My car is a 2024 Volvo XC60 PHEV. I was exiting a parking garage on the evening of April 24, 2026, going very slowly as I tried to get into a line of cars, when the brakes suddenly lost pressure. The brake pedal did nothing; I had no control over the car, which lurched forward, hitting the car in front of me. Then my car did the same thing again, and hit the next car again. I strongly suspect mechanical failure, namely that the transition from regenerative to friction braking failed. Exactly the same thing happened with the first XC60 PHEV that I had. In the summer of 2023, that acted as above on two different occasions. I turned that car into the dealer and in its place got the 2024.

Automatic braking failure: Another vehicle slammed into my vehicle as I was moving at a slow pace. That other vehicle pushed the front of my vehicle into a dumpster in a parking lot. The automatic braking should have engaged and stopped the car so it wouldn't have crashed into the dumpster. It is unlikely the vehicle is available for inspection as it was towed to a salvage yard. My safety and that of others was put at risk. The problem could not be reproduced - the vehicle was severely damaged and undrivable. The manufacturer (Volvo) refused to inspect it. There were no warning lamps, messages or other symptoms. Unsure if this is relevant, but the vehicle recently (less than 1 week ago) received an over-the-air (OTA) update to software version 5.0.5.

I was backing into my garage in reverse gear when there was a sudden power surge and the car accelerated rapidly backwards (without me pushing the gas pedal), which led to a crash that caused substantial damage to the car. The car was out of my control, with it accelerating without reason, so something failed on the car. I do not know what component failed, only that there was a sudden and rapid acceleration in reverse. There are no warning lights and the car was taken for an estimate and damages exceed $10,000.

SEAT BELTS
Back to my Initial seat belt on the same car. The car went in for service. The seat belt data showed it was fine. Again the belt pulled so aggressively it tore off my insulin pump patch by sliding upward aggressive as possible. After I took the car to Volvo they had some guy from corporate come look at computer data about the vehicle and test drive it and deemed it safe. I get the car back and zero work was done to the seat belt. The seat belt is harder to pull than it was before it went in. Multiple people in the service department agreed it’s much harder to pull. And literally said there’s nothing we can do cause the data did not show a srs notification. It’s my only car, I’m pretty sure the seat belt shot off or partially did and because their computer data says it’s ok they did nothing to actually investigate it. Allowing me their customer to drive around with a broken seat belt. the belt has a permanent, severe mechanical resistance to being pulled out that occurred simultaneously with the unprovoked tightening event. the vehicle manufacturer is refusing to repair a structurally compromised, physically stiffened safety restraint due to a lack of digital software codes. Glitches in software data on cars can easily happen and Volvo is entirely relying on computer data rather then physical investigation. The worst part is that people in service agreed the belt does not pull right but will do zero to look at it.

On multiple occasions that center screen goes blank while I am driving, the indicator lights do not click, there is no display of vehicle speed, and the map function disappears. These issues cause uncertainly if not panic. Three (Right Front & Rear, Left Rear) of four factory-installed Pirelli Scorpion Zero All Season tires (255/40R21, DOT 1XB RM582P 3923) are exhibiting tread block chunking, cracking, and physical material separation. All four tires share an identical DOT code from a single production run. Failure modes include lateral cracks across multiple tread blocks, physical loss of tread block material in multiple locations, and one tire with a deep tread void consistent with delamination. At 36,176 miles on a 2024 vehicle, this level of structural failure represents a safety hazard. The tread separation creates risk of sudden air loss or blowout at highway speed. I am aware of other NHTSA complaints filed for this exact tire size and model describing identical failure modes. I am requesting NHTSA investigate whether a safety defect exists in this production batch.

