The contact owned a 2019 Chevrolet Silverado HD Crew. The contact stated that while the vehicle was parked outside of the residence, unoccupied, the vehicle engine block heater was plugged in, the vehicle caught of fire under the hood on the passenger side. The fire department was called and extinguished the fire however the vehicle along with another vehicle in the driveway was deemed a total loss by the insurance company. The additional property damage due to the fire was siding, window, cause smoke damage to insulation in the attic. The dealer was not notified of the issue. The manufacturer was notified of the issue. The approximate failure mileage was 156,000.

2016 Chevrolet Silverado 2500 Clarence, NY

EQUIPMENT

Had new Western Wideout snowplow (recommended by western) installed by Western dealership on a 2016 Chevrolet Silverado 2500hd with snow plow prep package. Vehicle's Instrument cluster, HVAS, Radio and other electrical systems cut out while operating snowplow. GM is well aware of this issue which has been discussed and documented per GM Service Bulletin No.: PIT5387C (Feb 2016)- PIT5387P (10/17/2024), as well as GM Upfitter Integration UI Bulletin #124k (11/7/2019). Western states the fix for this issue is for GM to supply and install a wiring harness (per the above GM bulletins) GM dealerships as well as GM consumer assistance hotline state that GM part number 84043394 (fitting 2014-2019 vehicles) referenced to in PIT5387 and other bulletins has been discontinued. Therefore GM can not nor will ever be able to obtain or install this wiring harness in the future. GM Upfitter Integration Bulletin #124K instructs to install a different part number 84288774 VYU Snow Low Jumper harness per the latest Version of GM Service Bulletin PIT5387. "Note: that this jumper harness and installation will be provided (one time) without charge." GM dealerships state PN 84288774 does not appear in their system as a valid GM part. Parts. The official GM parts store at [XXX] , list this part but shows it does not fit this vehicle. Therefore neither GM nor Western is able to provide any actionable solution to this KNOWN safety issue. Western (and other manufactures) still manufacturing, recommending and installing snowplows for this specific vehicle. GM, manufactured, sold, promoted, advertises and continues to recommend snowplows to be installed on these vehicles (with the GM factory snow plow prep option), and continues to issue bulletins instructing GM dealerships how to fix this issue despite being fully aware GM cannot supply these parts. The manufacturer requested that the vehicle be taken to the dealer so that the third-party investigation could be performed. The vehicle was picked up by the dealer. 2015 Chevrolet Silverado 2500 Chino, CA

The contact owns a 2015 Chevrolet Silverado 2500 equipped with a Fastway Trailer Hitch manufactured in 2018. The contact stated while the vehicle was parked with an enclosed Carson trailer hitched to the vehicle, the contact became aware that the trailer hitch ball was lower than normal. Upon inspecting the trailer hitch, the contact noticed that the trailer hitch bolt was fractured. The trailer hitch manufacturer was notified of the failure and offered a discount on a replacement part. The contact declined and requested an improved part. The manufacturer informed the contact that the product might be under engineering review, but there was no time frame for the process. The part was not replaced. The VIN was not available.

The contact stated while driving at 30-35 MPH with a 2017 Heartland Bighorn trailer hitched to the vehicle, the message “Check Trailer Wiring” was displayed. The brake pedal was depressed, however, the trailer brakes failed to respond and the trailer did not slow down as intended. The contact stated that the failure had been reoccurring for an extended period of time while driving. The contact stated that the failure persisted for 4 hours while driving at 60 MPH. The contact stated that several unknown repairs had been completed on the trailer. However, the failure persisted. The vehicle was taken to the dealer where it was diagnosed that no issues were found. The vehicle was then taken to an independent mechanic who was unable to duplicate the failure. The vehicle was taken to a second dealer Graff Chevy Durand (9009 Lansing Rd, Durand, MI 48429), where it was diagnosed that no issues were found. The contact stated the Pollak 7-Way Plug Connector was installed on the vehicle under GM Technical Service Bulletin Number :PIT5311E. However, the failure persisted. The contact stated that the failure was intermittent, which made it difficult to diagnose. The manufacturer was not notified of the failure. The vehicle was not repaired. The failure mileage was approximately 11,000. The contact stated that they paid for repairs on the trailer but feels that the issue is coming from the truck too.

