From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2010-2016 4Runner, 2003-2006 Tundra, 2003-2013 Corolla, 2009-2010 Corolla Matrix, 2004-2005 RAV4, 2002-2007 Sequoia, 2011-2013 Sienna, 2008-2012 Scion xB, 2008-2009 Lexus IS-F, 2007-2012 Yaris and Lexus ES350, 2010-2017 Lexus GX460, 2002-2010 Lexus SC430, 2006-2012 Lexus IS250 and IS350 and 2010-2015 Lexus IS250C and IS350C vehicles. On January 9, 2018, Toyota Motor Engineering & Manufacturing (Toyota) notified NHTSA that they are recalling 14,585 model year 2010-2012 Yaris vehicles ever registered in the states of Alaska, Arizona, Arkansas, Colorado, Connecticut, Delaware, District of Columbia, Idaho, Illinois, Indiana, Iowa, Kansas, Kentucky, Maine, Maryland, Massachusetts, Michigan, Minnesota, Missouri, Montana, Nebraska, Nevada, New Hampshire, New Jersey, New Mexico, New York, North Carolina, North Dakota, Ohio, Oklahoma, Oregon, Pennsylvania, Rhode Island, South Dakota, Tennessee, Utah, Vermont, Virginia, Washington, West Virginia, Wisconsin, and Wyoming. Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2012-2015 Yaris vehicles manufactured August 31, 2011, to February 9, 2015, and sold or registered in Puerto Rico. The affected vehicles may have been manufactured with a roof headliner that does not provide the proper occupant protection in the event of a crash. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) No. 201, " Occupant Protection in Interior Impact."
Risk: If the vehicles are equipped with a headliner that does not meet the impact requirements, there is an increased risk of occupantl injury in the event of a crash.

STEERING:ELECTRIC POWER ASSIST SYSTEM
Toyota is recalling certain model year 2012 Yaris vehicles manufactured August 21, 2011, through August 31, 2011. A relay in the Power Steering Control Module of the electronic power steering system could experience a short circuit due to insufficient distance between terminals in the relay and moisture contamination of relay coil windings. A short circuit in the relay could disable power steering assistance.
Risk: If the power steering assistance shuts off, the driver may have difficulty steering the vehicle at low speeds, increasing the risk of a crash.

Issue: This recall is for certain block heater kits installed on certain vehicles or sold as accessories at Toyota or Lexus dealerships. On certain vehicles, the engine block heater cable may not have enough heat protection and could be damaged by high temperatures. If this happens, the cable could short circuit while plugged in, and cause the wiring to overheat or melt. Airbag
On certain vehicles, long-term exposure to high absolute humidity and temperature, combined with high temperature cycling, could eventually degrade the propellant contained in the passenger frontal airbag. This could cause the airbag to deploy with more force than normal. As a result, if the vehicle were involved in a collision that warrants airbag deployment, fragments could be propelled toward vehicle occupants, and the airbag assembly could be damaged, preventing proper function. These issues could increase the risk of injury. Correction: Dealers will replace the passenger frontal airbag assembly. <br><br> Note: Canadian climate results in the propellant degrading slowly. The recall is being conducted as a precaution to address future risk, and is expected to replace airbag inflators before their function would be affected. Matrix model vehicles may have had the original front passenger airbag inflator replaced as a “like-for-like” replacement under a previous recall as a temporary remedy while evaluating a permanent remedy. These vehicles will receive the permanent remedy passenger airbag assembly under this recall.

On certain vehicles, a relay within the electric power steering control module could short circuit and fail. The sudden loss of steering power assist would trigger a warning lamp to illuminate and a buzzer to sound. A vehicle experiencing a loss of steering power assist would revert to a manual steering mode which would require greater driver effort, especially at low vehicle speeds. A loss of steering power assist could ultimately result in a crash causing property damage and/or personal injury. Correction: Dealers will replace the electric power steering control module with an updated version.

