Issue: On certain vans converted for wheelchair access by BraunAbility, the fuel tank can be overfilled and fuel can enter the charcoal canister. If this happens, fuel can leak from the canister onto nearby components and the ground. Note: This problem only affects Honda Odyssey wheelchair vans with a power infloor side-entry ramp, which were altered between September 13, 2018 and December 3, 2020. Safety Risk: A fuel leak can create the risk of a fire. Corrective Actions: BraunAbility will notify owners by mail and instruct you to take your vehicle to a BraunAbility service center for an inspection. If there is evidence of a fuel leak, the dealer will install a new fuel tank assembly that has a different fill limit vent valve. The dealer will also replace the charcoal canister. SEAT BELTS
While my child was riding in the second row drivers side captains chair of my 2018 Odyssey, she fell asleep and unknowingly moved the shoulder strap portion of the seat belt behind her head/ upper body. I was driving and could not see that the belt had been moved because she was seated behind me and was also covered by a blanket. After the vehicle was parked, she unbuckled the seat belt and attempted to exit the vehicle. As she got out, the seat belt retracted and tightened around her abdomen and torso, trapping her against her seat. The belt became extremely tight and we were unable to free her after 20 minutes of struggling to create slack and slide her out.The pressure from the belt cause visible injuries to her abdomen. Because we could not safely release her, we were forced to cut the seat belt. The damaged seat belt was inspected by a Honda dealership, which recommended replacement of the entire seat assembly, costing $3,000. Honda customer relations was also notified and provided us with a case number. I am submitting this report because a child was trapped by the seat belt during normal vehicle use, requiring the belt to be cut to safely free her.

SEAT BELTS
On 4/30/26 my 10 year old son was sitting in the middle row behind the driver buckled in his seatbelt. The middle row, middle seatbelt (that comes down from ceiling) got wrapped around his neck and kept tightening. He yelled for help. I was driving and saw in my rearview mirror that my son was being pulled up by his neck from the seatbelt in the ceiling. I pulled into a shopping center immediately to try to help him. I could not reach down in between the seats to unhook it because the more I tried to release him, the more it would tighten. That seatbelt requires a tool to be pressed into the lock to release it and the more I tried to get to the seatbelt to unfasten it the more it was tightening and pulling my son up (he was literally starting to hang from the ceiling). I unhooked his seatbelt so he could push himself up to try to release some of the tension around his neck. I wound up running into a Michaels store screaming that I needed scissors because my son was hanging from a seatbelt. She gave me scissors and I went back out and cut the seatbelt to release my son from the seatbelt noose around his neck. He had red marks around his neck afterwards. I still have the cut seatbelt and the other half retracted into the ceiling. Research has confirmed that there have been other reports of issues with children having to be cut out of seatbelts due to the Automatic Locking Retractor (ALR) in Honda Odysseys, model years 2011, 2012, 2013, 2014 and my model 2018. I called Honda corporate on 6/8/26 and filed a complaint. I was told I would receive an email response. I have not received an email or any response. So I called Honda corporate again on 6/25/26 and was told someone would reach out to me, but they could not tell me anything over the phone at that time. I made an appointment at my Honda dealership to have the seatbelt replaced, but have been waiting on a response from Honda corporate before doing any repairs.

He snuck infront of me and was standing in the path of the sliding door, it did what it normally does, the same door did not detect his body and began shutting on him. As it was squeezing his shoulders I was pushing back on the sliding door with all my strength, and after a few seconds it opened back up. I'm not sure whether the sensor failed, or it can't detect past a certain height, but either way it was a very scary situation. This had just happened an hour ago, so I haven't reported it to any authorities or anything. I came home and googled how to make a manufacturer report and it lead me here. If I had not been standing behind my son, I believe the door would have continued shutting more, enough to sustain serious injuries if not more. My son physically was okay, aside from a red mark on his upper arm where the door was shutting on him, he did need at least 5 minutes of consoling after, which only happens when something truly hurt him. I was travelling on a two lane surface road with a speed of 45 mph, travelling at or below posted speed and just starting to slow to make a right hand turn when I encountered / wandered over the solid white line into a pothole at the edge of the road where there is no shoulder. Airbags on the passenger side including the sidecurtain and airbag inside the passenger seat deployed instantly. There was no collision and no deployment of the front passenger airbag or any other bags in the vehicle. There was an immediate call from Honda regarding the airbag deployment along with an automated notification to local police. Police in [XXX] responded and made a report and created a case number: #[XXX] I had a passenger in the front seat at the time who received a facial laceration, bruises to the right arm and hip. This is part of a known recall for this model and year of vehicle. Police and local dealership ( Casey Honda of XXX [XXX] ) have both confirmed visually that the airbags have deployed. No warnings, lights preceded the event. I just received the letter in the last few days regarding the recall. Honda has issued a case number for the incident: #[XXX]. I will be following up Honda for an inspection of other accident sensor and systems for safety which can be affected by the deployment. A warning for Power Tailgate Problem keeps coming up. It tried to open multiple times while driving, beeping at us. It wouldn’t open with the buttons at all. We had to manually open and close it. This makes the second time it’s done it. And it always has to do when it rains. Honda knows this is a well known issue with their odyssey’s and it needs recalled. The dealership wants us to pay for a well known issue. We had a friend with the same year odyssey and the same thing happened but their tailgate completely opened a few times while driving.

