The contact owns a 2015 Toyota Yaris. The contact stated while driving at an undisclosed speed, the seat belt light flashed on the instrument panel, indicating that the seat belt was not being worn. The air bag warning light was illuminated. The vehicle was taken to the dealer, who diagnosed a failure with the wiring harness. The vehicle was repaired, but the failure reoccurred. Vehicle was rear-ended on November 4, 2022. The driver was wearing a seatbelt but the driver airbag did not deploy after being rear-ended. After the accident, the dashboard airbag indicator warning light turned on. The vehicle did not show any airbag issues or warnings prior to the accident. The emergency parking brake does not properly engage after the accident.

I was driving a 2012 yaris when I was in an accident that totaled the vehicles. I was hospitalized for 3 weeks which I believe the airbags had played a major role in the seriousness of the injuries I sustained but not sure how to prove that they failed to protect me and possible they made things worse with the scar across my forehead from being sliced open needing numerous staples as well as shattered pelvis. Is there any way by looking at the pictures can you tell me if the air bags did their job

2008 Toyota Yaris Indianapolis, IN

My fiancee bought her 2008 Toyota Yaris on about the 26 Apr 2021. Right off she had brake failure pretty quickly after buying the car. Brake pads were suspected of failing. When the front tires were removed the calipers had built up thick rust on inside surface where the pads sit on both front calipers. The pads were pushed into the rotors heavily causing the driver side rotor to be damaged beyond acceptable limits. The right side rotor was turned successfully with enough rotor minimum thickness remaining. So the rust caused me to replace 1 rotor, both calipers and pads on front and rear brakes. The rust built up on the calipers threaded input and output brake fluid lines threaded attach points. This could have cause a fluid leak that could of been very dangerous leading to injury or death of occupants. I have not reported this to The Toyota dealership or anyone else. I didn't know I could report dangerous items that could be set for a safety recall by the NHTSA. As for the airbags, the driver airbag light stays on and passenger light comes on when 10 pounds of weight is sat in passenger seat. The belt is not latched. My driver seat belt is always latched when I drive. I weigh 130 but the light remains on. So, will the driver air bag inflator explode or fail due to the warning light illuminatimg. I was unaware of a recall for this and 3 others have not been repaired yet by previous owners. The third item is backseat seatbelt female receptical will not latch to the male end on 2 of the 3 seating positions. This is a real safety issue when we place children in the backseat. We can only take 1 child at each drive, so even adults riding in the backseat will be not buckled up, which is un-law and very dangerous causing severe injuries and even death in an accidental collision. I will take my Fiancee's 2008 Yaris to a Toyota dealership for the 4 recalls noted today by VIN,7/29/2021. Thanks, Richard Peavler

THE CONTACT STATED THAT WHILE HER DAUGHTER WAS DRIVING, AND STOPPED AT A STOP SIGN, THE VEHICLE WAS REAR-ENDED BY THE VEHICLE BEHIND HER WHICH CAUSED HER VEHICLE TO REAR-END THE VEHICLE AHEAD OF HER. I WAS INVOLVED IN A HEAD ON COLLISION INTO A UTILITY POLE ON A TWO LANE ROADWAY. I WAS FOUND UNCONSCIOUS IN MY CAR. I HAVE NO RECOLLECTION OF THE COLLISION ITSELF BUT NO OTHER CAR WAS FOUND TO BE INVOLVED. THE DRIVERS SIDE OF THE VEHICLE EXPERIENCED THE MOST DAMAGE. I HAD MY SEATS BELT ON BUT NOT A SINGLE AIRBAG IN THE ENTIRE VEHICLE DEPLOYED, DESPITE THE DISABLING DAMAGE THAT OCCURRED TO THE VEHICLE. I HAVE REACHED OUT TO TOYOTA MULTIPLE TIMES SINCE THE INCIDENT OCCURRED YET ABOUT THE CONCERN OF NO AIRBAGS DEPLOYING YET WAS NEVER NOTIFIED THAT THIS WAS EVEN AN OPTION TO REPORT THE FAILURE OF THE AIRBAG TO DEPLOY IN AN ACCIDENT, ESPECIALLY WHERE THE HEAD ON COLLISION TOTALED THE CAR. THE PICTURES OF THE INCIDENT ATTACHED TO THIS REPORT WERE OBTAINED BY MY FATHER IN THE DAYS FOLLOWING THE ACCIDENT AND INCLUDE THE ROAD OF THE ACCIDENT AND THE CAR. THE STEERING WHEEL IS VISIBLY DEFORMED FROM THE IMPACT MY BODY HAD ON IT DURING THE CRASH. I FRACTURED THE C2 SEGMENT IN MY NECK AND TO THIS DAY STILL SUFFER FROM COMPLICATIONS FROM THE CONCUSSION AND INJURIES OF THIS CRASH ON A DAILY BASIS. I HAVE BEEN THROUGH NEUROLOGICALLY TESTING AND THERAPIES RELATED TO THE INJURIES. THESE INJURIES HAVE IMPACTED MY LIFE GREATLY AND I FEEL AS THOUGH MY CONCERN IS BEING DISREGARDED BY TOYOTA. I STRONGLY FEEL AS THOUGH MY INJURIES WOULD NOT HAVE BEEN NEARLY AS SEVERE IF THE AIRBAG DEPLOYED AND I WOULD REALLY APPRECIATE IT IF YOU COULD LOOK INTO THE CASE. I DO HAVE ADDITIONAL INFORMATION AND PICTURES IF NEEDED.
