Detailed Description of the Safety Failure: Improper Calibration Environment: The technician attempted to perform the headlight alignment procedure inside a closed residential garage with approximately 12 inches of clearance between the front bumper and the garage door. Standard automotive and manufacturer procedures require either a level 25-foot standoff from an aiming target or a dedicated optical aiming tool. At 12 inches, optical beam geometry and cutoff steps cannot be evaluated. Extreme Downward Misadjustment: The technician bypassed standard cutoff alignment and manually adjusted both headlights down to their absolute lowest mechanical limit using the vehicle interface. Safety Impact: The usable beam throw is now restricted to only a few feet in front of the vehicle's bumper. When cruise control is engaged on a Tesla (all models to my knowledge) the Auto Wipers automatically come on and then randomly wipe. There is no option to disabled the Auto Wipers, as you can only disable by manually selecting control panel and then turn off each time you engage cruise control. This creates a significant risk to driver safety as I have had numerous instances of wipers randomly starting (without any rain) and smearing the windshield. This forces me to look away from the road and go through a manual disable every time I use cruise control. NOTE: this is a software default that cannot be removed

What component or system failed or malfunctioned, and is it available for inspection upon request? The front windshield (Acoustic Glass) failed due to a spontaneous stress fracture. The crack originated from the edge of the glass without any external impact. The vehicle has been repaired, but the service records and high-resolution photos of the crack are available for inspection upon request. How was your safety or the safety of others put at risk? The spontaneous crack occurred suddenly during a heatwave, significantly obstructing the driver’s forward visibility. Furthermore, the crack expanded rapidly during a subsequent rainstorm and temperature change, compromising the structural integrity of the windshield and potentially interfering with the front-facing ADAS cameras (Autopilot/Safety features) located behind the rearview mirror. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. The problem was formally inspected and confirmed by the Tesla Burlingame Service Center. They issued a written evaluation report stating the damage was a "Stress Impact" (a manufacturing/structural defect) rather than a rock chip. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Yes, the component was inspected by the manufacturer (Tesla Service Centers in both Burlingame and San Francisco). I am writing to file a formal safety complaint against Tesla, Inc. regarding a software update failure that has disabled legally required vehicle functions on my Tesla for over two years. For more than 26 months, my vehicle has been unable to complete a software update through Tesla's over-the-air (OTA) update system. As a direct result of this failure, my vehicle's display screen does not function for the first several minutes of every drive. This prevents me from accessing my speedometer, turn signals, and other legally required safety features, rendering the vehicle non-compliant with federal and state motor vehicle safety standards during this period. I have made multiple documented attempts to resolve this issue through Tesla's customer support channels over the course of these two years. Despite these efforts, the issue remains unresolved. Tesla has now informed me that resolving the defect will require a paid service visit — despite the fact that the failure originates within Tesla's own software update infrastructure, not from any action or negligence on my part. I am requesting that NHTSA investigate this matter on the following grounds: 1. The software defect disables legally required vehicle safety functions, including the speedometer, turn signals, hazard lights, and more. 2. The defect has persisted for over two years despite repeated good-faith efforts by the vehicle owner to resolve it through the manufacturer. 3. Tesla's OTA update system is manufacturer-controlled infrastructure. A failure within that system constitutes a manufacturer defect, not a maintenance issue. 4. Tesla is requiring payment from the consumer to correct a defect in their own systems. I believe this issue may not be isolated to my vehicle and could affect other Tesla owners experiencing similar OTA update failures. I respectfully request that NHTSA investigate whether a broader safety defect exists and whether a recall or corrective action is warranted.

My Tesla Model 3 was brought to Tesla Princeton Service Center on February 27, 2025 for a 12V battery replacement. During the service visit, Tesla also replaced the rear glass, citing the rear defroster as the root cause of the battery drain — a repair that was added without prior discussion and increased the total invoice from an original estimate of $180 to a final charge of $1,730. Upon picking up the vehicle, the rear backup camera, blind spot cameras, navigation system, Sentry Mode, and windshield wiper sensor were all non-functional. None of these systems showed any malfunction prior to the service visit. Tesla Princeton’s service advisor denied responsibility without performing diagnostics, and in a subsequent message admitted he could not determine the cause with certainty without diagnostics — which have still not been performed. HVAC system went out at 7500ish miles. It was repaired under warranty, and a "fix" was administered through software by tesla. My HVAC has gone out again at 84k miles FOR THE SAME REASON. about 77k miles after it was just repaired. Quick check online. this is still a very common issue. No heat creates a significant safety hazard due to lack of the ability to defrost the vehicle or frost building up on cold days.

This car uses auto-dimming rear view and side mirrors. However, the rear view mirror is only able to be dimmed automatically, or be disabled in the settings. The issue is that the dimming is not based on a light sensor at all, it is merely timed to dim around 30 minutes after sunset and return to normal 30 minutes before sunrise. I have confirmed this timing on many drives and it is repeatable. The safety concern is that there is no way to manually dim the rear view mirror, which is a big issue around sunrise and sunset when cars headlights turn on before the mirrors dim. This is especially an issue with larger trucks behind a sedan like my Model 3. The only option I have to fight the blinding glare from headlights is to manually move my mirror so that I can no longer see out of the rear window, which makes lane changes much more dangerous.

I am submitting this complaint to report a safety issue with the turn-signal buttons on my steering wheel of the 2024–2025 Tesla Model 3 Highland. The turn-signal buttons are located on the steering wheel surface instead of a traditional stalk. When the steering wheel is turned at any angle, the buttons are difficult to locate by feel, often requiring the driver to look down to activate them and take his eyes off the road. Trying to signal during turns, lane changes, or roundabouts is not intuitive, and I have nearly had several collisions. This design presents a potentially dangerous safety hazard and could increase the risk of collisions. Tesla should be required to recall and retrofit these vehicles with turn signal stalks on the steering wheel column.
