Owners have filed 183 complaints
about the 2004 Toyota RAV4 with NHTSA between 2004 and 2026.
The largest single group is the airbags.
Across this model's run it ranks 22 of 30 worst.
The letter says how bad. This says what kind — whether this year drew
an unusual number of complaints, unusually dangerous
ones, or failures that arrived unusually early.
The dashed ring is 50 — the national norm. Further out is worse.
Complaint volume — 23
Against the median year of the Toyota RAV4. 50 means typical for this
model; 100 means roughly double.
Severity — 100
23% of these complaints
involved a crash, fire, injury or death, against a national rate of
9.7%.
Early failure — 34
20% of failures with a mileage on record happened under
36,000 miles, against a national rate of
29%.
NHTSA files every complaint against a named sub-assembly, not just a
system. This is the difference between knowing it was "the engine" and
knowing which part of it — the list to hand a mechanic.
Odometer readings owners gave when they filed. About half of all
complaints include one.

When owners complained
A spike years after the car went on sale usually means a part wearing
out on schedule across the whole fleet — the kind of pattern no single
owner can see from inside their own car.

Recalls on the 2004 Toyota RAV4
A complaint is one owner's allegation. A recall is the point at which
the manufacturer or a regulator agreed the defect is real and dangerous
enough to fix for free — however old the car is.
Canada — Transport Canada
Canada runs its own recall programme, and its campaigns do not
always mirror the American ones.
Recall 202172355,683 units in Canada
Electrical
Issue: This recall is for certain block heater kits installed on certain vehicles or sold as accessories at Toyota or Lexus dealerships. On certain vehicles, the engine block heater cable may not have enough heat protection and could be damaged by high temperatures. If this happens, the cable could short circuit while plugged in, and cause the wiring to overheat or melt. Note: This recall only affects the following block heater/cord part numbers: C014003034 PK5A408J09 PU14000904 C014003151 PK5A408J13 PU14000905 C014000885 PK5A489J11 PU14000907 C014000888 PK5A489J22 PU14000909 C014000907 PK5A410H00 PU14000911 C014100904 PU14000900 PU14000913 C014100907 PU14000901 PU14003571 C014100911 PU14000902 PK5A489J14 PK5A408J05 PU14000903 Safety Risk: Engine block heater wiring that overheats or melts could create the risk of a fire. Corrective Actions: Toyota recommends that you should stop using the engine block heater immediately. Toyota will notify owners by mail and instruct you to take your vehicle to a dealer to disable the engine block heater. For some vehicles, the corrective actions for this recall are under development. For others, a new engine block heater cannot be installed, and the owner will receive a refund of the purchase price of the engine block heater.
Recall 201519814,570 units in Canada
Airbag
On certain vehicles, the driver's (frontal) airbag inflator has a potential for intrusion of moisture over time due to insufficient air sealing. This could result in the inflator rupturing during a deployment, and/or an abnormal deployment of the driver's (frontal) airbag during a crash (where deployment is warranted), which could increase the risk of injury to the seat occupant. Correction: Dealers will replace the driver's (frontal) air bag inflator with an updated part.

MY SON AND I WERE ON WAY TO PEYTON MANNING CHILDREN'S HOSPITAL IN MY 2004 TOYOTA RAV4 WHEN THE DRIVER IN FRONT OF ME ABRUPTLY STOPPED, WHICH MADE ME ABRUPTLY STOP, HOWEVER, THE DRIVER BEHIND ME WAS NOT PAYING ATTENTION AND REAR ENDED ME AS I WAS COMPLETELY STOPPED AND HE WAS GOING ABOUT 30 MPH. THIS OCCURRED ON THE PASSENGER BACK OF THE VEHICLE CAUSING ALMOST $4,000 WORTH OF DAMAGE, HOWEVER, MY SON NOR MY SEAT BELTS LOCKED INTO PLACE, THEREFORE, HE SUSTAINED A SIGNIFICANT CONCUSSION, WHIP LASH, MILD MEMORY LOSS, AND NEEDING PHYSICAL THERAPY. BOTH OF MY HANDS WERE ON THE WHEEL, BUT I SUSTAINED WHIP LASH, INJURY TO BOTH MY LEFT & RIGHT SHOULDERS, NECK, AND BACK. I MUST REINFORCE THAT STATEMENT THAT IF THE SEAT BELTS WOULD HAVE LOCKED PROPERLY MY SON AND I MAY HAVE NOT SUSTAINED THIS TYPE OF INJURY, THEREFORE, THIS ACCIDENT WAS LIKE WEARING NO SEATBELT WHEN WE WERE BOTH SECURELY BUCKLED IN AND THE SEAT BELTS NOT WORKING MADE IT LIKE WE WERE DRIVING WITH NO SEAT BELTS FOR PROTECTION.

