NHTSA investigations into the 2016 Ford Fusion

An investigation is the step before a recall: the agency has seen enough complaints to look into a pattern itself. An open one means it thinks something may be wrong and nothing has been fixed yet.

1 investigation is still open.

Desiccated Air Bag Inflator Rupture

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. Recalls on the 2016 Ford Fusion

A complaint is one owner's allegation. A recall is the point at which the manufacturer or a regulator agreed the defect is real and dangerous enough to fix for free — however old the car is.

United States — NHTSA

26V376000

POWER TRAIN:MANUAL TRANSMISSION

Ford Motor Company (Ford) is recalling certain 2015-2018 Focus and 2013-2016 Fusion vehicles previously repaired incorrectly under recall numbers 18V169 and 18V845. The clutch may fracture, resulting in damage to the transmission assembly and possibly a transmission fluid leak.

Risk: A transmission fluid leak in the presence of an ignition source such as hot engine or exhaust components can increase the risk of a fire.

25V695000

BACK OVER PREVENTION: SENSING SYSTEM: CAMERA

Ford Motor Company (Ford) is recalling certain 2015-2019 Flex, 2015 Explorer, Lincoln MKT, Lincoln MKZ, 2015-2016 C-Max, Escape, Taurus, 2016 Fusion, 2018-2019 Taurus, 2019 Lincoln MKT, Fiesta, and 2020 Mustang vehicles. The rearview camera may display a distorted, intermittent, or blank image when the vehicle is in reverse.

Risk: A rearview camera that fails to properly display an image can reduce the driver’s view behind the vehicle, increasing the risk of a crash.

23V775000

LATCHES/LOCKS/LINKAGES:DOORS:LATCH

Ford Motor Company (Ford) is recalling certain 2015 Fiesta, 2016 Fusion, and 2016 Lincoln MKZ vehicles sold, or ever registered in Alabama, Arkansas, Arizona, California, Florida, Georgia, Hawaii, Louisiana, Mississippi, New Mexico, Nevada, Oklahoma, Oregon, South Carolina, Texas, Utah, Washington, Puerto Rico, American Samoa, Guam, the Northern Mariana Islands (Saipan), and the U.S. Virgin Islands. A component inside the door latches may break, which can make it difficult to latch or possibly allow the doors to open while driving.

Risk: Vehicle doors opening unexpectedly while driving can increase the risk of a crash or injury.

23V162000

SERVICE BRAKES, HYDRAULIC:FOUNDATION COMPONENTS:HOSES, LINES/PIPING, AND FITTINGS

Ford Motor Company (Ford) is recalling certain 2013-2018 Fusion and Lincoln MKZ vehicles. The front brake hoses may rupture and leak brake fluid.

Risk: A brake fluid leak will increase brake pedal travel and extend the distance needed to stop the vehicle, increasing the risk of a crash.

22V413000

POWER TRAIN:AUTOMATIC TRANSMISSION:GEAR POSITION INDICATION (PRNDL)

Ford Motor Company (Ford) is recalling certain 2013-2019 Escape, 2013-2018 C-Max, 2013-2016 Fusion, 2013-2021 Transit Connect, and 2015-2018 Edge vehicles. The bushing that attaches the shifter cable to the transmission may degrade or detach.

Risk: A damaged or missing bushing may prevent the vehicle from shifting into the intended gear, and cause the vehicle to move in an unexpected direction. Additionally, the vehicle may roll after the driver selects the 'Park' position. Either scenario increases the risk of a crash or injury.

20V177000

LATCHES/LOCKS/LINKAGES:DOORS:LATCH

Ford Motor Company (Ford) is recalling certain 2014-2015 Fiesta, 2014-2016 Fusion and Lincoln MKZ vehicles sold, or ever registered in Alabama, Arkansas, Arizona, California, Florida, Georgia, Hawaii, Louisiana, Mississippi, New Mexico, Nevada, Oklahoma, Oregon, South Carolina, Texas, Utah, Washington, Puerto Rico, American Samoa, Guam, the Northern Mariana Islands (Saipan), and the U.S. Virgin Islands. A component within the door latches may break making the doors difficult to latch and/or leading the driver or a passenger to believe a door is securely closed when, in fact, it is not.

Risk: A door that is not securely latched could open while the vehicle is in motion, increasing the risk of injury to a vehicle occupant.

