Inadvertent Deployment of Side Air Bags

On October 28, 2025, the Office of Defects Investigations (ODI) opened PE25018 to investigate complaints of inadvertent deployment of the side curtain and/or side thorax air bags. These complaints concern model year (MY) 2018-2022 Honda Odyssey vehicles manufactured by American Honda Motor Co. (Honda). Honda issued recall 26V227 on April 9, 2026, to address concerns of inadvertent side air bag deployment. Due to a faulty Media Oriented Systems Transport (MOST) communication coaxial cable connector, the rearview camera image may not appear on the display. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 111, "Rear Visibility."

Risk: A rearview camera that does not display an image can reduce the driver's rear view, increasing the risk of a crash or injury.

The heating pads behind both side-view mirrors may not be bonded properly, allowing the mirror glass to detach. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 111, "Rear Visibility."

If the weight sensor fails, the SRS warning light would turn on in the instrument panel. Safety Risk: An airbag system that doesn't work properly could increase the risk of injury in a crash. Note: This recall only affects certain Q'Straint QRT-DELUXE and QRT-MAX wheelchair retractors with the part number Q8-6200 or Q8-6209. Safety Risk: A wheelchair that moves in a crash could increase the risk of injury. Corrective Actions: To reduce the safety risk, Vantage Mobility recommends you confirm the wheelchair is secured properly by attempting to move it before driving until the recall repairs have been completed. Issue: Under certain conditions, a software problem could cause the wrong view mode to display when the transmission is shifted into reverse. As a result, the rearview camera image may not appear correctly on the display screen. Canadian regulations allow only certain view modes to be displayed when the vehicle is reversing. Note: Vehicles that received a software update previously under a Honda product update, campaign no. P01 - 'Incorrect Rear View Camera View Mode', don't require this repair. Safety Risk: A rearview camera image that doesn't display the correct view mode could reduce a driver's field of view behind the vehicle. This could increase the risk of a crash. Corrective Actions: Honda will notify owners by mail and have sent a wireless over-the-air software update to eligible vehicles to update the vehicle software. Alternatively, you may schedule an appointment to have the update performed at a Honda dealership.

As a result, the audio display unit could lose its connection, and the rearview camera image may not display when the transmission is shifted into reverse gear. Safety Risk: A rearview camera image that doesn't display could reduce a driver's ability to see behind the vehicle while backing up. This could increase the risk of a crash. Corrective Actions: Honda will notify owners by mail and advise you to take your vehicle to a dealership to replace the media oriented systems transport (MOST) cable harness, and install a cover over the audio display unit connector.

Issue: On certain vehicles, the side mirror glass may not be attached properly. As a result, the mirror glass could vibrate or fall off. Safety Risk: A missing mirror will reduce rear visibility and could increase the risk of a crash. Corrective Actions: Honda will notify owners by mail. Due to a parts supply issue, Honda advises that you should only take your vehicle to a dealership if the mirror is loose, vibrating or has fully detached. Honda will send you a second notice when the replacement parts are available, and instruct you to take your vehicle to a dealership to replace the glass on both side mirrors.

While child was seated in the rear right second-row seat of a 2022 Honda Odyssey Touring, the seat belt became stuck and jammed, trapping the child in the car seat. Multiple attempts were made to release the child but the seat belt mechanism would not release. The child became distressed and complained of difficulty breathing. As an emergency measure, the child car seat had to be physically cut and destroyed to free the child. The vehicle was subsequently taken to the dealer. The dealer confirmed that the rear right second-row seat belt was found cut in half and that the seat frame had to be replaced. Dealer stated the belt mechanism tested normal during post-incident inspection but was unable to reproduce the entrapment condition. The seat belt tensioner is suspected of having gotten stuck, causing the entrapment. This is a serious child safety incident involving potential entrapment and breathing distress in the second-row seating area.

