FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

June 29, 2025, Driving on the St. Francis loop in Amarillo, TX, brake lights start flashing, the warning sounds happening, no brakes. I had already slowed down to 35-45 mph due to road construction ahead. I weaved through the cones and as soon as possible, hit the ditch and then coasted to a stop. (I didn’t think about the emergency brake, in my panic.) I turned the vehicle off and called my husband. I decided that I wasn’t pulled over far enough over in the ditch, so I started the pickup again. The lights and warning signals for the brakes were gone and when I pressed on the brake pedal, there were brakes again. I drove home at 35 mph, knowing I could use the emergency brake at that speed, if needed, and had brakes all the way home. GMC Bender repaired truck. After driving approximately 3 miles since repaired, our daughter took the truck to Grease Monkey. She was at the garage door; employee said the pickup didn't need an oil change. She backed out and didn’t have brakes (no warning lights or sounds) and as the truck continued backing, it hit the Grease Monkey sign at the east end of their lot and broke it, knocked the sign over. An employee walked over to see what happened, she told him that she didn’t have brakes. He sat in the pickup seat and pushed on the brakes and said, yes, you don’t have brakes! The police were called, and the officer who showed up took pictures, gave us his card, talked with my husband and talked to the Grease Monkey boss. August 6, 2025, I called GMC Bender Service Department (11 mins. 8 secs.) I told them what had happened. They sent a tow truck to bring the pickup back to their shop. September 16, 2025, my husband texted me saying GMC Bender said that the pickup shows another EBS Modulator Fault. No new parts… used one would be $1293 installed. A week to get it here, no guarantees. There's more details, but the vehicle cannot be repaired to guarantee brakes will work properly. Please help us know what to do next.

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

Current Unsafe Conditions: Post-Replacement Active Safety Failures. The vehicle presently exhibits two distinct categories of active, ongoing, and dangerous safety failures, both of which were introduced as a direct result of the engine replacement performed by Monument Chevrolet: (a) Severe Drivability Failure. Between approximately 1,500 and 5,000 RPM, the engine repeatedly and without warning lunges forward, drops all power, and lunges again — cycling in this manner between three (3) and six (6) times before stabilizing. This behavior occurs across all gears and driving conditions, which creates a severe and unpredictable hazard particularly when merging into traffic or accelerating onto highways. This failure constitutes an immediate and ongoing risk to the safety of myself and other motorists. (b) Complete ADAS System Failure. The frontal crash response warning light is permanently illuminated on the instrument cluster, rendering the following federally relevant safety systems entirely non-functional: (i) Automatic Emergency Braking / Forward Collision Alert; (ii) Cruise Control; and (iii) Lane Change Assist / Lane Control System. All of these systems were fully operational and properly functioning before the vehicle was submitted for the engine swap. Their failure is an assembly defect introduced by Monument Chevrolet during the course of the repair and constitutes a warranty and safety defect for which General Motors, as the manufacturer and warrantor, bears full responsibility. Medical Hardship and Urgency. This vehicle is my sole personal transportation. It is essential for travel to active medical appointments and upcoming medical procedures. The ongoing failure of General Motors and its authorized service network to restore this vehicle to a safe and fully operable condition has created a serious hardship that directly affects my ability to access needed medical care. Yes, it was driven by a service department supervisor. He did experience the issues.

The most common fix (if the windshield is not dirty) was remove the F 11 fuse, in the fuse panel beside passenger door for 10 seconds, that would allow the system to reboot. That worked for 3 months with more than 10 occurrences of the light illuminating on the dash. Now the crash indicator light is on continuously and will not reset by removing the fuse. The computer is up-to-date and all the settings in the infotainment system for the crash avoidance system are activated. Check numerous Internet websites, the cameras for this system is on backorder. So there must be a parent problem with the system. I was required to purchase 36 months of OnStar, I did not purchase a renewal agreement after the 36 month and this issue started soon after.

ENGINE FAILURE, 5/3/22, 23,987 MILES, ALL LIFTERS WAS REPLACED UNDER WARRANTY, ENGINE FAILURE, 7/6/26, 46004 MILES, TOOK TRUCK IN FOR OIL CHANGE AND WHEN RETURNED HAD A TICKING NOISE. Upper front grill trim piece was repaired under recall. After being fixed I was driving and it broke off flying up in the air and behind the truck. It did not hit another vehicle. Adaptive cruise control stopped working. Lane assist stopped working. Brake pad wear sensor error message stating it doesn't work. Vehicle has 19,000 miles.