The rear axle electric motor casing cracked with significant oil leaking requiring replacement, taking 15 hours of labor. Part numbers were 36003926-7 and 36009926-1. The mechanic at Hunter Volvo Cars in [XXX] relayed that he had just performed the same repair for the same problem the week before, in the same dealership. This raises my concern of a system problem rather than a one-off problem. This is the PHEV version of the Volvo C60 (the T8 model). With a range in EV mode of 35-40 miles, 95% of driving is performed in EV mode. It is unclear If the electric motor attached to the rear axle that was added to the standard Volvo C60 engine for the front axle is engineered for that amount of use and torque. This failure could lead to sudden failure of the electric motor. The car is owned and registered under my wife's name. Tire size: 255/40 R21 The original equipment Pirelli Scorpion Zero All Season VOL PNCS tires (255/40 R21 102V, DOT JXB RW582P) on my 2024 Volvo XC60 CPO developed significant tread chunking and active delamination, discovered at approximately 19,225 miles — less than 40% of the expected tire lifespan. Component: Both front tires are available for inspection upon request. Safety risk: Tread chunks are physically separating from the tire at the tread edge. Active delamination at highway speeds presents a serious blowout risk to myself, passengers, and other drivers. Prior symptoms: No warning lamps illuminated. Initial delamination was noticed earlier by a family member but was not fully examined at that time. Upon closer inspection at 19,225 miles, the extent of the tread chunking and delamination on both front tires was confirmed. The condition appears to have been progressive. Additional note: These tires are original equipment and have never been replaced. The vehicle has been driven exclusively on paved roads under normal conditions with no impacts, curb strikes, or overloading. This is consistent with a known manufacturing defect affecting Pirelli Scorpion Zero tires produced in 2021–2022 installed as OEM equipment on Volvo XC60 vehicles.

I decided to clean off my windshield with the windshield wiper spray and wipers and noticed my windshield would not turn on at all. I heard no noise from the motor coming on nothing happened when I attempted to turn them on. I knew rain was coming in the next few days so I was worried I was going to have visibility issues. Contacted the dealer who stated they had no loaner available. I had to drive in the rain with no windshield wipers. When I finally dropped my vehicle off to get serviced and explained the issue, the tech explained this has been a common with issue with many people especially because of the snow, he states “people keep turning the wipers on with snow on the windshield.” He also said his 6 month old Volvo had the same issue. However, I have never turned my windshield wipers on after snow without removing the snow off first so this was not the case. With multiple issues with different people, the tech issue’s with his own 6 month old car it sounded like a recall issue to me. I was told the motor had burned out on my 2024 Volvo XC60 that I have had for just under 2 years and it was no longer under warranty because I was 1,000 miles over my warranty. No error messages appeared on my screen to let me know there were any issues with my windshield wipers at all. Brakes sometimes do not engage when parked and putting the car into drive or reverse gears. The car performs automatic brake checks when the car is parked. Sometimes, if I happen to press the brake pedal as one of these checks is starting, the brake pedal doesn’t press down all the way. I can still start the car though and put it into drive or reverse while pressing the brake pedal until it stops, though the pedal only depresses partway. Once the car is in drive or reverse, it rolls 1-2 feet, with my foot still pressing the brake pedal, before the brake actually engages. This has happened twice in the past 3 weeks. I am using the latest Volvo car software.

ENGINE
In April 2025, the engine on our 2024 Volvo XC60 B5 Plus AWD failed suddenly with only 7,000 miles on the vehicle. The vehicle is leased and under full factory warranty. Volvo’s dealership confirmed the engine requires a full OEM replacement. However, our insurer, State Farm, is refusing to authorize a new OEM engine and is instead forcing the installation of a salvaged engine from a wrecked vehicle. The original dealership admitted they never performed a full internal inspection of the failed engine, relying only on spark plug checks and a borescope scan. State Farm denied coverage for a $1,299 tear-down at another Volvo dealer that would have confirmed the root cause. Instead, State Farm arranged a “second opinion” at the same original dealership without notifying us and presented this repeat review as independent confirmation. We have now been without a safe, functioning vehicle for over 80 days. State Farm’s insistence on using salvaged parts instead of OEM components not only violates our lease terms but undermines manufacturer safety standards and exposes us, and potentially other consumers, to heightened safety risks if this practice is systemic. No warning lamps appeared prior to the engine failure. This refusal to restore the vehicle to OEM condition represents a serious safety concern that should be reviewed.
