My mother was involved in a motor vehicle collision while properly restrained by her seat belt. During the collision, the vehicle sustained a significant impact; however, the airbags did not deploy. Following the accident, my mother went to the ER with significant bruising and injuries consistent with the force of the collision and seat-belt restraint. She has a fractured sternum, cardiac contusion and bad bruising. We are concerned that the airbags did not deploy and would like the airbag/restraint system investigated to determine whether it functioned as designed during the crash. We are reporting this incident to NHTSA so that the airbag non-deployment can be documented and reviewed as a potential vehicle safety issue.

Four days after I took delivery of this used vehicle (April 25, 2026), multiple warning lights appeared at once: BRAKE, ABS, PARK, Check Engine, Blind Spot Monitor Unavailable, and RCTA OFF. An independent Toyota dealership diagnosed the vehicle April 27-28, 2026 (approx. 109,000 miles) and found (1) water leaking into the rear hybrid battery area through a sunroof drain "installed incorrectly," causing a Brake System Control Module low-voltage code, starting issues, a headlight only partially functioning on one side, possible air in the brake system, and a musty HVAC odor; and (2) blind spot monitor fault code C1AC2 (Slave Module Horizontal Axis Misalignment) - the blind spot and rear cross-traffic monitoring systems do not function reliably. At a second visit on May 4, 2026, recalibration was unsuccessful; per Toyota procedure the only remaining option is replacing the right-rear blind spot sensor. The technician wrote the cause is a mystery, "whether it be body damage related or electrical short." Toyota service records show the same C1AC2 code was documented during the selling dealer's own pre-sale inspection, and that in December 2024 another Toyota dealership pulled the rear bumper and found the RR blind spot sensor bracket bent, causing incorrect readings - a recurring safety defect that prior calibrations and repairs have not resolved. Separately, federal safety recall 23V865 (occupant classification sensors; the passenger airbag may not deploy as designed) was open and incomplete when I purchased the vehicle and remains incomplete, and Toyota service campaign 25TD01 (rear traction motor cable corrosion) is also outstanding. The defect has been reproduced and confirmed by two Toyota dealerships, reported to the selling dealer in writing, and reported to Toyota's customer-care center. The vehicle is available for inspection upon request. No crash, fire, or injury.

No warning lights were illuminated. The contact stated that the air bags did not deploy. The contact stated that she sustained pain in her right chest, head, and left arm, and medical attention was sought. A police report was filed. The vehicle was towed to a local tow lot, where it remained for further investigation. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 89,083.

The air bags did not deploy. In addition, the contact stated that it was a very hard impact on an older model vehicle. The contact stated that the older model vehicle was made of steel. The vehicle was towed to a local tow yard. The contact was transported to the local medical center. The contact sustained severe sternum bruising, full spine whiplash, severe pain in the neck, both shoulders, and the upper back area. A Police report was filed. On April 5, 2026, my 2017 Toyota Highlander was involved in a side‑impact (T‑bone) collision on the front & rear passenger side. I was seated in the front passenger seat. The impact was hard enough that my insurance company declared the vehicle a total loss. During the crash, I struck my head, but no airbags deployed (front, side, or curtain). The vehicle is being released to Liberty Mutual Insurance. There were no warning lamps, messages or other symptoms of the problem prior to the failure. I am concerned that the airbag/SRS system may not have functioned properly in a crash severe enough to total the vehicle and cause a head injury to a front passenger. I am requesting that NHTSA record this incident and consider whether there may be a safety defect related to side or curtain airbag non‑deployment in this vehicle. The contact stated that while driving at 35 MPH, occupied with her children in the rear passenger seats, the contact was attempting to merge onto the intersection, and a state trooper failed to yield at 55-60 MPH vehicle crashed into the front driver's side of the vehicle, causing the whole front end of the vehicle to detach from the vehicle. The contact stated that no air bags were deployed in her vehicle. The contact stated that she and her children sustained injuries, however under direct legal advice, she cannot disclose this information. Medical attention was required. The contact stated that a police report was filed. The contact mentioned the vehicle was towed to a collision center where it was declared destroyed. The manufacturer was not notified of the failure. The approximate failure mileage was 140,000.

While driving down the road about 30mph on a paved road, heard a loud bang from the back. Stopped and inspected. The stability button shot off the door into the door jam, the door panel coming off, and the weather stripping separating from the door. The passenger rear airbag went off. Nothing visible on outside. Everything mechanically like it should be. 55,000 miles on car. Bought new from dealership. Done all maintenance and safety recalls. Vehicle is at dealership getting inspected.

Upon receiving the pdf file of black box raw data, it states that no incident happened. I asked a relative who works for a major airline manufacturer and the opinion of their analyst for data recorders is that the computer seems to have been defective. Toyota however states the accident did not meet the parameters to register an incident. I find this answer highly suspicious at best.

A vehicle turned in front of me as I was going down the highway and I collided with the vehicle at 44 mph. The airbags of my car did not deploy but the airbags of the other vehicle did deploy.

When right front pax sits in seat and fastens seatbelt, the airbag indicator correctly says "ON." But after a few minutes, the airbag turns "OFF" and the driver's dash gets an "unfastened seatbelt" warning and a "faulty airbag" warning. All of these occur while the front right pax has remain seated and buckled. If the pax unbuckles and exits the seat, these incorrect warnings all remain the same. The right front pax seat belt buckle has been replaced with no effect. The seat has been recalibrated (seat sensors) with no effect. The wires under the seat have been disconnected, cleaned, and re-connected with no effect. This is a safety issue in that the airbag on that front right side de-activates when a passenger is in that seat. There is a Toyota Recall Campaign for this identical problem, but it does not include my model year (2015). Requesting NHTSA get Toyota to extend their recall to include 2015 models, please.
