FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
The contact owns a 2027 Chevrolet Bolt EV. The contact stated that while driving approximately 65 MPH with the Adaptive Cruise Control activated, the braking system suddenly activated causing the vehicle to abruptly decelerate when another vehicle passed by. The contact stated that the failure had occurred on two separate occasions while being overtaken by another vehicle on the passenger’s and driver’s sides. The vehicle was taken to the local dealer to be inspected; however, no cause for the failure was found. The manufacturer was notified of the failure. The failure mileage was 900.
NHTSA complaint #11751617 — an unverified report by a vehicle owner.
Vehicle infotainment system on 2023 Chevrolet Bolt completely stopped functioning in May of May of 2026. Screen/radio/bluetooth and associated components had no functionality, including backup camera, safety warning features, etc. Dealership confirmed that the system needed replacement entirely and was non-functional. Vehicle was not inspected by any other parties but it was confirmed by the dealership that this is a "common issue" for Chevrolet vehicles, even newer models. There were no warnings prior to the issue - the system simply did not work one day and never functioned again.
NHTSA complaint #11745689 — an unverified report by a vehicle owner.
Without any warning or physical impact, the central infotainment screen completely blanked out and suffered an irreversible hardware failure. This screen acts as the mandatory display terminal for the vehicle's rear-visibility backup camera, as well as the audio generation module for the vehicle's driver alerts. Because of this component failure, the vehicle is in direct non-compliance with Federal Motor Vehicle Safety Standards (FMVSS): 1.FMVSS 111 (Rear Visibility): The vehicle completely fails to display a rear-view image when shifted into reverse, creating an immediate blind-spot hazard for pedestrians. 2. FMVSS 101 (Controls and Displays): All critical audible safety alerts, pedestrian alerts, and driver warning chimes have been entirely silenced. The manufacturer (General Motors) and the authorized franchise dealership (F.H. Dailey Chevrolet) have acknowledged the failure and have the part on backorder, but refuse to fully cover the safety restoration under warranty or provide a temporary loaner vehicle. This forces the primary driver, a [XXX] senior citizen, to choose between total isolation or operating a hazardous, federally non-compliant vehicle. This appears to be a systemic manufacturing or design defect inherently tied to the internal HMI module architecture on the late-model Chevrolet Bolt EV platform, rather than individual wear and tear. A formal investigation is requested to mandate a safety recall and enforce full manufacturer accountability. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

NHTSA complaint #11740712 — an unverified report by a vehicle owner.
Issue: Total failure of Frontview Camera ADAS sensor, disabling Automatic Emergency Braking (AEB), Front Pedestrian Braking, and Lane Keep Assist. Technical Evidence: GDS2 telemetry confirms a "No Communication" status for the camera hardware. Core safety modules including the Electronic Brake Control (EBCM) and the Inflatable Restraint (Airbag) Sensing Module report active communication failures (DTC U026A/U216A). These safety-critical modules are currently unable to receive the sensor data required for crash avoidance and occupant protection. Forensic Timeline: Vehicle NVRAM reveals a manual "Clear Codes" event occurred at 46,838.3 miles—exactly 0.4 miles after the sale contract was signed. The fault reappeared with a 0 km offset (immediate return) and has been active for 162 ignition cycles, proving the defect existed throughout the dealership's possession. Dealer Response: The selling dealer delivered the vehicle with an unsigned 125-Point Inspection. Despite being presented with GDS2 telemetry proof of the braking and airbag communication blackout, management refused repair, mischaracterizing the disabled emergency braking system as a "luxury feature" and claiming the vehicle is "safe" to drive. Symptoms: "Service Front Camera" warning, intermittent loss of HVAC controls, and gear display (PRNDL) blackout due to systemic serial data bus interference caused by the hardware failure.

Water has infiltrated the lower trunk area and fried the sound amplifier. The result is no sounds can be heard from the speakers. This includes sounds and warnings from the safety systems. Seat belt, ADAS, BLIS, OnStar, Turn Signals, Radio. The rear hatch seals seem to be intact and it is unclear how water is entering this lower area as there is no evidence of water anywhere else in the rear cargo area. A google search reveals many other Bolt owners are experiencing the same issue

This tends to only happen below 74 degrees Fahrenheit and happens excessively between 72 and 74 degrees Fahrenheit. While driving with one pedal engaged (labeled as low on the shifter since they re-used another model's shifter for the first several years of the Bolt EV), the cruise control will randomly disengage and force the vehicle to immediately slam on the brakes. Usually its not a big deal, but when it happens at highway speed as you're just passing a semi or large vehicle and merging back into the lane in front of it, it becomes dangerous very quickly, which has happened a couple of times now. I would venture a guess that its not related to the cruise control directly, but some system that its relying on is forcing it to deactivate the cruise control. The only other issue this vehicle is having that might be related is that the blind spot detection system is not working correctly on the left side. I know there's a TSB for the aluminum Butyl tape becoming damaged or missing, but if that's causing my car to slam on its brakes in front of fully loaded semis, Chevy should probably be persuaded into re-coding how their cruise control gets disengaged. Especially since this blind spot detection system issue seems somewhat common. Either way, something is causing my car to randomly slam on its brakes in the middle of the highway and Chevy says there's nothing they can do about it because its not setting a code. This is definitely a dangerous safety issue that they should be forced into actually diagnosing more than just plugging a computer in and saying: "whelp, nothing to see here." and returning it to the customer with a "we can't do anything, sorry."

The backup camera is the vehicle works intermittently. In fact, due to the defect of the camera, I hit a wall as it shut off and I hit a wall. Upon bringing to the dealer to have it replaced, the dealership admitted that it was a faulty. They replaced with another faulty camera that works worse than the previous replaced one. This is a dangerous situation that will hurt someone.

Putting car into reverse, rear camera starts to glitch sporadically. Very dangerous when this happens in a busy parking lot. This car also features a 360 overhead view and this also glitches at the same time as reverse camera mode.

Electric pole broken onto the street and hit the car, cause severe damage to the front-left-corner on the Chevy Bolt EV 2023. The airbags did not deploy. Safety of the driver and infant was diminished greatly.

The car was parked and when we went out to it, the stereo was crackling very loud static and popping. This continued when the stereo is off. When we reset the stereo, it stopped momentarily and then resumed. Eventually the speakers stopped and just crackled intermittently. After finding a slew of similar incidents online, I opened the rear to find it totally filled with water (see picture, where it is parked on a hill). This filled the subwoofer and drowned the amp, causing global failure of the stereo system. No liquid has been in the rear of my car, and you can tell it is dirty like rainwater. I believe this to be a design flaw due to the other cases reported here [XXX] This doesn't sound like a safety issue, except everything runs through the stereo. Not just music, but the turn signals audio functions, blind spot indicator and forward collision indicators all run through the stereo. Additionally I have lost all hands free functionality for my phone. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

multiple observations of water egress that collects in the rear hatch error, water puddle shorts out audio amplifier and ALL SOUNDS are disabled. No warnings, no pedestrian warning, no turnsignal click, no audible notifications at all . This is noted on many vehicles and GM has been uncertain as to water access point at this time, meanwhile the customer pays for the repair.
