I am reporting a recurring catastrophic high-pressure fuel-pump/fuel-system failure in my 2018 BMW X5 after BMW previously repaired the same type of failure, and subsequently performed the NHTSA HPFP recall on my vehicle. I am a 71 year old driver. This was a repeat catastrophic failure after Recall 21V-586 was performed and the loss of propulsion on the freeway was terrifying, and I almost crashed into other cars! At 42,763 miles, the vehicle experienced a catastrophic failure while being driven. The vehicle shut off while driving and would not restart. Santa Monica BMW documented a “HIGH PRESSURE FUEL PUMP MALFUNCTION” and fault codes. BMW's technician followed the fuel-system test plan and reported that the fuel quality control valve had metal shavings. See Invoice 42,763.pdf BMW subsequently issued NHTSA Recall 21V-586 concerning the HPFP on affected BMW diesel vehicles, including my vehicle. BMW performed the recall repair at Weatherford BMW at approximately 58,292 miles. The repair order identifies the work as “PERFORM RECALL B130121 HIGH PRESSURE FUEL PUMP” and documents replacement of the high-pressure fuel pump. See Invoice 58,292. At 120,945 miles, the vehicle experienced another, similar catastrophic failure while I was again driving on a freeway. Weatherford BMW subsequently diagnosed multiple fuel/rail-pressure faults. The technician removed the fuel quantity control valve to check for swarf and found magnetic metal shavings in the fuel system. See Invoice 120,945 These failures are particularly concerning to my safety. I am submitting this complaint to the NHTSA because I believe the recurrence of a catastrophic system failure, including metal contamination and loss of propulsion, after BMW previously replaced the complete fuel system under warranty and subsequently performed the HPFP recall remedy, warrants investigation to determine whether the recall remedy or the underlying HPFP/fuel-system defect can cause a subsequent failure of this nature.

The high-voltage battery system (this is a plug-in hybrid) began intermittently displaying a "battery not properly charging" warning approximately 6 months ago; the issue would resolve after plugging in and charging. On July 14, 2026, while driving, the vehicle displayed a "critical battery failure - pull over immediately and drive slowly" warning. The driver pulled over and shut off the vehicle; the warning cleared and the car appeared to drive normally afterward. This warning recurred several times over the following weeks under the same pattern. On August 6, 2026, the same critical battery failure warning appeared while driving; this time steering feel and drivetrain response were abnormal, and the air conditioning and several electronic features stopped working. Our safety, and the safety of other drivers on the road, was put at risk by the sudden loss of full vehicle control while in motion. The vehicle has been at an authorized BMW dealership for inspection since August 7, 2026; a full diagnosis is pending. Dealer service staff independently remarked that two other BMW X-series plug-in hybrids came in that same morning with the identical warning and symptoms, suggesting this may be a widespread component defect rather than an isolated issue.

On March 9, 2026, I brought the vehicle to BMW of Fairfax for a scheduled appointment. In the week leading up to the visit, the vehicle intermittently failed to start and required jump-starting using a portable battery starter. Despite this issue, the car drove normally, showed no warning lights or error messages, and had approximately 69,000 miles on the odometer at the time. After jump-starting the vehicle, I personally drove it to the dealership. I paid for a diagnostic evaluation and also authorized completion of a minor recall. Two days later, I was informed that the vehicle was no longer able to shift into gear and was now completely inoperable. I was told I would be responsible for towing the vehicle off their premises. According to the service department, the vehicle now requires replacement of the EME module, at an estimated cost of $12,000. This assessment was particularly troubling because the vehicle was fully drivable when it arrived at the dealership. At no point prior to service did the car have difficulty shifting into gear. It is deeply concerning that the vehicle became a “brick” while under the care of an authorized BMW service center, with no accountability taken for what occurred during their diagnostic process.