BRAKE FAILURE ON JUNE 21 2017, CAUSED ACCIDENT SAME ACCIDENT ON NOVEMBER 17 2017 AND AGAIN ON SEPTEMBER 1 2019. I HAVE HAD THE TRUCK IN SHOP TO BE FIXED NUMEROUS TIMES... ABS SENSOR IN FRONT RIGHT WHEEL WAS SUPPOSEDLY BAD. IT DID NOT FIX PROBLEM BC 3 RD ACCIDENT HAPPENED WEEKS AFTER. 9/11/19 GM RELEASED RECALL FOR BREAK FAILURE DUE TO THE VACUUM BOOSTER. MY ISSUE IS EXACTLY WHAT GM HAD JUST ANNOUNCED. CHEVY DEALER AND GM CORPORATE BOTH SAID THERE WAS NOTHING WRONG WITH BRAKES BUT THEY STILL GIVE OUT SLOWING DOWN INTERMITTENTLY. WHY ISNT MY VIN ASSOCIATED WITH THE MOST RECENT RECALL? BECAUSE FOR 3 YEARS NO ONE BELIEVED ME THAT THE BRAKES WERE FAILING.. I LOST MONTHS OF MY WARRANTY AND NOTHING WAS REPAIRED BC THEY SAID THEY WERE IN WORKING CONDITION AND THAT THERE WAS NO ISSUES. ALSO AT HIGH SPEEDS THE TRUCK SHAKES AND/OR VIBRATES, ALSO TOLD NOTHING WAS WRONG, I HAVE ELECTRICAL ISSUE THAT CAUSE ELECTRONIC SCREEN TO GO OUT, THEY SEEN VIDEO OF IT HAPPENING BUT WONT REPLACE IT UNLESS IT HAPPENS AT DEALERSHIP WHEN I BRING IT. THIS TRUCK HAS ALL ISSUES PERTAINING TO RECALLS BUT NOT ACCORDING TO VIN#. THE VEHICLE IS NOT SAFE TO DRIVE. ISSUE ARE SPORADIC AND INTERMITTENT WITH NO WARNING. I HAVE RECEIVED TICKETS, POINTS, AND BEING SUED BC OF THIS . LOST OUT ON MONTHS OF WARRANTY BC SERVICE CENTERS WERE TOLD NOT TO WORK ON IT UNTIL GM CORP DID THERE INVESTIGATION WHICH THEY SAID NOTHING IS WRONG ALSO.... NOW FINALLY THE LATEST RECALL FOR BREAK FAILURE CAME OUT ON 9/6/19 BUT MY VIN IS NOT UNDER IT AND MUST BE BECAUSE THE RECALL IS 100%ACCURATE TO WHAT IM EXPERIENCING WITH THE BRAKE FAILURE. THE CONTACT MOVED OFF THE ROADWAY AND DISCOVERED THAT THE TIRES WERE FLAT. HE THEN TRIED TO REMOVE THE SPARE TIRE FROM THE VEHICLE BUT TO NO AVAIL. THE VEHICLE WAS LATER TOWED TO THE CONTACT'S HOME AND THE TIRE WAS REPLACED. THE MANUFACTURER WAS CONTACTED AND A COMPLAINT WAS FILED. THE MANUFACTURER ALSO ADVISED HIM TO TAKE THE VEHICLE TO THE DEALER SO THAT THE DEALER COULD SHOW HIM HOW TO REMOVE THE SPARE TIRE. THE FAILURE AND THE CURRENT MILEAGES WERE APPROXIMATELY 46,000. THE CONSUMER STATED THE JACK IS UNSTABLE. THE TIRE WOULD NOT RELEASE FROM THE SECONDARY LATCH SYSTEM.

EQUIPMENT ADAPTIVE/MOBILITY

TRAVELING AT 65 MPH BRAKES ALL OF A SUDDEN THE BRAKES WERE APPLIED W/O APPLYING THEM LOSS OF STEERING CONTROL ETC. EVALUATING THIS PROBLEM II SAW THAT THE BRAKE PEDAL WAS GOING DOWN TO THE FLOOR, SO I PULLED UP ON IT WITH MY FOOT AND THE BRAKES RELEASED BUT STEERING CONTROL DID NOT RESUME. I GOT OFF AT THE NEXT EXIT AND VERY SLOWLY MADE MY WAY ON HOME. ANY APPLICATION OF THE BRAKE PEDAL RESULTED IN BRAKES LOCKING UP AND THEY WERE APPLIED SO TIGHTLY THAT ANY ACCELERATION COULD NOT SPEED UP UNLESS I PULLED UP ON THE BRAKE PEDAL. UPON ARRIVING HOME LOOKED THINGS OVER NO POWER STEERING UNLESS MOTOR WAS AT HIGH RPM,POWER STEERING PUMP LOOKED AS IF IT EXPELLED FLUID OUT THE BREATHER CAP AND THE BRAKES SMELLED AS IF VERY HOT. NOW THAT THIS HAPPENED I RECOGNIZED THIS HAD HAPPENED BEFORE BUT SOMEHOW REPAIRED ITSELF,BUT NOT TO THIS EXTREME. I SUFFERED BRUISES FROM SHOULDER HARNESS AND SEATBELT AND SOME KNOTS ON MY HEAD FROM BEING THROWN AROUND I CAME SO CLOSE TO OUT OF CONTROL AND A ROLLOVER SIMPLY COULDN'T CONTROL MY TRUCK.
