Despite the attempt, the van rear-ended the stopped vehicle. The contact immediately initiated an evasive maneuver to avoid the sudden collision occurring ahead. However, due to proximity and immediate lane obstruction, the contact was unable to avoid an impact and struck the vehicle. The severe front-end impact resulted in catastrophic damage to the contact's vehicle before it came to a complete stop. The airbags failed to deploy, and no warning lights were illuminated. The vehicle was towed to a repair shop. A police report was filed. The driver and her pet were the only occupants of the vehicle, and the contact was the only injured passenger. The contact sustained moderate chest pain, but no medical assistance was received. A police report was filed. The dealer was not notified of the failure. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was 62,000.

My airbag light came on and I received a letter saying there was a recall but unfortunately I threw out the letter and when I called Toyota said there was a recall but it was fixed in 2012 when I didn’t own the vehicle and it seems like it didn’t last long because my airbags are not working so this will be the last time I buy a Toyota!! 2020 Toyota RAV4 at 110,000 miles Upland, CA

2021 Toyota RAV4 Thornton, CO Crash

I was in a car accident that ended up being a total loss for my vehicle. I was hit head on to the front drivers side and none of my airbags went off. I am aware there was a recall for the passenger airbags, and was actually scheduled to get the repair the following week, but the drivers side airbags are not included in the recall. After the collision I received an error message on my display that advised there was an airbag malfunction and to contact my toyota dealer. Thankfully I did not sustain any known injuries, however I am pregnant and none of my airbags went off appropriately.

2021 Toyota RAV4 San Jose, CA Crash 1 injured

The contact owns a 2021 Toyota Rav4. The contact stated that while driving on the freeway at approximately 35 MPH, another vehicle approaching from the opposite direction attempted to make a left turn and collided with the contact's vehicle. The point of impact was the front driver's side wheel well and side bumper area, with additional damage noted on the front passenger side bumper. No warning lights illuminated. The other vehicle was driving at an undisclosed speed and sustained damage to its front-end frame. There was no property damage reported. Both vehicles came to a stop in the roadway at the intersection of [XXX] and [XXX] in San Jose, California. The contact's vehicle remained drivable and was driven to the residence; towing was not required. The airbags did not deploy. The driver sustained injuries, including a fractured vertebra and a fractured sternum, and received medical treatment. The driver believes that the injuries may have been prevented if the airbags had deployed. The vehicle was not diagnosed or repaired. The manufacturer was not made aware of the issue. 2018 Toyota RAV4 Virginia Beach, VA Crash 2 injured

3/4/2026 5:10 pm Vehicle (2018 RAV4) was sitting in backed up traffic waiting to exist highway and was hit from behind by a sedan travelling at 65+ mph, pushed into a pickup truck in front, then deflected left into a guardrail. SO, three separate impacts with extensive damage to the entire rear of the car, the front end, and right front quarter. Two occupants in the car, driver and front passenger, no airbags deployed. Driver's head contacted the steerring wheel (back eye, cut lip, swelling on forehead and around eye, headache), and legs and arms of driver and passenger contacted other portions of the interior of the vehicle and experienced severe whiplash, but no broken bones per initial ER xrays and scans. Vehicle towed and is currently available for inspection.

2022 Toyota RAV4 Muskegon, MI Crash

We were traveling across country from Michigan to Arizona. We were 50 miles outside Tucson, AZ on [XXX]. Traveling 80 mph in a 75 mph zone, in passing lane and felt like car exploded and all airbags deployed and we came to a quick stop. No major injuries to either of us. One vehicle incident, did not collide with anything or hit any debris. It was a clear day, road conditions good, no noises, lights etc. My husband was able to keep from rolling and we came to a stop with many semi's etc flying by, very scary. When we were able to safely get out and call 911, there was significant drivers side front end damage around tire area and inside near axle. Looked like something broke inside car, sheered off bottom of tire. Police arrived and had to push vehicle on to shoulder of road. He assessed scene and thought it had to be a mechanicla failure as there was no other vehicle or debris on site. We did contact our insurance company and subsequently it was totaled. We also have Toyota investigating what the cause was if it was a faulty part or mechanical failure. We have not heard back but did receive the "recording of time of crash". 2014 Toyota RAV4 Plano, TX Crash 1 injured

I was in a car accident and the side and steering airbags deployed. While the main steering airbag deflated successfully following the accident, the driver's side door side airbag never deflated. Once I was trying to get out of the car and had the door open, this meant it was much harder and took longer to get out as I had to slide out from underneath it. Because I wasn't able to get out quickly, I remained in a car in direct line of traffic with my driver's side door where it would have been hit first putting me in more danger.

2024 Toyota RAV4 at 10,000 miles Porterville, CA Crash 2 injured

The contact's daughter owns a 2024 Toyota Rav4. The contact stated that while his daughter was driving at approximately 35 MPH, she had crossed an intersection when she was crashed into by another vehicle making a U-turn from a cross street. The other vehicle's front end crashed into the driver's side tire and front door. The contact stated that the force of the crash had fractured the left front axle and pushed the tire into the engine compartment. The contact stated that the frame was bent and the driver's door was difficult to open. The contact stated that the side air bags had not deployed and that the seat belts on the driver's side and the passenger side could not be unbuckled. The contact stated that his daughter had sustained injuries to her left shoulder, left arm, and leg as well as head and neck injuries. The contact stated that his wife had sustained injuries to her right ribs, right arm and shoulder, and lower back injuries. The contact stated that his wife and daughter were taken by ambulance to the hospital. The contact stated that the police were on scene and wrote a report, but he did not have the report number. The contact stated that both vehicles were towed away from the scene of the crash. The contact stated that their vehicle was repaired. The manufacturer was not informed of the failure. The failure mileage was approximately 10,000.
