The Multi-Information Display (MID) on the instrument cluster (combination meter) has failed completely. The display shows full-width horizontal RGB scan lines across the entire screen with no readable information. The analog gauges (speedometer and tachometer) remain functional but all digital display information is unavailable including speed, warnings, fuel economy data, and safety system status indicators. This failure occurred suddenly with no prior warning. Vehicle has approximately 50,000 miles. I am aware that Toyota issued Safety Recall 25V595 in September 2025 covering the identical combination meter failure on 2023-2025 model year Tacomas and other Toyota vehicles. The root cause identified in that recall — accelerated deterioration of the memory device inside the combination meter due to improper programming — appears to be the same failure mechanism present in my 2021 model year vehicle. I am filing this complaint to formally document that this known defect extends to 2021 model year Tacoma vehicles and to request that NHTSA investigate expanding recall 25V595 to include earlier model years. Toyota should be required to repair or replace the combination meter on affected 2021 vehicles at no cost to owners, consistent with the remedy already established for 2023-2025 vehicles. A Toyota case has been opened with the Brand Engagement Center at [XXX]. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
NHTSA complaint #11746832 — an unverified report by a vehicle owner.
The key fob separate from the key dangles while driving and with manual seats there is no way to adjust the seat up or down and while accelerating or braking, while moving leg and foot the knee hits the key fob and causes the key to turn while driving leading to a hazardous safety condition, Toyota needs to integrate the key fob into the key so nothing is dangling to get hit by the knee and knock key out of position

The 2024 Tacoma has a recall out for the screen because it blacks out. We took it to the dealer to remedy this recall, even though we were not experiencing symptoms associated with the recall. After they did the update, our screen blacks out and the Bluetooth does not work. Our screen is blacking out and our Bluetooth does not work after the Toyota dealership allegedly fixed the recall.

The contact extinguished the fire with a jug of water that was in the vehicle. The vehicle was not diagnosed or repaired by an independent mechanic or dealer. The manufacturer was made aware of the failure and a case was filed. In addition, the manufacturer requested an inspection of the vehicle. The failure mileage was approximately 42,000.

The factory Heads Up Display (HUD) on my 2024 Toyota Tacoma Limited (VIN: [XXX] ) intermittently resets its motorized vertical position to the fully raised/home position, most often triggered when unlocking the vehicle and opening the driver door. The HUD panel actuates upward without input, partially removing projected speed, navigation, and driver-assist data from the driver's sightline. The component is available for inspection. Safety risk: The displacement is unpredictable and intermittent. A driver relying on the HUD for speed or warning information may not notice the shift until already in motion, creating a distraction hazard. Dealer confirmation: Three visits to Envision Toyota of [XXX] . RO #XXX (11/30/2024, 2,259 mi) — FTS contacted, could not duplicate, no TSB found, no repair. RO #XXX (03/03/2026, 15,039 mi) — noted as 3rd visit for same concern. Toyota Technical Assistance Center contacted; TAC Case #XXX opened. Toyota's documented response: condition acknowledged, no repair procedure available, concern under investigation. Manufacturer inspection: Yes — Toyota TAC has formally acknowledged the defect via the above case number. Warning lamps/messages: None. No warning indicators accompany the reset. First observed shortly after delivery (September 2024). INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

