The contact owns a 2025 Toyota 4Runner. The contact stated that the message “Headlights On” was erroneously displayed. The fuel gauge indicated 71 percent. Additionally, the 12-volt charging system malfunctioned with the message “See the Dealer” displayed. Several unknown orange warning lights were illuminated. The vehicle was restarted, and the warning lights turned off. The window on the passenger’s side was violently shaking. The vehicle was taken to the local dealer, who was unable to determine the cause of the failure, but an undisclosed software was reprogrammed. The vehicle was taken back to the local dealer, where it was diagnosed and determined that the fuel pump was replaced. The vehicle was repaired. The dealer was contacted and informed about a diagnostic test. The manufacturer was contacted, and a claim was filed. The failure mileage was unknown.
NHTSA complaint #11751751 — an unverified report by a vehicle owner.
We were traveling north on hwy 23 in southern Minnesota pulling a 5000 lb trailer in the rain and it was windy. When the road split to 4 lane I stepped on the gas to speed up from 55 to 65 and the vehicle studderd and lost all power. Many warning lights cam on and I think it said stop within .3 miles. We pulled to the side of the road and basically coasted to a stop. It was on hybrid power. We called the nearest Toyota dealership and they suggested restarting the vehicle and see if we could make it to them. The engine did restart still with all the warnings on and we were able to drive it to the dealer being really gentle with the throttle. They pulled codes and it was intake related. They removed the air filter and it was dripping wet. The raised intake ingests water when it's raining and if it's working hard enough it pulls it through the filter into the intake and causes the vehicle to suddenly lose power which could possibly cause a accident. I am also concerned about water pulling dirt with it into the motor and causing damage. I have driven the vehicle in the rain since this happened but not pulling a trailer and really working it and so far it hasn't shut down on me yet but I've pulled the filter and it was wet. I have brought this issue to my dealer but they said they are not aware of a problem. I will attach a video of the wet filter I pulled from the air box after driving in a rain storm and a video of the warning lights that appeared. I don't think the dealership I took it too has any records of it because it was just after closing and they used a handheld scanner. They just dried our filter off and cleared the codes.

At approximately 520 miles, while performing a low speed left hand turn, the vehicle experienced a sudden mechanical "thunk" in the drivetrain followed by an immediate loss of motive power (Limp Mode) in my new 2026 Toyota 4Runner TRD Pro. The engine revved significantly, but the vehicle failed to accelerate as intended, creating a hazardous situation in the intersection of oncoming traffic. A diagnostic health check at an authorized dealer confirmed stored DTC fault codes P281671 and P08BA71, indicating that Pressure Control Solenoids Actuators "H" and "L" were in a "stuck" state. The vehicle's Freeze Frame Data recorded a catastrophic internal transmission slip, showing an engine speed of 1767 RPM while the vehicle was moving at only 4 MPH in 2nd gear. There are no previous warning signs or symptoms of this problem before the transmission failure. It was sudden and unexpected. The manufacturer’s technical service protocols (such as T-SB-0094-24/T-SB-0076-24) for these specific solenoid codes on this specific 8-speed hybrid transmission (L580F) – which is equipped on my vehicle – identify this as an internal mechanical actuator failure requiring full transmission, torque converter, and hybrid module replacement. However, the manufacturer has denied the repair because the vehicle’s specific VIN is not yet listed on a service bulletin and that they could not reproduce it in a short test drive. The manufacturer’s official recommendation is to continue operating the vehicle with a documented and non-repaired powertrain defect until a second failure occurs in traffic. I am reporting this as a significant safety risk, as a recurring loss of motive power during critical driving maneuvers puts myself and other motorists in danger. The vehicle and its stored diagnostic data are available for inspection.

My 4Runner is hard starting all the time. I’ve replaced the battery, tested the alternator etc. It sounds exactly like the problem of the fuel pump recall. The 4Runner was recalled from 2014-2015, my car was technically put together in Sept 2015, but is labeled a 2016. I believe my fuel pump is failing. I am extremely nervous it is going to not start one day while I am out camping.

Vehicle was diagnosed from a mechanic with no fuel pressure due to not starting. Vehicle has approximately 135,000 miles and had no warnings or indications of problem. It appears when VIN was ran on gov. recall that Honda had issues with fuel pump. Vehicle will be inspected by Toyota dealer.

TOYOTA 4RUNNER 2013 VIN# [XXX] LP# XXX The gas needle will be above the half (middle line), and the fuel range on top of the dash would read anywhere from 160-213 miles, going 45 mph up a hill or 70 mph on the freeway, all of a sudden starts to stall and completely shut off while still driving. I would then only have a few seconds to try to pull over due to losing all power, and not being able to shift into neutral because it's a push/start button. The car will only turn back on after putting some gas into it, so I make sure to never let the gas needle go below the middle line or have the gas range reading go below anything around 160-200 miles since the vehicle has shut down at a different amount each time, and there's been multiple times. Also, the fuel level needle never matches with how many miles is shown on the range, displaying inaccurate readings. It has poor acceleration, poor mileage, and now making a loud whining noise that is heard from outside the vehicle near gas tank. I have tried looking up the VIN and it does not show that it falls under the recall, despite it having all the symptoms of a faulty fuel pump/sending unit. I love my 4Runner but I am in fear for my life whenever I drive it and not showing accurate fuel levels. I pray that nobody runs into me if the vehicle stalls out in the roadway, while going 30-80 mph (it has done it on regular roads and on the highway). There are no warning lights or check engine light. It's at about 208,000 miles, with the issue slowly getting worse over time. I am requesting for it to be inspected free of charge, as well as getting the necessary repairs that the recall is for free of charge. 2017 Toyota 4 runner SR5 While driving on the highway the vehicle stopped apparently the fuel pump stopped working and could not get going anymore. I had a mechanic come to the house with a PC and test it. He told me that’s a change that will need to take place in a garage. Luckily I was able to stop on the side and paid someone to bring car home

Throttle has a lag when depressing gas pedal (occasionally a dead petal symptom) research indicates a TSB T-SB-0166-10 exist that relates to this issue. Lack of Toyota dealer assistance with this issue when contacted (Toyota of Round Rock Texas 78664) This issues is a safety concern when operating vehicle due to lack of throttle response when accelerating into traffic. When issue occurs, vehicle "stalls" and creates a dangerous situation that eventually will result is an accident.

Car keeps running out of gas while driving, due to faulty readings on the gas gauge and electronic readout. I have been driving this car for 11 years and never ran out of gas until running out 3 times in the past 2 months. Today was the 3rd occurrence, gas gauge showed over 1/4 tank and 85 miles to go until empty. Previous incident was similar with 1/4 tank and showing 65 miles to go. (Pictures attached). The miles to go reading seems to be increasing with each incident (1st incident showed 25 miles, second incident 65 miles, then 85 miles today). No low fuel indicator light or warning has appeared on ANY of these 3 occasions. This is a very dangerous situation potentially causing loss of life as drivers have SECONDS to navigate their vehicle off the highway, while quickly losing speed and loss of steering. No indication that fuel is low, while instead providing incorrect (inflated) dashboard data. Vehicle is going into the shop tomorrow. Found multiple reports of this same incident on [XXX] [XXX] ) which indicate problem with sending unit. Please review and consider further action or communication to other 4Runner owners. I notice my gas wasn't lasting long. One day I was pumping gas and noticed that underneath my 4-runner gas was pouring out from the top of my fuel tank. Took it to the dealership and they repaired the charcoal canister, canister assembly and purge valve assembly. Cost me $953.48 with parts and labor. Very dangerous due to it could have started a fire while driving.
