Desiccated Air Bag Inflator Rupture

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2010-2016 4Runner, 2003-2006 Tundra, 2003-2013 Corolla, 2009-2010 Corolla Matrix, 2004-2005 RAV4, 2002-2007 Sequoia, 2011-2013 Sienna, 2008-2012 Scion xB, 2008-2009 Lexus IS-F, 2007-2012 Yaris and Lexus ES350, 2010-2017 Lexus GX460, 2002-2010 Lexus SC430, 2006-2012 Lexus IS250 and IS350 and 2010-2015 Lexus IS250C and IS350C vehicles. These vehicles had their driver or passenger frontal air bag inflators previously replaced under a prior recall using inflators of the same design. The inflators may explode due to propellant degradation occurring after long-term exposure to high absolute humidity, temperature and temperature cycling.

On January 9, 2018, Toyota Motor Engineering & Manufacturing (Toyota) notified NHTSA that they are recalling 14,585 model year 2010-2012 Yaris vehicles ever registered in the states of Alaska, Arizona, Arkansas, Colorado, Connecticut, Delaware, District of Columbia, Idaho, Illinois, Indiana, Iowa, Kansas, Kentucky, Maine, Maryland, Massachusetts, Michigan, Minnesota, Missouri, Montana, Nebraska, Nevada, New Hampshire, New Jersey, New Mexico, New York, North Carolina, North Dakota, Ohio, Oklahoma, Oregon, Pennsylvania, Rhode Island, South Dakota, Tennessee, Utah, Vermont, Virginia, Washington, West Virginia, Wisconsin, and Wyoming. As of January 30, 2018, Toyota is additionally recalling 3,347 model year 2010 Pontiac Vibe vehicles located in states listed above. Toyota Motor Company (Toyota) is recalling certain model year 2009-2011 Tundra, Sequoia, Corolla, Corolla Matrix and Scion xB, 2008-2011 Highlander and Highlander Hybrid, 2007 Camry and Camry Hybrid, 2009 Camry and Camry Hybrid, 2006-2011 RAV4, 2006-2010 Yaris, and 2009-2010 Scion xD and Pontiac Vibe vehicles. During the manufacturing of the Power Window Master Switch (PWMS), grease lubricant may have been inconsistently applied to the sliding electrical contacts.

Risk: If the sliding electrical contacts are not protected by lubricant, debris and moisture that get into the switch may cause a short circuit and the switch assembly may overheat and melt, increasing the risk of a fire.

AIR BAGS:FRONTAL:DRIVER SIDE:INFLATOR MODULE

Toyota is recalling certain model year 2006-2008 RAV4, 2006-2010 Yaris, 2008-2010 Highlander, and 2009-2010 Tacoma, Corolla, Corolla Matrix and General Motors Pontiac Vibe vehicles. In the affected vehicles, the steering column assembly contains electrical connections to the driver's airbag module housed in a spiral cable assembly, which includes a Flexible Flat Cable (FFC). Due to the shape and location of the FFC's retainer, the FFC could become damaged when the steering wheel is turned.

Risk: If the FFC is damaged, connectivity to the driver's air bag module could be lost and the air bag deactivated. The failure of the driver's air bag to deploy in the event of a crash that typically necessitates deployment increases the risk of injury to the driver.

Toyota is recalling certain model year 2006-2010 Yaris vehicles manufactured August 22, 2005, through May 12, 2010, and 2008-2010 Scion xD vehicles manufactured April 4, 2007, through May 12, 2010. In the affected vehicles, the springs used for the seat rails on the driver's seat and the seat rails of the front passenger seat of three-door models may break.

Risk: If the spring breaks, the seat may not lock in position. In the event of a vehicle crash, the seat could move increasing the risk of injury to the seat occupant.

Issue: This recall is for certain block heater kits installed on certain vehicles or sold as accessories at Toyota or Lexus dealerships. On certain vehicles, the engine block heater cable may not have enough heat protection and could be damaged by high temperatures. If this happens, the cable could short circuit while plugged in, and cause the wiring to overheat or melt. For some vehicles, the corrective actions for this recall are under development. For others, a new engine block heater cannot be installed, and the owner will receive a refund of the purchase price of the engine block heater.

Airbag

On certain vehicles, long-term exposure to high absolute humidity and temperature, combined with high temperature cycling, could eventually degrade the propellant contained in the passenger frontal airbag. This could cause the airbag to deploy with more force than normal. As a result, if the vehicle were involved in a collision that warrants airbag deployment, fragments could be propelled toward vehicle occupants, and the airbag assembly could be damaged, preventing proper function. These issues could increase the risk of injury. Correction: Dealers will replace the passenger frontal airbag assembly. <br><br> Note: Canadian climate results in the propellant degrading slowly. The recall is being conducted as a precaution to address future risk, and is expected to replace airbag inflators before their function would be affected. Matrix model vehicles may have had the original front passenger airbag inflator replaced as a “like-for-like” replacement under a previous recall as a temporary remedy while evaluating a permanent remedy. These vehicles will receive the permanent remedy passenger airbag assembly under this recall.