19V632000

STEERING:ELECTRIC POWER ASSIST SYSTEM

Ford Motor Company (Ford) is recalling certain 2013-2016 Ford Fusion and Lincoln MKZ, 2015-2016 Ford Edge, and 2016 Lincoln MKX vehicles originally sold, or currently registered in Connecticut, Delaware, the District of Columbia, Illinois, Indiana, Iowa, Maine, Maryland, Massachusetts, Michigan, Minnesota, Missouri, New Hampshire, New Jersey, New York, Ohio, Pennsylvania, Rhode Island, Vermont, West Virginia, and Wisconsin. Road salt or other contaminants may corrode the electric power steering gear motor attachment bolts.

Risk: If the bolts corrode, the steering gear motor may detach from the gear housing resulting in a loss of power steering assist, requiring a greater steering effort, especially at lower speeds, and increasing the risk of a crash.

19V362000

POWER TRAIN:AUTOMATIC TRANSMISSION:LEVER AND LINKAGE:FLOOR SHIFT

Ford Motor Company (Ford) is recalling certain 2013-2016 Fusion vehicles equipped with 2.5 liter engines. The bushing that attaches the transmission shifter cable to the transmission may degrade over time and cause the bushing to detach from the transmission. The condition could allow the driver to move the shift lever to Park and remove the ignition key, while the transmission may not be in Park, with no warning message or audible chime.

Risk: If the vehicle is exited without the transmission being in Park and without the parking brake being applied, the vehicle may unexpectedly move, increasing the risk of a crash.

Canada — Transport Canada

Canada runs its own recall programme, and its campaigns do not always mirror the American ones.

Recall 2023132 63,269 units in Canada

Brakes

Issue: On certain vehicles, the front brake hoses could fail and cause a progressive brake fluid leak. If this happens, the driver may experience a change in brake pedal feel and travel. This can cause reduced braking. Note: A brake fluid leak would cause the brake warning light to turn on. This recall doesn't affect models with a Hybrid engine. Vehicles repaired under Ford warranty extension program 22N02 already have updated brake hoses, and don't require this repair. Safety Risk: Reduced braking can cause an extended stopping distance and increase the risk of a crash. Corrective Actions: Ford will notify owners by mail and instruct you to take your vehicle to a dealership to replace the front brake hoses.

Recall 2022311 362,062 units in Canada

Powertrain

Issue: On certain vehicles, the shift cable may detach from the transmission. This could cause the transmission not to shift gears when the shifter is moved. As a result, the transmission could be in a different gear than the gear shifter indicates. If a driver attempts to exit the vehicle while the transmission is not in Park, and the parking brake is not applied, the vehicle could move suddenly. Safety Risk: Sudden and unexpected movement of the vehicle could increase the risk of a crash. Corrective Actions: Ford will notify owners by mail and instruct you to take your vehicle to a dealership to replace the transmission shifter cable bushing and install a protective service cap. Ford advises that you should apply the parking brake each time the vehicle is parked.

Recall 2019431 9,906 units in Canada

Steering

Issue: On certain vehicles, the bolts that attach the electric power steering assist motor to the steering gear may corrode and break. This could cause a loss of power steering assist. This recall applies to vehicles originally sold or currently registered in areas of heavy road salt usage (Ontario, Quebec, New Brunswick, Nova Scotia, Prince Edward Island, and Newfoundland & Labrador). Safety Risk: A loss of power steering assist may result in a sudden increase in steering effort, especially at low vehicle speeds. This could increase the risk of a crash. Corrective Actions: The company will notify owners by mail and instruct you to take your vehicle to a Ford or Lincoln dealer to replace the bolts and apply a wax sealer. If one or more of the bolts are broken or missing, a new steering gear will be installed.

Recall 2019420 5,327 units in Canada

Other

Issue: This recall is for aftermarket replacement rear window glass produced by Vitro Automotriz. Certain rear window glass may not have been properly tempered. If the window breaks, the glass may separate into large pieces instead of very small ones. Note: This is a voluntary recall that is not being conducted under the requirements of the Motor Vehicle Safety Act. Safety Risk: Glass that breaks in to large pieces could create a risk of injury. Corrective Actions: The company will notify consumers by mail and instruct them to inspect the designation code marked on the rear window glass. Products affected by this recall have the designation “DOT 287” along with the lettering “VMR” (Replacement Market) and a date code that is before July 2019. This information is printed on the bottom corner of the glass. Consumers should contact Vitro's customer service at 1-412-995-6457 or argbacklites@vitro.com to arrange for replacement of the glass.