I WAS PULLING INTO A PARKING SPOT AT LESS THAN 10 MPH WHEN THE ENGINE REVVED LOUDLY, LURCHED FORWARD, JUMPED THE CURB AND CONTINUED UP A SLOPE. I WAS UNABLE TO STOP THE CAR. I WAS ABLE TO TURN THE WHEEL TO AVOID A HEAD-ON COLLISION WITH A BUILDING, AND INSTEAD HIT THE BUILDING ON AN ANGLE ON THE LEFT FRONT SIDE. THE CAR STOPPED ONLY AFTER HITTING THE BUILDING. THIS IS THE THIRD INSTANCE OF "UNINTENDED ACCELERATION" I HAVE EXPERIENCED WITH THIS VEHICLE. THIS WAS THE MOST SERIOUS EPISODE. THE OTHER TWO EVENTS WERE SIMILAR. IN THE FIRST INSTANCE THE CAR AGAIN ACCELERATED WHILE I WAS PARKING AT LOW SPEED AND HIT A FENCE. IN THE SECOND INSTANCE THERE WAS NO DAMAGE OR INJURY AS I WAS ABLE TO PUT THE CAR INTO PARK.

MY MOTHER WAS INVOLVED IN A SIDE IMPACT CRASH (PASSENGERS SIDE) IN 2014. HER CAR WAS TOTALED AS THE OTHER CAR ESSENTIALLY SPEARED HERS CAVING IN THE RIGHT SIDE. SHE WAS WEARING SEATBELT AND THE AIRBAG DEPLOYED. MOM SURVIVED WITH NO INTERNAL INJURIES. HOWEVER SHE WAS LEFT WITH SEVERE LACERATIONS TO THE SCALP, FACE AND HEMATOMA TO THE HEAD WHICH NEEDED SURGERY. I SUSPECT THIS INJURY WAS FROM THE AIRBAG DEPLOYING VIOLENTLY. MY HUSBAND WHO IS A RETIRED FIREFIGHTER/MEDIC INSPECTED THE CAR AFTER THE WRECK AND HE FEELS THE SAME. THERE WAS NO INDICATION THAT HER HEAD HIT THE SIDE WINDOW OR WINDSHIELD. THERE WAS BLOOD ON THE AIRBAG AND ALL OVER HER SEAT. THIS INCIDENT HAPPENED IN FLORIDA ON A HOT AUGUST DAY. THE CAR HAS BEEN IN FL FOR THE LAST TEN YEARS.

SUSPENSION
Rear suspension control arm assembly (that supports the springs, control arm rods, shocks, as well as the the hub, brakes and wheel ) breaks completely due to rust and corrosion. It is a poor design that you can see this would happen at the weakest and narrowest section of the part. This causes the entire wheel assembly to drop or twist off, as it did mine and fortunately not on highway however on a busy local road. This vehicle was bought in [XXX], but original owner primarily had it in [XXX] where I can understand there was lots of salt on the road during snow time. I was driving on a highway, there was a red light and I pressed on the brakes. The car kept going forward, the brakes became resistant to any pressure and the car continued to roll forward. There were no warning lights when the car was started or during the incident. I kept trying to brake the car, but the car kept moving forward and was about to hit the car in front of me. The cars around me started honking, trying to move out of the way; Luckily the light turned red and the car in front of me was able to move out of the way. I could not press on the gas or the break, I decided to turn the ignition off and the car was able to turn off. I re-started the car and then was able to go again. Had this been on the highway and I was moving at a faster speed, I probably wouldn't be typing this right now and would be in the ER with potentially others. This is the second time an issue like this happened. The first time the car was in reverse and it wouldn't brake and I almost slammed into the car behind me. The brakes and gas were not responsive. I took it to a mechanic right away, got new brakes, rotors, engine and transmission was checked. A couple months later this happened again.

10 days ago transmission on 2004 Rav started to act funny. Slow/jerky to engage in lower gears. Check engine lights came on. Checked out by mechanic who said it looked like a transmission problem and should have it looked at by transmission specialist who said it was not safe to drive as is since it could simply die at any time, and it needed a new transmission. When replacing trans, the trans man found the ECM had to be replaced. He said this often happened with 2003 thru 2005 RAV4s. On looking through your site, discovered others had similar issues with their 2004 Ravs. I also found out that the electronic control module for these vehicles is defective and must be repaired to correct the transmission issues. What did Toyota have to say when you looked into this, or do you not think it’s a safety issue?
