STEERING
Toyota is recalling certain model year 2004-2009 Prius vehicles manufactured August 5, 2003, through March 30, 2009. The affected vehicles have a steering intermediate extension shaft assembly built with metal splines that complete the connection from the steering wheel to the steering gear. These splines may have been manufactured with an insufficient hardness.
Risk: Over time, the splines may wear and eventually fail resulting in a loss of steering ability, increasing the risk of a vehicle crash.

Toyota is recalling certain model year 2004-2009 Prius vehicles manufactured August 6, 2003, through March 30, 2009 and FCHV-adv vehicles manufactured December 12, 2008, through September 13, 2011. During manufacturing, a scratch may have occurred inside of the electrically driven water pump at the coil wire. The coil wire may corrode at the scratched portion.
Risk: The corroded coil wire may break and the water pump could stop. The corroded coil wire could cause a short circuit between coil wires and a possible open fuse, creating a stall-like condition of the hybrid system while the vehicle is being driven. This may increase the risk of a vehicle crash.

Issue: This recall is for certain block heater kits installed on certain vehicles or sold as accessories at Toyota or Lexus dealerships. On certain vehicles, the engine block heater cable may not have enough heat protection and could be damaged by high temperatures. If this happens, the cable could short circuit while plugged in, and cause the wiring to overheat or melt. On certain vehicles, the steering intermediate shaft splines may have been improperly hardened. If the steering wheel is held firmly on the steering lock while turning sharply at low speeds, the splines could deform and wear, causing a backlash in the steering system. The splines could also become fully stripped, resulting in a potential loss of control causing a crash resulting in injury and/or damage to property. Correction: Dealers will inspect the vehicles and replace affected steering shafts.

Powertrain
On certain vehicles, the electrical water pump assembly, used to circulate coolant through the hybrid components, may fail. If this were to occur, coolant would stop circulating, illuminating various warning lights in the instrument panel of the vehicle. In certain cases, the water pump motor could short-circuit, causing the hybrid system to stop functioning and the engine to stall. A loss of propulsion, in conjunction with traffic and road conditions, and the driver's reactions, could increase the risk of a crash causing personal injury and/or property damage. Correction: Dealers will replace the electrical water pump.

On certain vehicles, the accelerator pedal may become stuck in the wide open position due to an unsecured or incompatible driver's floor mat. A stuck open accelerator pedal may result in very high vehicle speeds and make it difficult to stop the vehicle, which could cause a crash, serious injury or death. Correction: Dealers will reconfigure the shape of the accelerator pedal. Certain models will also have the shape of the floor underneath the accelerator pedal modified and/or a brake override system installed.

On certain vehicles, the connection at the intermediate shaft or the intermediate extension shaft may become loose or the intermediate shaft sleeve may develop a crack under certain operating conditions. The connection may separate or the shaft sleeve may fracture, which could result in the loss of steering control and an increased risk of a crash. Correction: Dealers will replace the steering intermediate shaft. In addition, the dealer will inspect the sliding yoke and the intermediate extension shaft and replace them if necessary.

On certain vehicles equipped with a light emitting diode (LED) stop lamp, there is a possibility that silicon oxide may be generated on the surface of the contact point inside the stop lamp switch. Consequently, current may not flow at the contact, causing the stop lamp to become inoperative. The loss of stop lamps would result in failure to warn following drivers that the vehicle is braking and/or is stopped, which could lead to a vehicle crash. I WAS DRIVING APPROX. 15 MPH ON A STREET ON 5/14/20 AND WAS MAKING A SOFT TURN AND HEARD A 'CLICK' NOISE. THE MECHANIC CHECKED THE SHAFT AND SPLINES FOR DAMAGE AND FOUND NONE, THEN PROCEEDED TO REATTACH BOLT. I WAS ALSO INFORMED BY THIS MECHANIC THAT TOYOTA HAD TWO RELATED RECALLS: ONE FOR THE STEERING SHAFT, AND ONE FOR THE INTERMEDIATE STEERING SHAFT EXTENSION. BOTH OF THESE RECALLS HAD BEEN ADDRESSED AND REPLACEMENT PARTS INSTALLED FOR MY VEHICLE. The odometer on my 2004 Prius stops at 299,999. This is a known problem in the 04-05 models that was corrected in 2006. Leaves me unable to track miles for maintenance and other uses. This also has the problem with the entire digital dash going dark which means no speedometer or gas gauge. Making the vehicle almost unable to be driven safely at this point. I have reached out to Toyota and they verify that it was an issue with this year range but because I have owned it too long they don't do anything about it

During a hot period of summer, my drive selector decided to not stay in the drive position unless I held it there. I could make the initial selection of drive and then stop on the gas, but it would not stay unless I physically held the lever down in the drive position. I never had an issue with the reverse position; only drive. I could always get the car back into Park. This is an extremely unsafe driving condition because it happens without warning and the fix is not obvious. Once the driver is aware of the fix, it forces them into an unsafe driving condition: driving with one hand on the parking selector. Once cooler weather came, the issue went away. It didn't happen on all hot days and was intermittent. I replaced the drive selector in 2022 and have only had the issue come up one other time since replacing this part.