Recall 2019226 10,282 units in Canada

Powertrain

Issue: On certain cars equipped with a 2.5 litre engine, the shift cable may detach from the transmission. This could cause the transmission not to shift gears when the shifter is moved. As a result, the transmission could be in a different gear than the gear shifter indicates. If a driver attempts to exit the car while the transmission is not in Park, and the parking brake is not applied, the car could move suddenly. Safety Risk: Sudden and unexpected movement of the car could increase the risk of a crash. Corrective Actions: Ford will notify owners by mail and instruct you to take your vehicle to a Ford or Lincoln dealer to replace the transmission shifter cable bushing and install a protective service cap. Ford advises that you should apply the parking brake each time the vehicle is parked.

Recall 2018379 36,887 units in Canada

Powertrain

On certain vehicles, the bushing that attaches the shift cable to the transmission may degrade over time and could detach. This could allow the transmission to be in a gear state different than the gear shift position selected by the driver. This condition could also allow the driver to move the shifter to Park and remove the ignition key while the transmission gear may not be in Park. If the parking brake is not applied, this could result in unintended vehicle movement, which would increase the risk of crash causing injury and/or damage to property. Correction: Dealers will replace the shifter cable bushing.

Recall 2018130 59,266 units in Canada

Steering

On certain vehicles, the steering wheel bolt torque retention feature may not fully engage into the steering shaft. If the bolt loosens, the steering wheel may appear to be loose, which could affect vehicle control. If a loose bolt is left unrepaired, the steering wheel may detach from the steering column, which could result in a loss of steering control, increasing the risk of a crash causing injury and/or damage to property. Correction: Dealers will replace the steering wheel bolt with a longer bolt.

Recall 2016601 34,417 units in Canada

Seats And Restraints

On certain vehicles, deployment of the driver or front passenger seatbelt pretensioner in a crash may generate internal pretensioner temperatures that can degrade the tensile strength of the outboard seat belt anchor cable. This could result in the seat belt not adequately restraining the seat occupant in a crash, increasing the risk of injury and/or damage to property. Correction: Dealers will apply a coating to protect the pretensioner's cables.

SERVICE BRAKES

I would like to make a complaint for malpractice during a recall repair. I was notified on aug 4, 2026 that after the dealership did the recall 23S12 brake hose that during the test drive that my master seal and brake booster started to leak and needs to be replaced. They asked me if i have had any brake problems and we have not. They wanted to charge me over $1000. I declined because i had no issues prior to my vehicle going to them. They then stated they would complete the job for a little over $600. i declined. They said they reached out to Ford to see if they could cover the master seal and brake booster but they said FORD declined. However, they never asked ford. I have the OASIAS report verifying that information. They did not inform me that this is a saftey issue, they never said do not drive the vehicle, the documentation they gave me does not warn this is a issue. However, on the way home I realized I had no brakes. I assumed my vehicle would be in the same shape as i had it taken in and I would have brakes. I do not. I brought my vehicle to my home called the dealership who basically said unless i want to pay for the job in full i am on my own. I need help. I have the information on July 30 when they did the original inspection and they did a brake inpection and there is no mention of any brake issues. However, after the recall the inspection was done, they took a camera went into the brake booster and master seal. All of this was done with out informing us until it came time to pay.

SERVICE BRAKES

The brake booster vacuum pump coupler broke off of the vacuum pump and a piece of the coupler was left stuck in the camshaft. This caused the brakes to become very stiff and hard to push down to stop the vehicle. It wasn't inspected by anyone. No warning messages or icons appeared on the dash. Car had to be towed.

POWER TRAIN

The engine experienced a sudden failure that caused the vehicle to become completely inoperable. Prior to the failure, the vehicle exhibited intermittent hesitation, rough idling, and loss of power during acceleration. These symptoms began several weeks before the complete failure. No warning lamps were illuminated at first, but shortly before the breakdown, the vehicle displayed a check engine light and a “Service Engine Soon” message. While driving, the vehicle abruptly lost power and became unsafe to operate. I was forced to pull over immediately, which created a safety risk to myself and other drivers due to the sudden deceleration and inability to maintain speed in traffic. The vehicle would not restart afterward. A repair facility has confirmed that the engine has suffered a major internal failure and requires replacement. The estimated cost of repair starts upwards of $5,000. The vehicle is currently non‑drivable and remains available for inspection. The issue has been reproduced and confirmed by an independent service center. The vehicle has not yet been inspected by the manufacturer, police, or insurance representatives. However, I have found multiple similar complaints from other owners reporting the same type of powertrain failure, suggesting a potential pattern or defect affecting safety.
