FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

2023 Honda CR-V, purchased new in Mar 2023 from Friendly Honda, Poughkeepsie, NY. All regular service completed at Lia Honda, Brewster, NY. On [XXX], car was driven without incident and parked inside a garage. On [XXX] upon ignition, car immediately displayed a “christmas tree” of warnings for every major system, including but not limited to: Electric Power Steering, AWD, Adaptive Cruise Control, Collision Mitigation, Hill Descent, Automatic Emergency Braking, Lane Keeping Assistance. Behavior has continued every time the car is started. Car remains drivable, but all safety and driver assistance systems are disabled. In addition, on [XXX] and [XXX], after stopping at a stop sign, car had very limited power and could only accelerate to 30 mph on a flat road and 20 mph on an incline. Problem was resolved only by coming to a complete stop then accelerating again. This is a significant safety concern, as if this were to happen in stop-and-go traffic on a highway, the car would be unable to accelerate to highway speeds. On 6/20/26, I took the car to Lia Honda in Brewster, NY for diagnosis. They reported a failed Electric Power Steering Control Unit/Steering Rack (trouble code U0420-68). They explained that this single electrical component failure also caused all other components to report failures because they are connected in a ring. Estimated repair cost $4,595.98. I have filed a complaint with Honda to request out-of-warranty coverage. Car is ≈3 years old, 3 months out of warranty, and only lightly used at 17,484 miles. Note that this car falls under NHTSA recall 24V-744, which pertains to a manufacturing defect in the worm gear of the Electric Power Steering. We had the recall service completed on 12/26/24, but it seems possible that continued friction in the worm gear may result in overloading of the EPS control unit. 2024 Honda Cr-V Hybrid Boston, MA

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

While driving under normal conditions on a dry road (no snow or ice), the vehicle experienced a sudden loss of braking performance while in motion. The vehicle was traveling approximately 10–15 mph and approaching an intersection with a red traffic light. A warning message appeared stating: “Brake system problem. Brake performance may be reduced. See your dealer.” Immediately after this message, braking performance was significantly reduced. Multiple attempts to slow the vehicle using the brake pedal resulted in minimal deceleration. The stopping distance was much longer than expected for the speed and conditions. The vehicle only came to a stop after the brake pedal was pressed with full force. Shortly after the initial brake warning, multiple additional safety-related systems went offline, including collision avoidance and other braking-related driver assistance systems. Approximately five to six warning messages appeared, indicating a cascading failure of safety systems while the vehicle was still in motion. The driver was able to slowly maneuver the vehicle to the side of the road and park safely. No collision occurred. However, shortly before this incident, the vehicle had been driven on a highway. If the same failure had occurred at higher speeds or in traffic, it could have resulted in a serious crash or injury. The vehicle is relatively new (approximately two years old, low mileage) and has been routinely maintained. There were no prior warning signs before this event. The vehicle is currently available for inspection at a dealership. A preliminary dealer explanation suggested a possible low or discharged battery; however, the cause of the braking failure has not been clearly established. This incident involved loss of braking performance while the vehicle was in motion, presenting a significant safety risk.

2021 Honda Cr-V Hybrid Winston Salem, NC

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

While traveling northwest at 37 mph, the automatic brake system engaged jolting the driver. The camera shows no objects in front of but there were two vehicles traveling southeast in the opposite lane prior to the brakes applying. There were shadows from trees. If there was a vehicle behind, and the driver did not react fast enough, this would have been a rearend collision. This is the third time that the automatic brake system engaged for no reason since we have owned this vehicle (Dec 2021) and each time it has been reported to the NTSB. In the past two cases, there were no vehicles or shadows from trees, since we were in the desert. The previous two times, the vehicle was inspected by the dealership with no defect noted (under warranty). There were no dashboard warning notices indicating an issue. The vehicle will be resubmitted to a dealership for inspection for this third incident (not under warranty).

2022 Honda Cr-V Hybrid Baton Rouge, LA

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

on [XXX] around 5 PM, when driving below 30 miles per hour within a residential community, automatic braking was activated without any obstruction in front of the vehicle, leading to sudden decrease in speed. Vehicle is in Baton Rouge, LA and all service/maintenance were done according to recommended schedule by Honda at the authorized dealership where the vehicle was purchased. Through online search, I found NHTSA opened an investigation for the same problem. If possible, I would appreciate an update about the status of NHTSA investigation and what action I should take. 2024 Honda Cr-V Hybrid at 100 miles Placida, FL

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL

The contact owns a 2024 Honda CR-V Hybrid. The contact stated that while driving at various speeds with the Adaptive Cruise Control activated, the braking system erroneously activated, causing the vehicle to abruptly slow down and decelerate. The contact stated that the failure had occurred on several occasions with a second vehicle in next lane slowing down. The vehicle was taken to the local dealer who was unable to duplicate the failure. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was 100.

2022 Honda Cr-V Hybrid MC Calla, AL Crash 1 injured

LANE DEPARTURE: BLIND SPOT DETECTION

As I was backing out of a driveway the car suddenly accelerated in reverse. The brake did not stop or slow the vehicle. As a result the car's right fender hit a large oak tree. There was damage to the fender, the door and to the undercarriage. In addition, my wife who was sitting in the passenger seat was thrown back and she struck her head on the top seat belt connection. She had a cut in the back of her head that required an emergency room visit to the local hiospital. They did a CAT scan and they used seven staples close the wound. There were several messages flashed on the dash that indicated brake and cruise control issues. These messages wer of very short duration and could not be recazlled.