My 2025 Toyota Tacoma TRD Pro (VIN [XXX] , purchased from Delaney Toyota, DuBois PA) exhibited a recurring Smart Key and door lock malfunction beginning in late October/early November 2025 and continuing through the date the vehicle was traded in February 2026 with the defect unresolved. Symptoms observed: Doors locking autonomously on exit, sometimes before occupants could exit the vehicle System falsely detecting the key fob as being inside the vehicle when it was in the owner's pocket outside, causing the truck to refuse to lock Touch-lock refusal at the door handle Random unprompted beeps while parked and locked with no corresponding lock movement Key card unable to lock the vehicle Dealership witness: During a January 2026 service visit, Delaney Toyota Service Advisor Ricky Carlson was personally locked inside the vehicle by this malfunction before he could exit. This was witnessed firsthand by a trained Toyota service employee and is part of the service history, though it was not captured in the official repair order. Service history: December 22, 2025 — RO #XXXX, no codes found, TAS case opened January 21, 2026 — RO #XXX, TAS case XXX, technician unable to duplicate, no codes found February 9, 2026 — RO #XXX, TAS case XXX, dealership driver witnessed false key-in-vehicle behavior directly when owner demonstrated it on arrival, behavior self-resolved before service bay access, no codes found Additional facts This defect never generated a diagnostic trouble code across three service visits Toyota Corporate case XXX was opened and closed without resolution Toyota arbitration case XXX was also closed without resolution The identical defect has since appeared on a replacement 2026 Toyota Tacoma TRD Pro (VIN [XXX] ), currently under active Toyota Corporate case #XXX The vehicle was traded in February 2026 because Toyota Corporate failed to resolve the defect within the warranty window INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
NHTSA complaint #11725346 — an unverified report by a vehicle owner.
My 2026 Toyota Tacoma TRD Pro (VIN [XXX] , delivered 02/28/2026, purchased from Delaney Toyota, DuBois PA) has exhibited a recurring Smart Key and door lock malfunction beginning at approximately 250 miles and continuing through at least 1,814 miles as of March 18, 2026. Symptoms observed: Doors locking autonomously without any input while occupants are entering or exiting the vehicle System falsely detecting the key fob as being inside the vehicle when it is in the owner's pocket outside, causing the truck to refuse to lock Touch-lock refusal at the door handle Random single beeps while the vehicle is parked and locked with no corresponding lock movement Dashboard warning "Key Not Detected" with audible alert while vehicle was in motion (March 18, 2026, approximately 11:06 AM) Dashboard warning "Key Not Detected" with audible alert while attempting to start the vehicle (March 18, 2026, approximately 2:30 PM) Safety concern: On March 18, 2026, a Toyota Brand Engagement Center case manager (case #XXX) confirmed on a recorded call that if the vehicle loses key fob detection while driving and is subsequently shut off, there is a possibility it will not restart even if the owner holds the fob directly to the start button. This creates a real stranding risk on public roads. Additional facts: This defect has never generated a diagnostic trouble code across multiple service visits A full-day diagnostic on March 9, 2026 (RO #XXX) produced no fault codes Immediately after that diagnostic, the vehicle autonomously locked out two Delaney Toyota employees on dealership property — witnessed by the dealership's General Sales Manager and a Product Specialist The identical defect appeared on a prior vehicle, a 2025 Toyota Tacoma TRD Pro (VIN [XXX] ), which was traded in February 2026 with the defect unresolved after three service visits Toyota Corporate case #XXX is currently open INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

I am reporting a safety issue with my 2024 Toyota Tacoma regarding Recall 25V595 / 25TA08 (Blank Instrument Cluster Display). Multiple authorized dealers have refused to perform the repair, claiming that parts are unavailable. This defect prevents me from obtaining commercial insurance and fully utilizing the vehicle for my business needs, as a non-functional speedometer is a major safety hazard. This situation is significantly impacting my work and safe operation of the vehicle.

I had 8022 miles on my tacoma , toyota employee of almost a decade , drove like normal no adds , one morning drove to a conference and was on the road no more than 20 min and then I see the bed of my truck on fire , im on the interstate so i pull over quickly and call 911 and then it just burnt up and gone. Now a year later and toyota refuses to assist with anything saying they cant find out despite the insurance companies ruling me out and fowl play out. I feel strongly toyota has an obligation to make this right.

The contact owns a 2024 Toyota Tacoma equipped with Michelin Tires, Tire Line: Defender LTX M/S2, Tire Size: 245/70/R17, DOT Number: (N/A). The contact stated that two weeks after the vehicle was inspected and passed the inspection, while driving at an undisclosed speed and depressing the brake pedal to stop at a stop sign, the vehicle failed to stop and slid past the stop sign. The contact pulled over to inspect the vehicle and saw that the steel belts were protruding from the tires. The vehicle was taken to an independent mechanic, and the contact was informed that the vehicle should not have passed the inspection with the tires currently on the vehicle. Additionally, the contact was informed that there was a recall on the vehicle for the back-over prevention camera screen. The contact stated that the back-over prevention screen frequently went blank. The dealer was contacted, and the contact was informed that there was no remedy yet for the recall repair. The manufacturer was made aware of the failures, and the contact requested a buyback of the vehicle; however, the buyback was declined by the manufacturer. The failure mileage was unknown.
