Desiccated Air Bag Inflator Rupture

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. BMW of North America, LLC (BMW) is recalling certain model year 2016 X5 xDrive40e vehicles manufactured July 29, 2015, to September 9, 2015. The affected vehicles have a Digital Motor Electronic (DME) control unit that may have a loose electrical ground cable, possibly causing an under-voltage condition which would result in the transmission shifting to neutral.

Risk: If the vehicle unexpectedly shifts to neutral, there would be an increased risk of a crash.

Label

Issue: On certain vehicles, the tire and loading information label does not list that there is no spare tire in both official languages. The label reads as "NONE" when it should read as "NONE / AUCUN". Canadian regulations require the information to be listed on the label in both English and French. Safety Risk: This error does not affect vehicle safety. Corrective Actions: This is not a recall. The company has corrected the tire and loading information label for all new vehicles.

Engine

Issue: On certain vehicles, fluid from the positive crankcase ventilation (PCV) system could leak on the intake air hose. As a result, the fluid could enter the electrical connector for the water pump and cause a short circuit. Safety Risk: A short circuit can create the risk of a fire, even while the vehicle is parked and turned off. Corrective Actions: To reduce the safety risk, BMW advises you to park your vehicle outdoors and away from other vehicles or structures until the recall repairs are completed. BMW will notify owners by mail and advise you to take your vehicle to a dealership to inspect and, if necessary, replace the water pump and electrical connector. The dealer will also install a shield on the connector.

Airbag

Issue: This recall applies to vehicles that may be equipped with a replacement sport or M-sport steering wheel that has a Takata airbag. Long-term exposure to high absolute humidity and temperature, combined with high temperature cycling, may eventually degrade the propellant contained in the driver-front airbag. This could cause the airbag to deploy with more force than normal. If the airbag inflator ruptures, fragments could be propelled toward vehicle occupants or cause damage to the airbag assembly, preventing its proper function. Safety Risk: If the vehicle is involved in a crash requiring an airbag deployment, the airbag inflator could rupture and create a risk of injury. Corrective Actions: BMW will notify owners by mail and advise you to take your vehicle to a dealership to inspect and, if necessary, replace the driver-front airbag module. Issue: On certain vehicles, the exhaust gas recirculation (EGR) cooler could crack internally and leak. If this happens, you may notice the following symptoms: a warning symbol in the instrument cluster displaying a loss of coolant, reduced engine power, an unpleasant exhaust odour, an unusual noise from the engine and/or smoke from the engine compartment. Note: This recall replaces Transport Canada Recall no. 2018-587. Safety Risk: A cracked EGR cooler could create the risk of an engine fire. Corrective Actions: BMW will notify owners by mail and instruct you to take your vehicle to a dealer to replace the EGR cooler. The dealer will also inspect the intake manifold, and replace it, if necessary.

Issue: On certain vehicles equipped with a diesel engine, the high-pressure fuel pump may wear prematurely. This can cause reduced engine performance and could lead to a sudden engine stall. Note: A warning lamp on the instrument cluster will turn on if the fuel pressure drops. Safety Risk: A loss of engine power could increase the risk of a crash. Corrective Actions: BMW will notify owners by mail and instruct you to take your vehicle to a dealer to replace the high-pressure fuel pump.

This recall was replaced by Transport Canada Recall No. 2021-711. On certain vehicles, the cooler for Exhaust Gas Recirculation (EGR) module may develop an internal leak and coolant could mix with diesel engine soot/sediment. In rare circumstances, this condition could result in smouldering particles and lead to the melting of the intake manifold, increasing the risk of a fire causing injury and/or damage to property. Correction: Dealers will inspect the EGR cooler, and if necessary, replace it. If it is determined that the EGR cooler has leaked internally, the intake manifold will also be replaced. The EGR pipe connecting the cooler to the manifold will be cleaned. Note: An operator may be alerted to this condition by one or more of the following symptoms: A warning symbol in the instrument cluster displaying a loss of engine coolant, a reduction in engine power, an unpleasant odour (exhaust gas), an unusual noise from the engine compartment, and/or smoke from the engine compartment.

During a recent auto collision my airbags did not deploy. I swerved to avoid another vehicle entering the intersection from my left, he hit my rear left driver's side quarter panel and I subsequently hit a tree. This was noted by the paramedics first on scene as well as by the firemen and police when they arrived. I have notified BMW of North America, USAA insurance (my insurance company), and now you. The vehicle is a total loss and was towed to Leesburg Virginia by roadrunner wrecker service.com

I PARKED MY X5 ON AN INCLINE, PUT THE CAR IN PARK, SET THE PARKING BREAK AND TURNED THE VEHICLE OFF. LOCKED IT AND WENT INSIDE. THERE WERE WORKERS OUTSIDE FOR ABOUT 10 MINUTES BEFORE COMING IN TO THE HOUSE THAT WITNESSED IT SITTING THERE FOR THAT AMOUNT OF TIME. AFTER BEING INSIDE FOR ABOUT 5 MORE MINUTES OR BETTER, I WENT TO LEAVE AND FOUND THAT MY CAR HAD ROLLED AND HIT A TREE. THE PARKING BREAK WORKED SEEING THAT THERE WAS SNOW BUILT UP ON THE BACK TIRES, HOWEVER, YOU CAN SEE FROM THE PICTURES THAT BOTH FRONT TIRES ROLLED. BMW LOOKED INTO IT AND SAID THE CAR DIAGNOSTICALLY CHECKED OUT FINE AND THAT IT IS NORMAL FOR BOTH FRONT TIRES TO ROLL WHILE IN PARK???