It appeared that vehicle safety systems, including stability or braking systems, engaged automatically. Over the next several seconds, I attempted to regain control by applying the brakes multiple times and making steering corrections. However, the vehicle did not respond as expected. Despite these inputs, the vehicle continued toward the shoulder and ultimately impacted a barrier. I was unable to regain effective steering control during this period. AIR BAGS

The contact owns a 2022 Honda Odyssey. The contact stated that while their wife was driving approximately 50 -55 MPH, the front passenger side air bag and the curtain air bag, which were expanded from the front passenger seat to the rear passenger seat, suddenly deployed without impact. The sudden deployment caused the driver to immediately stop the vehicle to avoid a further incident. The contact stated their wife had informed them of a pot hole being in the road; however, they could not confirm if the air bags deployed after driving over the pot hole specifically. The driver pulled to the side of the road, and the local police were contacted. A police report was filed. The contact stated that two children had received injuries, one child received a bruise and swelling to the side of their head, and the other child received bruising and scratches to their arms. The contact's wife received bruising and whiplash from the incident. The other two children in the vehicle at the time of the incident received mild whiplash. Medical attention was needed. The vehicle was towed to a body shop, where a mechanic confirmed that no impact had occurred and was unable to diagnose the cause of failure. The manufacturer was not made aware of the failure. The failure mileage was approximately 75,000.

West Lafayette, IN

ELECTRICAL SYSTEM

The engine fails to restart on its own after the Auto Idle Stop (auto start/stop) system shuts it off at a stop. When this occurs the vehicle is left without power and I must shift to Park and restart the engine manually with the ignition button. This has happened while I was stopped at an intersection in traffic, leaving the vehicle disabled in a live lane until I could restart it. This is a serious safety hazard — the vehicle cannot move when traffic begins to move, and it cannot be moved out of the path of cross traffic. Honda's service campaign remedy for this condition (PGM-FI software update) was performed on my vehicle in March 2025. The failure recurred after that repair and has continued for approximately nine months. On [XXX] I returned to the selling dealer (Bob Rohrman Honda, Lafayette, Indiana, RO #[XXX]) and was charged $170 for diagnosis. The technician road-tested the vehicle, performed a battery test, and reported that the system was operating correctly with "no problem found." No cause was identified and the condition was not corrected. The failure has continued to occur since that visit. My vehicle is within the scope of NHTSA Engineering Analysis EA25004. I am filing this report because my vehicle demonstrates the specific pattern cited in that investigation: the failure persisting after the manufacturer's service campaign repair was completed. SUSPENSION

I have pictures, but come to find out 2018-2024 odysseys have rear shocks that come apart at the top weld. The shock never got painted originally and they rot under the rubber boot and cause the shock to come apart into two pieces. Which causes the rear of the van to become “loose” I. Turns and bumps . Honda knows about this issue but does nothing about it

The contact stated that on several occasions while the vehicle was parked, the passenger’s side door was opened, and the passenger’s side mirror fell off the vehicle. The vehicle was taken to a dealer on approximately four occasions, and it was determined that the mirror glass needed to be replaced. The vehicle was repaired each time. I went to open back tailgate with key fob, no sound or anything - tailgate did not open. I tried inside vehicle buttons for back tailgate and would not respond - no sounds or anything. Nothing was obstructing the back gate on the outside or interior of vehicle to prevent it from opening. I found a video of a 2021 Odyssey having water leaks into the car from the sliding door and rusts the power module causing it to short. So, my husband opened the back fuse box and got into the back panel and removed and sure enough there is water damage to the module. It is about $400 to replace and it is only a Honda dealership part. This is a safety issue because there is no way to manually open back tailgate in case of an emergency. If water is slowly leaking and causing corrosion, this needs to be fixed by Honda.

The right side airbags deployed without any damage or accident occurring to the vehicle. The safety of the driver and backseat passenger was put at risk because the airbags deployed without any warning and completely disabled the vehicle forcing us to the side of the road and trapping the backseat passenger who was in a wheelchair. The vehicle has been assessed by police and reported there was no damage done to the vehicle. The dealership has evaluated and reported there was no reason the airbags should have deployed. The contact stated that while their wife was driving approximately 50 -55 MPH, the front passenger side air bag and the curtain air bag, which were expanded from the front passenger seat to the rear passenger seat, suddenly deployed without impact.
