Airbags, seats, and headrest all failed. State Farm insurance currently has the vehicle. I do not know if the people has been inspected by anybody. The vehicle had no warning lamps message or other symptoms of problems or failure before the collision. I was traveling in the far left lane when traffic came to abrupt stop and I was able to stop 10 to 15 ft from the vehicle in front of me. Two vehicles in my line behind me swerved out of the way. Then a semi truck with a trailer rear-ended me. The front end of the semi truck struck the rear end of my vehicle getting lodged inside my vehicle. My car was then pushed into the car in front of me and dragged 100 to 200 yards down The highway by the semi. The seat belts did not lock. The airbags did not deploy. Both front seat frames were bent and ripped apart. The rear seat that's became detached. The headrest for the front seats flew off. All four seats were occupied by adult males all weighing around 190 pounds or above. All four occupants were wearing seat belts. I the driver was knocked unconscious and received a concussion plus facial damage/injuries. Rear passenger side occupant received a broken jaw and a skull fracture. The other two passengers were also injured. All four passengers of the vehicle were transport to emergency room via ambulance.

This creates a safety risk because the driver is unable to adjust the seat position to reach the pedals and steering wheel properly, and cannot reposition the seat in the event of an emergency requiring rapid exit from the vehicle. Seating position also affects airbag deployment safety, since proper occupant positioning relative to the airbag is a factor in crash protection. I have a 2021 TYTA RAV4 HYBRID XLE PREMIUM AWD, the driver seat is very slow moving forward and backwards twisting and preventing to fully go forward and backwards. I need to get out and in of my vehicle. So I have to move in back and forth to get out and move it forward to see over the dash board. I am concerned it makes a load noise. What if it fails to move. This is a critical danger to me the driver. I have passengers to consider. This is absolutely unsafe. This is allowed? Where is the recall? This is unlike Toyota to do nothing. I have platinum coverage purchased at time of purchase of this brand new vehicle on 12/31/2021. Apparently, it causes it other damage to other components and parts. There is alot of other components and parts under the front driver seat. I went it to have it looked at on March 30, 2026. Service advisor states: They ordered the parts its a seat frame sub assembly with motor. $1785.92 Covered by platinum Coverage, not all the parts are covered that are damaged. it will cost me: $448.87 for components and parts damaged but are not covered plus labor for Shield (71812-0R140-C0) and Holder (7181B-0R010) After further investigation it is a symptom of a failed internal component within this assembly—most commonly a stripped plastic gear or a bent drive shaft. So looking it up many Toyota Brand owners are experiencing the same thing and are having to replace other parts caused by this. This is a dangerous situation for me and others. What if it fails to move forward or reverse or motor stops? A partial repair is unacceptable. Leaving damaged trim and holders creates a risk of wire-chaffing and further mechanical stress on the new assembly. For the safety of the driver and the longevity of the vehicle, the repair must be 100% complete. This would be out of pocket monies families don't have. $2,234.79 total repair cost acts as a "safety tax" that prevents families from fixing this critical defect. I can not afford: $448.

A recall was issued for a roof leak defect that allowed water intrusion into my vehicle. The recall repair was performed at Autonation Toyota of Weston dealership, where the roof was replaced due to water staining. However, water intrusion which led to black mold growth into the interior carpeting and floor board remained after the repair. The dealership stated that the remaining water intrusion affecting the carpeting was caused by another source. Documentation showing that the recall repair procedure was completed according to the manufacturer’s requirements was requested but has not been provided. The defect involving water entering the vehicle interior remains unresolved. I was last told by the General Manager that my requests and concerns were forwarded to legal, but I have not heard back from October of 2025.

My driver seat suddenly stopped working correctly when trying to move the seat forward or backward. It would make this lagging noise and feel like it was jumping. The dealership diagnosed the issue as a clogged sunroof leak and clogged drains but did not provide clear documentation that they properly inspected the roof rail gasket. [2/18] – Dealership denied repair under 22TE05 but then stated they could replace the roof rail gaskets if my insurance paid for it—which contradicts their claim. [2/18] – I escalated my case to Toyota Corporate for review, but I am still waiting for a response. My car was left outside in the rain, potentially worsening the water damage. The dealership never provided me with a written repair estimate. Safety Concerns & Impact on Vehicle Use: I am worried about driving my car due to potential electrical system failure and airbag malfunctions caused by water damage.
