What component or system failed or malfunctioned? Seatbelt sensor system and cruise control system. The seatbelt sensor prevents cruise control activation unless the belt is fastened. How was your safety or the safety of others put at risk? A driver who cannot wear a seatbelt for a valid medical reason (recognized by state law) is forced to drive without cruise control. Manually maintaining speed over long distances increases driver fatigue and distraction, raising the risk of error. The dealer also disabled the factory seatbelt chime without consent, removing an audible safety warning. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. Peppers Toyota ([XXX]) confirmed on March 17, 2026 that cruise control will not activate without a fastened seatbelt. They attempted no fix for cruise control. Instead, they disabled the seatbelt chime without my request or consent. I asked them to restore the chime. They refused. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Yes. Peppers Toyota inspected the vehicle on March 17, 2026. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Yes. The factory seatbelt chime operated normally until March 17, 2026. On that date, Peppers Toyota intentionally disabled the chime without my consent. The chime no longer functions. The vehicle will not activate cruise control without a fastened seatbelt. If you are uncertain, simply state UNKNOWN and provide your best assessment of the incident and cause in your description. UNKNOWN why Peppers Toyota disabled the seatbelt chime without my consent or authorization. UNKNOWN why they refuse to restore it to factory settings. UNKNOWN why the cruise control cannot be modified to work independently of the seatbelt sensor, given that service staff stated this modification had been done on older Toyota models. Within a week of buying my vehicle in May of 2021 I went to Clearwater Toyota where I bought the vehicle from and complaining about the three seat belts in the back malfunctioning. When nobody is sitting in the backseat they sound off as if somebody is sitting there. I know that there is a recall on the front seat belt sensor doing this same issue for the Toyota Corollas but everyone I have spoken to that owns a 2021 Toyota Corolla complains about the back seats doing it as well the sensors misfire or sound off every hour as I'm driving at least once an hour. As an Uber Lyft driver I am concerned about my passengers in the backseat wearing the seat belts and the seat belt malfunctioning due to the sensors malfunctioning since I bought the vehicle in May of 2021. Any of the above questions I would have to stay unknown.

On March 12, 2025, I was involved in a frontal collision when another vehicle unexpectedly pulled out in front of me. Impact occurred to the front-left area of my vehicle. Despite the nature of the collision, the front airbags did not deploy. The vehicle is subject to Safety Recall 20TA03, which addresses incomplete or non-deployment of airbags and/or seat belt pretensioners due to ECU susceptibility to electrical noise in certain crash scenarios. The vehicle was declared a total loss by my insurance company and is no longer available for inspection. The manufacturer was notified in May 2025 and again in November 2025. The manufacturer closed the case citing inability to inspect the vehicle and later relied on photographs only to rule out airbag deployment concerns. No warning lights or messages related to the airbag system were observed prior to the collision. The non-deployment of the airbags placed my safety at risk during a frontal impact.

While the vehicle was parked and powered off, a fire originated inside the driver seat area. There was no collision, no aftermarket electrical equipment installed, and no heated seat or accessory usage at the time of the incident. The vehicle was stationary when the fire began. The fire damage was concentrated within the driver seat assembly, consistent with a suspected electrical failure involving seat wiring, seat-mounted components, or associated harnesses. This is not consistent with owner misuse or external ignition sources. The insurer (Allstate Insurance Company) declared the vehicle a total loss but declined further fire-origin or electrical defect investigation. The vehicle is at risk of being sold or dismantled through a salvage auction, which would destroy critical evidence related to a potential manufacturer defect. The vehicle is currently listed on Copart (Lot# 99091095 highlighted as ‘Run and Drive’) I have completed a Power of Attorney (HSMV 82053) per Allstate requirement, along with a limited conditions notification for evidence preservation however Allstate is refusing to accept the conditions stating their intent to sell it as salvage. This incident presents a serious safety concern involving spontaneous ignition within the passenger compartment. Similar failures pose a risk of injury, death, or property damage, particularly if ignition occurs while occupied. I am requesting that NHTSA review this incident for potential electrical system or seat assembly defects and preserve this complaint as part of any pattern or trend analysis involving Toyota vehicles.

The contact owns a 2017 Toyota Corolla. The contact stated while driving at an undisclosed speed, the air bags and seat belt warning lights were illuminated. The contact stated that upon starting the vehicle, the air bags and the passenger's side seat belt warning lights illuminated. The contact stated that the front passenger's side seat was not occupied. The vehicle was taken to an unknown dealer, where the contact declined to pay a diagnostic fee because the failure was a safety issue. The vehicle was not diagnosed or repaired. The contact stated that the front passenger's side air bag and the dashboard were replaced in 2022, and the front driver's side seat belt was replaced in 2023. In addition, the wiring harness was replaced due to water intrusion in March 2022. The contact was informed that the repair was not covered under warranty, and the 12 months warranty had expired. The driver seat belt dont work and once in a great while it will work but even whenever it does latch it still shows the seat belt isn't on and that noise keeps going off but after a while the noise goes off but the symbol is still on the dash board. I recently got hit from behind (09/15/2025) at a red light and no air bags deployed. Im not sure if they were suppose to deploy but either way my seat belt dont work. And I know this isn't a safety matter but my radio doesn't turn off when I turn off my car and I can't lock/unlock my doors with the key fob I have to lock it with the key on the driver door. I have to take the fuse out for the radio so my battery won't die and so the radio turns off.

Component/system failed: The SRS Airbag System malfunctioned. After a rear-end collision on October 3, 2025, in Gwinnett County, GA, the airbags did not deploy. The vehicle displayed an alert: “SRS Airbag System – Malfunction detected” at 16:02. Safety risk: My safety was put at risk because the airbags did not function during a significant impact. This failure could have resulted in severe injury if the collision had been more forceful or from another angle. Inspection/reproduction: On October 6, 2025, I brought the vehicle to Atlanta Toyota. Instead of documenting the diagnostic trouble code (DTC) and providing a report, the dealer cleared the fault code and recalibrated the system without my authorization, effectively erasing critical evidence of the malfunction. The invoice confirms this. Inspections by others: The vehicle was inspected only by the Toyota dealer. It has not yet been inspected by the manufacturer, police, or insurance representatives, but I have legal counsel and am preserving the evidence for further independent inspection. I purchased this vehicle in November 2024 from a dealership. On July 8, 2025, I was involved in a crash where the airbag failed to deploy. After the accident, I learned there was an active recall from 2020 (NHTSA Recall No. 20V024) related to the airbag control system. The recall had not been repaired, and I was never informed of it when I bought the car. This defect may have worsened my injuries and left me completely unprotected during the crash.

I was in a car accident .A woman failed to yield at a stop sign and rammed into the front of my car at a high accelerated speed. I was restrained but the right side of my forehead hit the rearview mirror .causing two huge knots on my head. My eye glasses also smashed into my right eye and I had a cut above my eye .I was thrown to the right passenger seat area , my right hip hit the gear shift and my knees and hip , were bruised. Apparently the seatbelt failed as well , but my main complaint is that the airbags failed to deploy. I suffered damage strain and bruising to both hands, the left was worse , I was made to wear a brace . I was taken to the ER via ambulance . One of the officers assumed I wasn’t wearing a seatbelt , which was not true at all . He thought I hit the windshield but I did not , it was the mirror . I have proof , photos and all EMT and ER records report that I was restrained . A paramedic helped me remove the seatbelt to exit my car . I feel I wouldn't have suffered the head, eye and other injuries if the airbag had deployed.
