My 2014 Ford F-150 was previously included in Ford Safety Recall 24S37 regarding unintended transmission downshifts caused by an intermittent Output Shaft Speed (OSS) sensor issue. The vehicle was inspected by a Ford mobile technician on January 20, 2026. No qualifying codes were found at that time, and the recall was closed under my VIN. The recall involved the risk of an unintended downshift into first gear that could result in loss of vehicle control. On August 20, 2026, while my husband was driving approximately 40-50 mph, the truck suddenly downshifted and rapidly lost speed. The vehicle would not accelerate beyond approximately 10-15 mph and had to be moved into a parking lot to avoid remaining in traffic. After parking, the truck would no longer move under its own power. The wrench warning light illuminated and the vehicle became undrivable. The symptoms experienced appear substantially similar to those described in Recall 24S37. The dealership has indicated that the current failure may involve the transmission lead frame and speed sensor system, but I am concerned that this event may be related to the same transmission downshift condition addressed by the recall. I am requesting that NHTSA document and review this incident.

Vehicle Information: Year: 2014 Make: Ford Model: F-150 Component: Power Train (Transmission / Powertrain Control Module) Dealership Involved:Midway Motors Ford, 2075 E Kansas Ave, McPherson, KS 67460 Complaint Description:I am filing a formal safety complaint against Ford Motor Company and Midway Motors Ford in McPherson, KS, for failing to provide a free remedy under active federal safety recall 24S37 (NHTSA Campaign 24V444).My 2014 Ford F-150 was brought to the dealership to complete safety recall 24S37, which addresses severe, sudden transmission downshifts. The dealership performed a software update to the Powertrain Control Module (PCM). Immediately following this update, the truck's drivability severely deteriorated, and the transmission began violently slamming into 3rd gear, locking itself into a dangerous transmission fault "limp mode." The diagnostic scans post-update indicate a failure of the molded lead frame/speed sensor. According to Ford's official technical service bulletin for recall 24S37, physical replacement of the molded lead frame is an explicitly authorized remedy when diagnostic codes appear following the software flash. However, both the local servicing dealership (Midway Motors Ford) and Ford Corporate Customer Service are actively refusing to perform this physical safety repair free of charge. They are wrongfully claiming that because my vehicle has over 200,000 miles, it is disqualified from financial assistance. They are confusing a federal, non-expiring safety recall with an older, expired warranty extension program (19N01).By refusing to replace the defective lead frame that their own software update triggered into an active failure state, the manufacturer and dealer are leaving my vehicle in an unsafe, undrivable condition and violating federal recall mandates. I request that NHTSA intervene to compel Ford to complete the mandatory safety recall remedy at zero cost to me.

On July 2, 2026, the vehicle was brought to an authorized dealer for a scheduled appointment to resolve active Safety Recall 26S45 / NHTSA Campaign 26V378 (6R80 transmission sudden downshift defect). The dealership refused to perform the recall software update, claiming it was not needed for the vehicle. On July 12, 2026, while traveling at highway speeds on a public highway, the transmission suffered the exact un-commanded sudden downshift described in the recall. The violent mechanical force instantly snapped the rear driveshaft in half and severed the undercarriage air conditioning lines, creating a sudden, life-threatening loss of control and wheel speed reduction on a live highway. The vehicle is now permanently disabled due to the dealer's refusal to fulfill a federal safety mandate.

I am the owner of a 2014 Ford F-150 that is experiencing sudden, unexpected downshifts into a lower gear while driving at highway speed. This has happened on multiple occasions and has nearly caused accidents, including loss of vehicle control momentarily. This defect matches the description of Ford safety recall 26S45 (loss of signal between the transmission output shaft speed sensor and the powertrain control module, causing unexpected downshift into first gear). Despite this, my vehicle was not included when I checked recall status, even though I have an open Ford claim number: Cxh-08412521-F6L5j4. I am filing this complaint to ensure this incident is on record given the ongoing safety risk, and to request that NHTSA confirm whether my VIN should be captured under recall 26S45 or a related campaign (24S37/24V444). Description: Transmission suddenly and dramatically downshifts at highway speed without warning, causing loss of vehicle control and near-accidents.