2025 Honda Cr-V Hybrid Pittsburg, CA Crash 1 injured

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

I was parking in our apartment's parking lot when I attempted a left turn to pull in front-facing toward the wall. Within seconds, and without understanding how or why, the car continued forward and hit the wall. Initially, I reported to the insurance company that I either didn’t brake or found the brake too late. However, the truth is—I do not clearly remember what happened. My mph had to be low. When the car hit the wall, several emergency systems were triggered: an automatic emergency call was made, collision alerts sounded, and the brakes activated. During the collision, my left arm was burned by the steering wheel where the Honda logo is located. The burn left a visible imprint of the Honda logo on my skin. Only some of the airbags deployed: the steering wheel and right-side window airbags were activated. However, the driver's side window airbags did not deploy. My concerns and complaints include: 1. The failure of the driver’s window airbag to deploy 2. The burn on my arm caused by the steering wheel’s Honda emblem 3. The inconsistent performance of the emergency braking system, which only works sometimes

2025 Honda Cr-V Hybrid Long Beach, CA

BACK OVER PREVENTION: REARVIEW SYSTEM BRAKING

Rear Traffic Alert and e-braking not working or working intermittently. Vehicle is supposed to alert you to cross traffic and does not...Maybe works 1 out of 6 occurences. When you place the vehicle in reverse and proceed backwards, the rear of the vehicle appears on the screen but, does not alert you when a vehicle is approaching on the street that you are backing onto. Does not matter if it the vehicle comes from the left or right. Same can be said for pedestrians or bicycles - which it is supposed to detect. This has resulted in emergency stops nearly striking vehicle or bicyclist. This safety feature is pretty much useless and as such have turned it off. I researched this issue on reddit and various forums. It appears to be common in 2023-2025 Honda CRV Hybrid Sport L. We owned a 2019 Honda CRV with the same cross-traffic alert and it worked very reliably. Not the case for our brand new 2025 CRV Hybrid

There have been several times where my forward collision warning goes off when I am just driving along an open highway with no other vehicles nearby and no precipitation. In broad daylight. Another time, a car in front of me slammed on their brakes. There wasn't enough time for me to come to a complete stop, so I navigated my car to the shoulder (I was only going about 30 mph to begin with). However, while I was turning the steering wheel, the Automatic Emergency Braking system engaged, making it impossible for me to turn the steering wheel further. I missed the car in front of me by only and inch or two. If I would have hit that car, it would have been because the AEB system locked up my steering wheel and made it impossible to control my vehicle.

2021 Honda Cr-V Hybrid Hillsborough, NJ

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

I was driving the vehicle on route I287 and all of a sudden the break system got activated without touching any button and the car speed was damped down and as a result the car was pulled aside to see what had happened. The break system completely collapsed with a break sign on the dashboard appearing persistently which could not be deactivated or was not able to remove from the dashboard with several attempts made in restarting the engine. Due to the involuntary break activation on highway driving, my life as well others would have been in extreme danger and jeopardized the whole situation in continuing driving further. The car was towed on a flat bed to the the dealer The Honda Princenton, New Jersey for further investigation and got analyzed for the root cause and they suggested a fix of head gasket replacement, a major repair, asking us to fix the vehicle at the cost of 6000 dollars. Previously, I had driven several Hondas and never experienced break issues on the cars and we had invested in buying this time the Honda CR-V and what we went through this vehicle something like this is not fair for the customer has to go through a major repair on the vehicle at such a low milage of 127,000 put on the car which never was involved in an accident and driving the car on high way every day to the work place. We want a complete restoration of the vehicle at free of cost and get it certified and drivable safely on the road.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL

On highway, engaged cruise control at 70 mph. Engine light came on, simultaneously with loss of power (pressing on gas pedal did nothing) and Apple CarPlay disconnect. We were near an exit and managed to drift off, at lower off-ramp speed engine again could respond to accelerator however was chugging very badly. Went to nearby oil change place. Mechanic there advised going straight to dealer a short distance further. Drove on to the Honda dealer, car still chugging and stopped the car until I could talk to the service person. When the car restarted it was no longer chugging. Service person diagnosed the following and said i basically needed a new engine: TECH SCANNED VEHICLE AND FOUND CODES P0300 RANDOM CYL MISFIRE, P0302 CYL 2 MISFIRE. TECH CHECKED LIVE DATA AND HAD VERY LITTLE MISFIRE COMING FROM CYL 3, PULLED ALL COILS AND PLUGS AND FOUND CYL 2 AND 4 HAD THE INJECTORS STILL SPRAYING WITH THE VEHICLE OFF INDICATING FAILURE OF THE INJECTORS IN THOSE 2 CYLINDERS, IN CYLINDER 3 TECH FOUND EXCESSIVE OIL BURNING ON THE PISTON, INDICATING OIL LEAKING INTO THE COMBUSTION CHAMBER OF THE MOTOR.

El volante al girar a lado del pasajero rechina al igual cuando va de reversa, fui con el comerciante BIG O TIRES por porque el ruido es bastante (esa fue la primera vez) no le encontraron nada, pensé que era falta de líquido de frenos, pero después de hacer el chequeo y revisar que estuviera bien igual siguió con el chirrido, luego lo lleve por segunda vez allí mismo a BIG O y encontraron que era un problema con la parte del volante que conecta con el rack and pinion, que lo tenía que llevar al dealership, entonces en eso ando. El parking collision no funciona, con objetos sin movimiento como un carro estacionado atrás, por lo cual si no se pone atención puede causar un accidente.