My Odyssey randomly began displaying all of the ADAS warning messages: The Brake System indicator light comes on, Forward Collision Warning, Lane Departure Warning, Lane-Keeping Assist, Auto High Beam System error, etc. These came on beginning 7/25/26 and have come on everyday since causing the entire ADAS system to be inoperable. There have been a few days that they didn't come on immediately. I have had a few days where they came on half-way through my drive. This is a safety issue due to a defect in either the front camera or wiring to it, a nuisance, and something Honda needs to correct. It seems to be way too common of an issue according to my research. There hasn't been any damage to the van and no reason for these to come on. I have done research and see this has happened to many people. The replacement camera is costly as well as the calibration. Therefore, I haven't taken it to the dealership but I have discussed the issue with them. They agree that the camera is getting too hot and failing, which is causing the errors and of course all of those safety features fail simultaneously.

The contact stated that the side wall on the front passenger tire had begun to form bubbles. The tire distributor was notified of the failure, and the tire was replaced however shortly after having the new tire replaced with the front driver's side, and the front drivers side placed on the front passenger side, the contact stated that shortly after upon inspection it was discovered that the tire was shredding and chunking on the shoulder. The cause of the failure was not yet determined. The manufacturer was notified of the failure. The tire was a replacement tire. Despite these two repair attempts, the network defect continues to escalate.The vehicle experiences sudden, violent, and near-constant loud popping and clicking noises through the audio speakers while driving. This occurs both at random and predictably when driving over bumps. Following the increase in these network pops, the vehicle's Blu-ray player completely failed and stopped working. Furthermore, the central infotainment screen frequently freezes, reboots, and displays an "Audio Connection Lost" error message, which also cuts out the FM radio tuner entirely.Crucially, this fiber-optic network failure recently caused a total loss of the digital instrument cluster and speedometer while driving. While this complete cluster blackout has only occurred one time so far, it presents an unpredictable and dangerous safety hazard, as I am left with no knowledge of my current speed or critical vehicle safety alerts while in motion.The manufacturer’s standard recall remedy of swapping plastic connector clips is ineffective for my vehicle. The recurring failures after two repairs indicate a permanently compromised factory wiring harness or a damaged network module that is continuously dropping the signal. The manufacturer needs to authorize a thorough diagnostic and full harness/module replacement to permanently resolve this safety defect before the instrument cluster fails again.

Early in a driving session, the accelerator pedal, when applied pressure, will not rev the engine and provide power to the wheels for 1-3 seconds. This issue happens once or twice in a driving session and then doesn't do it again until a future driving session. It is random and doesn't happen every session. This issue causes me to have concerns entering traffic, particularly left turns, as I am unsure if the vehicle will accelerate when needed to safely enter traffic. I have had Service Bulletin 19-124 completed on my vehicle and it did not fix the issue.

These include "Collision mitigation system problem", "auto high-beam problem", "road departure mitigation system problem", "lane keeping assist problem", "adaptive cruise control problem", and also alerts that there is an issue with our brakes. We took the van into our local Honda dealership and were told the ADAS camera was broken and would need to be replaced. We were told this would cost $2,733 and that there's a chance we will also have to replace the windshield (which is physically fine) for another $1200. The camera breaking is not normal maintenance or expected due to wear and tear. There is nothing physically wrong with our windshield, we have had no accidents, we have kept up on all needed vehicle maintenance. It's my understanding the location of the camera is simply causing it to overheat and blow out the electrical system. Additionally concerning is that even if I did have the money to fix the issue Honda does not have any cameras available. They are on backorder. It means as a driver to three small children I have to assume the nonstop alerts are all benign for at least a month. In the off chance there is a true safety concern such as brakes malfunctioning, I will not know as the alert system is going off at all times.

Front camera failure shut off multiple safety features. Confirmed by the dealership service department. Safety features shut off adaptive cruise control, lane keeping assist, collision mitigation system, road departure mitigation system and blind spot info system. Driving this car under 43000 miles with multiple safety features off with messages flashing is stressful, distracting, uncomfortable and unsafe especially this car is mainly used for children. The worse part is that the dealership service department can not fix the problems because the part is back ordered. We have to continue to use this car in this unsafe condition.