I need your help to get Toyota to fix 2 items: -ABS Accumulator: I understand Toyota at one time had a recall on the ‘ABS Accumulator’, that may have expired. I had to get new brakes & they get locked because of the ABS malfunction. It is an expensive item – more than the cost of the car. I intend to keep the car for a long time, if Toyota can help me get the ABS Accumulator fixed? -MFD: The Multi-Functional Display (MFD) in the middle of the dashboard, has gone blank. It used to just partially blank out before, but now the whole screen is blank all the time, making it very hazardous to drive. Toyota has issued time-limited 'Warranty enhancement program ZG1' instead of working with the NHSTA to issue a comprehensive BRAKE ACTUATOR ASSEMBLY RECALL. ZG1 was explicitly in place only from April 2016 to Dec 2017, requiring dealer verification of an actual system failure prior to 31 Dec 2017. My failure occurred in 2021 and Toyota will not extend ZG1 for me (case 211015000390). Obviously the BRAKE ACTUATOR is a key safety component. Obviously this is a design flaw - brake systems MUST be designed to last the life of the vehicle. Obviously Toyota has had an excessive number of failures. Toyota is improperly using this 'warranty enhancement program ZG1' to avoid the NHSTA and to avoid responsibility for the poor BRAKE ACTUATOR ASSEMBLY design. Per their letter to owners, Toyota was NOT replacing the BRAKE ACTUATOR ASSEMBLY on a preventive basis prior to actual brake failure. Because of the importance and severity, Toyota should have replaced the BRAKE ACTUATORS via a recall. I would like Toyota to fix my faulty brakes at their expense.

I SEARCHED THE INTERNET AND DISCOVERED THIS [PART IS BAD IN MOST PRIUS VEHICLES AND TOYOTA EXTENDED THE COVERAGE ON THE 2010-2015 UNTIL AUGUST 31, 2021. UNFORTUNATELY MY CAR IS 2004 AND DOES NOT GET THE BENEFIT OF THE EXTENSION. THE COST TO REPAIR IS ALMOST $4,000. TOYOTA KNOWS OF THIS ISSUE AND SHOULD REPAIR MY CAR AS THEY ARE WITH THE OTHER YEARS 2010-2015. THIS IS A DRIVER SAFETY ISSUE.

ABOUT 1.5 YEARS AGO DURING A VACATION. MY CAR DECIDED TO NOT SHUT OFF. I DROVE TO THE NEAREST DEALERSHIP THAT WAS ABOUT THE CLOSE. I RECENTLY HAD THE FRONT DASH LIGHT GO COMPLETELY OUT. I DID NOT HAVE AN INSTRUMENT PANEL AT ALL. THE WAS NOT A WAY TO SEE MPH, GAS GAUGE, WARNING LIGHTS, NOTHING. THE CENTER MONITOR STAYED ON. IT TOOK 2 DAYS BUT IT CAME BACK ON. A WEEK LATER IT HAPPENED AGAIN AND AGAIN CAME BACK ON. WARNING LIGHTS CAME ON AND I DROVE TO DEALERSHIP. IT WAS THE 2ND WATER PUMP GOING OUT. AS I WAS LEAVING MY DASH LIGHTS WENT OUT AGAIN. I TOOK IT BACK INTO THE PRIUS DEALERSHIP. THEY HAD PAPERWORK STATING THERE HAD BEEN ISSUES WITH THE METERED DASH BEFORE. IS THERE A RECALL ON IT OR NOT? I HAD TO HAVE A CAR AND WAS ALLOWED TO LEAVE WITH THE DASH NOT WORKING. I DROVE TO A GAS STATION. AFTER FILLING WITH GAS THE DASH LIGHTS CAME ON AGAIN. I HAD TO PAY FOR THE METERED DASH PART TO BE ORDERED UP FRONT BECAUSE THE MECHANIC SAID IT MAY COME BACK ON. THERE SHOULD BE A RECALL ON THIS AS IT IS AN ONGOING ISSUE. STEERING
I WAS DRIVING APPROX. 15 MPH ON A STREET ON 5/14/20 AND WAS MAKING A SOFT TURN AND HEARD A 'CLICK' NOISE. IMMEDIATELY AFTER MY STEERING WHEEL BEGAN TO SPIN FREELY WHILE TOTALLY DISENGAGED FROM THE FRONT WHEELS CAUSING TOTAL INABILITY TO CONTROL DIRECTION OF VEHICLE TRAVEL. THIS CAUSED A MINOR COLLISION WITH ROCK /DEBRIS WHICH CAUSED MINOR FRONT BUMPER DAMAGE. I THEN HAD VEHICLE TOWED TO NEAREST MECHANIC WHO DETERMINED THAT THE MAIN BOLT AT THE INTERMEDIATE STEERING SHAFT AND STEERING RACK WAS DISCONNECTED. THE MECHANIC CHECKED THE SHAFT AND SPLINES FOR DAMAGE AND FOUND NONE, THEN PROCEEDED TO REATTACH BOLT. I WAS ALSO INFORMED BY THIS MECHANIC THAT TOYOTA HAD TWO RELATED RECALLS: ONE FOR THE STEERING SHAFT, AND ONE FOR THE INTERMEDIATE STEERING SHAFT EXTENSION. BOTH OF THESE RECALLS HAD BEEN ADDRESSED AND REPLACEMENT PARTS INSTALLED FOR MY VEHICLE. THERE ARE TWO RELATED RECALLS: 06V188000 (STEERING SHAFT) AND 12V537000 (INTERMEDIATE STEERING EXTENSION SHAFT).

THIS SAME ISSUE OCCURRED AND I DID NOT HAVE TIME TO RELEASE THE BRAKES AND PRESS THEM AGAIN CAUSING ME TO HIT THE BACK OF THE CAR THAT I WAS FOLLOWING. I MAINTAIN MY CARS VERY WELL AND THIS CAR WAS IN TOP CONDITION WITH BRAND NEW TIRES. THE BRAKES DO NOT EVEN ACTIVATE ENOUGH TO SQUEAL OR LEAVE ANY RUBBER BEHIND.
