The contact stated that while driving approximately 30 MPH, the Forward Collision Avoidance system failed to activate as needed and the vehicle nearly crashed into the rear of another vehicle. The contact stated that during the failure, the vehicle in front struck a pedestrian, and the pedestrian went airborne and landed on the hood of the contact's vehicle. The contact stated that the Forward Collision Avoidance system failed to immediately activate, and the brake pedal was depressed to avoid crashing into the rear of the vehicle ahead. The pedestrian sustained unknown injuries and was transported to the hospital. The contact mentioned that the vehicle was taken to a dealer on several occasions for the seat belts failing to latch and secure the occupants. The dealer mentioned that there might be crumbs preventing the latch however, the contact mentioned that theory was not possible for the driver's seat belt. The failure was present with the seat belts in the second and third row seats. The cause of the failure was not yet determined. A Police report was filed at the scene. Front driver seat belt does not retract and has caused damage to door. Vehicle is available for inspection. We are worried that the seatbelt will not function properly in an accident since is does not function properly now. No confirmation by dealer, going in next week for service, however, dealer representative did say on phone that Mitsibishi s don't have seat belt problems??? First noticed problem last winter when I noticed the dents in the door frame being caused by non retracting seatbelt.

The seat belts are much slower to retract than with other cars, including our 2019 Mitsubishi Outlander we also own. My wife has had the front passenger seat belt get stuck in the door at least 3 times (likely more) which has required us to make multiple stops at the dealership to fix it. This is certainly a safety risk since the door remains partially open this certainly is something that should be recalled and fixed. Most recent day was today 5/7/26 but has happened many times.

The seatbelt retraction system failed to retract fast enough, which caused the seatbelt to become jammed in the passenger door. This resulted in damage to the passenger front and back door and opening of the passenger door. Yes, the vehicle and the affected components (seatbelt assembly and passenger door) are available for inspection upon request. 2. How was your safety or the safety of others put at risk? The slow seatbelt retraction created a door jam condition, which could have prevented or delayed exit from the vehicle in an emergency situation. This posed a safety risk to the passenger, as the door could not open or close properly. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? The issue has been observed by Glassman of Southfield Michigan the service men was not interested at all. In fact when I arrived to have the vehicle inspected the driver seatbelt did not retract back at all and the service guy stated, I should pay attention when unfasten my seatbelt. The guy at the collision shop stated that the seatbelt on the passenger side was not retracting back as it should. I have had two incidents with the passenger seatbelt becoming jammed in the door since I leased the vehicle in August of 2025. 4.Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others. The contact stated that the microscrews used to secure the seat belt buckle had failed. The cause of the failure was not determined. The contact stated that a clicking noise was present while opening and closing the front driver’s side door. Upon inspection, the contact discovered that the front driver’s door check arm bolts had become unsecured. The contact tightened the bolts, and the failure was remedied. I have a safety recall on my vehicle. Mitsubishi says I will have to dry from oklahoma to Arkansas because they no longer have places in Oklahoma that will fix a recall. But there are many dealerships that are selling Mitsubishis. Those dealerships also tell me I would have to take it to Arkansas. This is a huge problem, I bought this car brand new and unable to get recalls fixed on it unless I drive to another state. This is awful. I’m a nurse and work a lot I don’t have time to dry to Arkansas and have my car worked on. Not only that time, gas and mileage. That’s not fair at all to me. They should have to have a center of some sort in Oklahoma to fix these recalls if they are going to be selling new Mitsubishis here in Oklahoma.

I have had multiple issues with this SUV. The main issue I am writing this for is because the car was hit stopped at a red light by another car going over 40 mph. The car rolled back then slammed into my car again going just as fast. The airbags did not deploy and the seatbelts did not lock. The impact actually broke the seats the passenger and I were sitting in. There were extensive injuries that happened because of this. The SUV was deemed a total loss because of the extensive damage done to the car. There were no warning messages, no recalls, etc. The SUV was inspected by a mechanic and insurance company and both said that the airbags should have gone off and the seatbelts should have locked based on the amount of damage to the SUV. I also took this SUV into the Mitsubishi dealership for all services/maintenance and nothing ever came up about any issues.

AFTER ARRANGING FOR PEOPLE TO MOVE AROUND TO THE OTHER SIDE (LEFT-HAND SEAT) FOR A WHILE, I FINALLY QUIT DRIVING TO DEAL WITH THE ISSUE MYSELF. AFTER NUMEROUS ATTEMPTS TO REPAIR THE BUCKLE, I GAVE UP AND STARTED CALLING AROUND FOR THE PART--BUT COULD NOT FIND IT. FINALLY I NOTICED A RECALL NOTICE ONLINE FOR THIS EXACT SAME ISSUE IN 2014-2019 MITSUBISHI OUTLANDERS. ON MONDAY (APRIL 26) I CALLED SCOTT ELDER MITSUBISHI IN CEDAR PARK, TEXAS TO SCHEDULE THE RECALL REPAIR, BUT WAS TOLD THAT MY VIN NUMBER IS NOT COVERED UNDER THE RECALL. THEY ALSO DID NOT HAVE THE PART AND COULD NOT OBTAIN ONE FOR AT LEAST A WEEK. I THEN CALLED THE MANUFACTURER DIRECTLY TO LEARN WHY MY VEHICLE IS NOT COVERED FOR THE RECALL EVEN THOUGH IT IS A 2019 AND HAS THE EXACT SAME PROBLEM AS DESCRIBED IN THE RECALL NOTICE. THEY COULD NOT GIVE ME A SATISFACTORY ANSWER, NOR ANY ALTERNATIVE RECOURSE TO RESOLVE MY ISSUE. AS STATED ABOVE, I AM A RIDESHARE DRIVER. I CANNOT DO MY JOB WITH AN UNUSABLE SEAT BELT. SWITCHING PASSENGERS TO THE OTHER SEAT WON'T ALWAYS WORK BECAUSE I OFTEN PICK UP 3 OR MORE PASSENGERS.