The electric A/C compressor on my 2022 BMW X5 xDrive45e (VIN [XXX] ) failed internally at approximately 59,000 miles, causing metal debris to circulate throughout the refrigerant system. The failed compressor and contaminated components are at BMW of Sterling (Sterling, VA) and available for inspection. This is a safety concern because the A/C refrigerant circuit provides thermal management for the high-voltage hybrid battery. BMW of Sterling's service advisor confirmed in writing that the A/C system is "a critical component of your vehicle's HV system," that there is no independent backup cooling system, and that the failure "can cause potential high voltage battery failures." Without active cooling, the HV battery is at risk of overheating, which could result in a thermal event. The failure was diagnosed and confirmed by BMW of Sterling. They categorized the repair as "EHV: FULLY ELECTRIC OR HYBRID VEHICLE DRIVETRAIN DIAGNOSTICS" with the complaint "HIGH VOLTAGE SYSTEM ERROR." The repair estimate is $11,356.59 and requires replacement of the compressor, condenser, all expansion valves, and a double system flush. BMW Service Information Bulletin SI B64 09 21 documents this exact failure pattern for the G05 X5 xDrive45e, and BMW created a dedicated repair kit (part 64 50 5 A5B 590) for it. This is my second major HV component failure. A battery cell replacement was performed under warranty in 2025. Both failures were confirmed by the dealership to be component defects unrelated to owner use. The vehicle will not accept a charge from any home or public charging source. The high-voltage battery was replaced approximately one year ago; however, the charging failure persists. The vehicle is available for inspection upon request. In addition to the charging failure, the vehicle displayed drivetrain malfunction and start/stop malfunction warnings. While driving on the highway at normal speed, the vehicle lost power and stopped, creating a hazardous situation in active traffic. This sudden loss of propulsion placed my safety and the safety of other drivers at risk, as surrounding vehicles were traveling at highway speeds. Prior to the failure, warning messages appeared intermittently, including charging system faults and start/stop malfunction alerts. These warnings began approximately one year ago around the time the charging issue started. The vehicle was previously serviced and the high-voltage battery was replaced; however, the underlying issue was not corrected. The root cause is UNKNOWN, but the symptoms suggest a failure within the hybrid control system, charging module, DC-DC converter, or related electrical components. The failure has been persistent and unresolved. I am concerned that defects within the hybrid propulsion system may lead to additional sudden loss of power events.

I am writing to formally express my concern regarding serious and repeated safety-related failures with my 2022 BMW X5 xDrive45e, VIN [XXX] . I purchased this vehicle in 2022 through a custom build, at a purchase price exceeding $80,000, with the expectation of reliability, safety, and premium quality consistent with the BMW brand. In October 2025, while my wife was driving with my son in the vehicle, the hybrid component safety box failed without warning, causing the vehicle to stall on the roadway. This created a dangerous situation for my family. The vehicle was towed to the dealership, repaired, and returned to me on November 4, 2025. Unfortunately, on February 3, 2026, a second serious incident occurred. After my wife picked up my son from school, the vehicle began jerking violently while driving, and a strong sulphur-like odor entered the cabin. Fearing for their safety, she immediately stopped driving. The vehicle was again towed to BMW of Jackson. I was informed today by the service advisor that the failure is due to another hybrid-related component, specifically the EME Hybrid Inverter, and that BMW North America is declining to cover this repair under warranty. These repeated hybrid system failures are deeply concerning, particularly given that both incidents involved my wife and children and posed clear safety risks. As a result, both of my children are now fearful of riding in the vehicle, and this situation has caused significant emotional distress to my family. Given the nature of these failures, the vehicle’s purchase price, and BMW’s emphasis on safety and engineering excellence, I find it unacceptable that a critical hybrid component failure would not be covered under warranty—especially following a prior hybrid system failure less than four months earlier. We are reporting a safety issue with the BMW X5 xDrive50e model year 2025. I purchased this car on December 9, 2024, from BMW of Pembroke Pines, 14800 Sheridan Street Pembroke Pines FL 33331. Within two months of purchase, we found that car has unintended acceleration while it is about to be parked at very slow speed. I reported this safety issue to BMW dealer in Pembroke Pines FL where I purchased the Car. The dealership kept this car of investigation. After about five weeks they returned the car with a comment on my invoice that car is working normally. On October 24, 2025, while on vacation to St. Augustine FL and parking at the visitor center SUV accelerated and hit the wall of the visitor center. All the airbags opened and saved us from serious injuries. The crash report of this incident is attached to this report. BMW failed to investigate this issue and put us and others at high risk.