The contact owned a 2012 Toyota Yaris The contact stated that while slowing down and coming to a stoplight, a second vehicle traveling 54 mph crashed into the rear of the vehicle. During the crash, both the driver-side and passenger-side seat belts malfunctioned and did not secure the occupants. The driver sustained head and torso injuries and the front passenger who was the driver's wife sustained broken ribs, and head and spine injuries. Medical attention was received. The vehicle was destroyed. A police report was taken at the scene and the vehicle was towed away. The local dealer and manufacturer were not notified of the failure. The failure mileage was 76,200.

I was hospitalized for 3 weeks which I believe the airbags had played a major role in the seriousness of the injuries I sustained but not sure how to prove that they failed to protect me and possible they made things worse with the scar across my forehead from being sliced open needing numerous staples as well as shattered pelvis. Is there any way by looking at the pictures can you tell me if the air bags did their job

MY PROBLEM IS WITH THE REAR HATCH GLASS HAS BLOWN OUT TWICE WITH NO UNDERSTANDING HOW IT COULD OF HAPPENED BOTH TIMES WAS WITH THE VEHICLE IN A PARKED SITUATION NO PRESSURE ON THE GLASS THE SECOND TIME MY WIFE AND DOG WERE INSIDE THE CAR WAITING FOR ME IN THE AFTERNOON TO PICK ME UP FROM WORK THE FIRST TIME IT OCCURRED IT WAS JUST PARKED NO ONE IN IT AND IT HAPPENED IN THE MORNING TIME AND JUST SHATTERED MY WIFE SAID WHEN IT BLEW IT WAS LIKE A BOMB GOING OFF THE SOUND WAS VERY LOUD THERE WAS NOTHING AGAINST THE GLASS IN EITHER INCIDENT THIS HAD COST ME 375.00 DOLLARS THE FIRST TIME AND I HAVEN'T REPLACED THE GLASS ON THE SECOND TIME COSTLY REPLACEMENT IS THERE ANY HISTORY OR REPORTS OF THIS OCCURRING TO OTHER TOYOTA HATCHBACKS MINE IS THE 5 DOOR MODEL PLEASE ADVISE THE INCIDENTS HAPPENED IN THE SUMMER MONTHS AND THEY WERE 2 YEARS APART AND IT HAPPENED THE 2ND YEAR I OWNED IT STILL HAVE IT NOW.

TL* THE CONTACT OWNED A 2012 TOYOTA YARIS. THE CONTACT STATED THAT WHILE DRIVING DOWN THE HILL AT 40 MPH, LOST CONTROL AND CRASHED INTO THE TREE IN FRONT. ALL THE AIR BAGS FAILED TO DEPLOY. A POLICE REPORT WAS FILED. THE VEHICLE WAS NOT TAKEN TO THE DEALER. THE MANUFACTURER WAS NOT CONTACTED AND WAS NOT MADE AWARE OF THE FAILURE. THE CONTACT SUSTAINED SEVERAL BRUISES AND SUFFERED FROM CHEST CONGESTION THAT REQUIRED MEDICAL ATTENTION. THE VEHICLE WAS DESTROYED. THE APPROXIMATE FAILURE MILEAGE WAS 80,000.

MOTORWORLD TOYOTA (150 MOTORWORLD DR #4, WILKES-BARRE, PA 18702, (866) 652-1257) STATED THAT THE CONTACT WOULD NEED TO WAIT FOR A SECOND LETTER BECAUSE THE REMEDY AND PARTS WERE NOT AVAILABLE. IN ADDITION, THE RECALL NOTICE STATED THAT A LOANER VEHICLE WOULD BE PROVIDED IN THE INTERIM UNTIL THE REMEDY AND PARTS WERE SUPPLIED TO THE DEALER. REPAIRS WERE ATTEMPTED SEVERAL TIMES OVER A PERIOD OF APPROXIMATELY 10 MONTHS FROM NOV 2016 TO SEPT 2017. SPEED SENSOR: I WAS DRIVING 65 MPH ON THE INTERSTATE IN THE MIDDLE LANE WHEN THE SPEED SENSOR STOPPED WORKING, THE CAR ACTED LIKE IT WAS IN NEUTRAL AND I COULD NOT GET OVER OR CONTINUE DRIVING IT.