The transmission system failed, consistent with an active federal safety recall issued for this vehicle. The primary symptom was a complete failure to start. The vehicle is currently at Helfman, where it was towed for recall repair. It is available for inspection upon request at that location. A vehicle that fails to start due to a transmission defect under active recall creates a safety hazard in that the owner may be stranded — including in unsafe locations or conditions — without warning. The vehicle became completely inoperable with no ability to move it under its own power, requiring emergency towing. Additionally, a transmission defect can create unpredictable vehicle behavior risk if the failure had occurred while the vehicle was in motion rather than at rest. The vehicle was towed to an authorized Ford dealership, which received the vehicle for recall-related repair and initially indicated it was awaiting parts — implying the recall defect and the vehicle's no-start condition were acknowledged. The dealership subsequently refused to complete the repair without providing a clear written explanation. UNKNOWN whether a formal written diagnosis was issued.

My 2014 Ford F150 truck went into “limp mode” last week. A wrench showed up on my dashboard, my speedometer didn’t function, and it sounded like the transmission was “winding up” if I tried to accelerate. After hitting “ok” to remove the wrench and restarting the truck a couple of times, my check engine light illuminated. I was driving at about 40 MPH when this occurred and it was a scary moment. Luckily I was able to keep my truck under control and drove very slowly back home considering I was less than 4 miles from my home. The truck went into a Ford dealership in 2024 for the “transmission recall” in which the only thing I noticed different was that I couldn’t see the digital shifting of the transmission on the screen anymore and they “killed” my battery. I still had issues every so often with power loss or the transmission shifting down abruptly, but it wouldn’t throw a code. Last week it threw a code. P0720 to be exact, the reason for the transmission recall in the first place. There was no fix done with the recall and it made it so that I had to watch my RPM’s to track my transmission shifting. This is extremely concerning especially since I was having the issues prior to a computer reprogram and it wasn’t addressed. I have also seen that there’s yet another Ford transmission recall that could affect my truck considering it was assembled in June 2014 and fits the transmission 6R80 that needs fixed. Could someone please help? Thank you.

Complaint Description: Unintended Transmission Downshift Vehicle: 2014 Ford F-150 Issue: Intermittent Transmission Malfunction / Severe Unintended Downshifting Detailed Symptoms: Sudden Downshifting: While traveling at cruising speeds (typically in 4th or 5th gear), the vehicle occasionally performs an uncommanded downshift directly into 1st gear without driver input. Start-up Stalling: The vehicle intermittently becomes "stuck" in 1st gear upon acceleration from a stop, refusing to upshift into higher gears. Lack of Warning: These events occur with zero warning signs. No "Check Engine" light, "Wrench" icon, or transmission temperature warnings appear on the dashboard before, during, or after the incident. Recall History: The vehicle previously received the official Ford recall repair for the Lead Frame/Output Speed Sensor issue, yet the life-threatening symptoms have persisted or returned. Diagnostic History: The vehicle has been inspected by an authorized Ford dealership on two separate occasions. The technicians reported that they were "unable to duplicate the failure" and found no stored error codes, leaving the safety defect unresolved. Safety Concern: I am filing this complaint because the sudden shift to 1st gear at high speeds causes a violent deceleration (similar to slamming on the brakes), which risks a loss of vehicle control or a high-speed rear-end collision. I have raised this issue to Ford customer relation department as well. Case# CXH07719550, Date: 04/20/2026

It was Tuesday 04/14 I had just left a store. Drove to the corner stop sign and applied the breaks which have always been just fine, easy to stop and slow. Until thst day I eased on the breaks and all of a sudden the peddle went to the floor. I came home and called my mechanic shop. They told me to bring it in. When I did they told me the master cylinder had gone out and was leaking all into the booster. They felt it was unsafe for me to drive it. They gave me a loaner vehicle. Upon returning to my home that's when I saw all the break fluid running down my driveway where I had parked when I got home from the store the day prior. Which I hadn't noticed before. So, now I'm without my vehicle because I simply can't afford a $900 repair bill. I've looked it up and on your website it shows in 2025 there was a recall on the 2014 Ford F-150 that named the master cylinder as a problem. It also stated that on Google. Please help.