The engine warning system came on and warned of low oil pressure. Knowing the oil level was good I stopped the vehicle to check oil leaks. Found none. Knowing engine mechanics I assumed it was a faulty oil pump. had it towed to BMW and told it was so. The replacement would be $8,000. Found out after lengthy research that the oil pump of the 2019 X5 (which I have) was made with plastic internal parts that would be a matter of when not if, the plastic parts would fail because the acidic nature of used oil. Presented this info to BMW and they lowered the price to $3986.16. Then I discovered BMW knew about the problem and changed the oil pump design from 2020 on.

The HPFP failed and left my child and me dead in the water, stalled in the middle lane of a 50mph street for a half hour, nearly getting hit several times (two other vehicles crashed behind us trying to avoid us). Eventually a Sheriff helped us get the vehicle out of the road and into a nearby parking lot to wait for the tow truck. After we got to the parking lot, the Sheriff got another radio call and left without taking information or referring to a report or report number. This part has an open recall 21V-586, but both the local BMW dealership and BMW NA refuse to repair my vehicle (withou paying $18k), saying my VIN is not a part of the recall, despite my vehicle being the same engine within the production time, and my problem being exactly what was described in the recall. The pump failure has been confirmed by a crank-no-start condition, error code for low fuel rail pressure, and finding metal shavings in the fuel metering valve on the fuel pump. I am seeking assistance to be included in the recall that already exists and am frustrated to have been turned away when my vehicle meets all the criteria listed on the recall document. All of my correspondence with BMW was by telephone, so I do not have emails to attach below.

In September 2023, at approximately 78,000 miles, this vehicle exhibited 'Drivetrain Malfunctions' including Fault Code 0x222626 (EME: internal fault - active short circuit requested) and 0x22271C (Overvoltage in intermediate circuit). The BMW dealer (Fields BMW) failed to diagnose the root cause and instructed me to 'keep driving'. Since the release of Recall 24V-608, it is clear these 2023 faults were early indicators of the coolant pump short-circuiting. This persistent electrical instability has now resulted in a permanent failure of Cell Supervision Circuit #3, causing the high-voltage contactors to open and leaving the vehicle disabled. The dealer is currently attempting to charge for diagnostics and battery removal. I am asserting that the lack of a timely remedy for the coolant pump (which was only recently shielded per the recall) caused a power surge that 'fried' the internal battery circuitry. This is a safety issue as the vehicle experienced a sudden loss of propulsion while in motion. I request that NHTSA investigate BMW’s refusal to cover battery failures caused by the electrical interference of the recalled coolant pump."

The electronic oil level sensor was unable to complete an oil level measurement on the iDrive screen. Diagnostic inspection at a BMW Dealership (Mt. Kiso) and EuroTech Auto Repair (Larchmont, NY) revealed that the oil pump had degraded internally, specifically due to failure of plastic components. All Sensors were working fine and showed no faults. This failure could not be detected by regular maintenance and posed a risk to engine reliability and potential sudden loss of lubrication. The car had approx 55,000 miles, was purchased purchased after my 3-year lease (original owner of new vehicle), and the incident occurred during normal use. There were no warning lights or symptoms before the oil measurement stopped working. This defect appears to be a design/materials issue affecting other similar vehicles. Before the recall repaire, the vehicale was in normal working condition: -No check engine lights or warning indicators -No unusual noises -Fully drivable on a daily basis After the recall was completed, the dealership informed me that: -multiple check engine/warning lights appeared -The vehicle developed a very loud/high pitch noise -They do not recommend driving the vehicle in its currently condition. They did not warn us about his initial, they did after informed them we do not feel safe driving the car in this condition. The dealership explained this happened because they had open a valve to perform the recall, which they claim disturbed "sludge". They are now refusing to correct the problem unless we pay out of pocket. The issue did not exist before the recall and occurred only as a direct result of the recall repair. The recall left my car unsafe and undriveable, and I am being told to pay for a problem that only arose due to a federally mandated recall work. The recall resulted in a vehicle that is no longer safe to drive, with multiple warning lights and severe engine noise. The dealers refusal to fix this without charing me undermines the purpose of a recall, which is to ensure safety at no cost to the owner. I ask NHTSA to investigate this matter and ensure BMW and its dealerships perform recall repairs properly, at no cost to consumers, and do not return vehicles in unsafe conditions.

Driving the vehicle a short distance, I can smell oil burning after the car is stopped. I have had to put oil in the car twice because the warning light came on suggesting that a quart of oil is needed. The vehicle has less than 122,000 miles.