Electrical

On certain vehicles, inconsistent application of grease during the manufacturing process could cause the driver's side power window master switch to stick or become inoperative due to electrical contact point wear. This could cause debris to accumulate between the contact points, potentially resulting in an electrical short circuit. If a short circuit occurs, the switch assembly may overheat, produce smoke, melt and/or potentially lead to a fire causing injury and/or property damage. Correction: Dealers will inspect the driver's power window master switch and apply a specialized grease that inhibits heat build-up, or replace the power window master switch circuit board, as necessary. Note: This is an expansion of recall 2012-338.

As a result, the driver's airbag may not function as intended, causing the instrument panel airbag warning lamp to illuminate. Correction: Dealers will replace the spiral cable assembly.

Accessories

Toyota Canada, through its network of dealerships, offered for sale an accessory cruise control unit manufactured by Rostra Precision Controls, Inc., Model AP800 for 2006-2010 Yaris models. The Rostra accessory cruise control unit is mounted to the left of and below the steering wheel, behind the dashboard (below the cup-holder) with a switch on the steering column to enable the driver to activate it. Rostra has confirmed that, in very rare circumstances, when a mobile two-way radio (licensed for use by emergency personnel, under specific regulatory conditions and frequencies in Canada) is operated in the vehicle, the engine speed (RPM) can fluctuate or vary mildly, and for a short period of time. This can occur when the mobile handheld two-way radio is operated in the "transmit" mode at certain specific frequencies in very close proximity to the Rostra accessory cruise control unit under the dashboard. Also, if this condition does occur, the driver may notice a Malfunction Indicator Lamp (MIL) on the dash and a reduction of engine power, allowing the operator to drive the vehicle to the shoulder or another safe location. Toyota has confirmed that this problem is not due to a defect in the vehicle's original equipment nor does this condition occur where a handheld Citizens' Band (CB) radio, cell phone or Bluetooth transceiver is operated in the vehicle. THE YARIS'S LEFT DRIVER SIDE DOOR WAS COLLAPSED INTO THE YARIS CABIN WITH ENOUGH FORCE TO BREAK MY SAMSUNG NOTE 4 PHONE DISPLAY & DAMAGE ITS INTERNAL SOLID STATE DRIVE. THE SAMSUNG NOTE 4 WAS ENCASED IN AN OTTER BOX AND ATTACHED TO MY LEFT HIP VIA OTTER BELT CLIP DURING THE TIME OF ACCIDENT. MY HEAD STRUCK A HARD SURFACE IN THE YARIS, RESULTING IN A LARGE HEMATOMA ON MY LEFT FRONTAL LOBE. TOYOTA INVESTIGATED AND FOUND THAT WITHIN THE SPAN OF 68 MILLISECONDS, THE LATERAL DELTA-V AIRBAG SENSOR WAS ACCELERATED FROM 0.0 MPH TO 11.1 MPH. YET THE SIDE AIRBAG DID NOT DEPLOY. I would like to point out that both Driver and Police Officer safety may be improved if all vehicle designers and manufacturers were to disrupt the convention of locating caches for the drivers vehicle registration information at or below driver shoulder height. It may be a fairly inconsequential to implement a design standard so that the paper copy of a drivers registration and insurance would be neatly and preferentially stored at a height relativistic of the drivers shoulders so that every driver would naturally move through the motion of raising their hands into the air to retrieve requested registration information

Malfunctioning components 1. Door locks do not lock all the time when the lock button is pressed. 2. Randomly I will find my doors unlocked, when I closed them when I got out. 3. Doors unlock by themselves and that poses a problem for me and my family. To be able to figure out the issue I have to take it to the dealer where they charge an arm and a leg to diagnose the issue, due to Covid-19 and the hardship it has caused my family I don’t have the funds to do that. Somebody has to push the dealers to help the consumers out for a change. Thank you.

THE YARIS'S LEFT DRIVER SIDE DOOR WAS COLLAPSED INTO THE YARIS CABIN WITH ENOUGH FORCE TO BREAK MY SAMSUNG NOTE 4 PHONE DISPLAY & DAMAGE ITS INTERNAL SOLID STATE DRIVE. THE SAMSUNG NOTE 4 WAS ENCASED IN AN OTTER BOX AND ATTACHED TO MY LEFT HIP VIA OTTER BELT CLIP DURING THE TIME OF ACCIDENT. MY HEAD STRUCK A HARD SURFACE IN THE YARIS, RESULTING IN A LARGE HEMATOMA ON MY LEFT FRONTAL LOBE. TOYOTA INVESTIGATED AND FOUND THAT WITHIN THE SPAN OF 68 MILLISECONDS, THE LATERAL DELTA-V AIRBAG SENSOR WAS ACCELERATED FROM 0.0 MPH TO 11.1 MPH. YET THE SIDE AIRBAG DID NOT DEPLOY. PER TOYOTA, SINCE THE SIDE AIRBAG SENSOR WAS DEEMED IN PROPER WORKING ORDER AND DID NOT DESIGNATE THE COLLISION WARRANTED DEPLOYMENT OF AIRBAGS, EVERYTHING FUNCTIONED NORMALLY. WHEN QUERIED WHAT THRESHOLD WAS REQUIRED TO DEPLOY THE AIRBAGS (NOTING THE INSTANTANEOUS LATERAL ACCELERATION OF A 2300+ LBS VEHICLE FROM 0 MPH TO 11 MPH WAS INSUFFICIENT), TOYOTA INSISTED I HIRE MY OWN ENGINEERS TO RESEARCH.
