Desiccated Air Bag Inflator Rupture

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. If the power sliding door is unable to be opened when commanded, such as if the door is frozen shut, the door may subsequently open unexpectedly, possibly while the vehicle is moving.

Risk: If door opens while the vehicle is moving, there would be an increased risk of injury to the vehicle occupants.

The affected vehicles have a spare tire carrier assembly mounted under the vehicle whose attachment cable may corrode due to high concentrations of road salt splashing onto the spare tire carrier.

Risk: If the carrier cable is corroded due to road salt, the spare tire may separate from the vehicle, increasing the risk of a crash.

Risk: An inaccurate label could lead to owners overloading their vehicles and tires. An overloaded vehicle can result in a tire failure which may result in a vehicle crash, personal injury, or property damage.

Issue: This recall is for certain block heater kits installed on certain vehicles or sold as accessories at Toyota or Lexus dealerships. On certain vehicles, the engine block heater cable may not have enough heat protection and could be damaged by high temperatures. If this happens, the cable could short circuit while plugged in, and cause the wiring to overheat or melt. For some vehicles, the corrective actions for this recall are under development. For others, a new engine block heater cannot be installed, and the owner will receive a refund of the purchase price of the engine block heater.

Airbag

Issue: On certain vehicles, long-term exposure to high absolute humidity and temperature, combined with high temperature cycling, may eventually degrade the propellant contained in the passenger-front airbag. This could cause the airbag to deploy with more force than normal. If the airbag inflator ruptures, fragments could be propelled toward vehicle occupants or cause damage to the airbag assembly, preventing its proper function. Safety Risk: If the vehicle is involved in a crash requiring an airbag deployment, the airbag inflator could rupture and create a risk of injury. Corrective Action: Owners will be notified by mail and instructed to take their vehicle to a dealer to have the passenger-front airbag inflator or airbag module replaced. Note: The Canadian climate results in the propellant degrading slowly. This recall is being conducted as a precaution to address future risk. It is expected that all airbag inflators will be replaced before their function would be affected.

Structure

On certain vehicles equipped with power sliding doors, under certain conditions which impede the opening of the door (such as if the door becomes frozen with ice), the sliding door motor could stall when the door is operated. If the motor stalls, high current in the door motor circuit could be generated, operating the circuit protection for the door motor. If this occurs with the sliding door latch mechanism in an unlatched position, the door could open while driving, increasing the risk of injury to a vehicle occupant. Correction: Dealers will replace the existing junction block and the rear sliding door wire harnesses with newly designed ones. The dealer will also install two sub wire harnesses to connect the junction block to the two rear door wire harnesses.

Structure

On certain vehicles operated where road salts are frequently used, corrosion of the spare tire carrier assembly cable may cause the separation of the spare tire stowed under the vehicle. The spare tire falling on the road surface creates a safety hazard that may result in a crash causing injury and/or property damage. Correction: As an interim measure, dealers will remove the spare tire and store it in the cargo area of the vehicle. Dealers will replace the spare tire carrier assembly when parts become available. Note: This recall supersedes recall 2010-123. Vehicles that were inspected and/or repaired under the previous recall may still be at risk of spare tire separation and will require re-inspection and/or repair.

Label

Certain vehicles fail to comply with the requirements of Canada Motor Vehicle Safety Standard 110 - Tire Selection and Rims. The combined weight of occupants and cargo, as displayed on the Tire and Loading Information label, does not meet the requirements of the Standard. As such, the vehicle could be unwittingly overloaded, which would result in poor vehicle handling characteristics, increasing the risk of a crash causing property damage and/or personal injury. Correction: Dealers will install an updated Tire and Loading Information label, as well as an owner's manual overlay label. Customers with certain vehicles equipped with a genuine Toyota trailer hitch and 18” run flat tires or 19” wheels will be presented with an option (1) to remove the trailer hitch or (2) replace the tires (and rims) with new Extra Load Tires.

