Desiccated Air Bag Inflator Rupture

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. Vantage Mobility International, LLC (Vantage) is recalling certain 2012-2018 AMS Genesis wheelchair vans, built on a Toyota Sienna vehicles. The rubber brake hoses that attach to the rear wheel assembly may be too short, causing them to rub against the rear axle trailing arm.

Risk: The contact may cause failure of the rear brake hoses, resulting in a loss of vehicle control and reduced braking ability, increasing the risk of a crash.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2011-2016 Toyota Sienna minivans manufactured January 4, 2010, to August 12, 2016. If the power sliding door is unable to be opened when commanded, such as if the door is frozen shut, the door may subsequently open unexpectedly, possibly while the vehicle is moving.

Risk: If door opens while the vehicle is moving, there would be an increased risk of injury to the vehicle occupants.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2014 Toyota Avalon, Camry, Sienna, and Highlander and model year 2015 Lexus RX350 vehicles. Fuel may leak from the one of the fuel delivery pipes in the engine compartment.

Risk: A fuel leak in the presence of an ignition source increases the risk of a fire.

POWER TRAIN:AUTOMATIC TRANSMISSION:GEAR POSITION INDICATION (PRNDL)

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2014 Sienna vehicles manufactured June 17, 2014, through June 24, 2014. The affected vehicles have a transmission shift control cable whose protective cable sleeve may have been damaged during assembly. This could cause the control cable to bind or separate during gear shift operation causing incorrect gear selection. As such, these vehicles fail to comply with the requirements of the Federal Motor Vehicle Safety Standard No. 102, "Transmission Shift Lever Sequence, Starter Interlock, and Transmission Braking Effect."

Risk: If the transmission shift cable separates while the vehicle is being driven, the transmission gear selection may not match the indicated gear and the vehicle may move in an unintended or unexpected direction, increasing the risk of a crash. Furthermore, when the driver goes to stop and park the vehicle, despite selecting the 'PARK' position, the transmission may not be in 'PARK.' If the vehicle is not in the 'PARK' position there is a risk the vehicle will roll away as the driver and other occupants exit the vehicle or anytime thereafter. A vehicle rollaway increases the risk of injury to exiting occupants and bystanders.

Issue: This recall is for certain block heater kits installed on certain vehicles or sold as accessories at Toyota or Lexus dealerships. On certain vehicles, the engine block heater cable may not have enough heat protection and could be damaged by high temperatures. If this happens, the cable could short circuit while plugged in, and cause the wiring to overheat or melt. Toyota will notify owners by mail and instruct you to take your vehicle to a dealer to disable the engine block heater. For some vehicles, the corrective actions for this recall are under development. For others, a new engine block heater cannot be installed, and the owner will receive a refund of the purchase price of the engine block heater.

Brakes

Issue: On certain vans converted to a rear-entry mobility vehicle by BraunAbility, the fittings for the brake line extensions may be incorrect. This could cause a brake fluid leak that may result in reducing braking. Note: This problem affects conversions that were built between January 9, 2018, and May 20, 2019. Safety Risk: Reduced braking could cause extended stopping distances and increase the risk of a crash. Corrective Actions: The company will notify owners by mail and instruct them to take their vehicle to a BraunAbility dealer to inspect the brake line connections. If necessary, an adapter fitting will be installed and the brake system will be bled.

Recall 2019008 376,282 units in Canada

Airbag

Issue: On certain vehicles, long-term exposure to high absolute humidity and temperature, combined with high temperature cycling, may eventually degrade the propellant contained in the passenger-front airbag. This could cause the airbag to deploy with more force than normal. If the airbag inflator ruptures, fragments could be propelled toward vehicle occupants or cause damage to the airbag assembly, preventing its proper function. Safety Risk: If the vehicle is involved in a crash requiring an airbag deployment, the airbag inflator could rupture and create a risk of injury. Corrective Action: Owners will be notified by mail and instructed to take their vehicle to a dealer to have the passenger-front airbag inflator or airbag module replaced. Note: The Canadian climate results in the propellant degrading slowly. This recall is being conducted as a precaution to address future risk. It is expected that all airbag inflators will be replaced before their function would be affected.

Structure

On certain vehicles equipped with power sliding doors, under certain conditions which impede the opening of the door (such as if the door becomes frozen with ice), the sliding door motor could stall when the door is operated. If the motor stalls, high current in the door motor circuit could be generated, operating the circuit protection for the door motor. If this occurs with the sliding door latch mechanism in an unlatched position, the door could open while driving, increasing the risk of injury to a vehicle occupant. Correction: Dealers will replace the existing junction block and the rear sliding door wire harnesses with newly designed ones. The dealer will also install two sub wire harnesses to connect the junction block to the two rear door wire harnesses.

Fuel System

On certain vehicles, the end cap on the right-hand side fuel delivery pipe, located in the engine compartment, could have been improperly welded at time of manufacture. This could result in a fuel leak which, in the presence of an ignition source, could result in a fire causing injury and/or property damage. Correction: Dealers will inspect the fuel delivery pipe and replace with a revised part if necessary.

