Having two issues major issues that’s causing me a $10,000 bill. The timing cover is leaking and there is a coolant bypass hose that is leaking. I have spend a lot of maintenance on this truck on maintaining it to have to remove the engine. As for the forward collision warning ive had it go off multiple times witout a vehicle there. While in cruise control ive had it unexpectedly break on me when there was no visible vehicle infront or next to me.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
My father and I both purchased a 2025 Toyota Tacoma SR5 2WD double cab last year brand new from different dealerships, both were in FL. My father was driving his new Tacoma with adaptive cruise control engaged, this was on an interstate, and the vehicle in front of him moved to the left lane next to him and started braking and this caused his vehicle to start braking extremely hard with no vehicles in front of him (he was driving about 55mph when this happened). He said this has happened to him before. I was in the vehicle with him when this happened. When I was driving from his home in GA a few days later, back to my home in FL, I was on the interstate ([XXX]) with the adaptive cruise control engaged and the exact same thing happened with my truck. A semi truck in front of me moved into the lane to the right and started braking. This caused my vehicle to start braking extremely hard with no vehicles in front of me. Had a vehicle been behind me, this could have caused me to be rear-ended going about 70mph. The sudden automatic braking with NO vehicles in front of you is aggressive enough to cause a rear-end collision and possibly cause the driver to make a startled, impulsive, corrective mistake to the sudden aggressive braking. Problem has not been confirmed or reproduced. Neither my father's or my vehicle has been inspected for this issue. Im betting its a software issue. My 2025 Toyota Tacoma engaged the emergency braking/pre-collision system system while my vehicle was going under 10 mph on a steep road in winter conditions. My vehicle was completely under control before the system engaged and there was no risk of a collision with any vehicles in front of me. The emergency braking system engaged and rendered my vehicle completely out of control. I tried desperately to pump the brakes in hopes I could regain control of the vehicle, but the brakes were fully locked. Had the brakes dis-engaged, I could have easily avoided the collision. The car not only did not decelerate, it accelerated down the hill while the brakes were locked for several seconds. My vehicle eventually slid into another vehicle that had been parked on the side of the road. From doing extensive research I believe a perfect storm of events caused this malfunction. Previously on this day, several inches of snow had fallen. The road had not been plowed and cars were parked on the side of the road. There was also a curve in the road near my vehicle's collision where the other vehicle was parked. My windshield was also still defrosting in the uppermost section near the rear view mirror. It is also possible the radar block on the front of the vehicle had snow or ice on it. It was also actively snowing. From what I have read, any of the above factors can cause a malfunction in the emergency braking pre-collision system. I had never been in an accident in over 20 years of driving and my vehicle did the least safe maneuver it could have in the given situation. There is no circumstance where one would purposefully slam the brakes on a snowy/icy down hill road if they know how to drive in the snow (even if you were going under 10mph as I was). At the most basic level of failure, I can't imagine this system should ever engage if going as slow as I was. Also, the system is designed to decelerate (was accelerating) and I was pumping brakes in effort to disengage

What component failed? The rear brake system (rotors and pads) and the components associated with Safety Recall 25TA04 (rear brake hoses). The parts are available for inspection. * How was safety put at risk? The dealer (Island Toyota, St. Croix) labeled my 2024 Tacoma "UNSAFE TO DRIVE" on Invoice #[XXX] but refused to perform the Federal Safety Recall (25TA04) for rear brake hoses unless I paid for an $800 brake job first. This violates federal law by making a safety recall conditional on customer-paid maintenance. I am currently driving a vehicle the dealer documented as "unsafe" because they withheld the free safety remedy. * Has the problem been confirmed? Yes, the dealer confirmed the rear brakes are failed but refused the recall and the 36,000-mile warranty claim. * Has it been inspected by others? Not yet. However, the dealer’s technician failed to record actual millimeter measurements for the pads or rotors on the official invoice. * Warning lamps or symptoms? Metal on metal grinding noise from the rear brakes. No warning lamps appeared prior to the failure. * Additional Note on Records: The dealer falsified my service records on Line C (Recall 25TC07), claiming I was "unwilling to wait" for a repair that was never even mentioned to me. The dealer also claims I denied service on Line B, but what actually happened is I refused to pay $800 for a premature brake failure that should be covered by my 36,000 mile warranty or as consequential damage from the recall. The dealer then used my refusal to pay for pads as an excuse to withhold the Federal Safety Recall (25TA04). I never denied the safety repair I denied a bill for a factory defect. I have uploaded photos of both rear wheels. The left side is healthy, but the right side is completely destroyed with deep scoring. Due to my husband driving the vehicle, ISP states it looks like he crossed into an oncoming semi-chip truck trailer, causing a fatal accident and killing him instantly. The 2025 Toyota Tacoma's Safety Sense obviously failed since my husband is now dead. We had the 2025 Tacoma in the prior week due to my complaints that the lane assist was not working properly and the screen did not respond to my touch to turn it on. After the fatal accident, ISP removed the black box to download information for the accident report. I am now waiting for isp report. No jaws of life was used to extract husband they didn't need it Tacoma metal already peeled back like a tuna can.

There are three concerns in this write up: at 117000 miles the blind spot monitoring system has fails and will cost $300 to repair. More concerning is that it was recommended I replace the Power Steering Lines at a cost of $927 and the Transmission Lines at a cost of $797 due to severe rusting and corrosion. Its recommended before they rust through and break/leak.

The contact owned a 2025 Toyota Tacoma. The contact stated that while driving approximately 75 MPH, a Semi Truck attempting to merge onto the highway crashed into the passenger side of the vehicle, causing the driver to lose control of the steering and cross three lanes of traffic before crashing into the concrete median. During the crash, no air bags in the vehicle deployed, the collision avoidance systems did not operate, and twelve error messages were activated, including a brake error, a Four Wheel Drive error, and an ABS error. The driver sustained a concussion from the head contacting the steering wheel and lacerations from shards of glass entering the vehicle during the crash. The front seat passenger sustained lacerations to the legs and arms, the rear driver side passenger sustained fractured ribs and lacerations to the arms leg, and face. The rear passenger side occupant sustained lacerations to the arms and legs. All occupants in the vehicle were transported to the hospital with injuries, along with the semi-truck driver. No further information was available. A Police report was taken, and the vehicle was towed away. The manufacturer and local dealer were notified of the incident. A representative from the manufacturer was sent to inspect the vehicle and deemed no failure had occurred. The vehicle was destroyed. The failure mileage was 4,000.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
Toyota has a manufacturing defect going on with their v6 in Gen 3Tacoma's. I have a 2020 Limited with 48,200 miles that needs a new cylinder head. It threw a check engine light on me, and luckily enough I took it to the dealership within about 30 miles, otherwise my engine could have exploded. I have videos from the dealership inspection that I cannot attach here. I have since found via the error code a history of these issues on newer trucks. [XXX] [XXX] [XXX] [XXX] [XXX] [XXX] I believe that this issue could pose serious safety issues for someone who is not as immediate on stopping driving, as it was just a check engine light. If you were to drive on this light down the highway, and have catastrophic failure, the Tacoma becomes a completely unpowered hazard. When the radar becomes covered with ice and snow, the truck continuously spawns alerts for each Safety Sense feature that is disabled, including a pop up the occludes the instrument cluster and can not be dismissed. Multiple warnings interfere with focus in poor conditions and create more of a safety hazard due to driver distraction.