I was driving the vehicle this morning; the temperature was about 16 degrees Freiheit outside and the truck had been sitting out all night. I had the heater on and the rear defroster on. I drove the truck for about 2 minutes on a 25 mph residential street and heard a loud bang. I continued to drive the truck and noticed the driver side pane of the rear sliding glass window had shattered. I did some research online and found several reports from individuals with F-Series trucks that have reported similar incidents.

I am reporting an odometer rollback that occurred before I purchased my vehicle. CARFAX shows the vehicle had 206,627 miles when it was sold at an auto auction on February 27, 2023. Four months later, in June 2023, the vehicle was retitled in Illinois at 69,000 miles. The odometer reading at my first service visit on June 26, 2023 also showed 68,380 miles. This is a discrepancy of approximately 138,000 miles, which indicates the odometer was rolled back sometime between the auction sale and the Illinois title issuance. I purchased the vehicle in June 2023 without knowing the mileage had been altered. The issue affects the accuracy of the vehicle’s history and could impact safety and maintenance expectations. I am reporting this so the rollback can be documented and investigated by the appropriate authorities.

After receiving a call from Mullinax Ford in Mobile, AL regarding a recall for our 2014 Ford F-150 Raptor (recall related to unexpected downshifting), I brought the vehicle to my workplace on [XXX], where a technician performed the software update (flash) on-site. Before the update, the vehicle was functioning without issue. That same evening while driving home, I experienced extremely hard downshifting, initially in lower gears, and worsening over time. The truck now violently downshifts, primarily into 4th gear, and sounds as if the transmission is slipping. I drove the truck again the next morning and the symptoms persisted. I contacted Zachary, the original representative at the dealership, who said it may be part of the “relearning” process. My husband drove it a week later after returning from a business trip and he confirmed the issue. It has progressively worsened to the point of being undrivable. We returned the truck to Mullinax Ford for evaluation on 7/29/25, and they claimed the failure was unrelated to the update and that the transmission needed to be replaced, at our expense. The truck unpredictably drops gears at high speeds, creating a sudden jerk that could lead to loss of control. This poses a serious safety risk to both the driver and others on the road. The dealership performed the recall software update and later diagnosed the truck with transmission failure, but is denying responsibility. No other third-party inspections have been done at this time. There were no warning lights or issues prior to the update. The issue started immediately after the recall software was flashed and worsened over the next 24–48 hours. The vehicle is currently at the dealership and available for inspection. We believe the issue stems directly from the PCM/TCM update applied under recall conditions. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

This 2014 pickup was started at about 8:00 AM and the manager went inside the building for a few minutes, (less than 5). A vender that was delivered petroleum product saw that the truck was smoking and on fire in the cab and he told the technicians in the shop who ran out with fire extinguishers to put the fire out. The doors had locked automatically with the keys inside the vehicle. A spare set of keys were acquired shortly and the vehicle was opened with the key. The vehicle was turned off and the hood was popped. a fire extinguisher was deployed while another technician disconnected the battery which then extinguished the fire. The heater motor had melted out of the dash and the dash had started on fire. The dealer was notified and we were told to call Ford and report to our insurance. We are self insured so there is no one to report to. There were no symptoms, problems leading up to this thermal event. The vehicle has 78,000 miles on it and is in great mechanical shape. We are the original vehicle owners. The backup camera was replaced at a Ford Dealership on 3/26/20 because the screens were showing blank images and sometimes distorted images. at that time the mileage was 82564. Now the backup camera is doing the exact same thing, and the mileage is 144236. This is a safety issue backing up and can cause an accident. There have been recalls on most models but 2014 was not included. Ford has stated that the recalls are tied to the VIN., thus even though we paid Ford and replaced the camera in 2020, the camera is not covered although it is the same parts of model year VINS that were recalled. The 2014 should have been included in the recalls, as well as specific part recalls.