Seats And Restraints

Certain vehicles fail to comply with the requirements of Canada Motor Vehicle Safety Standard 210.2 - Lower Universal Anchorage Systems for Restraint Systems and Booster Cushions. The plastic buttons on the seatback of second row seats, which identify the location of the UAS anchor bars, may have been installed slightly outside the limits prescribed by the Standard. Correction: A production change has been undertaken to remedy the technical non-compliance. No corrective recall action is required as the technical non-compliance is deemed to be non-safety related.

Brakes

On certain vehicles, the brake stop lamp switch is mounted on a bracket welded on the service brake pedal support assembly. This bracket is relatively close to the parking brake pedal when the parking brake pedal is fully applied. Due to its proximity to the parking brake pedal, in some instances, the stop lamp switch bracket could be damaged by the operator's foot. Depending on the amount of bracket deformation, this could result in the brake stop lamps remaining "ON". If the deformation is significant, the service brakes could become partially engaged, resulting in brake drag with associated brake noise, vibration and/or illumination of the brake warning light. Should the vehicle continue to be operated in this condition, brake drag could cause excessive heat, which may result in reduced braking performance. This could increase stopping distances, possibly resulting in a crash causing property damage and/or personal injury. Correction: Dealers will replace the service brake pedal support assembly.

VEHICLE SPEED CONTROL

I was pulling my 2011 Toyota Siena into a parking space. My foot was on the brake and the car was going about 5 mph. The car made a slight sound, along with a perceptible yet small physical shift, and (while my foot was on the brake) independently accelerated with tremendous force. It was like it was ignited. The car hit a concrete pole and was totaled. The acceleration over-rode the brake. No one was hurt. The concrete pole averted what could have been a catastrophic accident, had we hit pedestrians, or had we rear ended a car or truck on the freeway. The brakes failed as the car jumped into independent acceleration. My safety and the safety of my passenger were both at risk. As I said, in a different situation, we might have hit pedestrians, or gone through the wall of the grocery store and hit customers. The concrete pole saved us and others. The mechanic at the service center where the car was towed said it was impossible to discern if there had been a failure in the system of the car - though clearly both my passenger and myself experienced that failure in the car's ability to override an independent, forceful acceleration not caused by the driver. There was no warning of any kind. On the Edmunds site, there are many, many testimonials by other drivers of the Toyota Siena of similar, frightening experiences of independent acceleration. It is terribly dangerous and should be investigated. If there is not a recall on this vehicle, there should be. This is a potentially catastrophic defect.

Rear ac system the line is too close to the rear tire which was replaced once yet it is so close it gets friction and wears a hole in the line . Dangerous for people in the rear that may be inhaling a/c vapor while ac is running with a leak . Also bad for environment. There is no clearance the tire is almost right on the line so when you hit a bump you are rubbing it . Repair man says he has had to repair several lines more than once because of them being placed too close to the rear tire . Date below is third time this happened same vehicle. The car hit a concrete pole and was totaled. The acceleration over-rode the brake. No one was hurt. The concrete pole averted what could have been a catastrophic accident, had we hit pedestrians, or had we rear ended a car or truck on the freeway. The brakes failed as the car jumped into independent acceleration. My safety and the safety of my passenger were both at risk. As I said, in a different situation, we might have hit pedestrians, or gone through the wall of the grocery store and hit customers. The concrete pole saved us and others. The mechanic at the service center where the car was towed said it was impossible to discern if there had been a failure in the system of the car - though clearly both my passenger and myself experienced that failure in the car's ability to override an independent, forceful acceleration not caused by the driver. There was no warning of any kind. On the Edmunds site, there are many, many testimonials by other drivers of the Toyota Siena of similar, frightening experiences of independent acceleration. It is terribly dangerous and should be investigated. If there is not a recall on this vehicle, there should be. This is a potentially catastrophic defect.