SERVICE BRAKES

I STOPPED AT THE ENTRANCE OF AN APARTMENT COMPLEX AND WAS WAITING FOR THE SECURITY GATE TO OPEN. ONCE I CAME TO A COMPLETE STOP THE AUTO START STOP FUNCTION TURNED THE ENGINE OFF. WHILE STOPPED, WITH MY FOOT STILL ON THE BRAKE, SUDDENLY AND UNEXPECTEDLY, THE ENGINE STARTED AND THE VEHICLE TOOK OFF STRIKING THE SECURITY GATE AS IT WAS OPENING. THE BRAKES DID NOT WORK. NO AIRBAG DEPLOYED EITHER.

2016 BMW X5 xDrive40e. Experiencing intermittent/sudden drivetrain malfunction warnings. Vehicle loses power or enters limp mode while driving. Max eDrive / electric drive becomes disabled even with battery charged. Multiple warning lights appear. Issue appears related to the hybrid system (possible EME, SME, Safety Box, or high-voltage battery faults). This creates a safety risk because power can be lost unexpectedly while moving. Mileage at time of most recent occurrence: 126,500. Problem first noticed 118,000.

The contact owns a 2016 BMW X5 PHEV. The vehicle was serviced under NHTSA Campaign Number: 24V608000 (ENGINE AND ENGINE COOLING, ELECTRICAL SYSTEM), but the failure persisted. The contact stated that a month after the repairs were completed, the coolant connector would stay on and drain the battery. The contact stated that after the vehicle was turned off at night, the coolant connector overheated and the battery drained. The contact needs to jump-start the battery every day. The vehicle was taken to the dealer, and the dealer refused to repair the vehicle, stating that it was already repaired. The contact stated that the coolant connector damaged the EME (Electrical Machine Electronics). The dealer confirmed that the problem may be EME. The vehicle remained at the dealer unrepaired. The contact stated that the failure is worsening. The manufacturer was not made aware of the failure. The failure mileage was 114,865.

The component that failed appears to be the hybrid propulsion and charging system, including the high-voltage battery management system and/or integrated charging electronics. The vehicle will not accept a charge from any home or public charging source. The high-voltage battery was replaced approximately one year ago; however, the charging failure persists. The vehicle is available for inspection upon request. In addition to the charging failure, the vehicle displayed drivetrain malfunction and start/stop malfunction warnings. While driving on the highway at normal speed, the vehicle lost power and stopped, creating a hazardous situation in active traffic. This sudden loss of propulsion placed my safety and the safety of other drivers at risk, as surrounding vehicles were traveling at highway speeds. Prior to the failure, warning messages appeared intermittently, including charging system faults and start/stop malfunction alerts. These warnings began approximately one year ago around the time the charging issue started. The vehicle was previously serviced and the high-voltage battery was replaced; however, the underlying issue was not corrected. The root cause is UNKNOWN, but the symptoms suggest a failure within the hybrid control system, charging module, DC-DC converter, or related electrical components. The failure has been persistent and unresolved. I am concerned that defects within the hybrid propulsion system may lead to additional sudden loss of power events.

ELECTRICAL SYSTEM

Subject: Consequential Damage and Failure of Remedy regarding Recall 24V-608 Incident Description: "I am reporting a failure of the manufacturer to provide an effective remedy for Safety Recall 24V-608 (Electric Water Pump) and subsequent consequential damage to the High-Voltage battery system. In September 2023, at approximately 78,000 miles, this vehicle exhibited 'Drivetrain Malfunctions' including Fault Code 0x222626 (EME: internal fault - active short circuit requested) and 0x22271C (Overvoltage in intermediate circuit). The BMW dealer (Fields BMW) failed to diagnose the root cause and instructed me to 'keep driving'. Since the release of Recall 24V-608, it is clear these 2023 faults were early indicators of the coolant pump short-circuiting. This persistent electrical instability has now resulted in a permanent failure of Cell Supervision Circuit #3, causing the high-voltage contactors to open and leaving the vehicle disabled. The dealer is currently attempting to charge for diagnostics and battery removal. I am asserting that the lack of a timely remedy for the coolant pump (which was only recently shielded per the recall) caused a power surge that 'fried' the internal battery circuitry. This is a safety issue as the vehicle experienced a sudden loss of propulsion while in motion. I request that NHTSA investigate BMW’s refusal to cover battery failures caused by the electrical interference of the recalled coolant pump."