On [XXX], about 20 minutes into my 30-minute commute, while driving at highway speed, my trucks transmission suddenly downshifted dramatically, jerking and throwing me forward in my seat and causing the truck to slow down so abruptly that I was almost rear ended, putting drivers around and behind me at risk of high-speed collisions. Less than 5 minutes later it happened a second time. This is the exact same abrupt transmission downshift issue that happened to me in this vehicle a few times before in early 2024. I received a Safety Recall notice on this known issue about a week after the first incident in 2024. I took my vehicle into the AutoNation Ford Dealer in Littleton, Colorado immediately after I received that notice and they supposedly 'fixed' the issue under the recall 24S37. Obviously, the issue has not been resolved. I am asking for a deeper look into the safety issue as I am sure I am not the only one this is happening to. I am also concerned that there were no warning lamps or messages that would indicate a problem that needed to be addressed! The truck is currently back at the AutoNation Ford Dealer in Littleton and is available for inspection. They are telling me there is no current recall for this issue. How can that be true if it’s the exact same problem and their Safety Recall ‘repair’ didn’t work? I believe the repair was just a software patch, but I can’t be sure. Can you please help me find out what the recall (24S37) was supposed to fix and why it didn’t fix the problem? Their suggestion now is to replace the Valve Body with a cost of $2,554.63 (in addition to a $219.95 diagnostic fee). My concern is that even this expensive repair might not actually solve the problem, and I am truly afraid to drive this vehicle. I received a recall (Safety recall notice 24S37 . NHTSA Recall 24v444 ) notice in the mail. I scheduled with the dealership to have the recall taken care of. When the recall issue was "fixed", I picked up my 2014 Ford f150 stx 3.7L truck and immediately noticed it was running vwry rough and the transmission felt like it was slipping. The truck ran great before going in for the recall issue and had even been fully serviced and tuned up two weeks prior. I called the Ford dealership and explained to them. The truck then would not start but it did continue to crank over. I plugged my computer to it and no codes. I called the dealership back and was told to have it towed (at my expense) back to the dealership so they could fix it and if the issue was under due to the recall service, which it was, I would be reimbursed the money for the tow and the money they charged for a diagnostics. They also found nothing wrong and couldn't start the truck. They recommended I allow them to perform work to it claiming the spark plugs showed that the engine was flooded but could tell they were new and correctly gapped. After another $700 (for 6 spark plugs to be replaced) it did not fix the issue and they're not sure what to do. I've called the dealership, spoke with the service manager Bill and the GM Bob. They claim there is nothing they can do. I have called Ford Motor customer service twice with no help to fixing the problem. I too my perfectly running truck in for a safety recall and received a broken truck Back. After over $1,200 out of pocket, I still have no truck and no answers and am stuck at the mercy of the dealership.

Took vehicle to dealer for safety recall in reference to the transmission issue of downshifting to a lower gear while driving at highway speeds. Dealer did a software reprogram and indicated that would revolve the issue. Within six months I had encountered the same issue. I notified Ford corporate and they instructed me to take it in to diagnose the issue, to which I did. They notified me that the lead frame was defective and would need to be replaced. they indicated that I would be charged close to $1700.00 for the service. contacted Ford Corporate and they claimed that it was not part of the recall. My problem with this that the software reprograms did not resolve the recall issue. It is known that the lead frame had manufacturing faults.