I purchased this new full-size heavy-duty pickup, and at purchase the selling dealer installed a 3-inch suspension lift kit with 37-inch tires on 22-inch wheels as a paid dealer upfit. The lift kit manufacturer publishes a maximum tire size of 35 inches on a 20-inch wheel for this kit; the tires and wheels the dealer installed exceed that spec. The lift manufacturer confirmed directly that they would never approve exceeding the 35-inch tire / 20-inch wheel recommendation. The dealer's assistant manager confirmed that NO calibrations of any kind were performed after the lift and 37-inch tires were installed, and said nothing is recalibrated when a lift is done with 37-inch tires. This vehicle is factory-equipped with forward collision alert, automatic emergency braking, front pedestrian braking, lane departure warning, and automatic high-beam control. These crash-avoidance systems rely on forward-facing camera and sensor aim, which depends on ride height and suspension geometry. A 3-inch lift with 37-inch tires materially changes that geometry. With no calibration performed, there is no assurance these safety systems are aimed or functioning to spec. The truck was also never aligned after the lift and the tires are cupping. The headlights were never re-aimed for the higher ride height, and the speedometer was never recalibrated and reads inaccurately. The oversized tires also contact the body. The left front tire has torn into the front fender, the fender well is misaligned, and it is forcing the lower front edge of the driver's door out of alignment. The dealer's only proposed remedy is to keep cutting the fender wells; the trimming already done has exposed electrical wiring behind the wheel well to mud, water, and road debris. I am reporting this because the assistant manager's statement suggests it may be the dealer's standing practice—other customers may be driving lifted trucks with uncalibrated crash-avoidance systems without knowing it.

2025 GMC Sierra 1500 Iola, KS

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL

The adaptive cruise control has twice unexpectedly applied the brakes with sufficient force to result in a minor whiplash situation of a sore neck. Both times were driving on a two-lane highway traveling between 65-70 mph (cruise control was set a 68 mph) with no vehicles in the lane in front of me. The adaptive cruise control appeared to interpret oncoming traffic as a vehicle in front of me and aggressively applied the brakes, rapidly dropping my speed from 68 mph to the 35 mph range. Brakes were applied with such force that my neck was sore for about two weeks after each incident. My vehicle also has automatic emergency braking (AEB), but I don't believe this system was activated as the warning lamp on the dash and the audible alert was not activated in either situation. These incidents are related to the adaptive cruise control feature, not the AEB system. The dealership has not had an opportunity to inspect the vehicle yet, or any other agencies. In both incidents, no warning lamps or audible alerts were activated.

2024 GMC Sierra 1500 Marshfield, MA

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL

The vehicle suffers from a persistent transmission defect including shuttering bucking slipping torque converter failure valve body failure twice metal debris improper down shifting chirping noises and related liability and safety issues. The vehicle has undergone multiple unsuccessful repair attempts at authorise GM dealerships. These repairs include torque converter replacement valve body replacement twice solenoid replacement fluid services software updates. The transmission problems have persisted despite these efforts and the vehicle has been out of service for more than 22 business days including a breakdown driving to Florida. I have filed a claim through GM but it seems like it just keeps getting pushed further and further away this is a serious safety issue as I fear for those driving around me and myself that I may cause an accident with the bucking slipping and failure to operate properly on the road.

2026 GMC Sierra 1500 Ellijay, GA

BACK OVER PREVENTION: WARNINGS

I bought my 2026 GMC in April. Several times the audio doesn't work work. When this happens audio related safety technology is affected too including turn signal sounds and back camera audio also. A person walking behind the truck can be seen on the screen but no warning beeps are heard. When backing up the warning for getting too close to an object also doesn't work. I called the dealer in early May and was told that this was a known issue and that it needed a software update. took the truck to the dealer on May 8th and the software update was performed. When the update had been completed when I started the truck after the update there was no audio. I told the service advisor it was currently happening and she took it back to the technician. They disconnected the battery cables to reset the system and told me to do that in the future until the issue is resolved. I opened Case #100978480 with GM on June 1st because I wanted to be sure that GM was also informed of the issue. I was hoping they would be proactive and report back to me what they were doing to resolve this issue. Instead I was told that I need to call the dealer and the dealer would call GM because an update might be available but dealers wouldn't be informed and wouldn't check unless I called.

2021 GMC Sierra 1500 Sedalia, MO

BACK OVER PREVENTION: WARNINGS

When I put the vehicle in park, the dash warning says it is not in park or shift to park. The components continue to work. I have taken it to the dealership and according to the service manger, this is a very common occurrence in these trucks. This seems to me that it would be a very serious safety issue if one assumes their vehicle is in park and it is actually not. The shift to park warning is a safety warning to alert the driver it is not in park so the vehicle doesn’t roll off and hit someone (especially a child playing in the driveway). Assuming that alert is just a malfunction could get someone killed. I did notice that this was considered a safety recall for some models of GMC vehicles but not others. I’m not sure why there hasn’t been a recall on these trucks when the service manager specifically said they are having this issue on a lot of trucks.
