Desiccated Air Bag Inflator Rupture

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. Label

Issue: On certain vehicles, the tire and loading information label does not list that there is no spare tire in both official languages. The label reads as "NONE" when it should read as "NONE / AUCUN". Canadian regulations require the information to be listed on the label in both English and French. Safety Risk: This error does not affect vehicle safety. Corrective Actions: This is not a recall. The company has corrected the tire and loading information label for all new vehicles.

Airbag

On certain vehicles, long-term exposure to high absolute humidity and temperature, combined with high temperature cycling, could eventually degrade the propellant contained in the passenger frontal airbag. This could cause the airbag to deploy with more force than normal. As a result, if the vehicle were involved in a collision that warrants airbag deployment, fragments could be propelled toward vehicle occupants, and the airbag assembly could be damaged, preventing proper function. These issues could increase the risk of injury. Correction: Dealers will replace the passenger frontal airbag inflator. Note: Canadian climate results in the propellant degrading slowly. The recall is being conducted as a precaution to address future risk, and is expected to replace airbag inflators before their function would be affected.

On certain vehicles equipped with electro-hydraulic power steering (“EHPS”), the alternator may fail suddenly. This could result in the engine stalling, causing a loss of motive power which could increase the risk of a crash as well as disable various vehicle safety systems, and could also cause a fire. Both issues could increase the risk of injury and/or damage to property. Correction: Dealers will inspect the part number on the alternator, and will replace the alternator as necessary. Note: Only specific engine and alternator combinations are affected by this campaign.

On certain vehicles, the gear shift position indicators and warnings may not be sufficient to deter drivers from exiting the vehicle when the transmission is inadvertently left in a gear other than “Park”. If this occurs and the driver exits the vehicle with the engine running and the parking brake has not been set, unintended vehicle movement can result, increasing the risk of injury and/or a crash. Correction: Dealers will update vehicle software to include additional features to protect against driver error.

If this were to occur while the vehicle is being driven, it could result in a loss of motive power. If this were to occur while the vehicle is parked without the parking brake applied, it could result in unintended vehicle movement. Both situations could increase the risk of a crash causing injury and/or property damage. Correction: Dealers will replace the transmission in affected vehicles.

On the gear shift lever of my vehicle, there are jagged, hard plastic pieces between the leather and the shift lever that continuously cuts my fingers while shifting gears. The cuts are similar to papercuts. One time, a piece of the hard plastic broke off and got stuck in my hand, which required tweezers to remove. Very painful.

IN MARCH 2019 I TOOK MY CAR TO THE DEALERSHIP BECAUSE A CHROMED SECTION ON THE STEERING WHEEL WAS PEELING BACK AND HAD CAUSED ME TO CUT MY THUMB ON IT. THEY LOOKED AT THE CAR AND THEY ALSO CALLED ON MY EXTENDED WARRANTY AND WAS TOLD IT WOULD NOT BE COVERED BECAUSE IT WAS COSMETIC IN NATURE. I TRIED WORKING DIRECTLY WITH THE WARRANTY (ZURICH) PERSONNEL AND THEY SEEMED AT FIRST AS THOUGH THEY WOULD TRY TO ASSIST AND THEN DECIDED NOT TO. I DID NOT PURSUE IT ANY FURTHER SINCE. ABOUT A MONTH AGO I WAS DRIVING THE VEHICLE AND MY RIGHT HAND THUMB RAN ACROSS ANOTHER SECTION OF THE STEERING WHEEL AND IT PUT A CONSIDERABLE CUT IN MY THUMB. I HAVE A PHOTO OF THAT AS WELL AS THE PIECE THAT BROKE OFF IN MY THUMB. THIS PROBLEM IS NOT COSMETIC IN NATURE BUT REALLY IS A SAFETY ISSUE. I AM NOT SURE IF THIS IS WHAT IS CAUSING THE PROBLEM BUT THE STEERING WHEEL HAS THE HEATED FEATURE AND I USE IT ALL THE TIME IN THE WINTER. IT DOES SEEM TO GET VERY WARM FROM THE HEAT AND I AM NOT SURE IF THAT COULD BE CONTRIBUTING TO THE PROBLEM. I AM VERY UPSET ABOUT THE FACT THAT THIS PROBLEM IS SOMETHING THAT WILL CONTINUE TO GET WORSE OVER TIME AS THE CHROME PLATING CONTINUES TO PEEL AWAY. THE PROBLEM DOES NOT EXIST FROM NEGLECTFUL USE, IS VERY REAL, AND AGAIN SHOULD BE LOOKED AT FROM A SAFETY STANDPOINT. WHEN I WAS DRIVING THE LAST TIME IT HAPPENED, IT CAUSED A QUICK REACTION TO LOOK DOWN AT MY THUMB TO SEE WHAT HAPPENED AND CERTAINLY DISTRACTED MY DRIVING THAT SUDDENLY. I WOULD APPRECIATE THIS BEING LOOKED INTO FOR ME AS WELL ANY OTHER DRIVER THAT MAY HAVE HAD SIMILAR PROBLEMS.

The alternator ceased up on me while I was driving on 7/12/2019. This almost caused an accident! I was driving around 55MPH and the steering and brakes completely locked up. I couldn't turn the wheel or stop my car. Eventually I slowed down and barely got the steering wheel to turn so I wasn't stuck right on the highway. My car was towed to a mechanic that replaced the alternator. I later received a recall notice in the mail for the alternator and have been trying to get reimbursed.

THE TRANSMISSION W5A580-NAG1-722.6 EITHER SHIFT EXTREMELY HARD 3-4 4-5 OR SLIPS IN BETWEEN THE GEARS CAUSING THE ENGINE RPMS TO GO WAY UP AND THEN EITHER FALLS BACK INTO GEAR OR CAUSES A NEUTRAL AND WILL NOT GO BACK INTO GEAR UNTIL YOU EITHER DROP BELOW 18 MPH OR COMPLETELY STOP AND EITHER PUT THE CAR IN PARK AND BACK INTO DRIVE OR SHUT THE ENGINE OFF FOR ABOUT 10 SECONDS AND START IT BACK UP. TL* THE CONTACT OWNS A 2013 CHRYSLER 300. THE CONTACT STATED THAT UPON PURCHASE OF THE VEHICLE, THE VEHICLE WOULD SHIFT OUT OF GEAR INDEPENDENTLY WITHOUT WARNING. THE CONTACT HAD TAKEN THE VEHICLE TO MCCLUSKEY CHEVROLET (9673 KINGS AUTO MALL RD, CINCINNATI, OH 45249) ON TWO SEPARATE OCCASIONS WHERE THE MECHANIC WAS UNABLE TO DETERMINE THE CAUSE FOR THE FAILURE. THE CONTACT STATED WHILE AT A COMPLETE STOP, THE VEHICLE STALLED WITHOUT WARNING WHILE DEPRESSING THE ACCELERATOR PEDAL. THE CONTACT WAS UNABLE TO RESTART THE VEHICLE. THE VEHICLE WAS TOWED TO AN INDEPENDENT MECHANIC WHERE THEY DIAGNOSED THE VEHICLE WITH TRANSMISSION FAILURE. THE MANUFACTURER HAD YET TO BE NOTIFIED OF THE FAILURE. THE VEHICLE HAD YET TO BE REPAIRED. THE FAILURE MILEAGE WAS UNKNOWN.