Was driving my sienna down he highway, as I was passing someone I lightly accelerated and I heard a loud bang sound. Seconds later I see smoke coming out of the hood so I pull over, about 30 seconds before I pulled over the engine bursted into flames. When the vehicle came to a stop it would not unlock and me and my 2 kids were stuck in a burning vehicle, we barley made it out and my arm got burnt from it, this is the 4th time I’ve had a problem with the engine and 6th time the transmission has had problems. I’ve heard this happen on many other siennas yet it’s not recalled…

The passenger-side power sliding door system failed to stop or reverse when it encountered physical interference with the vehicle’s open fuel door. The door continued opening under power despite clear obstruction by a fixed exterior body component. The affected components, including the sliding door mechanism and damaged fuel door latch, remain available for inspection upon request. A powered sliding door continuing to operate against an obstruction creates a foreseeable risk of injury to occupants or bystanders, as well as vehicle damage. If a person’s hand, arm, or clothing had been in the same position as the fuel door, injury could have occurred. The incident occurred during normal vehicle use and demonstrates inadequate safeguards against obstruction. The issue has been formally reproduced or confirmed by an independent service center which noted that the incident occurred as described during normal operation of the power sliding door, and no abnormal conditions were present at the time. No warning lamps, messages, or other symptoms appeared prior to the incident. The power sliding door operated normally and without indication of malfunction until it failed to stop when encountering the open fuel door. Fueling the vehicle with the fuel door open is routine and expected vehicle use. The absence of any interlock, sensor logic, or safeguard preventing a powered sliding door from operating against an open body panel represents a design deficiency rather than a maintenance or wear issue. This condition may affect other vehicles equipped with similar power sliding door systems and warrants evaluation.

The spare tire, which is normally secured under the minivan, fell off while driving at approximately 40 mph. My son, who was driving the vehicle, heard a sudden loud noise from the underside of the vehicle. He looked in his rearview mirror and saw something in the middle of the road. He pulled over to inspect the vehicle, and noticed that the spare tire was gone, which he was able to retrieve. I brought the vehicle to a local Toyota dealer, who indicated I needed a new spare tire carrier (at a cost of $329.07), since the original unit had failed due to rust. The spare tire had never been used or removed from its manufactured mounting position since original purchase of the vehicle in 2014. There were no warning signs at any time prior to the incident that the spare tire might come loose from its mounted position. The safety issues associated with this incident are obvious, e.g., another vehicle striking the spare tire or loosing control to avoid the spare tire; the tire rolling and striking a person, another vehicle, or damaging property, etc.

I HAD THE VEHICLE INSPECTED BY PERSONAL MECHANIC BECAUSE THE STEERING WHEEL WOULD TIGHTEN AND THEN RELEASE WHENEVER TURNED TO RIGHT AND RELEASE ON LEFT TURN. THIS WOULD HAPPEN ON A REGULAR BASIS WHEN TURNING THE WHEEL. THE MECHANIC DID RECOMMEND THAT I CALLED THE MANUFACTURER TO CHECK IF THERE WAS ANY RECALL FOR THE PROBLEM. WHICH I DID. RESPOND WAS NO AND SO I EXPLAINED THE PROBLEM TO THE REPRESENTATIVE. THERE IS NOT A FAILURE OR INCIDENT EXCEPT THE STEERING WHEEL KEEPS ON TIGHTENING AND I'M JUST BEING CAUTIOUS. NO MANUFACTURER, POLICE, OR INSURANCE HAS INSPECTED IT BECAUSE THERE IS NO ACCIDENT. THANK GOD! JUST CONCERNED BECAUSE MY MECHANIC SAID IT IS SOMETHING FOR THE DEALERSHIP. SOMETHING TO DO POSSIBLY WITHIN THE STEERING COLUMN/CHANNEL COMPONENT. NOT SURE WHICH IS RIGHT, BUT TO CHECK FOR RECALL BY MANUFACTURER. I TOOK IT TO TIRE KINGDOM BUT SOMETHING ELSE WAS REPAIRED. After a rain, water will leak into the driver and front passenger seatbelt coil housing. The front passenger seatbelt was not used in some time and mold developed with green and black stains all along the seatbelt. After it rained, the driver side seatbelt released about 1 cup of water when I pulled the seatbelt. With my wife as the passenger, she was conflicted about wearing the seatbelt while having mold pressed against her body, clothes, and breathing it in. The smell of mold is now present in the vehicle. Tire pressure monitor system dashboard icon is ridiculously non-intuitive. Both my wife and daughter have been baffled by it because the icon DOES NOT LOOK LIKE A TIRE! Neither are stupid, one is a college grad and the other is a junior at ASU. The icon is a cross-section of a tire, and none of them know what a tire cross-section looks like. The van seems to prematurely wear tires out before their service life expectancy. I've owned the van for seven years, and I've had to purchase over five complete sets of tires. The front tires wear-out much faster than the rear tires, and do not last more than 15K miles. I've had several alignment jobs done, both (L&R) control arms replaced, ball joints, and struts - the van still wears out tires regardless of tire brand. I suspect the must be a design flaw causing premature wear and tear on tires on the front of the van, as I just replaced another set of tires from the Yokohama brand recommended for the van. I've asked Toyota, and they claim ignorance of such a problem. It is my desire to make this problem known, and to have the Government investigate if there is an design flaw in the Sienna van which is causing rapid part failures.
